1.Research advances in uterine papillary serous carcinoma

Uterine papillary serous carcinoma is a special pathological type of endometrial cancer. It is similar to ovarian papillary serous carcinoma in histology and can have distant metastasis when it is still confined to the endometrium. As it is very aggressive and has a rather poor prognosis,complete surgical staging is necessary in the treatment,assisted by radiotherapy and chemotherapy. In this review,the author discussed the recent research advances in uterine papillary serous carcinoma.
2.Study on the directing role of multi-slice spiral CT and multiplanar reformation(MPR)in bronchoscopy.

Objective To study on the directing role of multi-slice spiral CT and multiplanar reformation(MPR)in bronchoscopy and enhance the yield of peripheral pulmonary carcinoma(PPC).Methods This study was carried in 60 PPC patients from Respiratory Department in the First Hospital Affilliated to Suzhou University during 2002—2005.They underwent MSCT.Images were reconstructed with MPR(CPR)to evaluate the relationship between PPC and bronchi and the type of BS was identified.Compare the diagnosed yields of bronchoscopic multiple diagnostic procedure(BMDPs)with types of BS.Results (1)In all patients the third to the seventh level branches of the bronchi were clearly shown.The tumor-bronchus relationship was identified as four types on MSCT.(2)MSCT demonstrated the BS in 22 of 25 adenocarcinomas and 13 of 17 squamous-cell carcinomas and 4 of 6 small cell lung carcinomas and 4 of 8 bronchiolo-alveolar carcinomas.(3)BS typeⅠwas shown in 15of 48(31.3%).Type Ⅱ was seen in 7 of 48(14.6%).Type Ⅲ was shown in 12 of 48(25%)squamous tumors.Type Ⅳ was seen in 14 of 48(29.2%).(4)Total positive rate of bronchoscopy was 58.3%,73.3% with BS and 13.3% without BS.Type of BS was closely associated with positive rate of BMDPs.Conclusion Axial CT and MPR can show the relationship between PPC and bronchi better.BS can direct fiberoptic bronchoscopy(FOB)diagnosis procedure and enhance the yield of PPC.
3.Study on the normal value of motor conduction velocity and F-wave parameters in median and ulnar nerves in normal adults

Objective To study the normal value on the motor conduction velocity(MCV) and F wave parameters in normal adults at different age,sex,and provide reasonable datum for clinic.Methods 155 healthy volunteers (male for 76 cases, female for 79 cases,age from 18 to 82 years old) were used as subjects.Distal latency,proximal latency, MCV,F wave latency,the duration between F and M waves (F MIPL),F wave conduction velocity (FCV) as well as the F wave ratio were recorded in bilateral median and ulnar nerves, F wave were elicited with stimulation at wrist.Results There were significant differences in M wave latency, MCV, FwPL, F MIPL and FCV between the different groups of age and sex, the latency of each individual wave was shorter in the female than in the male.the conduction velocity(CV) of the female was faster than the male. No significant differences in the F wave ratio were found between male and female, left and right limbs for the same nerve,median and ulnar nerves in the same Limb and in individuals at different age.MCV was usually faster in the proximal than in the distal.Conclusion MCV and F wave parameters changed progressively with increasing age. There were respective normal values in different age group and sex,F wave ratio might provide information in evaluating nerve conduction function in both of the proximal and distal end.
4.Effects of shRNA-IL-32γ-mediated gene silencing on the proliferation and apoptosis of fibroblast-like synoviocytes in patients with rheumatoid arthritis

