1.A randomized, controlled, multi-centre clinical study on the efficacy of flupenthixol melitracen in the treatment of patients with functional dyspepsia accompanied by depression

Objective To explore the efficacy of flupenthixol melitracen in the treatment of functional dyspepsia (FD) and its effects on receptivity expansion and visceral sensitivity.Methods A randomized,controlled,multi-centre study was carried out.The FD patients with depression were divided into anti-depression group and control group.The patients of both groups received omeprazole and mosapride treatment for eight weeks.The patients of anti-depression group added flupenthixol melitracen and psychotherapy.Before the treatment,by the end of the second,fourth,sixth and eighth week of the treatment,the scores of upper gastrointestinal symptoms and adverse effects of the patients of the two groups were recorded,the psychological status was evaluated by Hamilton depression scale and the gastric receptivity expansion and visceral sensitivity were assessed by water loading test (WLT).The data were analyzed by t test and chi-square test.Results Among two hundred and fifty enrolled FD patients with depression,245 patients (129 in anti-depression group,116 in control group) were enrolled in intention-to treat analysis.Before the treatment and by the end of the eighth week of the treatment,the scores of upper gastrointestinal symptoms of anti depression group were 12.23±4.45 and 2.31±2.13 (t=22.837,P=0.008),the control group were 12.29± 4.62 and 8.58±3.76 (t=6.708,P=0.184).By the end of the eighth week of treatment,the difference between the two groups was statistically significant (t =16.261,P<0.05).By the end of the fourth and eighth week of treatment,the total effective rates of the improvement of upper gastrointestinal symptoms of anti-depression group were 76.0% (98/129) and 83.7% (108/129),control group were 14.7 % (17/116) and 25.9 % (30/116),the differences between the two groups were statistically significant (x2 =92.191 and 83.117,both P<0.05).By the end of the eighth week of treatment,the total effective rate of the improvement of depression of anti depression group was 71.3% (92/129),which was higher than that of control group (23.3%,27/116),the difference between two groups was statistically significant (x2 54.434,P<0.01).Before the treatment,the maximum WLT volume of anti depression group was (850±130) mL,by the end of the eighth week of treatment was (1400±210) mL (t=25.292,P<0.01).Before the treatment and by the end of the eighth week of treatment,the maximum WLT volumes of control group were (800± 120) mL and (960±140) mL (t=9.345,P<0.01).All the adverse effects of anti depression therapy were mild.Conclusion The treatment of flupenthixol melitracen can improve the efficacy in patients with FD,relieve the depression status,improve gastric receptivity expansion and visceral sensitivity with a good safety and tolerability.
2.Microfracture technique plus bone marrow mesenchymal stem cell transplantation for repair of articular cartilage injury of the knee

BACKGROUND:It has been an urgent problem of how to promote cartilage repair of the knee and shorten the total course through a tissue engineering approach. Fortunately, microfracture plus stem cel transplantation may open up a new path for this issue. OBJECTIVE:To investigate the clinic feasibility of arthroscopic microfracture technique plus stem cel transplantation for repair of articular cartilage injury of the knee. METHODS:From October 2010 to March 2012, a total of 16 patients with articular cartilage injury of the knee were enrol ed, including 12 males and 4 females, with the average age of 38.6 years (16-52). Al cases of cartilage injury were confirmed by arthroscopy. Autologous bone marrow was extracted from patients at 2 weeks before treatment to isolate, culture and amplify bone marrow mesenchymal stem cel s in vitro. The cel culture solution of 3-5 mL (about 107 cel s) was harvested. The articular cavity was clean by arthroscopy and microfracture technique was performed at the area of cartilage injury that was then covered with hemostatic gauze through a minimal y invasive incision and the prepared bone marrow mesenchymal stem cel s were injected. The knee was bandaged with the elastic bandage after aspirating the joint cavity effusion by vacuum suction. Functional exercises were performed early by CPM.RESULTS AND CONCLUSION:After fol ow-up of 4-18 months, there were 13 cases of excel ent, 2 cases of valid and 1 case of ineffective. According to Lysholm knee scores, the average scores were improved from 42 points (33-67 points) to 89 points (75-99 points) at 4 weeks after treatment. The function was satisfied and al patients were fol owed up without recurrence or worse. Under the arthroscopy, the combination of microfracture technique and autologous bone marrow mesenchymal stem cel transplantation is proved to be effective for articular cartilage injury of the knee and it can notably improve the clinic symptoms and recover the function of the knee.
3.Therapeutic effect of aerobic exercise based on exercise load test assessment on heart function and psychological state in patients with myocardial infarction

