1.Correction of penile curvature by allogeneic acellular dermal matrix

ObjectiveTo evaluate the effect of lengthening the short-side albuginea of the bending penis by the allogeneic acellular dermal matrix ( Allo-ADM ) for the treatment of penile curvature.MethodsFrom Jun 2007 to Jun 2010,18 patients with penile curvature due to malformation of the albuginea cavernous body were treated.The age of the patients ranged from 15 to 26 years (mean,20 years).Twelve patients were married.The curvature degree ranged from 30° to 80° (mean,55°).There were 17cases of single curvature and 1 case of complex curvature.The grafts ( Allo-ADM ) of different sizes were sutured to the albuginea at the curvatus side of the penis according to the extent of penile curvature through a circumcision incision.The extent of penile curvature and complications were evaluated postoperatively.ResultsPenile curvature was corrected in all 18 patients after the operation.No infection,hematoma and abnormal erection occurred postoperatively. No erectile dysfunction and penile re-curvature was observed during the follow-up period of 3 to 24 months.ConclusionLengthening the short-side albuginea of the bending penis by Allo-ADM could be a safe and effective way to correct penile curvature.
2.The changes of serum IL-6,IL-10,PLA2 on intracranial infection and its relationship with prognosis

Objective To explore the changes of serum IL‐6 ,IL‐10 ,PLA2 on intracranial infection and its prognosis .Methods Totallly 100 patients with intracranial infection during February 2011 to December 2013 were selected as the research object .In 100 cases of intracranial infection patients ,30 cases were cured (group A) and 52 cases improved (group B) ,18 cases of illness or death as poor prognosis group (group C) .Serum IL‐6 ,IL‐10 ,PLA2 content of the subjects in different time point were detected .Results One day After infection ,serum IL‐6 ,IL‐10 ,PLA2 levels in intracranial infection group were obviously higher than that of healthy control group (P< 0 .01) ;7 d after infection ,IL‐6 ,IL‐10 average level among 3 groups had obvious changed(P< 0 .05) ,7 d after in‐fection ,IL‐6 ,IL‐10 level of group B and group C was significantly higher than group A (P< 0 .05) ;and in group C ,IL‐6 ,IL‐10 was higher than group B (P< 0 .05) .7 d after infection ,the IL‐6 IL‐ 10 level of group A declined ,the IL‐6 ,IL‐ 10 levels of group B be‐gan decreasing 14 d after infection ,and the IL‐6 ,IL‐10 levels of group C had been in a rising trend .3 d after infection ,the PLA2 leves among 3 groups had obvious changed(P< 0 .05) ,7 d after infection ,the change rate increased ,7 d after infection ,PLA2 level of group B and group C was significantly higher than group A (P< 0 .05) ,and in group C ,PLA2 was higher than group B (P< 0 . 05) .7 d after infection ,the PLA2 level of group A declined ,in group B and group C ,PLA2 level began to decline significantly 14 d after infection .Conclusion IL‐6 ,IL‐10 ,PLA2 are closely related to the occurrence and prognosis of intracranial infection .
3.Delayed operation on treatment of high-energy distal tibia Pilon fracture.

OBJECTIVETo investigate the selection of operative methods,timing of operation and the effect of hige-energy distal tibia Pilon fracture.
METHODSFrom July 2006 to December 2009, 29 patients with hige-energy distal tibia Pilon fractures were treated, including 23 males and 6 females with an average age of 36.8 years old ranging from 21 to 54 years. According to Ruedi-Allgower classification on Pilon fractures, there were 3 cases of type I, 16 of type II and 10 of type III. The type I patients were fixed by screws and Kirschner wires and the cases of type II and III were fixed by filmy clover steel plates closed up tibia medial border or tibia lateral anatomical steel plates. All patients were evaluated by the tumid algesic level of ankle joint, gait,the activity of ankle joint according to Mazur score.
RESULTSNone of patients occurred complications such as deep infection, fractured internal fixation and prolapsed internal fixation. All patients were followed up from 6 to 42 months (averaged 28 months). The time of fracture healing was from 10 to 32 weeks (means 15 weeks). According to the ankle score of Mazur, the results were excellent in 15 cases,good in 10 cases,fair in 3 cases, poor in 1 case.
CONCLUSIONThe step-by-step delayed open reduction and internal fixation for hige-energy distal tibia Pilon fracture is an effective method with fewer complications and good function after the recovery of soft tissue injury. The method can not only mitigate the level of soft tissue injury, but also is beneficial to the early joint motion with rigid fixation.

