1.A study on the homogeneity of the therapeutic effect on stroke patients in rehabilitation Medical Association

Objective:To investigate the therapeutic effect of stroke patients received rehabilitation treatment at different medical units in the Rehabilitation Medical Association.Method:Totally 183 stroke patients were recruited from October 12,2012 to February 5,2016 by Shenzhen Rehabilitation Medical Association(SRMA).SRMA consists of 4 members,including the Second People's Hospital of Shenzhen,Shenzhen Nan'ao People's Hospital,Shenzhen Baoxing Hospital and Community Healthcare Center in Futian District,which belong to tertiary hospital,secondary hospital,and primary healthcare unit,respectively.According to the time of onset of stroke,all the patients were divided into recovery phase or sequelae phase.The rehabilitation facilities were standardized.All patients received rehabilitation treatment for 4 weeks,which included functional rehabilitation training of limb,functional electrical stimulation training,activities of daily living training.Fugl-Meyer Assessment(FMA),balance subscale of the Fugl-Meyer test(FMB),National Institute of Health Stroke Scale(NIHSS) and modified Barthel Index(MBI) before and after rehabilitation treatment were used to evaluate the effects.Result:The scores of FMA,FMB,NIHSS and MBI were significantly improved in all the groups after rehabilitation treatment(P < 0.05).There was no significant difference in all assessments among the groups(P> 0.05).Conclusion:Based on the standardized rehabilitation treatment programs,the stroke patients come from the members of Rehabilitation Medical Association can achieve homogeneous effects after rehabilitation treatment.
2.Effects of frustration situation,resilience and positive music on attentional bias

Objective:To explore effects of frustration situation,resilience and positive music on attentional bias by a behavioral experiment.Methods:Totally 60 participants were chosen from 650 college students according to their scores in Connor-Davidson Resilience Scale (CD-RISC).Then,a 2(resilience,high/low)×3(experimental treatment,no frustration/frustration with positive music /frustration without any background)×2(vocabulary,posi-tive /passive)mixed design experiment was conducted to investigate the attentional bias of participants in emotional Stroop paradigm.Results:Reaction times to passive vocabulary were different significantly in three frustration situa-tions(P <0.01).Interaction of resilience and frustration on reaction time to positive vocabulary was significant (P<0.01).Among the participants with low resilience,reaction time to positive vocabulary in the situation without frustration or the frustration situation with positive music was longer significantly than that in the frustration situa-tion without any background [(476.3 ±85.6)vs.(456.0 ±54.0)vs.(427.1 ±61.1),P <0.05].Among the par-ticipants with high resilience,reaction time to positive vocabulary in the frustration situation without any background was longer than that in the situation without frustration [(472.1 ±82.9)vs.(432.4 ±53.2),P <0.05].Conclu-sion:It suggests that individuals may have obviously attentional bias to negative stimulus in frustration situation,and effects of resilience and positive music on attentional bias in situation may be obvious.
3.Chimney technique combined with thoracic endovascular aortic repair in treatment of type B aortic dissection

Objective To observe the clinical effects of chimney technique combined with thoracic endovascular aortic repair(TEVAR) in the treatment of type B aortic dissection.Methods The clinical datas of 56 patients with type B aortic dissection in our hospital were analyzed retrospectively.According to the surgical intervention method,they were divided into observation group (22 cases,TEVAR + chimney technique) and control group (34 cases,TEVAR alone).Surgery related indicators,incidence of postoperative short-term complications and mortality between the two groups were compared.Results The success rates in both groups were 100%,and the surgical time in observation group was longer than that in control group (P < 0.05).There were no significant differences in blood loss,eating time,off-bed time and hospitalization time between the two groups(P > 0.05).The incidence of complications of observation group and control group within 2 weeks after operation were 18.2% and 5.8 % respectively,the difference was not significant(P > 0.05),as well as mortality and incidence of longterm complications during the followed period (P > 0.05).Conclusion The perioperative indexes of single endovascular repair and its combination with chimney technique for the treatment of type B aortic dissection are similar.Chimney technique can reconstruct important branch flows,especially for patients with insufficiency of landing zone.However,particular attention needs to be paid to postoperative complications to avoid negative effect on the prognosis of patients.
4.Bone marrow mesenchymal stem cells combined with theaflavin for steroid-induced avascular necrosis of femoral head

