2.Cardiopulmonary Resuscitation Learning Experience, Knowledge, and Performance in Newly Graduated Nurses.

PURPOSE: The purpose of this study was to evaluate the learning experience, knowledge, and performance of cardiopulmonary resuscitation (CPR) in newly graduated nurses, and to identify differences related to learning experience. METHODS: The participants were 114 new nurses in the hospital. They were asked to complete a questionnaire, which included CPR learning experience. They were evaluated by a written test and a skill test using a manikin and check list. RESULTS: All participants attended CPR lectures and underwent practice while in university. Only 12.28% of participants were taught by a certified Basic Life Support (BLS) instructor. The mean scores of the written and skill tests were 79.82+/-12.69 and 64.41+/-11.71, respectively. The nurses lacked CPR knowledge related to checking breathing, the frequency of 30 chest compressions, compression rate, and automated external defibrillator use. They also lacked skill in performing CPR related to checking breathing and pulse and giving 2 breaths. CPR performance differed according to learning time (p=.047) and BLS educator (p=.029). CONCLUSION: The findings of this study reveal that CPR performance by newly graduated nurses is poor and suggest that CPR education by trained instructors, practice-based education, and reeducation programs must be provided to newly graduated nurses in the hospital.

3.A Case with Neuro-Sweet Syndrome.

Sweet's syndrome, or acute febrile neutrophilic dermatosis, is an unusual disease characterized by fever, leukocytosis, and distinctive skin lesions. Common complications include arthralgia, arthritis, conjunctivitis, and iridocyclitis. However, the involvement of the central nervous system in this disease, termed `neuro-Sweet syndrome' is rarely reported. We present a patient with recurrent encephalitis for 15 years, accompanied with neutrophilic dermatosis, and characterized by HLA-B54 and Cw1, with good responsiveness to corticosteroid.

4.Asymptomatic Extrapontine Myelinolysis in Diabetic Woman.

Osmotic myelinolysis syndrome in central nervous system is classified into central pontine myelinolysis(CPM) and extrapontine myelinolysis(EPM). The former can cause a spastic quadriplegia, pseudobulbar palsy, and varying degree of mental disorder, but the latter a movement disorder(tremor, dystonia, parkinsonism, and chorea, etc), behavior change(mood instability, personality change, agitated delirium, and disinhibition, etc), and cognitive dysfunction. Although a few cases of asymptomatic CPM have been reported, asymptomatic EPM were rare. A 67-year-old woman with diabetes mellitus and old cerebral infarction has suffered from the common bile duct stone. She had hyponatremia(Na=126mEq/L) on admission and was corrected rapidly. Incidental findings of her brain MRI showed EPM. We experienced a case of bilateral middle cerebellar peduncle myelinolysis incidentally without neurological presentations and report a rare case of asymptomatic EPM recoverd in diabetic woman after rapid correction of hyponatremia.

Systemic lupus erythematosus, an autoimmune disease with multisystem involvement, has been reported to be associated with a number of gastrointestinal complications and symptoms such as nausea, vomiting, and abdominal pain. However, acute pancreatitis only rarely has been reported as a complication of SLE. We report a case of SLE presenting drug unrelated acute pancreatitis as a initial manifestation.

7.Effects of gamma-irradiation on intracellular proliferation of Toxoplasma gondii RH tachyzoites.

A quantitative assay was performed on the effects of gamma-irradiation (30-300 Gy) on intracellular proliferation of Toxoplasma gondii RH tachyzoites in human leukemic HL-60 cells and murine peritoneal macrophages by means of 3H-uracil uptake assay. Infected non- irradiation group (NI) and uninfected group (incubating only host cells) were prepared. The 3H-uracil uptake by tachyzoites of NI group 12-24 hrs after infection was 2,190-4,787 counts per minute for macrophages and 2,967-8,254 for HL-60 cells, whereas the irradiated tachyzoites revealed only 381-703 (100 Gy) and 218-408 (300 Gy) for macrophages, and 1,911-2,618 (30 Gy), 1,253-1,384 (70 Gy), 1,013-1,090 (100 Gy), and 483-588 (300 Gy) for HL-60 cells. The proliferation inhibition rate was similar in macrophages and HL-60 cells, for example, 89-94% and 80-94% respectively by 300 Gy, 12-24 hrs after infection. It is concluded that RH tachyzoites of T. gondii are severely affected by gamma-irradiation in their capability of intracellular proliferation.

10.Effects of Nocodazole on Protein Synthesis Appratus of Tumor Cells.

Nocodazole is an anticancer agent that acts on microtubules or filaments. HeLa, Hep G2, A549, L929 and NIH/3T3 cell lines were cultivated in alpha-MEM with 3micrometer or 30micrometer nocodazole. To elucidate the associations between nocodazole`s antitumor actions and these effects, the influences of nocodazole on the cellular morphology and the organelles involving synthesis, secretion and destruction of proteins were investigated under light and electron microscopes. The changes of intermediate filaments such as pancytokeratins and vimentins that maybe suggest antimetastatic action of nocodazole were observed using immunocytochemical technique, PAP at light microscopic level. Rounded or micronucleate cells were induced by treatment with 3micrometer and 30micrometer nocodazole for 2 hours to 4 days. Multimicronucleate cells appeared in experimental groups of all cell lines. Nuclear foldings occurred in cells of experimental groups treated with nocodazole for 2-3 days. The numerical increases of rough endoplasmic reticulum were observed in HeLa cells treated with nocodazole for 3 days and the dilatation or numerical increases in L929 cells treated with nofodazole for 1-3 days. The fragmentations or dispersion of Golgi complex were observed in cells treated with nocodazole for 1-3 days. The amount of filaments increased in cells treated with nocodazole for 1-3 days. The number of lysosomes increased in cells treated with nocodazole for 1-3 days. The number of liposomes also increased in Hep G2 cells treated with 30micrometer nocodazole for 3 days and in 3micrometer & 30micrometer, 3 days group of 3T3 cells. The amount of pancytokeratins and vimentins increased in cells treated with nocodazole for 1-3 days. Taken together, depolymerization of microlubules was induced by nocodazole, and then the organization of cells was disintegrated. As a result, the rounded cells, the cells having multimicronuclei, and the changes of golgi complexes occurred. But there were relatively no great changes of rough endoplasmic reticulum. The amount of intermediate filaments that maintain the differentiated states of cells increased by nocodazole treatment. It was suggested that morphological changes of cells could be used in evaluation of actions of nocodazole. Especially, the increase of amount of intermediate filaments by nocodazole changed cells of each cell line from undifferentiated state to differentiated, and therefore the author hope that the changes in amount of intermediate filaments provide an important clue in anticancer and antimetastatic actions of nocodazole.
