Multilocular cystic renal cell carcinoma appears to be a distinct and rare subtype of renal cell carcinoma with characteristic gross and microscopic features. Multilocular cystic renal cell carcinoma should be separated from multilocular cyst, multilocular cystic nephroma, and renal cell carcinoma with cystic degeneration. We present a case of multilocular cystic renal cell carcinoma. A 61-year-old man presented with right flank pain for 4 years. The computerized tomography revealed multilocular cystic mass in the upper pole of right kidney. The cystic mass measured 4.5x4 cm. The cyst was multilocular and locules not communicated with each other. The solid area was not present. Microscopically, the locules were lined by flat or cuboidal neoplastic clear cells. The clear cells were focally aggregated in the septa. The nephron was not present in the septa.

It is important to predict the prognosis of patient with cancer. Well known parameters predicting the prognosis are staged and degree of differentiation. Recently, the prognostic and diagnostic significance of silver-stained nucleolar organizer regions(Ag-NORs) and flow cytometric DNA analysis are introduced. This study aims to detect the diagnostic significance of Ag-NORs in breast lesions, and to correlate the number of Ag-NORs and cellular ploidies in breast carcinoma. In addition, the existence of heterogeneity according to the sample in the same patient is studied Sixty six paraffin blocks(two primary lesions and the two metastatic lymph nodes in 13 patients, and one primary lesion and one metastatic lymph node I 10 patients) of breast carcinoma, 30 paraffin blocks of fibroadenoma and 23 paraffin blocks of normal breast are studied. The numbe of Ag-NORs increase from normal breast(mean 1.45) through fibroadenoma(mean 1.60) to carcinoma(mean 2.43). It was significantly different between each lsions(P<0.0005). The number of Ag-NORs increased in the aneuploidy carcinoma than that in the euploidy one(P<0.05). But, there was difference between the number of Ag-NORs and DNA index according to the sample area in the same patient with carcinoma. In conclusion, the number of Ag-NORs is diagnostically significant in breast lesions and is correlated with cellular ploidies in breast carcinoma. In addition, Ag-NORs and cellular ploidies are heterogenous in the same patient.

Fifty-four adenocarcinomas of stomach were investigated to assess the expression of p21 and p53 using an immunohistochemical method. The relationship between p21 and p53 expression and the clinicopathologic parameters were analysed. The staining pattern of p21/p53 were: p21+/p53+, p21-/p53+, p21+/p53-, and p21-/p53- in 30, 12, 8, and 4 cases, respectively. Loss of p21 expression was observed in 16 of 54 tumor tissues (29%). p21 expression, however, had an inverse correlation with vascular invasion and depth of tumor invasion. The p21 and p53 protein expression showed intratumoral heterogeneity. In 63% of the adenocarcinoma, a proportional relationship was found between p21 and p53 immunostaining. The present results suggest that p53 independent induction of p21 expression may be involved in the molecular mechanism of these tumors, and expression of p21 protein may be related to a favorable prognosis in gastric adenocarcinomas.

Hypoplastic left heart syndrome is a rare congenital heart disease. This is a universally fatal leison, and approximately 80% of patients die within the first week of life. The diagnosis should be considered in infants, with the sudden onset of heart failure, systemic hypoperfusion and cyanosis. 2 cases of hypoplastic left syndrome are presented with review of related literatures.

PURPOSE: The purpose of this study was to compare the cleansing performance of a distilled water, a diluted solution of sodium hypochlorite as a household bleaching cleanser and three alkaline peroxide cleansers in vivo plaque deposits by using scanning electron microscope. MATERIALS AND METHODS: Five individuals were selected from department of the prosthodontics in Pusan National University Hospital, and each of them was inserted with specimens for plaque accumulation in their temporary dentures for 48 hours. The specimens were removed and cleaned by each cleansing agents for 8 hours. Scanning electron micrographs were made from the specimens at a magnification of x2,000. A panel of ten persons with a dental or paradental background, but not directly involved in the study, was selected to analyze the photomicrographs to determine which denture cleanser was more effective in removing plaque. RESULTS: Diluted solution of sodium hypochlorite was the most effective at removing plaque following Polident(R), Cleadent(R)e, Bonyplus(R) and distilled water in order. But there was no significant difference of cleansing efficacy between diluted solution of sodium hypochlorite and Polident(R), Polident(R) and Cleadent(R)e, Cleadent(R)e and Bonyplus(R), respectively (P > .05). Alkaline peroxide cleansers by themselves cannot adequately remove accumulated plaque deposits, especially if the deposits are heavy. Corrosion could be seen on the surface of non-precious alloy specimens immersed in diluted solution of sodium hypochlorite. CONCLUSION: It is recommended to use of alkaline peroxide type cleansers with brushing whenever possible, since denture cleanliness is often poor due to the relative inefficiency of these cleansers.

There are considerable in vitro and in vivo evidences for remineralization and demineralization occurring simultaneously in incipient enamel caries. In order to "heal"the incipient dental caries, many experiments have been carried out to determine the optimal conditions for remineralization. It was shown that remineralization is affected by different pH, lactic acid concentrations, chemical composition of the enamel, fluoride concentrations, etc. Eighty specimens from sound permanent teeth without demineralization or cracks, 0.15 mm in thickness, were immersed in lactic acid buffered demineralization solutions for 3 days. Dental caries with a surface zone and subsurface lesion were artificially produced. Groups of 10 specimens were immersed for 10 or 12 days in lactic acid buffered remineralization solutions consisting of pH 4.3 or pH 6.0, and 100, 50, 25, or 10 mM lactic acid. After demineralization and remineralization, images were taken by polarizing microscopy (x100) and micro-computed tomography. The results were obtained by observing images of the specimens and the density of the caries lesions was determined. 1. As the lactic acid concentration of the remineralization solutions with pH 4.3 was higher, the surface zone of the carious enamel increased and an isotropic zone of the subsurface lesion was found. However, the total decalcification depth increased at the same time. 2. In the remineralization solutions with pH 6.0, only the surface zone increased slightly but there was no significant change in the total decalcification depth and subsurface zone. In the lactic acid buffer solutions with the lower pH and higher lactic acid concentration, there were dynamic changes at the deep area of the dental carious lesion.

