1.A clinical observation on Danggui Sini decoction for treatment of patients with chronic non-atrophic gastritis and spleen-stomach vacuity cold syndrome

Objective To observe the clinical effect of Danggui Sini decoction for treatment of patients with chronic non-atrophic gastritis and spleen-stomach deficiency cold syndrome.Methods A prospective randomized control trial was conducted, including 126 in- and out-patients with chronic non-atrophic gastritis diagnosed by gastroscopy and spleen-stomach deficiency cold syndrome differentiated by traditional Chinese medicine (TCM) method admitted to Chinese Medicine Hospital of Haikou City. They were randomly assigned to an observation group (65 cases) and a control group (61 cases). The patients in observation group were given Danggui Sini decoction with addition or/with subtraction of ingredients according to individual situation (ingredients: angelica 15 g, ramulus cinnamomi 15 g, paeonia 15 g, asari 5 g, glycyrrhizae 10 g, Chinese jujube 12, medulla tetrapanacis 10 g ). The warm 200 mL (one dose) decoction was divided into two parts, one part was taken orally before a meal, twice daily, the therapeutic course was 4 weeks and in one week only 6 doses were applied; while the patients in control group, omeprazole capsules (dissolved in intestine), each time 20 mg, once a day combined with domperidone (bite phenanthroline) 10 mg each time, 3 times a day were given. After 4 weeks of treatment, the clinical efficacy, the total integral of symptoms, the therapeutic effect of TCM symptoms score, the improvement of the therapeutic effect of mucosal histological and changes of the number of patients with the patholoyical changes of mucosa under gastroscopy were observed.Results The total effective rate in the observation group was significantly higher than that in the control group [96.9% (63/65) vs. 86.9% (53/61),P < 0.05]. After treatment, the total integral of symptoms and the integrals of main TCM symptoms after treatment were lower than those before treatment, the degree of decrease in the observation group being more marked (the total integral of symptoms score: 12.36±2.11 vs. 15.94±2.37, epigastric distention integral score: 1.23±0.72 vs. 2.12±0.78, epigastric pain score: 1.18±0.67 vs. 2.25±0.94, thermophilic and like pressure integral score: 1.03±0.46 vs. 1.62±0.79, anorexia: 1.06±0.57 vs. 1.43±0.53, integral of loose stools score: 1.36±0.43 vs. 1.67±0.58, lassitude integral score: 1.24±0.32 vs. 1.65±0.42, allP < 0.05). Under gastroscope, the efficacy of mucosal histological change was significantly higher in observation group than that in control group [90.77% (59/65) vs. 81.97% (50/61),P < 0.05]. After treatment, the numbers of patients with chronic inflammatory cell infiltration (47 cases vs. 54 cases), erythema exudation (28 cases vs. 30 cases), flat erosion (8 cases vs. 15 cases), swelling erosion (5 cases vs. 6 cases), bile reflux (2 cases vs. 3 cases) and positive pylorus spirobacteria (20 cases vs. 29 cases) were significantly lower in the observation group than those in the control group (allP < 0.05).Conclusion The therapeutic effects of Danggui Sini decoction for treatment of patients with chronic non-atrophic gastritis and TCM syndrome of spleen-stomach deficiency-cold are remarkable.
2.A study on the homogeneity of the therapeutic effect on stroke patients in rehabilitation Medical Association

Objective:To investigate the therapeutic effect of stroke patients received rehabilitation treatment at different medical units in the Rehabilitation Medical Association.Method:Totally 183 stroke patients were recruited from October 12,2012 to February 5,2016 by Shenzhen Rehabilitation Medical Association(SRMA).SRMA consists of 4 members,including the Second People's Hospital of Shenzhen,Shenzhen Nan'ao People's Hospital,Shenzhen Baoxing Hospital and Community Healthcare Center in Futian District,which belong to tertiary hospital,secondary hospital,and primary healthcare unit,respectively.According to the time of onset of stroke,all the patients were divided into recovery phase or sequelae phase.The rehabilitation facilities were standardized.All patients received rehabilitation treatment for 4 weeks,which included functional rehabilitation training of limb,functional electrical stimulation training,activities of daily living training.Fugl-Meyer Assessment(FMA),balance subscale of the Fugl-Meyer test(FMB),National Institute of Health Stroke Scale(NIHSS) and modified Barthel Index(MBI) before and after rehabilitation treatment were used to evaluate the effects.Result:The scores of FMA,FMB,NIHSS and MBI were significantly improved in all the groups after rehabilitation treatment(P < 0.05).There was no significant difference in all assessments among the groups(P> 0.05).Conclusion:Based on the standardized rehabilitation treatment programs,the stroke patients come from the members of Rehabilitation Medical Association can achieve homogeneous effects after rehabilitation treatment.
3.Meta analysis for the effect of clinical pathways on single disease management

