1.A modified speech enhancement algorithm for electronic cochlear implant and its digital signal processing realization.

In order to improve the speech quality and auditory perceptiveness of electronic cochlear implant under strong noise background, a speech enhancement system used for electronic cochlear implant front-end was constructed. Taking digital signal processing (DSP) as the core, the system combines its multi-channel buffered serial port (McBSP) data transmission channel with extended audio interface chip TLV320AIC10, so speech signal acquisition and output with high speed are realized. Meanwhile, due to the traditional speech enhancement method which has the problems as bad adaptability, slow convergence speed and big steady-state error, versiera function and de-correlation principle were used to improve the existing adaptive filtering algorithm, which effectively enhanced the quality of voice communications. Test results verified the stability of the system and the de-noising performance of the algorithm, and it also proved that they could provide clearer speech signals for the deaf or tinnitus patients.

2.Correction of penile curvature by allogeneic acellular dermal matrix

ObjectiveTo evaluate the effect of lengthening the short-side albuginea of the bending penis by the allogeneic acellular dermal matrix ( Allo-ADM ) for the treatment of penile curvature.MethodsFrom Jun 2007 to Jun 2010,18 patients with penile curvature due to malformation of the albuginea cavernous body were treated.The age of the patients ranged from 15 to 26 years (mean,20 years).Twelve patients were married.The curvature degree ranged from 30° to 80° (mean,55°).There were 17cases of single curvature and 1 case of complex curvature.The grafts ( Allo-ADM ) of different sizes were sutured to the albuginea at the curvatus side of the penis according to the extent of penile curvature through a circumcision incision.The extent of penile curvature and complications were evaluated postoperatively.ResultsPenile curvature was corrected in all 18 patients after the operation.No infection,hematoma and abnormal erection occurred postoperatively. No erectile dysfunction and penile re-curvature was observed during the follow-up period of 3 to 24 months.ConclusionLengthening the short-side albuginea of the bending penis by Allo-ADM could be a safe and effective way to correct penile curvature.
3.Research progress of the selective sphingosine-1-phosphate receptor 1 agonists.

Sphingosine-1-phosphate (S1P) is a lysophospholipid signaling molecule that regulates important biological functions in both intracellular and extracellular compartments. It interacts with five G protein-coupled receptors subtypes (S1PR(1-5)) to generate multiple downstream signaling. Activation of S1PR1 has been validated to be involved in the process of immune modulation. Fingolimod (FTY720), the novel S1PR1 agonist, has been approved for the treatment of multiple sclerosis in clinical trials. The study towards discovery of selective S1PR1 agonists has become hot spot for immunological diseases. This article summarized the research progress of S1PR1 agonists, emphasizing their structure types, structure-activity relationship and direction of development.
4.Anti-proliferative effect of arotinoid Ro40-8757 on human pancreatic carcinoma cell line JF-305

ObjectiveTo study the anti-proliferative effect and the mechanism of arotinoid Ro40-8757 on human pancreatic carcinoma cell line JF-305.MethodsMTT assay was used to evaluate the growth inhibition effect of Ro40-8757 on human pancreatic carcinoma cell line JF-305. The expression of p27 protein was measured by Western blotting, and flow cytometry for cell cycle analysis.ResultsRo40-8757 significantly inhibited the growth of JF-305 cells with a dose-, time-dependent manner. After 72 hours treatment with 10 -5mol/L Ro40-8757, the proportion of cells in G 1 phase increased from 56% to 76% while the cells in S phase decreased from 25% to 7%. Western blot analysis showed that the expression of p27 protein in cells treated with Ro40-8757 increased markedly.Conclusion Ro40-8757 inhibits the growth of pancreatic carcinoma cell line JF-305 possibly by up-regulating p27 protein expression and arresting cell cycle.
5.Evaluation of helicobacter pylori infection and its influencing factors in preschool children in Yulin, Shanxi province