In this study, trends in the sales of antimicrobials for use in livestock facilities and fisheries from 2003 to 2012 were investigated with regard to antimicrobial group, antimicrobial usage, and animal species. The overall amount of antimicrobials sold each year from 2003 to 2007 was 1,500 tons, after which they decreased, with the lowest sales being 936 tons in 2012. The total volume of antimicrobials used for feed additives decreased markedly by 94% from 2003 to 2012, which was mainly attributed to banning of feed additives. However, antimicrobial consumption through self prescription by farmers for disease prevention and treatment increased by 25% from 2003 to 2012. The largest volume of antimicrobials sold was for use in pigs (48~57%), followed by poultry (18~24%), fisheries (11~25%), and cattle (5~8%). Tetracycline was the highest selling antimicrobial, followed by penicillins and sulfonamides, although the overall sale of all three antimicrobials gradually decreased over the study period. This study demonstrated that the total consumption of antimicrobials has gradually decreased since 2008. Nevertheless, usage by nonprofessionals increased, which can ultimately cause emergence and spread of antimicrobial resistance. Thus, early establishment of veterinary prescription guidelines for prudent use of antimicrobials is urgently needed in Korea.

PURPOSE: The purpose of this study was to compare the wear characteristics of human enamel opposing 2 heat-pressed ceramics (e.max Press and Empress Esthetic), conventional feldspathic porcelain (Ceramco 3) and type III gold alloy. MATERIAL AND METHODS: Intact cusps of extracted premolars were used for enamel specimens. Five disk samples were made for each of two heat-pressed ceramics groups, conventional feldspathic porcelain group and type III gold alloy group. Wear tests were conducted in distilled water using a pin-on-disk tribometer. The amount of enamel wear was determined by weighing the enamel specimens before and after wear tests, and the weight was converted to volumes by average density. The wear tracks were analyzed by scanning electron microscopy and surface profilometer to elucidate the wear characteristics. RESULTS: 1. Ceramco 3 led to the greatest amount of enamel wear followed by Empress Esthetic, e.max Press and type III gold alloy. However, there was no significant difference between Ceramco 3 and Empress Esthetic (P > .05), and there were also no significant differences among Empress Esthetic, e.max Press and type III gold alloy (P > .05). 2. The average surface roughness of e.max Press after wear test was smallest followed by Empress Esthetic and Ceramco 3, but there was no significant difference between Empress Esthetic and Ceramco 3 (P > .05). 3. There were no significant differences among the depth of wear tracks of all the groups (P > .05). The group that showed the largest width of wear track was Ceramco 3 followed by Empress Esthetic, e.max Press and type III gold alloy. However, there was no significant difference between e.max Press and Empress Esthetic (P > .05), and there was also no significant difference between Empress Esthetic and Ceramco 3 (P > .05). CONCLUSION: Within the limits of this study, heat-pressed ceramics were not more abrasive than conventional feldspathic porcelain.

STATEMENT OF PROBLEM: Restoring and replacing teeth with fixed prostheses commonly used in dental practice. Because of improper oral hygiene care and inaccurate laboratory procedure, complications of fixed prostheses were found in the mouth of patients. Although many efforts have been continually made to obtain the data of long term prognosis of fixed prostheses, it was difficult to do it. PURPOSE: The purpose of this study was to evaluate the clinical status of fixed prostheses. MATERIAL AND METHODS: In order to assess the clinical status of fixed prostheses, a total of 161 individuals (aged 17-85, 99 women and 62 men with 1596 unit of fixed prostheses, and 1169 abutments) who first visited the Department of Prosthodontics, Pusan National University Hospital, between April to September, in 2007 were examined. RESULTS AND CONCLUSION: The results of this study were as follows: 1. Length of service of fixed prostheses was 8.6+/-0.6 years (mean), 10.0 years (median). 2. Location of fixed prostheses was found to have no statistically significant influence on longevity of fixed prostheses (P > .05). But, the success rate was high in posterior region and in mandible where the failure rate was high in combination (P < .05). 3. Longevity of fixed prostheses made of metal was longest (mean: 13.0+/-9.3, median: 14.0), gold, precious ceramic, non-precious ceramic trailing behind (P < .05). 4. Number of units in fixed prostheses was found to have no statistically significant influence on longevity of fixed prostheses (P > .05). But, the success rate was high in Single-unit and the failure rate was high in over 3-unit (P < .05). 5. Condition of opposing dentition was found to have no statistically significant influence on longevity of fixed prostheses (P > .05). But, the success rate was high in natural dentition (P < .05). 6. Defective margin (28.2%), dental caries (23.0%), periodontal disease (19.3%), periapical disease (16.9%) were frequent complications. In 30.1% of the cases, abutment state after removing fixed prostheses was needed to be extracted.

An unusual case of glroependymal cyst involving the lateral ventricle, 3rd ventricle, cerebellopontine angle cistern and prepontine cistern, simultaneously, is reported. The glroependymal cyst is benign developmental cyst that mainly occurs in the cerebrospinal axis in relation to ventricle, but occasionally occurs in relation to extraventricular system, such as subarachnoid space, brain stem and cerebral parenchyme. The histological characteristics of ependymal cyst wall are ependyma-like epithelium and neuroglial tissue.
