1.Investigation of the Proarrhythmic Effects of Antidepressants according to QT Interval, QT Dispersion and T Wave Peak-to-End Interval in the Clinical Setting

OBJECTIVE:
Some antidepressants have been implicated as risk factors for QT prolongation, which is a predictor of sudden cardiac death. However, the QT interval is considered an imperfect biomarker for proarrhythmic risk. Therefore, we reevaluated the risk of sudden cardiac death due to antidepressants using improved methods, namely, QT dispersion (QTD), T wave peak-to-end interval (Tp-e), and Tp-e/QT ratio.
METHODS:
We compared the effects of antidepressants on QTc (QT/RR1/3), QTD, Tp-e, and Tp-e/QT ratio in 378 patients with mood disorder. We also compared each index between 165 healthy controls and 215 randomly selected age-matched patients.
RESULTS:
Age (p < 0.01), sex (p < 0.05), tricyclic antidepressant (TCA) use (p < 0.05), and clomipramine (p < 0.01) and mianserin (p < 0.05) use in particular, significantly associated with a prolonged QTc. We also found that age (p < 0.01), TCA use (p < 0.05), and clomipramine (p < 0.01) and mianserin (p < 0.05) use in particular, significantly prolonged QTD. However, there was no correlation between each variable and Tp-e or Tp-e/QT ratio. Significant differences in QTc and QTD were found between the patients and healthy controls.
CONCLUSION
From our results, prediction of risk of sudden cardiac death by QTD, Tp-e, or Tp-e/QT ratio was inconsistent. Increased QTD may be more suitable for predicting sudden cardiac death due to antidepressants.
2.Cardiac Rehabilitation Increases Exercise Capacity with a Reduction of Oxidative Stress.

BACKGROUND AND OBJECTIVES: Reactive oxygen species (ROS) mediate various signaling pathways that underlie vascular inflammation in atherogenesis and cardiovascular diseases. Cardiac rehabilitation (CR) has a variety of multiple beneficial effects, including anti-inflammatory effects. The purpose of the present study was to investigate the effects of CR on ROS in patients with cardiovascular diseases. SUBJECTS AND METHODS: The serum level of derivatives of reactive oxidative metabolites, an index of oxidative stress, was measured in 100 patients with cardiovascular diseases before, and, subsequently, 3 and 6 months after, CR. A biological antioxidant potential (BAP) test was applied to assess the antioxidant power of the serum. RESULTS: The resting reactive oxidative metabolite levels decreased 3-6 months after CR {pre: 351+/-97 Carratelli unit (CARR U), 3 months: 329+/-77 CARR U, 6 months: 325+/-63 CARR U, all p<0.01} with the increase of the percentage of the predicted values of VO2 peak and the percentage of the predicted values of VO2 at the anaerobic threshold (VO2 AT) and the decrease of the B-type natriuretic peptide (BNP). The BAP test and antioxidative/oxidative stress ratio increased 6 months after CR. The % changes of the antioxidative/oxidative stress ratio was positively correlated with the % changes of VO2 AT, and negatively correlated with the % changes of the BNP. CONCLUSION: These results suggest that intensive supervised CR significantly improved exercise capacity, which may be attributable to an adaptive response involving more efficient oxidative metabolites or the increased capacity of endogenous anti-oxidative systems in patients with cardiovascular diseases.

3.Natural History of Paralytic Scoliosis

Introduction : We examined the characteristics of paralytic scoliosis using plain radiography.Subjects and methods : We recruited fourteen patients aged ≥ 15 years old with no history of bone surgery at the time of their final observation. Participants included those who had cerebral palsy or those who had a history of encephalitis and underwent spinal frontal plain radiography in the supine position at different time points. We evaluated gross motor function, position and direction of the curve, Cobb angle, rate of variability, and degree of progression at 5-year intervals. We measured the percentage of migration using hip frontal plain radiography to assess hip dislocation.Results : The final Cobb angles were 82.0°, 118.4°, and 92.3°for the thoracic, thoracolumbar (TL), and lumbar curvatures, respectively. TL curvatures showed the greatest progression, although this was not statistically significant. The progression was greatest in the 10-15-year age group (12.5°annually). The final Cobb angles in the hip dislocation, subluxation, and no dislocation groups were 102.8°, 108.8°, and 87.5°, respectively;the difference was not statistically significant. No relationship was observed between the location or progression of curvature and the state of the hip location.Discussion : Paralytic scoliosis progressed most rapidly in 10-15-year-old patients, especially in those with TL lesions. We did not detect any relationships between hip dislocation and Cobb angle, but these parameters progressed at different rates in different patients.
