3.Nitroblue Tetrazilium dye Reduction Test for Differential Diagnosis of Varilus Menigitis.

The usefullness of the nitroblue tetrazolium (NBT) test as an aid in the differential diagnosis of various meningitis was evaluated in infancy and childhood. 36 cases of patients with various meningitis who were admitted to the department of pediatrics in Severance Hospital from June, 1975 to March, 1976 and 8 cases of healthy children as control were the subject of this study. The following results were observed; the mean proportion of NBT-positive neutrophils was 4.0?2.0% in healthy control group, 17.0?.7% in patients with bacterial meningitis, 6.0?.3% in tuberculous menigitis, and 5.o?,1% in aspetic meningitis. And we couclued that NBT test is useful in differenciating various form of meningitis.

4.Distal Advancement of the Loose Anterior Cruciate Ligament

The anterior cruciate ligament has been considered one of the basic stabilizers in the knee as a very complex in its function. A large number of treatment regimens and philosophies showed controversies in its treatment. And we have experienced recurrence of the instability after operation of the injured ligament. So, the authors devised a new operative technique for the loose anterior cruciate ligament, that is, distal davancement of the loose anterior cruciate ligament with a bone-block as a biomechanical fixation. We have performed 11 cases of the anterior cruciate ligament using distal advancement method. The results were as follows; 1. Eleven cases (92%) showed good results by the rating system of Cincinnati sports-medicine and Orthopaedic Center. 2. The distal advancement of the anterior cruciate ligament is technically simple and biomechanically reasonable, but further investigation is necessary to answer the question regarding the long term results.

5.Clinical Study on NeonataI Meningitis.

A clinical study was made on 68 cases of neonatal meningitis occuring under the age of 1 month at the department of Pediatrics at Severance Hospital, Yonsei University College of Medicine from 1st Jan. 1965 to 31th Dec. 1981. The sex ratio of male to female was 1.8:1 approximately. Neonatal predisposing factors significantly associated with neonatal meningitis were omphalitis(14 cases), skin infection (13 cases), birth injury (9 cases) and pneumonia (8 cases) etc. The most common maternal predisposing factor was difficult labor (13 cases). In 27 out of the 68 cultured CSF, the most common organisms were E. coli (29.6%), Staphylococcus coagulase(+) (22.2%) and Beta meholytic streptococcus (22.2%). Gram negative organisms were found in 12 cases (44.4%). The most common presenting symptoms were non-specific in nature -an elevated or subnormal body temperature, convulsion, poor feeding, irritability, jaundice and vomiting in that order of frequency. The presence of a poor Moro reflex, neck stiffness, unconsciousness or convulsion correlated with the high mortality rate significantly. Complications and sequelae included convulsion(11 cases), subdural effusion (8 cases), candida infection (8 cases), hydrocephalus (2 cases) and cerebral hemorrhage (2 cases) in that order of frequency. In the 68 cases, there were 29 mortalities or 42.6%.

6.Chronic Cough and Sinusitis in Children: The Role of Antimicrobials.

We evaluated the role of antimicrobials in the treatment of chronic maxillary sinusitis in children with chronic cough. At the first visit 216 of 276 cases with chronic cough had radiologically abnormal maxillary sinuses (78.3%). By the Waters x-rays around 50 cases each were assigned randomly to four groups; among them, 48 cases were given an amoxicillin, 51 cases were given trimethoprim-sulfamethoxazole (TMS), 53 cases were given amoxcillin plus TMS and 50 cases were given expectorant plus decongestant (control) group. Thus, 202 children were included for data analysis. Treatment was given for two weeks in each group. The group treated with antimicrobials had a significantly higher cure rate than that of expectorant plus decongestant group (P < 0.001). Most of the patients who had roentgenographic improvement have successfully recovered from the chronic cough and the other cardinal signs of chronic sinusitis.

7.A Clinical Observation of Congenital Diaphragmatic Hernia.

Diaphragnatic hernia is one of the surgical emergencies of the childhood because of the life-threatening cardiorespiratory embarrassment frequently associated with it. This congenital condition is characterized by varying degrees of protrusion of the abdominal viscera into the thoracic cavity through an abnormal opening in the diaphragm this opening results from a defect in the complex embryologic developement of the diaphragm between the thoracic and abdominal cavities. We expenienced 16 cases of congenital diaphragmatic hernia which were diagnosed at Severance Hospital from May 1964 to April 1977, and obtained the following results. 1) In 16 cases, male were ten and female were six. They were eleven cases of bochdalek hernia, two cases of diaphragmntic eventration, one case of hiatal and two undetermined. 2) Onset of symptoms were within 24 hours in 7 cases and in the rest cases symptoms were occured up to 24 months. 3) Most common symptoms were dyspnea and cynosis. 4) Most common signs were decreased breathing sound on affected lung, PMI shifting, scaphoid abdomen and subcostal retraction. 5) Combined anomalies were band adhesion of intestinal loop, hypoplasia of left lung etc. 6) Small intestine were more frequently herniated as well as large intestine. 7) mortality rate between abdominal procedure and thoracic procedure was equal. 8) Cases whose symptoms were occured within 24 hours were 7, 5 of these 7 were expired, Expired cases were associated with marked respiratory difficulties and pulmonary hypoplasia.

9.Clinical Study on Spina Bifida and Cranium Bifidum.

Spina bifida or cranium bifidum result from a failure in fusion of skull(cranium bifidum) or the vertebral column(spina bifida). These entities are the most common anomalies of the nervous system which are associated with major abnormalities of cellular migration, and secondary mechanical deformities of the nervous system. During the last 12 years from Jan. 1967 to Dec. 1978, 83 cases of spina bifida and cranium bifidum had been admitted to the department of Pediatrics, Severance Hospital and evaluated clinically. 10.Selective Protein Index and Response to Treatment in the Childhood Nephrotic Syndrome.

In a group of 59 children with nephrotic syndrome who had been admitted to Severance Ho-spital, during the period of 2 years from July of 1978 till June of 1980, the histological picture seen on renal biopsy, the selective protein index, and the response to the steroid therapy were studied. The degree of selective protein index is not related to the total daily amount of urinary protein excretion and selective protein index remains relatively constant over months in the minimal lesion nephrotic syndrom. Those patients with primary renal disease and the nephrotic syndrome who displayed high selectivity types of proteinuria usually responds initially to steroid therapy within 8 weeks. Studies of selective protein index allow the pediatrician to avoid renal biopsy with safety in nephrotic children aged 1 to 5 years, but distinguish any given renal disease with certainty. The mesangial proliferative glomerulonephritis with immune deposits has relatively poor selectivity of proteinuria and without immune deposits, it has high or moder-ate selectivity of proteinuria. Response to treatment of steroid in the Henoch-Sch?lein Purpura was poor and not related with selective protein index.