4.Dual plane penile augmentation with human acellular dermal matrix through penile lengthening incision

Objective To illustrate the details and effects of a new technique of penile augmentation-a dual plane approach to enhance the penile girth with human acellular dermal matrix (ADM)through the incision on the dorsal penile root.Methods Firstly,a reversed V incision was made at the dorsal root of the penis and the superficial suspensory ligament of the penis was released.A Dartos fascia incision was then made and the plan between Dartos fascia and Buck's fascia was dorsally dissected toward the coronary sulcus.A Buck's fascia incision was made 1.5-2 cm from the coronary sulcus and the fascia was undermined distally.One or two sheets of ADM was dorsally placed by a dual plane method which combined partial sub-Buck's fascia plane and partial sub-Dartos fascia plane to enhance the penile circumference.Finally,the Dartos fascia incision was closed and followed by the closure of the wound with V-Y advancement.Results A total of 35 patients underwent dual plane penile augmentation No dorsal penile skin necrosis and exposure of ADM sheet happened in our group postoperatively.25 patients were followed from 6 to 24 months after operation.All patients were satisfied with their penile appearance and 21 married patients reported satisfied sexual activity.The increase of penile circumference was (2.65±0.41) cm at flaccid state and (1.85±0.35) cm at erectile state 6 months postoperatively.No pleat or absorb of the ADM sheet,losses of sensitivity of the penis occurred during the follow-up period.Conclusions By adjusting the plane of ADM placement,the dual plane penile augmentation ensures adequate soft-tissue coverage and sufficient blood supply of penile skin and offers increased benefits and fewer tradeoffs compared with a single plane.This technique could be an effective way to lengthen and enlarge the penis in one stage without major complication.
5.Effect of dual antiplatelet drugs on Fibulin-5, vWF and P-selection in serum of patients with acute cerebral infarction

Objective To analyse the effect of dual antiplatelet drugs on fibulin-5, vWF and P-selection in serum of patients with acute cerebral infarction.Methods Diagnosed 60 patients with acute cerebral infarction in our hospital and collected. All patients were randomly divided into experimental group and control group, 30 cases in each group.The control group was treated with conventional treatment and aspirin, and the experimental group was treated with clopidogrel on the basis of control group.After treatment, the serum levels of Fibulin-5, vWF, P-selection and adverse reactions were detected in all patients.ResuIts After treatment, compared with control group, the serum Fibulin-5 level was significantly lower in experimental group ( P<0.05 );the serum vWF level in experimental group was significantly lower ( P<0.05 );the serum P-selectin level in experimental group was significantly lower (P<0.05);there was no significant difference in the incidence of adverse reactions between two groups. ConcIusion Dual antiplatelet drugs can reduce serum vWF, P-selectin and fibulin-5 in serum of patients with acute cerebral infarction, adverse reactions do not significantly increase, have guiding significance to clinical application.
6.Relationship between Sonic hedgehog signaling pathways and radioresistance of esophageal cancer

AIM:To investigate the potential role of Sonic hedgehog (Shh) signaling pathways in the radioresistance of esophageal cancer.METHODS:Radioresistant cell line Eca109R was established by repeating X-ray irradiation at dose of 60 Gy in total using Eca109 cells as parental cells.The radiosensitivity of the parental and radioresistant cells was confirmed by colony formation assay.The cell viability was detected by CCK-8 assay.The intracellular protein levels of Shh and Gli1 were determined by Western blot and immunofluorescence.RESULTS:The survival fractions at dose of 2 Gy for Eca109R cells and Eca109 cells were 0.937±0.013 and 0.499±0.042, respectively.The inhibitory rate of cell viability decreased gradually in the Eca109R cells (P<0.05), suggesting that the radioresistant cell line was successfully established.The results of Western blot indicated that the protein expression of Shh and Gli1 was much higher in the Eca109R cells than that in the Eca109 cells (P<0.05).Immunofluorescence staining showed that Gli1 was expressed in the cytoplasm and nucleus, and presented nuclear clustering in the Eca109R cells.The positive rate of Gil1 expression in Eca109 cells was 52.3%± 0.035%, while that in Eca109R cells was 87.6%±0.021% (P<0.05).CONCLUSION:The radioresistance of esophageal cancer may be related to the activation of Shh signaling pathways with over-expression of Gli1 and other related proteins.
7.Latest research progress in integron capture and cleavage of exogenous gene cassettes and expression regulatory mechanisms

Integrons, an important system in bacteria that can capture, express, and excise exogenous genes (particularly antibiotic resistance genes), have received considerable attention in recent years. The integrons integrated onto transposons, conjugative plasmids, phages, or chromosomes can capture gene cassettes through site-specific recombination and facilitate their expression. Through a specific excision mechanism, gene cassettes can also be precisely excised and spread within bacterial populations via horizontal gene transfer and other means. This series of processes not only promotes the reconfiguration of gene cassettes and maintains diversity, but also provides flexibility for bacteria to adapt to constantly changing environments. This article will focus on the latest research advances in the capture, excision, and expression regulation mechanisms of integrons, in order to help researchers have a deeper understanding of the evolutionary trends of integrons and provide valuable insights for the study of bacterial antibiotic resistance dissemination mechanisms and the development of new antibiotics.
8.Correlation between sleep quality and parasympathetic intensity at nighttime