BACKGROUND:Pathological mechanism of femoral head necrosis is not fuly known, and high-dose corticosteroids are prone to induce femoral head necrosis.
OBJECTIVE:To investigate the clinical effect of bone marrow mesenchymal stem cels combined with theaflavin on steroid-induced avascular necrosis of femoral head in rats.
METHODS: Rat bone marrow mesenchymal stem cels were isolated and culturedin vitro and compounded onto gelatin sponge, and then, divided into four groups: model group, decompression group, cel transplantation group and combined group. Models of steroid-induced avascular necrosis of femoral head were established using liquid nitrogen method. In the combined group, gelatin sponge carrying bone marrow mesenchymal stem cels was transplanted and 250 mg theaflavin was given daily after modeling.
RESULTS AND CONCLUSION:At 4 weeks after modeling, the femoral head samples in the four groups were round with exfoliation of articular cartilage that was pale in color. Then, the exfoliation of articular cartilage was aggravated in the model group, and some specimens appeared to colapse; the femoral head specimens in the combined group were round and pale. Until the 8th week after modeling, bone necrosis was increased in the model group; osteoblasts were found in the decompression group with fibrous calus formation; a few of empty bone lacunae appeared with irregular medulary cavity in the cel transplantation group; a large amount of new bone tissues formed in the combined group, and intramedulary adiocytes were in regular array. At 4 and 8 weeks after modeling, the number of empty bone lacunae was significantly lower in the combined group than the other groups (P < 0.05). These findings indicate that bone marrow mesenchymal stem cels combined with theaflavin is an ideal therapy for steroid-induced avascular necrosis of femoral head.
5.Study on the Inhibitory Effect of Artesunate on Wnt / β-catenin Signaling Pathway and Improve Asthma rat Model of Airway Inflammation and Airway Remodeling

Objective To investigate the role of Wnt/β-catenin signaling pathway in airway inflammation and airway remodeling in asthmatic rats,and to explore the possible molecular mechanism of artesunate (ART) in the treatment of asthma.Methods Forty-eight SD rats were randomly divided into normal group,asthmatic group,dexamethasone group,low-dose artesunate group,middle-dose artesunate group and high-dose artesunate group.Asthmatic models were established by ovalbumin (OVA) challenge and aerosol inhalation,and were treated with drugs.The morphological changes of lung tissue,the white blood cell count and the white blood cell count of bronchoalveolar lavage fluid (BALF) were observed in each group.The bronchial wall thickness and smooth muscle thickness were measured by Image-Pro plus image analysis software.The expressions of β-catenin and WISP-1 in lung tissue were detected by immunohistochemistry and RT-PCR respectively.The expression of IL-6 in serum and BALF was detected by ELISA.Results The artesunate group rats,airway inflammatory cells infiltration,the smooth muscle thickness,bronchial wall thickness were significantly lower than those in asthma group,and the difference was statistically significant (P < 0.01).The expression of artesunate group β-catenin,WISP-1 protein and WISP-1 mRNA were significantly lower than those in asthma group,and the difference was statistically significant (P <0.01).The level of IL-6 in serum and BALF of artesunate group was significantly lower than that of asthmatic group (P < 0.01).Conclusion The possible mechanism of artesunate to improve airway inflammation and airway remodeling in asthmatic rats related to the inhibition of Wnt/ beta-catenin signaling pathway activity and down-regulation of IL-6 levels.
6.Clinicopathology and prognosis of gastric cancer patients with perigastric soft tissue involvement

