1.Central nervous system involvement in primary Sjgren's syndrome

Objective To investigate the clinical features of central nervous system involvement in pSS.Methods The clinical data of 21 CNS-SS patients in our hospital were retrospectively reviewed.Results There were a variety of neuropsychiatrie presentations in pSS,in which 12 patients had brain involvement and 9 patients had spinal cord involvement,accounting for 57.1% and 42.9% respectively.Seven patients had con- current peripheral nervous system involvement.Neurological involvement of 10 patients preceded other syrup- toms of pSS.The mean duration of pSS at the onset of CNS-SS was(4?6)years.Abnormal MRI results had been found in 13 patients,of which 8 patients had demyelination lesions,which was the most frequent presen- tation,and accounted for 38.1%.Cerebral spinal fluid abnormality was seen in 11 CNS-SS patients,among which protein elevation was found in 10,pressure elevation in 6 and white cell count elevation in 5.Conclu- sions Central nervous system may be involved in pSS.MRI and cerebral spinal fluid tests are essential for the diagnosis of CNS-SS.CNS-SS may features of spinal cord and the peripheral nervous system involvement CNS-SS usually carries a mild clinical process and frequently oceurrs in stable pSS.
2.Curative efficacy of Angelica containing selenium in treatment of chronic renal failure and its effects on Micro-inflammatory state

Objective To study curative efficacy of Angelica containing selenium in treatment of chronic renal failure and its effects on Micro-inflammatory state.Methods A total of 80 patients of chronic renal failure who received therapy from March 2014 to March 2015 in our hospital were selected as research objects,and divided into observation group and control group,40 patients in each group.The control group was treated with routine treatment,while the observation group was treated with routine treatment with selenium enriched Angelica.Then renal function indicators including blood urea nitrogen(BUN),serum creatinine (SCr),hemoglobin(Hb)levels changes,micro inflammatory state including C reactive protein(CRP),interleukin-6 (IL-6),tumor necrosis factor (TNF-a) level changes,TCM syndrome points,treatment efficacy of two groups before and after treatment were compared.Results After treatment,BUN,SCr in observation group were less than those in the control group[(16.08±3.29)mmol/L vs.(21.50±3.68)mmol/L,(330.21±81.96)μmol/L vs.(390.86±84.24)μmol/L],Hb was higher than that in the control group[(107.38±7.37)g/L vs.(98.27±6.36)g/L](P<0.05).CRP,IL-6,TNF-α in observation group were less than those the control group[(5.92±1.14)mg/L vs.(7.26±2.02)mg/L,(24.75±7.12)pg/mL vs.(41.08±9.07)pg/mL,(112.16±20.16)ng/L vs.(134.46±22.16)ng/L],the differences were statistically significant(P<0.05).The score of TCM syndrome in observation group was smaller than that in control group[(5.01±1.21)score vs.(9.36±2.43)score],the difference was statistically significant(P<0.05).The total effective rate of the observation group after therapy was better than the control group[95.00%(38/40) vs.75.00%(30/40)],The difference was statistically significant(P<0.05).Conclusion Angelica containing selenium is well for chronic renal failure,which could effectively improve the micro inflammatory state,and could improve the therapeutic effect.
3.Vasoactive intestinal peptide and methylprednisolone change intestinal Toll-like receptor mRNA expression in rats with endotoxic shock

