The most frequent complication of the open arthrotomy for the cruciate reconstruction are quadriceps weakness and flexion contracture of the knee. Besides complications such as chondromalacia, patellar maltracking, saphenous neuralgia and patellar crepitus are not infrequent in wide open arthrotomy. We adopted an open method with smaller incision like arthroscopic maneuver to reduce these complications. The pre-requisite of this mini-incision are arranging a proper instrument and graft material. We advocate Achilles autograft which is longer and more flexible than other autografts and by taking advantage of these characteristics, we could performed reconstruction with relatively small incision. The advantages of mini-incision technique are early recovery of quadriceps function and little complication. We applied this method in ACL, PCL and simultaneous ACL & PCL reconstruction, with the result of early recovery of qudriceps function.

2.A Case of Narcissistic Personality Disorder with Brief Psychotic Episode.

OBJECTIVES: The Objective of this case report is to present basic materials of clinical symptomatologic study in narcissistic personality disorder by reviewing a case diagnosed as narcissistic personality disorder with brief psychotic episode. METHODS: The subject was a psychiatric inpatient diagnosed concomitantly as narcissistic personality disorder and brief psychotic disorder by DSM- IV. Practical clinical diagnostic process for narcissistic personality disorder was applied and reviewed. Clinical characteristics of brief psychotic episode in narcissistic personality disorder such as precipitating psychological factors, main psychotic symptoms, and course were investigated as well. RESULTS AND CONCLUSION: For diagnosing narcissistic personality disorder, it is essential besides superficially manifested symptoms, to search for underlying psychological meanings of those symptoms, careful observations of behavior and attitude, and to take reliable informations from family or relatives. In case of encountering blow-out of pathologically inflated grandiose self mainly in middle age, brief psychotic episode could be broken out. Main features of the episode were affective symptoms, failure in impulse and behavior control, and perceptual disturbances rather than thought disorder.

4.Percutaneous Transgastric Stent Placement for Malignant Gastroduodenal Obstruction

Purpose:
To evaluate the technical feasibility and clinical efficacy of percutaneous transgastric stent placement after the failure of treatment attempt with the transoral approach in malignant gastroduodenal obstruction patients.
Materials and Methods:
From October 2008 to April 2016, nine patients (M:F = 4:5; mean age = 66 years) with malignant gastroduodenal obstruction underwent stent placement via a gastrostomy tract, which was attributed to the failure of the transoral approach. The primary etiologies of the obstruction were pancreatic (n = 5), gastric (n = 2), and metastatic (n = 2) cancers. Through percutaneous gastrostomy, dual stents (inner bare metal and outer polytetrafluoroethylene-covered) were deployed at the obstruction site. The technical and clinical success rates, as well as complications were evaluated during the follow-up period.
Results:
Stents were successfully inserted in eight patients (88%). We failed to insert stent in one patient due to the presence of a tight obstruction. After stent placement, symptoms improved in seven patients. Gastrostomy tube was removed 9 to 20 days (mean = 12 days) after the stent insertion. During the mean follow-up of 136 days (range, 3–387 days), one patient developed a recurrent symptom due to tumor overgrowth. However, there were no other major complications associated with the procedure.
Conclusion
Percutaneous transgastric stent placement appeared to be technically feasible and clinically effective in patients who underwent a failed transoral approach.
5.Two Cases of Olfactory Reference Syndrome.

Disease identity of dysmorphophobia and olfactory reference syndrome which used to be manifested by a kind of monosymptomatic somatic delusion is not yet established definitely. So many issues such as whether it be regarded as separate disease, or a spectrum of preexisting diseases such as schizophrenia or obsessive compulsive disorder and whether the monosymptom should be considered as a delusion or not are still on debate and the results are controversial. Consequently few is known about the etiology of the disorder.Obvioulsy, somatic delusion in dysmorphophobia and olfactory reference syndrome is the result of cognitive distortion of body image of the self. It reflects symbolic meaning of unresolved psychological conflict by defense mechanisms such as repression, dissociation, symbolization, projection, and so on. Though psychopharmacological and psychotherapeutic treatment modality has been reported, the treatment efficacy is unclear. Prognosis has been reported unfavorable and one of the leading causes has been thought to be treatment noncompliance. The authors experienced 2 cases of olfactory reference syndrome, so reported with literature review.

6.A Case Report of Triazolam Dependence: Focusing Anxiety Symptoms and Treatment.

Benzodiazepines, which used to and would be prescribed frequently as anxiolytic sedative hypnotics not only in psychiatric field but also in almost all clinical departments, are prone to develop tolerance, physical and psychological dependence, and withdrawal symptoms after abrupt discontinuation. Owing to shorter plasma half life, severe degree of tolerance, physical and psychological dependence, and withdrawal symptoms after abrupt discontinuation have been developed in a case of triazolam dependence. Nevertheless, no single case of triazolam dependence was reported yet in Korea. The authors experienced a single case of triazolam dependence, so report with literature review.

BACKGROUND: The purpose of this study was to identify the effects of Tai Chi exercise program on physical symptoms (pain, stiffness) and physical function (difficulty with physical function) related to joint, stress, depression, and sleep disturbance in middle aged women with osteoarthritis. METHODS: The study was conducted with a nonequivalent control group pretest-posttest design. Forty participants were assigned to either the experimental group (20) or control group (20). Tai Chi exercise program was administered twice a week for 12 weeks to participants in experimental group. Data were collected from October 4 to December 24, 2011 and analyzed using chi2-test and t-test. RESULTS: There were significant differences between groups in joint pain score (z=-2.029, P=0.047) of Korean-Western Ontario and McMaster University Osteoarthritis (K-WOMAC) index and depression score (z=-2.566, P=0.010). However, there were no significant differences in stiffness score and physical function score of K-WOMAC index, stress score and sleep disturbance score. CONCLUSIONS: Findings from this study indicate that Tai Chi exercise program are effective in reducing joint pain and depression in osteoarthritis women. It could be regarded as positive program for health promotion of self-care skills in middle-aged women with osteoarthritis.

9.A Study on Climacteric Symptoms, Knowledge of Menopause, Menopausal Management in Middle-Aged Women.

The purpose of this study was to identify the relationship between self reported climacteric symptoms, knowledge of menopause, menopausal management, in middle-aged women. The subjects of this study were 209 women from 40 to 64 years of age. The data was analyzed using SPSS program for t-test, ANOVA and Pearson's correlation coefficients. The results of the study were as follows : 1. The characteristic variables significantly related to self reported climacteric symptoms were education(F=6.371, p<0.001), income (F=4.481, p<0.05), mensturation state(t= -2.004, p<0.05), mensturation cycle(t=-2.039, p<0.05), hormone therapy(t=2.107, p<0.05), health condition(F=21.111, p<0.001).2. The characteristic variables significantly related to knowledge of menopause were education(F=6.580, p<0.001), health condition (F=3.487, p<0.05).3. The characteristic variable significantly related to menopausal management was income(F=3.080, p<0.05). 4. The relationship between self reported climacteric symptoms and menopausal management was statistically significant with a negative correlation(r=-0.323, p<0.05).In conclusion, climacteric symptoms - based complaints by middle-aged women was negatively correlated to menopausal management. Therefore, health promotions designed to increase climacteric women's health should be a planned program based on results of the study.
