Objective To study the influence of NS4B on host defense system. Methods After cell line stably expressed NS4B was established, cell expression profiling caused by NS4B was studied using DNA microarray, and the results of microarray were verified via IFNGR1 fluorescence intensity analysis. Results The data showed that HCV-NS4B could suppress host defense system-associated gene ex-pression, in particular, IFN-γsignal transduction-related genes. Conclusion NS4B could play a role in persistence and resistance to IFN therapy in HCV infection.

BACKGROUND: Previous artificial ligament is easy to break for material fatigue and to appear synonitis for materialbiocompatibility.OBJECTIVE: To observe the application effect of ligament advanced reinforcement system (LARS) artificial ligament inreconstruction of anterior cruciate ligament via follow-up.DESIGN, TIME AND SETTING: An own control, follow-up observation was performed between August 2004 andJanuary 2007 at the Department of Orthopedics and Joint Surgery, Shanghai Changzheng Hospital, Shanghai, China.PARTICIPANTS: Twenty-eight patients with anterior cruciate ligament injury, including 16 with sport injury and 12 withvehicle accident injury.MATERIALS: The main component of LARS artificial ligament (Laboratoire d'Applieation et de Recherche Scientifique,France) is the high-tenacity polyester fiber polyethylene terephthalate.METHODS: Twenty-eight patients with anterior cruciate ligament injury were performed arthroscopic reconstruction of theligament with LARS artificial ligament and were averagely followed up for 20 months.MAIN OUTCOME MEASURES: Patients' anterior drawer test, Lysholm score, range of motion prior to transplantationand in follow-up.RESULTS: The knee unstable symptom of 28 patients disappeared, anterior drawer test was negative, joint function wasgood with the flexion of 5.71 to 129.64 degrees. Average Lysholm score raised from (45.7±9.82) points prior totransplantation to (86.5±6.44) points after transplantation (P < 0.001). All patients resumed normal activities at 2 monthsafter transplantation, without acute or chronic synovitis and host response to materials.CONCLUSION: LARS artificial ligament is a relatively safe transplantation material and especially suitable to reconstructanterior cruciate ligament with sport injury. Mid-/short-term follow-up shows good result.
4.Comparison of Excel and CMRtools in Evaluating Myocardium and Liver T2* for Iron Overload

PurposeTo assess the level of agreement and interchangeability among Excel and CMRtools (2012) for calculation of T2* values in iron overload.Materials and Methods T2* images in 27 cases withβ-thalassemia major were imported into CMRtools (2012). The regions of interest (ROI) were drawn and interference signal removed to obtain T2* values. The T2* images were also measured using default MR software and manually entered into Excel to obtain T2* values after removal of the interference signal. The agreement between the two methods in calculating T2* values was compared.Results For 27 patients withβ-thalassemia major, 10 cases showed myocardial iron overload. 25 cases had liver iron overload. The T2* values from two methods were not statistically significant (t=-0.152 and-0.691,P>0.05) for the liver.Conclusion There is good agreement and interchangeability between Excel and CMRtools (2012) in calculating T2* values. Excel can be used clinically to evaluate iron overload.
5.The role of modular femoral component in restoring femoral offset after total hip arthroplasty