ObjectiveTo analyze the clinicopathological features and prognosis of gastric cancer patients with metastatic nodules of perigastric soft tissue. MethodsIn this study,1025 cases of gastric cancer received radical resection.According to the metastasis of perigastric soft tissue,patients were divided into metastatic group ( group MP,n =334 ),non-metastatic group ( group NMP,n =691 ).The clinicopathological features and prognosis were compared between the two groups. ResultsIn group MP,the ratio of upper,middle,lower,total gastric cancer was 25.8%,22.0%,51.4%,0.9% and the ratio in group NMP was 33.2%,21.3%,41.3%,4.2% respectively,showing significant higher ratio of upper and total gastric cancer in MP group(P =0.000). In group MP 47.3% cases with tumor size ≥5 cm,significantly higher than that in NMP group(27% ) (P =0.000).Lymph node metastatic ratio between 21% -40% and 41% -100% was found in 24.4% and 37.3% in MP group respectively,significantly higher than that of 12.9%,10.8% in NMP group(P =0.000).20.1% cases had distal metastasis in group MP,significantly higher than that of 4.1% in group NMP(P=0.000).In group MP and NMP group,the ratio of Borrmann infiltration typing was 82.1% vs.64.6%,the ratio of positive CEA was 21.2% vs.11.4%,the ratio of lower or undifferentiation typing was 78.7% vs.64.2%,all with significant difference (P =0.000 ). COX regression analysis showed the infiltration depth,organic invasion,lymph node metastatic ratio,M staging,Borrmann typing,metastatic nodules was the independent prognostic factors.Prognosis was significantly poorer in the cases with perigastric soft tissues than without ( P =0.000 ).Stratified analysis showed that irrespective of tumor size,infiltration depth,lymph node metastatic ratio,CEA value,Borrmann typing,differentiation degree,the mean survival time was significantly shorter in MP group than that in group NMP(P < 0.005).In cases without distal metastasis,the prognosis was significant poorer in group MP than that in group NMP ( P =0.000 ),however,there was no significant difference between two groups in cases without distal metastasis ( P =0.076).ConclusionsPerigastric soft tissue metastasis was common in gastric cancer,more frequently seen in tumor ≥5 cm,or with organic invasion,lymph nodemetastaticration ≥ 21%, distalmetastasis, Borrmanninfiltrationtyping, loweror undifferentiation typing,positive CEA. Perigastric soft tissues metastasis was the independent prognotic factor for gastric cancer.
7.Effect of δ-opioid receptor agonist on celluar immune function in a rat model of sepsis

Objective To investigate the effect of δ-opioid receptor agonist D-Ala2-D-Leu5-enkephalin (DADLE) on the celluar immune function in septic rats. Methods One hundred and fifty healthy male SD rats weighing 154-198 g were randomly divided into 3 groups (n = 50 each):group Ⅰ sham operation (group S);group Ⅱ sepsis (group SEP) and group Ⅲ DADLE. Sepsis was induced by cecum ligation and punture (CLP) in group Ⅱ and Ⅲ. In group Ⅲ 0.5 rmg/kg DADLE 10 ml/kg was injected intraperitoneally (IP) immediately after CLP operation. Seven day survival rate was calculated. Blood samples were collected from 10 animals at 4, 8 and 12 h after operation (T1-3) respectively in each group for determination of serum TNF-α and IL-10 concentrations by ELISA and changes in T-cell subsets by flow cytometry. Results CLP significantly increased serum TNF-α and IL-10 concentrations and TNF-α/IL-10 ratio at T1-3 and decreased CD4+ and CD4+/CD8+ ratio and increased CD8+ at T3 in group Ⅱ as compared with group S (group Ⅰ). DADLE treatment significantly attenuated the CLP-induced above changes. Seven-day survival rate was significantly higher in DADLE group than in CLP group.Conclusion δ-opioid receptor agonist DADLE can improve the celluar immune function of rats with sepsis and increase the survival rate.
8.Functional connectivity between the substantia nigra and the motor cortex in the rapid eye movement sleep behavior disorder

Objective To investigate the abnormal functional connectivity (FC) between the substantia nigra (SN) and the brain motor area in rapid eye movement sleep behavior disorder (RBD) by Unified Parkinson′s Disease Rating Scale (UPDRS), Hoehn-Yahr Scale and resting state functional magnetic resonance imaging (rfMRI).Methods A total of 34 subjects (14 with RBD (RBD group), 12 with Parkinson′s disease (PD group), and 8 age, sex-matched normal controls (control group)) recruited in the Department of Neurology, Henan Provincial People′s Hospital from 2014 to 2015 were enrolled in this study according to international diagnosis criteria.All subjects were examined by UPDRS, Hoehn-Yahr Scale and rfMRI.Results UPDRS scores and Hoehn-Yahr staging were 0.00 (0.00, 3.75) and 0.00 (0.00, 0.50) respectively in the RBD group, which were significantly different from that in the PD group (30.5 (18.75, 33.00) and 1.75 (1.50, 2.50), respectively;Z=-3.782, P<0.05 and Z=-2.963, P<0.05), whereas not significantly different from that in the control group (0.00 (0.00, 0.00),0.00 (0.00, 0.00), respectively;Z=1.16, P>0.05 and Z=1.10, P>0.05).The FC analysis showed alterations from the right SN to bilateral cerebellum respectively among the three groups (Fright cerebellum=12.975, Fleft cerebellum=6.144;P<0.05);furthermore, there were significant increases in the RBD group compared with the PD group (t=-4.602,-5.080, P<0.005).The FC analysis showed reduced correlation from the right SN to the left prefrontal cortex in the RBD and the PD groups compared with the control group (t=-4.899,-3.500, P<0.005), but there was no statistically significant difference between the RBD and the PD groups (t=2.035, P>0.05).Conclusions There was abnormal FC from the SN to motor areas in RBD patients, and partial alterations were similar as PD patients.rfMRI provided an evidence that RBD might be presymptom of PD.
9.Protection of Ndrg2 deficiency on renal ischemia-reperfusion injury via activating PINK1/Parkin-mediated mitophagy