Objective To analyze the effect of clinical pathways on single disease management.Methods Clinical controlled trials on tumors of uterine,benign biliary tract diseases and benign thyroid neoplasm were collected and related literatures were screened according to the criteria of inclusion.The literature so collected underwent a Meta analysis.Results A total of 21 literatures were included.Meta analysis indicated that statistical difference existed in the total cost of hospitalization(WMD=1046.06,95%CI:- 1281.15 ~ - 810.96,P<0.00001) and length of hospital stay (WMD=- 2.18,95%CI:-2.59~- 1.76,P<0.00001)between non-clinical pathways group and clinical pathways group.Conclusion Implementation of clinical pathways can further reduce hospital costs and shorten hospital days of the single disease management.
4.Impacts of rhG-CSF on neutrophilic granulocytes and their surface molecules expression

Objective To investigate injecting recombinant human granulocyte-colony stimulating factor(rhG-CSF) on the proportion of peripheral neutrophilic granulocyte and their surface molecules expression in healthy people.Methods The peripheral blood samples were collected from the healthy people who were injected with rhG-CSF.The expressions of CD62L,CD11a,CD44 and CD49d on the surface neutrophilic granulocytes were determined before and after injection with flow cytometery.Results The median percentage of neutrophilic granulocytes in peripheral blood leucocytes after injecting rhG-CSF significantly increased from 60% to 85%(P0.10).Conclusion Injecting rhG-CSF may obviously impact the expression of CD62L and CD49d on neutrophilic granulocytes.

Objective To investigate the prognostic significance of extended resection for locally advanced colorectal carcinoma. Methods Clinical data of 960 cases of colorectal cancer hospitalized for surgery between Jan 1995 and Dec 2002 were reviewed, and complication rates, perioperative mortality, 5-year survival were calcualated, prognostic factors were analyzed using Cox regression model. Results Sixty-six patients with locally advanced colorectal carcinoma were treated with extended resection, accounting for 6.9% of the total cases. Surgical complication rate in this subgroup was 27.3% (18/66), relatively higher than that of conventional resection for colorectal carcinoma (?~2 =8.82, P=0.002). The perioperative mortality was zero. Pathology showed that carcinoma invasion into the adjacent organs resected en bloc was 31% (27/88), the 5-year survival rate was 62%. The tumor spreading into the adjacent organ (Wald=7.42,P=0.005) and lymph node status (Wald=4.55, P=0.035) were found to be the independent prognostic factors. Conclusions Extended resection for locally advanced colorectal carcinoma resulted in a favorable prognosis. Postoperative complication was relatively high, but still acceptable.
8.Allicin suppresses atherosclerosis by up-regulating protein S-nitrosylation

Objective To investigate the effect of allicin on the development of atherosclerosis in apoE-/-mice and explore its underlying mechanism from the perspective of protein S-nitrosylation.Methods Thirty male apoE-/- mice were randomly divided into 3 groups:control group (saline,ig),low-dose group (allicin,9 mg/kg·d, ig)and high-dose group (allicin,18 mg/kg·d,ig).They were fed with high cholesterol diet for 12 weeks.The levels of plasma lipids,oxidized-LDL (ox-LDL),malondialdehyde,tumor necrosis factor-alpha and nitric oxide (NO)were measured.The atherosclerotic lesions in aortic root were evaluated after hematoxylin and eosin staining and elastica van Gieson and immunohistochemical staining,respectively.Furthermore,in vitro experiments were performed using human umbilical vein endothelial cells (HUVECs).The HUVECs were treated with allicin (10μmol/L or 20 μmol/L)for 24 hours in the presence of ox-LDL (50 μg/mL).The level of NO in supernatant was measured by a nitrate/nitrite assay. The protein S-nitrosylation of the HUVECs was detected through immunofluorescence.Results The histological analysis revealed that allicin treatment not only significantly decreased the areas of the atherosclerotic lesion (all P <0.05)but also suppressed the macrophage accumulation and smooth muscle cell proliferation in the lesion.There was no significant difference in the levels of plasma lipids between control and treated groups.However,allicin exerted obvious anti-oxidative and anti-inflammatory effects. Interestingly,the allicin treatment led to marked increase of the plasma NO level (P <0.05)and aortic protein S-nitrosylation.The experiments in vitro further proved that the allicin up-regulated the levels of NO and protein S-nitrosylation in HUVECs treated with ox-LDL (P < 0.01 ).Conclusion Allicin can inhibit the development of atherosclerosis.The mechanism is associated with the up-regulation of protein S-nitrosylation in endothelial cells, which plays an important role in anti-oxidization and anti-inflammation.