Objective: To study therapeutic effect of personalized aerobic exercise based on exercise load test assessment on heart function and psychological state in patients with acute myocardial infarction (AMI). Methods: According to random number table, a total of 80 AMI patients during recovery period were randomly and equally divided into routine treatment group (n=40, received routine drug therapy) and exercise group (n=40, received personalized aerobic exercise based on exercise load test assessment on the basis of routine treatment group). Serum levels of high-sensitivity cardiac troponin I (hs-cTnI) and creatine kinase isoenzyme MB (CK-MB), and left ventricular ejection fraction (LVEF), stroke volume (SV), left ventricular fractional shortening (LVFS), ventricular wall thickening rate (△T), motion amplitude of interventricular septum (AIS), and scores of self-rating anxiety scale (SAS) and self-rating depression scale (SDS) were compared between two groups before and after 12-week intervention. Results: Compared with before intervention, there were significant reductions in serum levels of hs-cTnI and CK-MB, and scores of SAS and SDS; and significant rise in LVEF, SV, LVFS, △T and AIS in both groups after 12-week intervention (except LVEF of routine treatment group), P<0.05 or <0.01; compared with routine treatment group, there were significant reductions in serum levels of hs-cTnI [(9.31±1.04) ng/L vs. (5.89±0.72) ng/L] and CK-MB [(11.34±1.25) U/L vs. (8.41±1.01) U/L],in scores of SAS [(51.34±6.54) scores vs. (42.32±5.29) scores] and SDS [(50.23±6.92) scores vs. (43.86±6.03) scores];and significant rise in LVEF [(57.12±6.67)% vs. (62.32±7.34)%], SV [(62.45±7.13) ml vs. (69.18±7.84) ml], LVFS [(23.93±2.96)% vs. (28.45±3.12)%], △T [(28.51±3.41)% vs. (34.52±4.87)%] and AIS [(9.56±1.02) mm vs. (12.45±1.46) mm] in exercise group, P<0.05 or <0.01. Conclusion: Personalized aerobic exercise based on exercise load test assessment can reduce myocardial cell injury, improve cardiac function, relieve negative emotions and promote patients’ rehabilitation rapidly.