ObjectiveTo investigate the effects and mechanisms of IL-32γ-shRNA-mediated gene silencing on the proliferation and apoptosis of fibroblast-like synoviocytes in patients with rheumatoid arthritis (RA).MethodsA eukaryotic expression plasmid of shRNA targeting IL-32γ was transfected into fibroblastlike synoviocytes by liposome in patients with rheumatoid arthritis.RT-PCR was used to determine the expression level of IL-32γ.Western blotting was used to detect the levels of cyclin D1 and p-Akt.The proliferation of RA-FLS was examined by MTT.Cell cycles were analyzed by flow-cytometry.The apoptosis of cells were measured by TUNNEL.Comparisons between groups were tested by t test.Results ① The expression of IL-32γwas significantly inhibited by shRNA-IL-32γ-expressing plamid PGCsi 3.0 targeting sequence 1,2 and 3,and the inhibition rate had reached 75.6%,66.2% and 64.1%,respectively.② The absorbance value of proliferation of RA-FLS in EASY-shRNA-IL-32γ group was significantly lower than that in the shRNA-control group and normal group at day 3 [(0.23±0.03) vs (0.35±0.03) and (0.36±0.04),P<0.05] and 5 [(0.27±0.03) vs (0.52±0.05) and (0.53±0.04),P<0.01 ] after transfection.③ The rate of RA-FLS at phase G1 in the EASY-shRNA-IL-32γ group was significantly higher than that in the shRNA-control group and normal group respectively [(88±6)% vs (69±5)% and (68±4)%,P<0.05],while those at phase S+G2 in the EASY-shRNA-IL-32γgroup was significantly lower than that in the shRNA-control group and normal group [ ( 13.6±3.0)% vs (30.2±4.1)% and(32.1±4.3)%,P<0.01].④The rate of RA-FLS apoptosis in the EASY-shRNA-IL-32γ group was significantly higher than that in the shRNA-control group and normal group[(20.50±3.21 )% vs (9.20±0.32)% and (8.60±0.22)%,P<0.01].⑤ The expression of cyclin D1(0.36±0.04) and p-Akt(0.31±0.03) in the EASY-shRNA-IL-32γ group was significantly lower than that in the shRNA-control group [ (0.59±0.08) and (0.53±0.06)] and normal group [(0.61±0.07) and (0.52±0.06),P<0.01].ConclusionEASYshRNA-IL-32γ can inhibit RA-FLS proliferation by down-regulating the expression of cyclin D1 and induce RA-FLS apoptosis by down-regulating the expression of p-Akt.
5.Comparison of two preliminary screening methods for obstructive sleep apnea hypoventilation syndrome

Both the sleep apnea scale of sleep disorders questionnaire (SA-SDQ) and the Epworth sleepiness scale (ESS) were used to evaluate 25 patients with snoring and 125 patients with obstructive sleep apnea hypoventilation syndrome ( OSAHS), and scores were calculated and compared to the results of polysomnography ( PSG). Results showed that there was significant difference in scores of ESS and SA-SDQ, apnea hyponea index (AHI) and the minimal saturation of blood oxygen (LSpO2) at night between patients with OSAHS and those with snoring (P < 0.05). Score of both ESS and SA-SDQ significantly correlated with AHI ( r = 0. 78 and r = 0. 66, respectively, P < 0. 01 ) , i. e. , more severe of the condition, higher the scores. Score of ESS is highly consistent with clinical presentations in patients with OSAHS and can be used as preliminary screening for its diagnosis, however, use of SA-SDQ score is limited in diagnosing OSAHS due to its lower consistency.
6.Innova 3D-guided percutaneous vertebraplasty puncture reduces bone cement breakage

BACKGROUND:Percutaneous vertebraplasty is a new means for the treatment of vertebral compression fractures and bone metastases. The multiplaner reconstruction with digital substraction angiography (DSA)-Innova 3D plays an important role in guiding the percutaneous vertebraplasty puncture, which can effectively prevent the leakage of bone cement. OBJECTIVE:To explore the value of DSA-Innova 3D in the guidance of puncture of percutaneous vertebroplasty. METHODS:From March 2014 to September 2014, a total of 72 patients with 94 vertebras were selected for percutaneous vertebraplasty, of whom 36 (48 vertebras) with Innova 3D (experimentalgroup) and 36 (46 vertebras) without Innova 3D (control group). Spiral CT scan was performed in al patients to assess the distribution of bone cement. The imaging quality in the two groups was compared by Mann-Whitney U test. RESULTS AND CONCLUSION:CT showed bone cement leakage occurred in 7 of the 36 patients in the control group. Leakages were into the lumber disc in two cases, paravertebral space in two cases, vertebral pedicle in onecase or veins in two cases. However, only one case of bone cement leakage was observed in the experimental group. With the guidance of DSA-Innova 3D, the experimental group had signficantly lower leakage and fluoroscopic time than the control group (P < 0.05). The multiplaner reconstruction with DSA-Innova 3D plays an important role in guiding the percutaneous vertebraplasty puncture, thereby effectively reducing the leakage of bone cement.
7.Clinical and pathologic analysis of 33 cases of uterine papillary serous carcinoma

Objective To analyze the clinico-pathological characteristics, prognostic factors and the rational therapy for the uterine papillary serous carcinoma(UPSC). Methods Thirty-three cases of UPSC were treated in our hospital between January 1996 and December 2005. The clinico-pathologic characteristics, treatment and prognosis were retrospectively analyzed. Results The three-year overall survival for patients with stage Ⅰ, Ⅲ and Ⅳ was 77.4%,53.3% and 12.1%, respectively. 8.Effects of simvastatin on the function of dendritic cells in patients with rheumatic arthritis.

