Neurofibroma, one of the peripheral nerve tumor, is a complex benign tumor arising from skin, deep soft tissue, nerve tissue and bone. The incidence of peripheral nerve tumor is relatively low and it may occur either solitarily or diffusely. Plexiform neurofibroma, a type of neurofibroma, develops characteristically in the patients with von Recklinghausen's disease and its exact incidence is unknown. The case reports of solitary plexiform neurofibroma not associated with von Recklinghausen's disease were extremely rare. We reported a case of solitary plexiform neurofibroma on the median nerve.

3.The Late Blink Reflex Response in Patients with Thalamic Hemorrhage.

BACKGROUND: This study was designed to understand the possible pathways of R2 responses by testing the blink reflex in patients with a thalamic hemorrhage. METHOD: Blink reflexes were obtained in 11 patients with a thalamic hemorrhage. The mean interval of the first test of the thalamic hemorrhage was 21.7(24.4days, and follow-up tests were performed in 5 patients about 10 months later. RESULTS: All subjects showed normal R1 responses on affected or normal sides. However, R2 responses were abnormal in 9 patients (81.8%). R2 responses were not recorded in 4, and prolonged in 3. Two patients showed both absent and prolonged R2 responses. The abnormal R2 responses were grouped as follows. First, R2 was abnormal bilaterally by the stimulation on the affected side (afferent type, N=4). Second, R2 was abnormal bilaterally by the stimulation on the affected side and abnormal contralaterally by the stimulation on the normal side (mixed type, N=2). Third, R2 was abnormal contralaterally by the stimulation on the affected side (undetermined type, N=2). One showed abnormal R2 bilaterally by the stimulation on the affected and normal side (coma type). On follow-up tests, all showed normalized R2 responses. CONCLUSIONS: Thalamic hemorrhages caused various abnormal R2 responses in blink reflexes, which were normalized in several months. It is suggested that the abnormal R2 responses of thalamic hemorrhage result from the removal of crossed cortical facilitation on brainstem interneurons rather than an interrupted central pathway of the R2 response.

4.Evaluation method for vinyl chloride exposed workers.

BACKGROUND: This study was conducted to evaluate vinly chloride exposure in vinyl chloride and poly vinyl chloride manufacturing factories. The object of this study was compare two different type of air sampling method that of, NIOSH and OSHA recommended and Investigate quantitative correlation between level of vinyl chloride exposure and urinary thiodiglycolic acid excretion. METHODS: Air sampling was conducted by two method and sampling mediums were located at workers breathing zone in the same location. Sampling mediums were changed in 60 minute interval and sampling pumps were recalibrated at the same time. Urine was collected before and end of shift were stored frozen and determined by GC/FID analysis. RESULTS: In NIOSH method, time weighted average was 3.562 +/- 2.898 ppm and OSHA method time weighted average was 4.051 +/- 3.700 ppm. Concentration of urinary TdGA in before shift was 0.527 +/- 0.828 g/g creatinine and end of shift was 4.190 +/- 7.665 mg/g creatinine. Difference of urinary TdGA between end of shift to before shift was 3.662 +/- 7.865 mg/g creatinine. In NIOSH method, correlation coefficient between 8hour-time weighted average (8hrs-TWA) and urinary concentration of TdGA was r=0.666. and last period vinyl chloride level (6th-TWA) and urinary TdGA concentration was r=0.972. In OSHA method, correlation coefficient between last period vinyl chloride level (6th-TWA) and urinary concentration of TdGA was r=0.976 and this was highest value. CONCLUSION: There was no statistically significant difference in NIOSH method and. OSHA method. A correlation were found between level of vinyl chloride exposure and urinary thiodiglycolic acid excretion. Also, vinyl chloride exposure had significantly effected on the urinary thiodiglycolic acid excretion.

6.Comparison of Blood Donor Reactions in Whole Blood Donations, Plasmapheresis and Plateletpheresis.

BACKGROUND: Several kinds of adverse reactions can occur during blood donation such as vasovagal reaction (VVR), hematoma, citrate toxicity, etc. These adverse reactions are not common, but they are important because they cause a decrease in re-donation. The cost for maintaining a repeat donation is very low compared to that for securing first-time donors. Whole blood donation differs from apheresis in some aspects, and this could have an influence on blood donor reactions. We compared whole blood donation with apheresis for blood donor reactions. METHODS: From January to December in 2007 at Busan Red Cross Blood Center, 109,004 donations were investigated for blood donor reactions. 76,098 (69.8%) donations were from male donors and 32,906 (30.2%) were from females. 77,813 (71.3%) donations were for whole blood, 25,224 (23.2%) were for plasmapheresis and 5,967 (5.5%) were for plateletpheresis. RESULTS: The frequencies of VVR were 0.10% (75/77,813) for the whole blood donations, 0.15% (37/25,224) for plasmapheresis and 0.03% (2/5,967) for plateletpheresis (P<0.05). The frequency of hematoma was 0.05% (37/77,813) for whole blood donation, 0.25% (62/25,224) for plasmapheresis and 0.27% (16/5,967) for plateletpheresis (P<0.05). Citrate toxicity was extremely rare. VVR was most common in plasmapheresis, and it was rare in plateletpheresis. CONCLUSION: The kinds of donated blood components had an influence on blood donor reactions. Understanding these characteristics helps to prevent adverse reaction. Having people re-donate is essential for keeping a large sized donor pool. So, appropriate management to prevent donor reactions is very important.

