1.Cytologic Heterogeneity of Signet Ring Cell Carcinoma of the Stomach: Histochemical and electron microscopic observations.

The cytologic heterogeneity of the tumor cells in gastric signet ring cell carcinoma was studied with 13 surgically early gastric carcinoma specimens by means of histochemical stainings on mucin(periodic acid Schiff-alcian blue at pH 2.5, paradoxical concanavalin A, high iron diamine) and electron microscopy. Of the 13 cases of early gastric cell carcinomas, 6 were mucosal type and 7 were submucosal type. Eleven cases consisted of mixture of gastric and intestinal type signet ring cells and the remaining 2 of the mucosal type were entirely made of gastric type. The colonic goblet cell type was found in 4 of the submucosal type. Within the mucosa the tumor cells showed a layering phenomenon; type A signet ring cells were distributed at the central zone and type B and C at the superficial or deeper zone. Each type of signet ring cell showed variable mucin histochemical stainability of gastric and/or intestinal nature. Above finding strongly suggest that the variable phenotypes of signet ring cells result from a heterogeneity of cytoplasmic mucin as well as different stages of differentiation of signet ring carcinoma cells.

4.Two Cases of Double-Chambered Right Ventricle by Abnomal Muscle Bundles.

The double-chambered right ventricle is congenital or acquired cardiac anomaly, which is characterized by aberrent hypertrophied muscular bands that divide the right ventriclar cavity into two different pressure chamber. This anomaly can complicate the natural history of patient with isolated ventricular septal defect. We experienced two cases of cases of acquired DCRV, which confirmed by two separate cardiac catherterization and angiography. The purpose of this report is to show that the aberrant muscular bands may be nonobstructive in early infancy and that the obstructive effect is developed with time as the bands become progressively more hypertrophied.

7.Primary Angiosarcoma of the Spleen associated with Kasabach-Merritt Syndrome: Report of a case.

A 46-year-old woman presented with multiple purpuric skin rashes together with splenomegaly and thrombocytopenia. The resected spleen weighed 720 gm, and contained multiple, discrete, dark red, bulging nodules throughout the entire splenic parenchyma. Microscopically, the nodules revealed various range of endothelial cell proliferation which included cavernous vasculature, anastomosing vascular channels, papillary intravascular growth and solid pattern. Intrasinusoidal sequestration of platelets along with extramedullary hematopoiesis was present in the neoplastic vascular spacws. Neither exposure to thorotrast nor vinyl chloride was nonfirmed. Ultrastructurally, the tumor cells demonstrated a row of pinocytotic vesicles, occasional intercellular junctional structures, and the paucity/absence of filamentous structures or Weibel-Palade bodies supports origin of sinus lining type cells. After splenectomy, the thrombocyte count returned to normal. We conclude this is the first case of primary splenic angiosarcoma with Kasabach-Merritt syndrome in Korean literature.

8.Clinical Observation of Acute Myocardial Infarction.

A Clinical study was done on 90 cases of acute myocardial infarction admitted to Busan Wallace Memorial Baptist Hospital from January, 1977 to Febuary, 1983. The following results have been obtained. 1) The ratio of male to female was 2.2:1. The most age group were 5th decades(35.6%) and over 5th decades(5th decades and 6th decades) were two-thirds of all age group. And annual incidency were increasing tendencies, in 1980-1982 than 1977-1979. 2) Between the preceding diseases and the risk factors, the most common cause was the smoking (60%). And other associated diseases and factors were hypertension(54.4%), hypercholesterolemia (43.8%), hyperlipidemia(51.4%) and angina pectoris (34.4%). 3) The most common inducing factor was physical exertion(33.3%). And others were emotional stress(23.3%), alcohol drinking(10%), rest and sleeping(5.6%). 4) The major symptoms of acute myocardial infarction were precordial or epigastric pain(96.7%), dyspnea(53.3%), radiating chest pain (51.7%) and painless infarction(3.3%). 5) On laboratory data, there were leukocytosis in 50.5% increased SGOT in 68.6%, increased serum LDH in 75.9% and cardiomegaly in chest x-ray film were 82 percents. 6) The ratio between the anterior and inferior infarction on EKG was 1.9:1. 7) When admission, normal sinus rhythms on EKG were 59 percents. And the common associated electrocardiographic abnormalities were ventricular premature beat (23.6%), atrial fibrillation(15.7%), first degree A-V block (15.7%), left ventricular hypertrophy(15.7%). The most common abnormality was ventricular premature beat(23.6%). 8) The mortality rate of acute myocardial infarction was 10 percents. The causes of death were heart failure(33.3%), ventricular arrhythmia(33.3%), sudden death (22.2%) and cardiogenic shock(11.1%).

9.Primary Appendiceal Papillary Adenocarcinoma of Colonic Type: Report of a case.

We report a case of colonic type-papillary adenocarcinoma of appendiceal origin in a 73-year-old male patient. The patient presented with right lower quadrant mass and was operated for a preoperative diagnosis of inflammatory small bowel mass. The mid one-third of the appendix showed a 3.5x3.3 cm sized, broad-based, intraluminal papillary mass. Microscopically, it was a well differentiated papillary adenocarcinoma and revealed a strong immunoreactivity to carcinoembryonic antigen. Tumor desmoplasia and acute inflammatory change were prominent.

10.The Changes of EEG Nonlinear Parameter in Sleep and Wakefulness States.

BACKGROUND: Up to now, sleep stages have traditionally been determined by the visual inspection of individual EEG waves. However, the exact physiological meaning of the sleep waves is not known. The purpose of this study was to try and find out the physiological parameters of the EEG of the sleep and wakefulness states by calculating one of the non-linear chaos parameter, the largest Lyapunov exponent (LLE), of EEG time series. METHODS: The digital EEG of the wakefulness with eye opening (WEO), wakefulness with eye closure (WEC), stage1 (S1), stage2 (S2), stage3 or 4 (S34) were recorded at centroparietal region (C4-P4 bipolar derivation) in 10 normal subjects. Lyapunov exponents of 50 EEG time series in different states were compared. RESULTS: LLE's of WEO, WEC, S1, S2, S34 showed an increas-ing tendency as states switched from wakefulness to sleep. LLE of sleep was larger than that of awake state. CONCLUSIONS: The EEG of the sleep state appeared to be more chaotic than that of the awake state. This nonlinear chaos parameter can be used as a physiological parameter of normal sleep and awake states.