Background::Renal ischemia-reperfusion (R-I/R) injury is the most prevalent cause of acute kidney injury, with high mortality and poor prognosis. However, the underlying pathological mechanisms are not yet fully understood. Therefore, this study aimed to investigate the role of N-myc downstream-regulated gene 2 ( Ndrg2) in R-I/R injury. Methods::We examined the expression of Ndrg2 in the kidney under normal physiological conditions and after R-I/R injury by immunofluorescence staining, real-time polymerase chain reaction, and western blotting. We then detected R-I/R injury in Ndrg2-deficient ( Ndrg2-/-) mice and wild type ( Ndrg2+/+) littermates in vivo, and detected oxygen and glucose deprivation and reperfusion (OGD-R) injury in HK-2 cells. We further conducted transcriptomic sequencing to investigate the role of Ndrg2 in R-I/R injury and detected levels of oxidative stress and mitochondrial damage by dihydroethidium staining, biochemical assays, and western blot. Finally, we measured the levels of mitophagy in Ndrg2+/+ and Ndrg2-/- mice after R-I/R injury or HK-2 cells in OGD-R injury. Results::Ndrg2 was primarily expressed in renal proximal tubules and its expression was significantly decreased 24 h after R-I/R injury. Ndrg2-/- mice exhibited significantly attenuated R-I/R injury compared to Ndrg2+/+ mice. Transcriptomics profiling showed that Ndrg2 deficiency induced perturbations of multiple signaling pathways, downregulated inflammatory responses and oxidative stress, and increased autophagy following R-I/R injury. Further studies revealed that Ndrg2 deficiency reduced oxidative stress and mitochondrial damage. Notably, Ndrg2 deficiency significantly activated phosphatase and tensin homologue on chromosome ten-induced putative kinase 1 (PINK1)/Parkin-mediated mitophagy. The downregulation of NDRG2 expression significantly increased cell viability after OGD-R injury, increased the expression of heme oxygenase-1, decreased the expression of nicotinamide adenine dinucleotide phosphate oxidase 4, and increased the expression of the PINK1/Parkin pathway. Conclusion::Ndrg2 deficiency might become a therapy target for R-I/R injury by decreasing oxidative stress, maintaining mitochondrial homeostasis, and activating PINK1/Parkin-mediated mitophagy.
10.Effect of Electrical Stimulation of Vagus Nerve on the Expression of Calcium-Binding Protein S100A8 in Septic Shock Rats

Objective:To establish septic shock rat model and to investigate the effect of electrical stimulation of vagus nerve on the expression level of plasma calcium-binding protein S100A8 in septic shock rats.Methods:Thirty male adult rats were per-formed cecal ligation and puncture (CLP)to establish septic shock model and then randomly divided into six group:Group I, sham electric acupuncture and normal;Group II,electric acupuncture and normal;Group III,sham electric acupuncture and CLP;Group IV,electric acupuncture and CLP;Group V,vagotomy,sham electric acupuncture and CLP;Group VI,vagoto-my,electric acupuncture and CLP.In Group III,IV,V and VI,indwelling catheter was placed in the common carotid artery for monitoring mean arterial blood pressure (MAP)continuously.Plasma calcium-binding protein S100A8 was determined by ELISA.Results:In groups with CLP,MAP gradually decreased,and serum S100A8 level at postoperative 12 h significantly increased (P <0.01).Compared to that in groups with CLP,the decreased extent of MAP reduced in rats treated with electri-cal stimulation (P <0.01).Moreover,the decreased extent of MAP reduced and the level of plasma calcium-binding protein S100A8 significantly decreased in Group VI,compared to that in Group V (P <0.01).Conclusions:The electrical stimulation of vagus nerve can alleviate gradual decrease of MAP which was caused by CLP induced septic shock in rats and reduce the level of plasma calcium-binding protein S100A8,so that it contributes to anti-shock treatment as supplementary role.