7.Study on the Comparison between Wide Excision and Mohs Micrographic Surgery for the Management of Dermatofibrosarcoma Protuberans: A Single Institution Experience.

BACKGROUND: Dermatofibrosarcoma protuberans is a mesenchymal tumor of the skin of intermediate-grade which is a rare condition. The slow growing and aggressive invasion on local tissues are characteristic features of dermatofibrosarcoma protuberans. The treatment for dermatofibrosarcoma protuberans is mainly a surgical excision such as a wide excision and Mohs micrographic surgery. OBJECTIVE: The aim of this study was to compare the result of wide excision and Mohs micrographic surgery for the treatment of dermatofibrosarcoma protuberans at a single institution in Korea. METHODS: A retrospective review was done for 24 patients diagnosed with dermatofibrosarcoma protuberans and treated surgically from 1999 to 2010 at Chonbuk National University Hospital. Patient demographics, tumor features, surgical features, and recurrence during the follow-up period were evaluated. RESULTS: 13 patients were treated with wide excision, and 11 with Mohs micrographic surgery. There was no metastasis for all the cases. Mean operation time for the wide excision group was 83 minutes whereas 182 minutes for the Mohs micrographic surgery group, and it was a statistically significant difference. However, no significant difference was observed in post-operative defect size, advanced surgical repair and local recurrence in our study. CONCLUSION: We suggest that wide excision and Mohs micrographic surgery are both successful modalities for the surgical treatment of dermatofibrosarcoma protuberans. Hence, individualized patient and tumor characteristics should be concerned when determining the surgical options for dermatofibrosarcoma protuberans.

8.Relationship of Aircraft-noise and the Result of Audiological Evaluation Among Residents Near U.S. Military Airbases in Pyeongtaek City.

Objective: To investigate the relationship between aircraft noise and the results of audiological evaluation on tinnitus and hearing loss among residents near United States military air-bases in Pyeongtaek city. METHODS: Residents (n=492) exposed to aircraft noise were selected from eight villages near U.S. military air-bases (K-55 and K-6) in Pyeongtaek city. Residents (n=200) from five villages located at least 10 km away from the air-base were selected for the control group. All participants completed a questionnaire on tinnitus and audiological evaluations included pure tone audiometry (PTA) and distortion product otoacoustic emissions (DPOAE). Statistical analyses involved a general linear model and multiple logistic regression stratified by gender and data was adjusted by age, educational level, stress response index and noise type (i.e. military, agricultural and occupational). RESULTS: The equivalent noise levels (Leq) in the vicinity of the air-field were 73.4~81.5 dB (A). Tinnitus prevalence was 47.3% and odds ratio (OR) was 2.06 (95% confidence interval (CI): 1.09~3.88) among noise-exposed males. Tinnitus prevalence was 50.8% and OR was 1.97(95% CI: 1.17~3.30) among noise-exposed females. Tinnitus handicap scores among exposed group were significantly higher in functional, emotional, and catastrophic subclasses (p<0.001). PTA revealed hearing thresholds among the exposed group of 0.5 and 1 kHz (male right ear), 0.5 and 1 kHz (female left ear) and 0.5, 1, 2, 3, 4 and 6 kHz on female right ear that were significantly higher than those of the unexposed group. No significant difference in DPOAE was observed among noise-exposed males. However, significant differences were evident for noise-exposed females at 3175 and 4007 Hz. ORs of right female ears 3175 and 4007 Hz were 1.73(95% CI: 1.01~2.99) and 1.78(95% CI: 1.01~3.15). ORs of left female ears at 3175 Hz and 4007 Hz were 1.92(95% CI: 1.10~3.36) and 2.71(95% CI: 1.49~4.91) CONCLUSIONS: Aircraft noise may adversely affect hearing function and tinnitus.

10.Group B Streptococcus Isolated from Bacteremic Patients: Serotypes and Antimicrobial Susceptibilities.

Group B Streptococcus (GBS, S. agalactiae) is known to be the leading cause of neonatal sepsis and meningitis and the infection has been increasingly noted in adults, particularly in those with underlying diseases. Penicillin G is the drug of choice for GBS infection. However, the MIC of penicillin for GBS is greater than that for S. pyogenes. Therefore some GBS infections may be difficult to be treated. However, in Korea, our knowledge on GBS infection is limited. We observed 7 cases of GBS bacteremia during 1993-1996 in a hospital, Of the 7 patients, 3 were less than one month of age with no known underlying disease and 4 were adults with liver cirrhosis or malignancy. One adult patient developed disseminated intravascular coagulopathy and expired. Among the GBS isolates, 4 were serotype III and 3 were Ib. All of the isolates were susceptible to ampicillin, teicoplanin and vancomycin, but most were intermediate or resistant to clindamycin, erythromycin or tetracycline. It is concluded that GBS also cause severe infections in adult with underlying diseases and the serotypes III and I b may be more virulent than other serotypes. Early detection and antimicrobial susceptibility test of GBS from severe infection may be necessary for the proper treatment of the patients.
