1.Change and clinical significance of insulin-like growth factor Ⅱ in patients with bone fractures accompnied by craniocerebral trauma

Objective To study the change and clinical significance of insulin-like growth factor Ⅱ (IGF-Ⅱ) in patients with bone fractures accompanied by craniocerebral trauma.Methods The level of IGF-Ⅱ was detected by ELISA in 40 patients with bone fractures accompanied by craniocerebral trauma (combined with craniocerebral trauma group) and 40 patients of simple fracture (simple fracture group) after injury 1,3,7,14 d.Forty healthy person was as control group.The rate of porosis after injury 4,8,12 weeks was compared.Results The level of IGF-Ⅱ in combined with craniocerebral trauma group after injury 1,3,7,14 d was (4.15 + 1.38),(7.69 ± 2.40),(11.97 ± 3.74) and (14.08 ± 4.69) μg/L,simple fracture group was (2.56 ± 0.85),(3.70 ± 1.16),(4.96 + 1.55) and (7.52 ± 2.51) μg/L,control group was (2.10 ±0.70) μg/L.The level of IGF-Ⅱ in combined with craniocerebral trauma group and simple fracture group was significantly higher than that in control group (t =8.363,14.121,16.403,15.968;2.636,7.495,10.635,13.171,P < 0.05).The level of IGF-Ⅱ in combined with craniocerebral trauma group was significantly higher than that in simple fracture group (t =6.187,9.449,10.950,7.799,P< 0.05).The rate of porosis after injury 4,8,12 weeks in combined with craniocerebral trauma group was 22.5% (9/40),77.5% (31/40),100.0%(40/40),significantly higher than that in simple fracture group [5.0% (2/40),27.5% (11/40),47.5% (19/40)] (x2 =5.165,29.463,28.475,P < 0.05).Conclusion The serum IGF-Ⅱ may become a new factor promoting the healing of fracture and this study provide new ideas for the treatment of fracture.
2.A nursing care experience of using transoral Orvil EEA stapler during total laparoscopic gastrectomy

Objective To summarize the nursing care and cooperation with surgeons using transoral Orvil EEA stapler during total laparoscopic gastrectomy (TLG).Methods We present our initial experience of nursing care and cooperation with surgeons for 8 patients undergoing TLG using a circular end-toside anastomosis created with the transoral directed EEA circular stapler Orvil.Results The surgeries of all 8 patients were successfully completed under our good cooperation with surgeons using the technique of Orvil system.Conclusions Good preparations and knowing well the surgical procedures were the key points to achieve success using the transoral Orvil EEA stapler for patients undergoing TLG.

Hepatic satellite cell (HSC) is a kind of nonparenchymal cells in the liver,which releases various collagenous fibers and cytoskeletal proteins,playing an important role in the pathophysiological changes of the liver.In normal,HSCs maintain normal tissue architecture via regulating the synthesis and degradation of extracellular matrix (ECM) proteins.During liver injury,HSCs were activated.Activated HSCs are the main cells that lead to the accumulation of extracellular matrix,which is the main reason of hepatic fibrosis,cirrhosis and liver failure.Therefore,lucubrating the role and mechanism of HSC in the progress of hepatic disease and investiating the HSC-related therapeutic strategies have practical significances for the prevention and treatment of liver injury and raise of patients' survival rates in clinical practice.

5.The effect of high concentration oxygen to the lipid peroxidation of the gerbils subjected to cerebral ischemia and reperfusion

Exposure of gerbils 120 min to a high concentration oxygen (HCO)atmosphere after 60 min cerebral ischemia resulted in a marked increase in cerebral malondialdehyde (MDA, 4.39?0.26 nmol/mg protein). Much less cerebral MDA was among the other groups (2.63?0.50 nmol/mg protein in sham-operated controls; 3.07?0.52 nmol/mg protein in gerbils exposed to HCO without ischemia; 2.96?0.41nmol/mg protein in ger. bils subjected to 60 min ischemia; 2.79?0.59nmol/mg protein in gerbils subjected to 60 min ischemia and 120 min reperfusion). The contents of cerebral water, sodium, increassd after 60 mln cerebral ischemia. The contents of TXB_2 increased, 6-Keto-PGF_(1?) decreased, the elevation of cerebral water and sodium were more obvious after 120 min repefusion following ischemia. There was no difference in these items except for MDA between the reperfusion groups within air and within HCO.The authors proposed the mechanism that HCO elevates the contents of cerebral MDH might be due to abnormal function of election transport chain of mitochondria.
6.Clinical Observation of Bloodletting plus Acupoint Sticking for Diabetic Peripheral Neuropathy