Objective To assess the clinical significance of modular femoral component in restoring femoral offset after total hip arthroplasty.Methods Data of 75 patients,who had undergone THA with modular femoral component from June 2009 to June 2012,were retrospectively analyzed.There were 42 males and 33 females,aged from 44 to 83 years (average,64.7±12 years).There were 40 cases of left hip and 35 cases of right hip.The average Harris hip score was 54±6 (range,32 to 65) before operation.The Zimmer M/L Taper hip prosthesis with Kinectiv technology was used in all patients.Preoperative templating was utilized to accurately predict the prosthetic size and the femoral offset.During the surgery,the femoral offset was measured accurately,and the appropriate femoral neck prosthesis was selected in order to restore the femoral offset and reconstruct hip abductor moment arm.The femoral offset and leg length were measured according to X-rays after operation.The Harris hip score was used to assess the clinical results.Results The femoral offset was reconstructed in all patients,ranging from-3.0 to 3.0 mm (average,1.3±0.3 mm).The average leg length discrepancy was improved to 3.0±0.8 mm.All patients were followed up for 6 to 36 months (average,19.7 months).The average hip abduction angle was 40°±10° at 12 weeks after operation.The average Harris hip score was 86±13 (range,65-94) at 12 weeks after operation and 95±15 (range,78 to 98) at final follow-up.There was no hip dislocation and femoral acetabular impingement syndrome.Conclusion Modular femoral component can restore femoral offset without changing the leg length,which can result in good hip abductor strength,range of motion of the hip and stability,as well as less complications.
6.Therapeutic effect of creatine phosphate in early diastolic dysfunction

Objective: To investigate the therapeutic effect of creatine phosphate sodium (CP, Neoton) at early stage of myocardium diastolic dysfunction. Methods: A total of 138 cases with myocardium diastolic dysfunction were divided into CP group (n=109) and routine treatment group (n=29). All the patients received routine treatment. The CP group received CP treatment based on routine treatment(CP 2g+5%glucose or 0.9% sodium chloride 100ml, iv. drip, for patients with mild diastolic dysfunction was 1time/d, patients with moderate diastolic dysfunction was 2 times/d), duration was 7~14 days. Color-coded Doppler echocardiography was used to detect transmitral peak early diastolic velocity (E-wave),transmitral peak late diastolic velocity (A-wave),peak early diastolic velocity of mitral annulus (Ea), and peak late diastolic velocity of mitral annulus(Aa), and the value of E/A ,Ea/Aa and E/Ea were calculated. Symptoms, signs and items from echocardiography were compared between the two groups before and after treatment. Results: After treatment, all symptoms and signs of patients in two groups significantly improved (P<0.05~0.01), and the marked effective rate in CP group was significantly higher than that of routine treatment group (96.2% vs. 57.1%, P<0.01). In CP group, the marked effective rate of E/A (81.65% vs. 10.34%), Ea/Aa (83.49% vs. 13.79%) and E/Ea (92.86% vs. 12.00%) were significantly higher than those in routine treatment group (P<0.01all). Conclusion: Creatine phosphate possesses marked effect for improve patient’s condition and diastolic dysfunction.
7.Effect of transcatheter arterial chemoembolization combined with radiofrequency ablation in the treatment of primary liver cancer and its influence on the levels of nuclear factor kappa B and p53

Objective To investigate the effect of transcatheter arterial chemoembolization combined with radiofrequency ablation in the treatment of primary hepatocellular carcinoma (HCC) and its influence on the levels of nuclear factor kappa B (NF-B) and p53.Methods 90 patients with primary HCC were selected,and they were randomly divided into control group (n =45) and observation group (n =45) according to the digital table.The control group received chemoembolization treatment,the observation group was given combined radiofrequency ablation treatment.The patients were continuously treated for 6 months.Immunohistochemical assay was used to detect the expression of NF-B and p53 in two groups.The clinical curative effect and influence on the expression of NF-B and p53 were compared between the two groups.Results After 6 months of treatment,the Karnofsky score of the observation group was (84.32 ± 12.31)points,which was higher than (64.32 ± 11.24) points of the control group (t =21.295,P < 0.05).After treatment for 6 months,the AFP level of the observation group was (121.23 ± 1.43) μg/L,which was lower than (189.44 ± 36.42) μg/L of the control group (t =19.853,P < 0.05).After treatment,the positive expression rates of NF-B and p53 of the observation group were 24.44% and 20.00%,respectively,which were lower than 40% and 33.33 % of the control group (x2 =6.597,5.784,all P < 0.05).The recurrence rate of the observation group was 15.56%,which was lower than 28.89% of the control group (x2 =6.126,P < 0.05).The 6-month and 12-month survival rates of the observation group were 86.67% and 82.22%,respectively,which were higher than 75.56% and 64.44% of the control group (x2 =4.927,7.192%,all P<0.05).Conclusion The effect of radiofrequency ablation combined with radiofrequency ablation for patients with primary liver cancer is satisfactory,which is worthy of popularization and application.
8.Progress of interventions for dysphagia after radiotherapy in nasopharyngeal carcinoma