Objective To explore the effects of sodium oligomannate combined with high-frequency repetitive transcranial magnetic stimulation on the activity of daily living and intelligence level of patients with Alzheimer's disease.Methods A total of 100 patients with Alzheimer's disease admitted to Henan Provincial Staff Hospital from September 2021 to September 2023 were selected as the research subjects.These patients were divided into an observation group and a control group according to the random number table method,with 50 patients in each group.Patients in the control group were given high-frequency repetitive transcranial magnetic stimulation,and patients in the observation group were given sodium oligomannate combined with high-fre-quency repetitive transcranial magnetic stimulation.Enzyme-linked immunosorbent assay was used to detect the levels of amyloid β-protein(Aβ)142,tumor necrosis factor-a(TNF-a)and interleukin-6(IL-6)in patients of the two groups before and after treatment.Chemiluminescence was used to detect the levels of neurotransmitters in patients of the two groups before and after treatment.The mental symptoms of patients in the two groups before and after treatment were evaluated by the behavioral pathology in Alzheimer's disease rating scale,the intelligence level of patients in the two groups before and after treatment was evaluated by the mini-mental state examination(MMSE)scale and the Montreal cognitive assessment(MoCA)scale,the activity of daily living of patients in the two groups before and after treatment was evaluated by Alzheimer's disease cooperative study-activity of daily living(ADCS-ADL),and the quality of life of patients in the two groups before and after treatment was evaluated by the quality of life-Alzheimer's disease scale.Results Before treatment,the two groups showed no significant difference in the levels of Aβ1-42,TNF-α and IL-6(P>0.05);after treatment,the levels of Aβ1-42,TNF-α and IL-6 in both groups were lower than those before treatment,and the levels of Aβ1-42,TNF-α and IL-6 in the observation group were lower than those in the control group(P<0.05).Before treatment,there were no statistically significant differences in the levels of 5-hydroxytryptamine(5-HT),acetylcholine(ACh),γ-aminobutyric acid(GABA),and Alzheimer-associated neuronal thread protein(AD7c-NTP)between the two groups(P>0.05);after treatment,the levels of 5-HT,ACh,and GABA in the two groups were higher than those before treatment,while the AD7c-NTP level was lower than that before treatment(P<0.05);after treatment,the levels of 5-HT,ACh,and GABA in the observation group were higher than those in the control group,while the AD7c-NTP level was lower than that in the control group(P<0.05).Before treatment,there were no significant differe-nces in the scores of paranoid and delusional ideas,hallucinations,behavioral disorders,aggressive behaviors,daily rhythm disorders,emotional disorders,anxiety and fear,and the total scores between the two groups(P>0.05);after treatment,the scores of paranoid and delusional ideas,hallucinations,behavioral disorders,aggressive behaviors,daily rhythm disorders,emotional disorders,anxiety and fear,and the total scores of patients in the two groups were lower than those before treatment,and the scores of paranoid and delusional ideas,hallucinations,behavioral disorders,aggressive behaviors,daily rhythm disorders,emotional disorders,anxiety and fear,and the total scores of patients in the observation group were lower than those in the control group(P<0.05).Before treatment,there were no significant differences in MMSE,MoCA and ADCS-ADL scores of patients between the two groups(P>0.05);after treatment,the MMSE,MoCA and ADCS-ADL scores of patientsin the two groups were higher than those before treatment,and the MMSE,MoCA and ADCS-ADL scores of patientsin the observation group were higher than those in the control group(P<0.05).Before treatment,there were no significant differences in the scores of physiological function,mental function,behavioral ability and interpersonal relationship,and the total scores of patients between the two groups(P>0.05).After treatment,the scores of physiological function,mental function,behavioral ability and interpersonal relationship,and the total scores of patients in the two groups were higher than those before treatment,and the scores of physiological function,mental function,behavioral ability and interpersonal relationship,and the total scores of patients in the observation group were higher than those in the control group(P<0.05).Conclusion Sodium oligomannate combined with high-frequency repetitive transcranial magnetic stimulation can improve the inflammatory state of patients with Alzheimer's disease,regulate neurotransmitter levels,alleviate psychiatric symptoms,enhance the intelligence level and the activity of daily living,and increase the quality of life.
6.The relationships between functional gastrointestinal diseases and psychological factors, diet and lifestyles: a network analysis

Objective:To investigate the relationship between common functional gastrointestinal diseases symptoms with psychological factors, diet and lifestyles by using the network analysis method which has achieved great success in the field of psychology in recent years.Method:A questionnaire survey was conducted in two military units using the cluster sampling method during July 2020, and a total of 1 805 subjects were included. Functional gastrointestinal disease symptoms were evaluated with the Gastrointestinal Symptom Rating Scale (GSRS). The state, trait anxiety scale and stress response scale were used to evaluate the mental and psychological state by self-evaluation. R was used to build the network and calculate statistical parameters.Results:1 486 of the 1 805 subjects (82.3%) had experienced functional gastrointestinal diseases symptoms within 2 weeks, but most of them were mild. Network analysis shows that there was a strong interaction between digestive system symptoms with different clinical manifestations (Spearman coefficient ranges 0.31-0.56). There was a clear relationship between functional gastrointestinal symptoms and mental and psychological factors (Spearman coefficient ranges 0.16-0.27), but there was no clear interaction with diet, age, education level, body mass index, etc. Functional gastrointestinal diseases symptoms were connected with mental and psychological factors through two nodes: stress and indigestion. The stability coefficient of node strength correlation was 0.75, indicating that the network was stable.Conclusions:The current study revealed the network structure and features of functional gastrointestinal diseases symptoms with mental and psychological factors. The key linking nodes provided potential interfering target for controlling functional gastrointestinal symptoms related to mental and psychological factors.
7.Construction and verification of a nomogram model for postoperative pulmonary embolism in patients with spontaneous cerebral hemorrhage