Objective To observe the clinical efficacy of bloodletting plus acupoint sticking in treating diabetic peripheral neuropathy (DPN). Method Eighty DPN patients were randomized into a treatment group of 40 cases and a control group of 40 cases. In addition to the basic treatment (general blood glucose control) in both groups, the treatment group was given bloodletting plus acupoint sticking with Chinese medication, while the control group was given oral administration of Mecobalamin tablets. Result The treatment group had a higher total effective rate (92.5%) than the control group (67.5%), and the improvement of nerve conduction velocity in the treatment group was more significant than that in the control group (P<0.05). Conclusion Bloodletting plus acupoint sticking is effective in treating DPN.
7.Diagnostic value of serum Fer, AFP and AFP-L3 combined detection in primary hepatic cancer

Objective To evaluate the diagnostic value of the combined detection of serum Fer,AFP and AFP-L3 in patients with primary hepatic cancer (PHC).Methods The serum levels of Fer,AFP and AFP-L3 were determined in 92 patients with PHC,84 patients with benign liver disease,and 45 control subjects.Results The PHC group serum Fer was (819.4±616.8) μg/L,AFP was (556.9±451.5) μg/L,AFP-L3 was (109.0±62.5) μg/L.The benign liver disease group Fer was (229.3±155.5) μg/L,AFP was (6.0±4.2) μg/L,AFP-L3 was (26.0±49.4) μg/L.The healthy control group Fer was (125.6±110.5) μg/L,AFP was (2.8±0.8) μg/L,AFP-L was (7.0±1.8) μg/L,there was significant difference between the three groups (all P < 0.01).The sensitivity of PHC group alone in detection of serum Fer was 85.9 %,the sensitivity of AFP was 71.0 %,the sensitivity of AFP-L3 was 83.7 %,the combined detection of Fer and AFP or AFP-L3 improved the sensitivity of the test to 90.2 % and 95.7 %,the three combined detection sensitivity was 98.9 %.Conclusion Combined detection of serum Fer,AFP and AFP-L3 is essential in the diagnosis of PHC.
8.Effects of Early before PCI and Immediate Application of Tirofiban on Coronary Blood Flow and Myocar-dial Perfusion in Patients with High Risk Acute Coronary syndrome

OBJECTIVE:To investigate the effects of early before PCI and immediate application of tirofiban on coronary blood flow and myocardial perfusion in patients with high risk acute coronary syndrome (ACS). METHODS:100 high risk ACS patients were randomly divided into observation group and control group,with 50 cases in each group. Two groups were given tiro-fiban;observation group was given medicine 10 μg/kg within 3 min,4-6 h before PCI,with the velocity of 0.15 μg/(kg·min)till 24 h after PCI. Control group was given medicine at the beginning of PCI,route of administration was same as observation group. The incidence of TIMI blood flow grading,TIMI myocardial perfusion grade (TMPG),cTn Ⅰ,PAR and main adverse cardiac events(MACE)were compared between 2 groups before and after treatment. RESULTS:After operation,the patients of 2-3 grade TIMI blood flow and 2-3 grade TMPG in 2 groups were significantly more than before;the patients of 2-3 grade TMPG in observa-tion group was significantly more than in control group,with statistical significance(P<0.05). After treatment,cTn Ⅰof 2 groups were significantly increased,while PAR were significantly decreased,with statistical significance compared to before operation (P<0.05);but there was no difference between 2 groups (P>0.05). There was no statistical significance in the incidence of MACE between observation group(8.0%)and control group(16.0%)(P>0.05). CONCLUSIONS:Early application of tirofiban before PCI can effectively improve coronary blood flow and myocardial blood supply in high risk ACS patients.

10.Viability and histological changes of encapsulated rat hepatocyte after transplantation

Objective To study the viability and histological change of encapsulated rat hepatocytes after being transplanted into abdominal cavity of rat. Methods The two-step collagenase perfusing method was used to separate hepatocytes from Wistar rat liver. The separated hepatocytes were purified with Percoll density gradient centrifugation and encapsulated by the alginate-barium method. Then the purified hepatocytes were transplanted into abdominal cavity of SD rats (group 1) and the encapsulated hepatocytes were transplanted into abdominal cavity of SD rats (group 2) and Wistar rats (group 3). At different time points post-transplantation, trypan blue stain exclusion was used to determine the viability of recovered hepatocytes. The histological changes of transplanted microencapsulated hepatocytes was examined using HE stain. Results Twenty-four h after transplantation, the viability of hepatocytes between group 1 and group 2 showed significant difference (P<0.01), but there was no significant difference between group 2 and group 3 (P>0.05). At day 4 and day 7 after transplantation, the viability of hepatocytes showed significant difference between group 1 and group 2, and group 2 and group 3 (P<0.01). At day 14 after transplantation, no significant difference was found in the viability of hepatocytes between group 2 and group 3 (P>0.05). From day 4 post-transplantation, fibrosis overgrowth was found around some microencapsules, and it was more obvious in group 2 than in group 3. Conclusions Microencapsulation can provide protection to transplanted hepatocytes from host immunorejection, and thus increase the viability of hepatocytes post transplantation. The existence of inadequately encapsulated microencapsule cause the fibrosis overgrowth around these capsules, resulting in ischemia and subsequent necrosis of the hepatocytes and decreasing hepatocyte viability.