Objective To investigate the effect of vasoactive intestinal peptide (VIP) and methylprednisolone (MP) on Toll-like receptor (TLR)2/4 mRNA expression in endotoxin (lipopolysaccharide,LPS) induced shock.Methods Ninety Sprague-Dawley rats were randomly divided into LPS group (n =20),LPS + VIP group (n =20),LPS + MP group (n =20),LPS + VIP + MP group (n =20) and control group (n =10).LPS group injected intravenously LPS (E Coli O55B5) 10 mg/kg.LPS + VIP group,LPS + MP group and LPS + VIP + MP group were injected intravenously VIP 5 nmol/kg,MP 3 mg/kg and VIP 5 nmoL/kg + MP 3 mg/kg after LPS 10 mg/kg injection.The control group injected normal saline intravenously instead of LPS.The rats were sacrificed at 6 h and 24 h after injection and the intestine samples were collected.Pathological changes of the intestine were observed by microscopy.RT-PCR was used to detect the intestinal TLR2 mRNA and TLR4 mRNA expressions.Results Intestinal mucosa showed edema or necrotic change with structure of the microvilli disappeared after LPS injection.The inestinal lesions in VIP,MP and VIP + MP groups were milder than LPS group.At 6 h after LPS injection,TLR2 mRNA and TLR4 mRNA expressions were significantly up-regulated in LPS group,LPS + VIP group,LPS + MP group and LPS + VIP + MP group (TLR2 mRNA:1.14 ±0.38,1.17 ±0.42,1.16 ±0.41,0.92 ± 0.29;TLR4 mRNA 1.21 ±0.18,1.04 ± 0.38,1.11 ± 0.34,1.01 ± 0.20) compared with the control group (0.32 ± 0.20,0.24 ± 0.17) (P < 0.01).But there was no significant difference between LPS group,LPS + VIP group,LPS + MP group and LPS + VIP + MP group (P > 0.05).At 24 h after LPS injection,TLR2 mRNA and TLR4 mRNA expressions in LPS + VIP group,LPS + MP group and LPS + VIP + MP group (TLR2 mRNA:0.63 ± 0.12,0.59 ± 0.13,0.52 ±0.19;TLR4 mRNA 0.67 ±0.09,0.64 ±0.09,0.51 ±0.13) were significantly lower than LPS group (1.04 ± 0.38,0.82 ±0.18) (P <0.01) (P <0.05).Conclusion VIP and/or MP can mitigate intestinal injury induced by LPS shock.The gastrointestinal protection of VIP and glucocorticoids were related to downregulation signaling TLR2 mRNA and TLR4 mRNA expression.But VIP/MP and VIP + MP have no significant effect on expression of intestinal TLR2/4 mRNA until 24 h after LPS shock.

Objective To investigate a cure for neonatal hypoxic-ischemic encephalopathy(HIE) by observing the curative effect on neonates with HIE admitted to neonatal intensive care unit (NICU).Methods Fifty neonates with HIE from May to Dec.in 2006 admitted to NICU for treatment(NICU group),which contrast to the 42 neonates with HIE in corresponding periods of 2005 year(control group).The curative effect of three supports and three expectant treatment were evaluated by observing vital signs,oxygen saturation,biochemical test,and the neonatal beha-vioral nerve assessment (NBNA) respectively; Clinical curative effect contained markedly effective and effective which as the total curative effect statistics.Results There were no obvious difference between each group from the means of blood sugar data while hospitalizing as well as the NBNA in 3 days. ObjectiveTo summarize experiences of diagnose and disposal of acute abdomen of patients with spinal cord injury (SCI) in order to raise the successful rate.MethodsThe data of 10 acute abdomen patients with SCI were analyzed retrospectively.ResultsAmong 10 cases, 9 cases were cured, 1 case died.ConclusionDiagnosing and managing acute abdomen of patients with SCI in time can get satisfied outcome.
7.Interlaboratory method validation of HPLC-FMA for determination of polysorbate 80 in monoclonal antibodies