Radiotherapy is one of the effective methods for the treatment of head and neck tumors, especially nasopharyngeal carcinoma, but the adverse reactions caused by radiotherapy are common. Patients with nasopharyngeal carcinoma have a high incidence rate of dysphagia after radiotherapy. This article reviews the research progress of intervention strategies for dysphagia after radiotherapy in patients with nasopharyngeal carcinoma, aiming to improve the prognosis of patients with nasopharyngeal carcinoma.
9.Role of P4HA2 in promoting cancer progression

P4HA2, one of the important genes encoding the α subunit of collagen prolyl 4-hydroxylase (C-P4H) , is overexpressed in a variety of tumors and promotes tumor progression, and its high expression correlates with the poor prognosis of patients. In different tumors, P4HA2 can promote the proliferation, migration and invasion of tumor cells, increase the proportion of cancer stem cell population, and promote tumor cells to escape from immune system. An in-depth understanding of the role of P4HA2 in tumor progression may provide new ideas and insights for targeting P4HA2 to prevent or reverse tumor progression.
10.Primary clinical application of shear wave dispersion in differentiating benign and malignant liver tumors

Objective:To analyze the viscosity characteristics of liver tumors and investigate the clinical value of shear wave dispersion (SWD) in the differentiation of benign and malignant liver tumors.Methods:A total of 103 patients with focal liver lesions were prospectively collected in Zhongshan Hospital Affiliated to Fudan University from October 2020 to July 2021, including 80 cases with single lesion and 23 cases with multiple lesions, and only the largest lesion was observed in patients with multiple lesions. SWD values were measured within the tumor and in the liver parenchyma 2 cm away from the tumor, and were compared between benign tumor group and maligant tumor group. The ROC curves of SWD value, SWD ratio and their combination in differentiating benign and malignant liver tumors were plotted respectively, and the optimal diagnostic threshold, the sensitivity, specificity and accuracy of different diagnostic methods were analyzed.Results:Among the 103 patients, 35 were benign and 68 were malignant. The SWD value of liver benign tumor group was lower than that of liver malignant tumor group [(16.38±3.58)m·s -1·kHz -1 vs (18.59±3.12)m·s -1·kHz -1], the SWD value of liver parenchyma background in liver benign tumor group was lower than that in liver malignant tumor group [(10.88±3.37)m·s -1·kHz -1 vs (14.31±3.34)m·s -1·kHz -1], and the differences were statistically significant (all P<0.05). The SWD ratio of benign tumor to surrounding liver parenchyma was higher than that of malignant tumor group [1.57(1.25, 2.00) vs 1.27(1.06, 1.57)], and the difference was statistically significant ( P<0.05). When the SWD value >15.60 m·s -1·kHz -1 was used as the cut-off value, the area under ROC curve (AUC) was 0.72, the sensitivity was 88.2%, the specificity was 51.4%, and the accuracy was 75.7%. The sensitivity, specificity, accuracy and AUC were 58.8%, 74.2%, 63.1%, and 0.68, respectively, when the ratio of SWD value<1.32 was used as the cut-off value. SWD value combined with SWD ratio for the diagnosis of liver malignant tumor, the AUC was 0.88, the sensitivity was 82.3%, the specificity was 83.0%, and accuracy was 81.6%. The diagnostic efficacy of the two in combination for liver malignant tumor was superior to SWD value ( Z=2.678, P=0.007 4) and SWD value ratio ( Z=3.822, P=0.000 1). Conclusions:SWD imaging can reflect the viscosity information of liver tumors and surrounding liver parenchyma, and has potential clinical application value in the differentiation of benign and malignant tumors.