The present study examined the functional profile of dendritic cells (DCs) in patients with rheumatoid arthritis (RA) and the effects of simvastatin on the function of DCs. A total of 40 patients who was recently diagnosed as having RA were equally assigned to two groups: the routine treatment group (group R) and the routine treatment plus simvastatin group (group R+S). Twenty healthy individuals served as control. The peripheral blood mononuclear cells (PBMCs) were isolated before and 4 weeks after the treatment and then cultured with interleukin-4 (IL-4) and granulocyte-macrophage colony stimulatory factor (GM-CSF) to prepare mature DCs. The expression of co-stimulating factor CD86 on the surface of DCs was assessed by flow cytometry. And the stimulating capacity of DCs was measured by mixed lymphocyte reaction (MLR). The contents of cytokines in culture supernatants of DCs in MLR were detected by ELISA. Blood lipids and high-sensitivity C-reactive protein (hs-CRP) were detected. The relationship between the expression of CD86 and the blood CRP level was also investigated. The results showed that, as compared with the control group, the CD86 expression and the level of cytokines secreted by DCs were significantly increased in RA patients and greater stimulating capacity of DCs in MLR was demonstrated in RA patients. T lymphocytes in MLR secreted higher levels of proinflammatory cytokines (IL-2, IL-17, TNF-α and INF-γ) and lower level of anti-inflammation cytokine (IL-10). The function of DCs was markedly weakened and the level of hs-CRP and low-density lipoprotein was substantially lowered in group R+S in comparison to group R. The CD86 expression was positively correlated with hs-CRP. It was concluded that DCs in RA are highly activated and DC-initiated immune reaction may play an important role in the pathogenesis of RA. Simvastatin administration can significantly inhibit the DCs function and reduce the level of hs-CRP, indicating the suppression on inflammatory reaction may be one of the mechanisms by which simvastatin exerts its effect in treating RA.
9.Clinical significance of Her-2/neu status in patients with uterine papillary serous carcinoma

Objective The purpose of this study was to evaluate gene amplification by chromogenic in situ hybridization (CISH) and the protein expression of Her-2/neu gene in patients with uterine papillary serous carcinoma ( UPSC) and to determine its prognostic value.Methods Thirty-six patients with confirmed pathologic diagnosis of UPSC in Cancer Hospital of Fudan University from Jan.1996 to Jan.2006,were analysed retrospectively.CISH was performed to assess Her-2/neu gene amplification,and protein expression was evaluated by immunohistochemistry (IHC).The prognostic factors were analyzed by log-rank test or Cox proportional hazard model.Results Among 36 cases with UPSC,13 patients (36.1% ) showed moderate staining (++) to strong staining (+++) for Her-2/neu protein,while amplification of the Her-2/neu gene by CISH was observed in 4 of the 36 (11.1% ) cases.Her-2/neu protein over-expression was significantly associated with advanced surgical stage and worse prognosis by univariate analysis ( P = 0.030 and P = 0.002,respectively),while the multivariate analysis shown that only Her-2/neu protein over-expression and deep myometrial invasion were associated with a poor prognosis ( P < 0.05 ).In 13patients with Her-2/neu protein over-expression,the mean survival period with chemotherapy was shorter than those without chemotherapy (20 vs.42 months,P = 0.370 ).Conclusion Her-2/neu protein over-expression is significantly associated with advanced surgical stage UPSC and poor survival outcome,and might reduce the chemotherapy sensitivity.
10.Expression of RhoGDIα in aortae of hypertensive rats

Objective To evaluate the role of angiotensin Ⅱ(AngⅡ) signal passway on the expression of Rho GDP dissociation inhibitor alpha (RhoGDIα) in hypertensive rats. Methods Protein and mRNA expressions of RhoGDIα in aortae of 4, 12 and 18 week-old spontaneously hypertensive rats (SHR, n = 4) and Wistar Kyoto rats (WKY, n= 4) were examined by Western blotting and real-time PCR. Aortas from SHR and WKY were analyzed using immonuchemical staining to locate the RhoGDIα in the aorta. The RhoGDIα expression in aorta of hypertensive rat model of aorta coarctation (ACR, n = 6) was also analyzed using Western blotting. Furthermore, The effect of mechanical strain at 10 % elongation on expression of RhoGDIα in vascular smoothmuscle cells (VSMCs) in the presence or absence of L-158809, an antagonist for AngⅡ type 1 receptor, was also evaluated by Western blotting. Results No significant difference of RhoGDIα expression was found between SHR and WKY at 4-week-old and 12-week-old. However, in 18-week-old group, RhoGDIα was significantly highly expressed in SHR than that of WKY at both mRNA and protein levels. RhoGDIα was located in the media of the aorta. Expression of RhoGDIα protein was upregulated in aortas of ACR at 2 and 4 weeks as compared with the controls. The expression of RhoGDIα in VSMCs was inhibited by mechanicalstrain at 10 % elongation, and further decreased by treatment of L-158809. Conclusion RhoGDIα is upregulated in aortae of the hypertensive rats. AngⅡ signal passway may be involved in the process of regulating expression of RhoGDIα.