The high performance liquid chromatography-fluorescence micelle assay (HPLC-FMA) method for the content determination of polysorbate 80 in monoclonal antibody drugs was validated to study its applicability and transferability between various laboratories, and the feasibility to be included in the Chinese Pharmacopoeia. Both J.T. Baker and Nanjing Well-sourced polysorbate 80 was used in the collaborative validation of polysorbate 80 content analysis in seven different laboratories. The results show that when the protein concentration was no more than 20 mg·mL-1 and the concentration of polysorbate 80 ranged from 0.05 to 0.5 mg·mL-1, the method had good specificity. The recovery rates of the spiked samples ranged from 92.20% to 117.70% for J.T.Baker and from 93.90% to 117.20% for Nanjing Well. The intra-laboratory precision (%RSD) was less than 4.30% for J.T. Baker, and less than 2.60% for Nanjing Well, while the overall precision was less than 5.45% for J.T. Baker, and less than 6.70% for Nanjing Well. The linear correlation coefficient was more than 0.98 for J.T. Baker and more than 0.99 for Nanjing Well. The results of the collaborative validation prove that the HPLC-FMA method has good accuracy, precision, linearity, and specificity, and could be used for release control analysis of polysorbate 80 content in monoclonal antibodies across different laboratories.
8.VATS thymectomy for myasthenia gravis: a retrospective review of 500 cases

Objective To review video-assisted thoracoscopic thymectomy as a treatment for myasthenia gravis (MG),compare outcomes of thoracoscopic thymectomy for thymoma and non-thymoma MG,and assess the efficacy of Video-assisted Thoracoscopic Extended Thymectomy (VATET) combined with mediastinoscopy.Methods A retrospective review of 500 patientswith MG who underwent VATS thymectomy between 2001 and 2011 has been done.They were divided into three groups:118 cases of thymoma MG group,thoracoscopy for non-thymoma MG group 301cases,and VATET for non-thymoma MG group 81cases.Results There was no mortality.Thoracoscopic thymectomy was successfully performed for 495 cases.In the thoracoscopy group for non-thymoma MG,the operating time is (111.3 ± 31.6) min,11.0% having post-operative myasthenic crises ; in the VATET group,the operating time is (145.0 ± 71.6) min,9.9% having post-operative myasthenic crises ;in the thymoma MG group,the operating time is (128.5 ± 77.8)min,24.6% having post-operative myasthenic crises.During the follow-up,CSR was 37.3%,36.5% and 28.7% in the groups of thoracoscopy for non-thymoma MG,VATET and thymoma MG respectively.However,the disease-free survival curve shows that CSR of the thymoma MG group became lower than other two groups 3 years after surgery,and CSR of the VATET group becoming higher than that of thoracoscopy for nou-thymoma MG group 5 years after surgery.CSRs of groups of thoracoscopy for non-thymoma MG,VATET and thymoma MG might reach 50%,60% and 36%.Conclusion The VATET combined with mediastinoscopy has a better long-term outcome because the more thymus might be removed comparing with non-thymoma MG,thoracoscopic thymectomy for thymoma MG had a worse long-term outcome.

Objective To investigate the metabolic changes and the metabolites distribution of different hippocampal regions (head,body and tail)in patients with Alzheimer’s disease (AD)using 1 H-MRS for early diagnosis.Methods The hippocampal multivoxel 1H-MRS at 3.0 T was scanned in 30 patients with AD and other 30 normal cognitive elders (NC)as contrast.The data obtained were processed at a workstation.The hippocampus was divided into 3 parts (head,body and tail),and the ratios of N-acetylaspartate (NAA)/creatine (Cr),myoinositol (MI)/Cr and MI/NAA were calculated respectively.The metabolite ratios and distribution changes were compared between group AD and NC.Results Compared with the group NC,the NAA/Cr in group AD in bilateral hippocampal body and tail was decreased,whereas the MI/Cr and MI/NAA in bilateral body and tail,MI/NAA in left head were opposite (P<0.01).In group NC,the NAA/Cr was gradually decreased from the bilateral hippocampal heads to tails (P<0.01),however,the MI/NAA was opposite (P<0.01).No distribution differences in every metabolic ratios of bilateral hippocam-pus were found in AD group (P>0.05).Conclusion Metabolic changes and disappearance of the normal distribution in different hippocampal regions detected by 1 H-MRS provide helpful clues for early diagnosis of the AD.
