1.Effects of intravenous anesthetics on the phagocytosis of human neutrophils

The effects of intravenous anesthetics on phagocytosis of human neutrophils were observed in vitro. 1?10~6/ml of neutrophils, isolated from heparinized venous blood of healthy human,was mixed at equivalent volume with normal saline (as control), four different concentrations (clinical doses) of droperidoI, midazolam, fentanyl,etomidate or sodium hydroxybutyrate respectively. All mixtures were incubated in each of four 24-pore culture plates. The equivalent volumes of staphylococci 6~9?10~9/ml were added to these mixtures, 10 mins, 30 mins, 2hs or 24hs after incubation individually. The successive incubations lasted 45 mins,and the neutrophil phagoeytosis rate (NPR) was calculated. The results showed that NPR was correlated negatively with the concentration of each anesthetic mentioned above. but was related to its acting-duration. It is demonstrated that these intravenous anesthetics may reduce the phagoeytosis of neutrophils in a dose-related way,therefore we suggest that antibiotics or antineoplastics should be used in combination with these anesthetics during operation.
2.Medical Ethical Analysis of Management Mode of Correspondence of Potencyscale- Taking the Department of Neurology as an Example

Objective:To explore the effect of management mode of correspondence of potencyscale on impro-ving the nurses′job satisfaction and patient satisfaction in the department of neurology and analyze the medical eth-ics. Methods:We selected 37 nurses in the department of neurology. From July 2013 to March 2014, they were provided with the traditional management mode, whereas from March 2014 to June 2015 with management mode of correspondence of potencyscale. The questionnaire survey was conducted to measure the job satisfaction of nurses and the satisfaction degree of patients. Results:After the implementation of management mode of correspondence of potencyscale, the research indexes were significantly higher than before. Conclusion:The management mode of correspondence of potencyscale can improve the nursing staff′s work enthusiasm, patient satisfaction, and the quali-ty of care. Meanwhile, it is the specific practice of nursing ethics.
3.Application of tongue-drawing at lateral position in nasogastric intubation for senile patients with conscious disturbance

Objective To observe the effect of tongue-drawing at lateral position in nasogastric intubation for senile patients with conscious disorders. Methods One hundred senile patients with conscious disorders were randomly divided into observation group and control group with 50 cases in each group. The patients in the control group were given routine nasogastric intubation while those in the observation group were given nasogastric intubation by tongue drawing at the lateral position. The two groups were compared in terms of success rate of intubation, the rate of adverse reactions and intubation effects. Result Compared with the control group, the observation group showed significantly higher in one-time intubation success rate , comfort rate and lower rate of adverse reaction (all P<0.01). Conclusions The nasogastric intubation by tongue-drawing at lateral position is superior to the routine intubation. It can improve the success rate of one-time intubation, decrease the rate of adverse reactions and relieve the pains of patients.
4.Evidence-based medicine and reform in the mode of clinical medical education

With the change in the modes of medicine, the traditional educational mode of clinical medicine is unable to meet the development demands of modern medicine. Evidence based medicine, a new mode of medical treatment, reflects the development trends of modern medicine and represents the direction of modern medical advancement. Judging from the perspective of medical education, evidence based medicine is a learning method which differs from the traditional educational mode and represents a new concept of education in clinical medicine. The rise of evidence based medicine demonstrates the direction of reform in medical education in the world today and will also bring about great changes in the mode of medical education in China. It is imperative for us to follow the trend in clinical medical education, update concepts on medical education in line with the basic ideas of evidence based medicine, and push forward reform in the mode of medical education.
5.Investigation and analysis of the perceived stigma among family members of people with schizophrenia

Objectives To explore the perceived stigma among family members of people with schizophrenia and to identify the influential factors related to perceived stigma in socio-demographic characters of family members and patient characters.Methods Total 127 family members of people with schizophrenia in a psychiatric hospital in Beijing were surveyed by the Perceived Devaluation-Discrimination Scale developed by Link.Results The mean score of the perceived devaluation-discrimination scale is 2.60±0.44.It was significantly higher than the 2.5 midpoint(t=2.53,P<0.05).Family member's perceived stigma was significantly different between different groups of residence area,education level,occupation and family income (P<0.05).Perceived stigma were not affected by patient characters (P>0.05).Family members'education level was the influencing factors of stigma,and the R~2 Square is 0.213.Conclusions The family members of people with schizophrenia suffered from high stigma.Education level of family member may be one of the influential factors.