Objective To clarify the effect of cardio pulmonary coupling (CPC) technology on sleep quality and the effect of Shichen-based physiological function index analysis on sleep health assessment.Methods CPC and Shichen-based physiological function index were used to evaluate the sleep quality of 242 volunteers from 9:00 pm to 9:00 am for 12 hours.Based on CPC technology score,volunteers wree seperated to three sleep quality groups (scoring 85-100 in 63 volunteers,scoring 60-84 in 80 volunteers,scoring < 60 in 99 volunteers).The results of sleep quality and parasympathetic intensity over the whole nighttime and the different time period of each sleep quality group were compared and the correlation analysis was done.Results The whole nighttime sleep quality test showed that there was significant difference (t =-12.754-19.030,all P < 0.01) in the time to the stable sleep ratio (52.78% ± 10.32%,43.99% ± 14.48,20.95% ± 10.41%),the disturbed sleep ratio (25.65% ± 7.81%,34.18% ± 12.71%,53.65% ± 14.81%),and the apnea-hypopnea index (2.79 ±2.95,7.93 ± 6.61,23.94 ± 16.09)in the different sleep quality groups.The Shichen-based sleep quality test showed that there was significant difference among the stable sleep ratio,the disturbed sleep ratio during the 23:00 pm to 5:00 am period in the different sleep quality groups.There existed significant difference with parasympathetic intensity in the different sleep quality groups.There was positive correlation between the parasympathetic intensity and the stable sleep ratio in the same sleep quality group (r =0.508,P < 0.05).Conclusions CPC evaluation can commendably distinguish and evaluate different sleep quality groups on the whole night sleep.The Shichen-based physiological function index can not only distinguish the sleep quality,but also evaluate the multiple physiological function,the parasympathetic intensity between different sleep quality groups during different sleep periods.Thus they can be widely used to assess the sleep quality.
9.Bone marrow mesenchymal stem cell transplantation for treating ischemic stroke

Objective:To observe any effect of transplanting bone marrow mesenchymal stem cells (BMSCs) after ischemic stroke, and explore its mechanism.Methods:Seventy-two spontaneous hypertensive male rats were randomly divided into a sham group, a model group and a BMSCs group, each of 24. A model of middle cerebral artery occlusion was induced in the rats of the model and BMSCs groups but not in the sham group. The BMSCs rats had 106 BMSCs injected into their tail veins 24 hours after the modeling, while the other 2 groups were injected with the same amount of phosphate buffer. Modified neurological deficit scoring and 2, 3, 5-triphenyltetrazole chloride staining were performed on the 3rd, 7th and 14th days after the operation. The mRNA and protein expressions of vascular endothelial growth factor (VEGF) and glial cell-derived neurotrophic factor (GDNF) in the rats′ brain tissues were detected by RT-PCR and western blotting.Results:On the 3rd, 7th and 14th days after the operation, the average neurological deficit score of the BMSCs group was significantly lower and the area of cerebral infarction was significantly smaller than among the model group. Moreover, the infarcted volume in the BMSCs group continued to decrease gradually as time went on. There was no significant difference between the sham and model group in the expression of VEGF or GDNF mRNA or protein 3, 7 or 14 days after the operation. They were, however, significantly higher in the ischemic brain tissue of the BMSCs group compared with the other two groups.Conclusions:BMSC transplantation has a neuroprotective effect on rats with hypertension modeling ischemic stroke. The mechanism may be that BMSCs can up-regulate the expression of VEGF and GDNF in ischemic brain tissue.
10.Effects and Mechanisms of Bone Marrow Mesenchymal Stem Cell Transplantation for Treatment of Ischemic Stroke in Hypertensive Rats

Background and Objectives:
Stroke is the most common cause of human death and functional disability, resulting in more than 5 million deaths worldwide each year. Bone marrow mesenchymal stem cells (BMSCs) are a kind of stem cell that are able to self-renew and differentiate into many types of tissues. Therefore, BMSCs have the potential to replace damaged neurons and promote the reconstruction of nerve conduction pathways and connective tissue. However, it remains unknown whether transplanted BMSCs promote angiogenesis or improve the tissue microenvironment directly or indirectly through paracrine interactions. This study aimed to determine the therapeutic effect of BMSCs on ischemic stroke with hypertension in a rodent model and to explore the possible mechanisms underlying any benefits.
Methods:
and Results: Middle cerebral artery occlusion was used to establish the experimental stroke model. The area of cerebral infarction, expression of vascular endothelial growth factor (VEGF) and glial cell line-derived neurotrophic factor (GDNF), and increment of astrocyte were measured by TTC staining, western blot, real-time quantitative polymerase chain reaction (RT-qPCR) and immunocytochemistry. The results showed a smaller area of cerebral infarction and improved neurological function scores in animals treated with BMSCs compared to controls. The results of RT-qPCR and western blot assays showed higher expression of VEGF and GDNF in BMSC-treated animals compared with controls. Our study also showed that one round of BMSCs transplantation significantly promoted the proliferation of subventricular zone and cortical cells, especially astrocytes, on the ischemic side following cerebral ischemia.
Conclusions
Above findings support that BMSCs have therapeutic effects for ischemic stroke complicated with hypertension, which may occur via up-regulated expression of VEGF and GDNF and reduction of neuronal apoptosis, thereby promoting the recovery of nerve function.