Objective To provide nutritional supportive scheme for patients with radiation injury through the treatment of the one exposed to Nanjing 192Ir source accident.Methods The reasonable nutrition treatment scheme was made on the basis of dietary survey and nutritional index monitoring during clinical stages of the patient,including body weight,body mass index(BMI),biochemical indexes,electrolyte,etc.,as well as metabolic cart determination of resting energy expenditure (REE).Results Patient on admission (days 5 post-irradiated) weighing 42.5 kg,172 days after the first irradiated (the first skin grafting) fell to a minimum of 36 kg,then gradually rise,hen rose back to normal range on days 383 before discharge.Normal admission hemoglobin was 135 g/L,172 d after irradiated to a minimum of 54 g/L,normal discharge;when lymphocytes admission low as 0.5 × 109/L,58 days back to normal after exposure,172 days after irradiated down to 0.4 × 109/L.Serum albumin was normal admission 41.2 g/L,172 days after irradiated down to 25.3 g/L.The normal level of serum prealbumin was 0.22 g/L,248 days to a minimum of 0.04 g/L,the basic return to normal at discharge was 0.17 g/L.Admission normal liver function,bilirubin index slightly higher,the all in one parenteral nutrition after about 2.5 months later,bilirubin and liver function indicators were gradually increased,the adjusted treatment and nutrition liver and gallbladder and other gradually returned to normal after treatment.REE and the body weight were determined by metabolic cart on days 294,308 and 342 for the energy requirements.Conclusions For patient with radiation injury,appropriate nutrition therapy is a key method for the clinical treatment and rehabilitation,which can maintain the nutritional status of patients and improve clinical treatment.
10.Limited internal fixation combined with external fixation for comminuted humerus shaft fractures

Objective To evaluate the clinical outcome in the treatment of humerus shaft comminuted fractures using limited open reduction and internal fixation combined with an external fixator.Methods Data of 80 patients with comminuted humerus shaft fractures treated from January 2005 to January 2013 were analysed retrospectively.All the patients underwent limited open reduction and internal fixation combined with an external fixator (treatment group) and open reduction and plate fixation (control group) according to the random number table.In the treatment group,there were 40 patients (28 males,12 females),at mean age of 33.5 years (range,21-54 years),with causes of injury including traffic accidents in five patients,falls in nine,crashes in seven and others in six.There were seven patients with open fractures and 33 with closed fractures.In the control group,there were 40 patients (25 males,15 females),at mean age of 32.9 years (range,19-55 years),with causes of injury including traffic accidents in 16 patients,tumbling in seven,crush in seven and others in ten.There were eight patients with open fractures and 32 with closed fractures.The operation time,intraoperative blood loss,bone union time and complications in both groups were recorded.Clinical efficacy was evaluated using the Stewart and Hundley standard.Results Mean follow-up was 19 months (range,15-24 months).Treatment and control groups showed significant differences in operation time [(55.5 ± 10.3) minutes vs.(120.5 ± 15.3) minutes],intraoperative blood loss [(120.4 ± 20.7) ml vs.(245.4 ± 26.7) ml] and bone union time [(11.6 ± 1.3) weeks vs.(14.9 ± 2.3) weeks] (P < 0.05).Rate of incision infection was 8% (3/40) in treatment group and 10% (4/40) in control group (P > 0.05).In treatment group the results were excellent in 31 patients and good in nine.In control group the results were excellent in 27 patients,good in nine,fair in one and poor in three.One patient with radial nerve injury after a second surgery for implant removal and two patients with osteomyelitis or bone nonunion were noted in control group.Conclusion Limited open reduction and internal fixation in combination with an external fixator is associated with small trauma,easy operation,short operation time,few bleeding,rigid fixation,early functional exercises and reduced bone nonunion for treatment of comminuted humerus shaft fractures,which exhibits great clinical value.