Objective To investigate the risk factors for postoperative pulmonary embolism in patients with spontaneous cerebral hemorrhage,and construct and verify the nomogram model.Methods A retrospective cohort study was conducted on 558 patients admitted in the First Affiliated Hospital of Chongqing Medical University and the Three Gorges Hospital Affiliated to Chongqing University.And 393 of them who hospitalized from January 2015 to January 2021 were assigned into a modeling group,and the other 165 patients from February 2021 to January 2023 into a validation group.Univariate and multivariate stepwise logistic regression analyses were used to screen out the risk factors associated with pulmonary embolism after spontaneous cerebral hemorrhage surgery.Then a nomogram model was build based on these factors and verified.Results Based on age,blood loss,Glasgow coma scale(GCS)score,surgical treatments,levels of fibrin degradation products,D-dimer and hemoglobin,plasma osmolality,and deep vein thrombosis,a risk model of pulmonary embolism was built.Receiver operating characteristic(ROC)curve analysis showed the model had good discriminability for the presence of pulmonary embolism,and the area under the curve(AUC)value was 0.908.Hosmer-Lemeshow goodness-fit test indicated that the model had a good fit to the verification set(Chi-square=14.805,df=8,P=0.063),the correction curve was close to the ideal curve,and the prediction probability of the model was close to the actual occurrence probability,suggesting the model having good accuracy.Decision curve analysis revealed that the established nomogram model can get benefits under a large range of threshold probabilities.Conclusion We develop a prediction model for postoperative pulmonary embolism in patients with spontaneous cerebral hemorrhage after surgical treatment,which shows good prediction performance in both the training and validation groups,and can be used for accurate,prompt and quick identification for the occurrence of pulmonary embolism in clinical practice.

9.O'Sullivan-McLeod Syndrome: A Case Report

O'Sullivan-Mcleod syndrome is a very rare variant of MND with a good prognosis. Its clinical feature is distal lower motor neuron syndrome of both upper limbs, and there is no effective treatment at present. We reported a case of O'Sullivan-Mcleod syndrome in this paper.The patient exhibited with middle-aged progressive distal muscle weakness and atrophy of both upper limbs, without sensory, cognitive or behavioral impairment and without pyramidal tract sign. Laboratory examination, imaging and genetic tests showed no obvious abnormalities. EMG revealed neurogenic damage to the small muscles of both hands. Now we retrospectively analyzed the clinical features of a patient with O'Sullivan-McLeod syndrome, and data from 18 cases for comparative analysis, in order to improve its understanding by clinicians.

Existing traditional Chinese medicine (TCM)-related databases are still insufficient in data standardization, integrity and precision, and need to be updated urgently. Herein, an Encyclopedia of Traditional Chinese Medicine version 2.0 (ETCM v2.0, http://www.tcmip.cn/ETCM2/front/#/) was constructed as the latest curated database hosting 48,442 TCM formulas recorded by ancient Chinese medical books, 9872 Chinese patent drugs, 2079 Chinese medicinal materials and 38,298 ingredients. To facilitate the mechanistic research and new drug discovery, we improved the target identification method based on a two-dimensional ligand similarity search module, which provides the confirmed and/or potential targets of each ingredient, as well as their binding activities. Importantly, five TCM formulas/Chinese patent drugs/herbs/ingredients with the highest Jaccard similarity scores to the submitted drugs are offered in ETCM v2.0, which may be of significance to identify prescriptions/herbs/ingredients with similar clinical efficacy, to summarize the rules of prescription use, and to find alternative drugs for endangered Chinese medicinal materials. Moreover, ETCM v2.0 provides an enhanced JavaScript-based network visualization tool for creating, modifying and exploring multi-scale biological networks. ETCM v2.0 may be a major data warehouse for the quality marker identification of TCMs, the TCM-derived drug discovery and repurposing, and the pharmacological mechanism investigation of TCMs against various human diseases.