Objective To analyze the infection situation and risk factors of Helicobacter pylori (Hp) in children in Yulin, Shaanxi province. Methods A total of 374 preschool children with digestive tract symptoms in Yulin area from December 2019 to December 2020 were selected. All the research object of personal data by using self-designed questionnaire (gender, age), personal experiences and health habits (common tableware, common koubei brushing your teeth, wash your hands before the rice then empress), the life habits (whether diet, koubei utilities, brushing your teeth regularly to suck finger), socioeconomic status, place of residence, with the number of living, family economic income), caregivers has a history of stomach problems (parents, nanny) And so on. The investigation method is the combination of telephone inquiry and on-site investigation. 3ml fasting venous blood was collected, and the supernatant was centrifuged. Serum IL-17 and IL-10 levels were determined by ENZYme-linked immunosorbent assay, and Treg and Th17 cell levels were determined by flow cytometry. Hp infection was detected by 14C urea breath test. Univariate analysis and logistic regression were used to analyze the independent risk factors of Hp infection in children. Results Eighty-three of the 374 children were infected with Hp, and the infection rate was 22.19%. The levels of Treg and IL-10 in peripheral blood of Hp infected children were significantly lower than those of non-hp infected children (P<0.05). The levels of Th17 and IL-17 in peripheral blood of children with Hp infection were significantly higher than those without Hp infection (P<0.05). In univariate analysis, there were statistically significant differences between the two groups in age, parental education level, per capita monthly income, sharing tableware, sharing toothbrushing cups, washing hands before and after meals, sucking fingers, parents' knowledge of Hp, and caregivers' history of stomach disease (P<0.05). Logistic regression analysis showed that sharing tableware, sharing toothbrushing cups, sucking fingers, parents' lack of knowledge about Hp, and caregivers' history of gastric diseases were independent risk factors for Hp infection in children (P<0.05). Conclusion The infection rate of Hp in children is high, and Hp infection can cause immune system dysfunction in children. It is necessary to take targeted and effective clinical measures to prevent Hp infection.
6.Changes and Clinical Meaning of Elements in the Gallbladder Bile in Patients With HBV Infection

Objective: Our aim was to examine the correlation infection of HBV and the formation of cholelithiasis. Methods: Gallbladder bile samples of 38 HBV-infection patients and 35 non-HBV-infection patients were determined. Results: Elevated levels of unconjugated bilirubin(UCB)(P<0.01)and Ga2+(P<0.05),decreased levels of total bile acid (TBA)(P<0.01)and cholesterol (TC)(P<0.01)were found. Conclusio: The changes of bile elements of HBV-infection and cholelithiasis are correlated.
7.Anesthesia for aortic valve insufficiency complicated with extremely dilated left ventvicle

Objective To retrospectively review the experience of anesthesia for aortic valve insufficiency complicated with extremely dilated left ventricle.Methods The patients were premedieated with morphine 10 mg and scopolamine 0.3 mg IM.Anesthesia was induced with etomidate 0.1-0.2 ms/ks.sufentanil 1μ/kg and pipecuronium 0.1 mg/kg.After the patients were intubated,anesthesia wag maintained beween 1.0%and 1.5% isoflurane and intermittent Ⅳ boluses of sufentanil and pipecuronium.ECG,direct BP,SpO2,PET CO2,CVP and body temperature were monitored during anesthesia.Result All patients Were rehabilitated without ventricular fibrillation and arrhythmia.Conlusions Measures are taken before the surgery to maintain preload,increase heart rate,reduce afterload moderately and maintain myocardial contractility.After the surgery,maintenance of preload is helpful for ensuring adequate forward flow and benefcial to patients using dopamine and nitrolycerin.
8.Pilot study of genetic counseling and prenatal diagnosis in androgen insensitivity syndrome: with 3 families reports