7.Capillary GC Determination of Fentanyl in Human Plasma

A GC method is described for measuring fentanyl in man plasma. Fentanyl was extracted at basic pH with cyclohexane-isobutyl alcohol (197:3) with Ro21-2212 added as internal standard and fentanyl derivative as earner. The drug was back-extracted in H2SO4, then the extract was made basic and recxtracted with ethyl ether-dichloromethane (9:1). The organic phase was evaporated at 40℃ under N2 and the residue was dissolved in ethanol. A. portion (2?l) was analyzed on a wide-bore capillary column (10 m ? 0.53 mm) of HP-1 (2.65 ?m), operated at 255℃ with N2 as carrier gas (8 ml/min) and N -P detector. The calibration graph was linear within the ranges of 0.25-100 ng/ml with a corelation coefficient of 0.9996 and the detected limit was 0.2 ng/ml. Mean recovery was 99.02% ? 6.81%. The coefficient of variation for the within- and between-run were all less than 8%. No interference was found from endogenous compounds, metabolites of parent drug or other commonly used drugs. The method was applied to therapeutic drug monitoring and pharmacokinetic studies.
8.Experimental study on differentiation of human hepatocellular carcinoma cell line Bel-7404 induced by matrine in vitro and its mechanism

Objectives:To investigate the differentiation of human hepatocellular carcinoma cell line Bel-7404 induced by Matrine in vitro,and discuss its mechanism.Methods:After dealing human hepatocellular carcinoma cell line Bel-7404 with different concentrations of Matrine,we drew cell growth curve by MTT colorimetry,and cell morphology was examined by light microscope.Then we evaluated the tumor cell biological characteristics changes through improved soft agarose clone forming test,cell adhesion assay,cell movement assay and cell invasion assay.Also we detemined cell cycle distributing by flow cytometry.Results:After 72 hours being treated with 0.5,0.75,1.0g/L,in the Matrine test groups,the cell proliferation was inhibited significantly,and cell shape transformed to normal.The amounts of clone were remarkably reduced.The cell adhesion ability increased in different level.At the same time cell ability of movement and invasion differently weakened.Analysis of cell cycle shown that along with rising of Matrine concentration,the cell amounts in G1 phase increased accordingly and cell amounts in G2 and S phase remarkbly decreased.Conclusion:During the appropriate concentration range,Matrine not only inhibited Bel-7404 growth and proliferation,but also induced tumor cell to differentiate.Its probably mechanism may related to cell cycle regulation.

10.Radiofrequency ablation for the treatment of solid malignant tumors with Cool- tip needle:report of 102 cases

Objective To evaluate ‘Cool-tip nee dl e’ radiofrequency ablation (RFA) for the treatment of solid tumors. MethodsIn this study, 102 patients w ith 157 tumors underwent RFA. Tumor size varied from 1.0cm to 20.0cm in d iameter. Result The comple te ablation(CA) rate was 83.5%(91/109)in tumors less than 5cm in diameter , and 35.4%(17/48)when tumors were larger than 5cm. RFA significantl y ameliorates symptoms in most lung cancer patients, and in 40%(4/10)patient s of pancreatic tumors. In the group receiving percutaneous RFA , no patients we re shifted to celiotomy, nor skin burn, or pneumothorax occurred. RFA for hepa tic tumors was complicated with abdominal pain in 13.4%(11/82), nausea in 6 .1%(5/82), jaundice in 9.7%(8/82), fever in 23.2%(19/82), and biliary leak age in 1.2%(1/82). Post RFA complications for pulmonary tumors included pain in 14.3%(1/7)and hypodermal gas accumulation in 14.3%(1/7). For pancrea tic tumors, pancreatic leakage in developed 20%(2/10), elevated blood amylas e in 70%(7/10), GI bleeding in 20%(2/10). Con clusions Treating solid tumors with ‘Cool-tip needle ’ RFA is efficient, reliable and safe, but great care should be taken in case o f pancreatic tumors.