Objective:To explore genetic counseling and prenatal diagnosis strategies for women who have androgen insensitivity syndrome (AIS) family history or pregnancy history of AIS proband.Methods:Three families of complete AIS (CAIS) were retrospectively reported and summarized. The subsequent pregnancies and processes of prenatal diagnosis were followed up.Results:Among three CAIS families, one family had androgen receptors (AR) gene mutation diagnosis; the other two families were diagnosed clinically without gene diagnosis. All three mothers of CAIS probands were in pregnant again when they sought counseling, with gestational weeks between 7-13 weeks. They underwent chorionic villi sampling or amniocentesis in their second trimester (at 12, 16, 17 weeks respectively). Chromosome gender of all three fetuses were 46,XY, which was inconsistent with the ultrasonographic phenotype of external genitalia. All patients chose selective abortion in their second trimester. The external genitalia of all aborted fetuses were female phenotype, which supported the diagnosis of CAIS.Conclusion:Genetic counseling and prenatal diagnosis should be provided to high-risk patients with family history of AIS or proband pregnancy history, so as to achieve the goal of good childbearing and sound childrearing.
9.Diagnosis and surgical treatment of pancreatic insulinoma: a report of 137 cases

Objective To summarize the methods of diagnosis and results of surgical treatment of pancreatic insulinoma. Methods The clinical data of 137 patients with insulinoma treated in our hospital during the past twenty-six years were reviewed retrospectively.Results There were 77 males and 60 females. All of them were characterized by the Whipple′s triad. The sensitivity of ultrasonography, CT and MRI for localization was 35.1%, 67.9% and 58.1% respectively. One hundred and tweenty-six patients underwent operation. Of them, 102 cases had tumor enucleation, 4 cases had pancreaticoduodenectomy, 16 cases had distal panreatectomy, and the other 4 cases had only laparotomy. Of the 122 patients, who underwent resection, the tumor was benign in 118(96.7%) and malignant in 4(3.3%). The diameter of the tumor was less than 2cm in 86.9% of cases. In 98.4% of cases the tumors were single and in 1.6% of cases were multiple. 13.1% of the tumors located in the head, 46.7% in the body, and 40.2% in the tail.Conclusions Whipple′s triad and the measurement of fasting glucose, IRI, IGR, C-peptide, and proinsulin levels contribute to the diagnosis of insulinoma. However, the preoperative tumor localization is still difficult. Tumor enucleation is the technique of choice when feasible. Patients in whom tumor localization is unsuccessful at operation should be carefully evaluated to be certain of the diagnosis, and in general should not undergo blind resection.
10.Transcatheter arterial embolization for the treatment of advanced bladder cancer:clinical analysis of ;22 cases

Objective To investigate the therapeutic efficacy of transcatheter arterial embolization (TAE) in treating advanced bladder cancer. Methods A total of 22 patients with advanced bladder cancer were included in this study. The clinical data and the imaging materials were retrospectively analyzed. The clinical efficacy of TAE in treating advanced bladder cancer was evaluated. Results TAE was successfully accomplished in all the 22 patients. The preoperative and the postoperative abdominal pain scores were 4.73± 1.91 and 2.45±1.29 respectively, the difference was statistically significant (P<0.01); the preoperative and the postoperative ECOG scores were 3.14±0.47 and 2.68±0.56 respectively (P<0.05); the preoperative and the postoperative hemoglobin levels were (61.4±11.8) g/L and (79.3±14.5) g/L respectively (P<0.01). Of 7 patients with urinary tract obstruction, after the treatment the obstruction disappeared in 5 and was improved in 2, although the urination was still not smooth. All patients were followed up for (7.1±3.0) months. The 6-month survival rate was 54.5% (12/22). No serious complications occurred in all patients. Conclusion TAE can not only effectively improve the hematuria and urinary tract obstruction symptoms caused by bladder cancer, but also effectively control the tumor growth, improve the quality of life and prolong the survival time as well.
