Objective To implement the informationization management in 80% medical treatment health vocation through the research of clinical information exchange and share system,and truly realize "patient as central" in comprehensive information management. Methods The theories and ways for system engineering and management information system were used and succeed experience of domestic and overseas were referenced by the middleware added to system. Results The clinical information exchange system can accelerate medical treatment decision and promote efficiency and level of management,and cut down cost of medical treatment,which can improve clinical scientific research in further. Conclusion This research not only has practical importance,but also embodiment advancement,prospective,operability and latent social benefit.
2.The influence of ambroxol on expression of IκBα and TGF-β1 in rats with chronic bronchitis induced by smoking

Objective To explore the roles of IκBα and TGF-β1 on airway inflammation in rats with chronic bronchitis induced by smoking and study the effects and mechanism of anti-inflammation of pretreatment with ambroxol. Methods Sixty male wistar rats were randomly divided into four groups: Normal dosage group, model group, high dosage group and low dosage group. The rats with chronic bronchitis were established by smoking. For high and low dosage group, rats were pretreated respectively with ambroxol group, rats were pretreated with normal saline through peritoneal injection as much as the low dosage group.After 76 days, the histopathologic changes stained in hemotoxylin and eosin (H. E.) of bronchopulmonary tissues were observed under opticalmicroscope, white cell counts and differential analysis were performed in BALF, the expression of IκBα and TGF-β1 were detected by immunohistochemistry. Results The pathological changes of model group were in consistent with that of chronic bronchitis, but the degrees were significantly reduced in high and low dosage groups. Compared with those in normal group, the white cell count and the neutrophilic granulocyte count of BALF in model group were significantly increased and the macrophagocyte count decreased, and the expression of IκBαwas significantly decreased(t =3.24,3.31,3.29,3.48, P <0. 05) and the TGF-β1 significantly increased (P <0. 05). Compared with those in the model group, the white cell count and the neutrophilic granulocyte count of BALF in high and low dosage group were significantly decreased and the macrophagocyte count increased, the expression of IκBαwas significantly increased (t =2. 86,2. 97,2. 92,3.52,2.42,2. 88,2. 58,3.48, P <0. 05) and the TGF-β1 significantly decreased (P <0. 05) . Compared with those in low dosage group, the expression of TGF-β1 was decreased and the expression of IκBαincreased in high dosage group (t =2. 82,3.64, P <0. 05). Conclusions Down-regulating the expression of IκBα and up-regulating of TGF-β1 may be involved in the process of airway inflammation in rats with chronic bronchitis induced by smoking. Ambroxol might have better effects on ameliorating airway inflammation by up-regulating the expression of IκBα and down-regulating of TGF-β1.
3.Effects of peiminine preconditioning on ventilator-induced lung injury in mice

Objective To evaluate the effect of peiminine preconditioning on ventilator-induced lung injury (VILI) in mice.Methods Thirty-two clean healthy male BALB/c mice,aged 9-10 weeks,weighing 20-25 g,were divided into 4 groups (n=8 each) using a random number table:control group (group C),VILI group,dexamethasone group (group D) and peiminine preconditioning group (group P).Normal saline 0.2 ml was administered by intragastric gavage for 7 days in C and VILI groups,dexamethasone 10 mg/kg was administered by intragastric gavage for 7 days in group D,and peiminine 1 mg/kg was administered by intragastric gavage for 7 days in group P.VILI model (respiratory rate 80 breaths/min,tidal volume 40 ml/kg,fraction of inspired oxygen 21%,inspiratory/expiratory ratio 1 ∶ 2,for 4 h) was established on the 7th day of intragastric administration.After the end of ventilation,blood samples were collected from hearts for determination of serum interleukin-8 (IL-8) and tumor necrosis factor-α (TNF-α) concentrations (by enzyme-linked immunosorbent assay),and bilateral lung tissues were obtained for examination of the pathological changes after haematoxylin and eosin staining and for determination of contents of aquaporin 1 (AQP-1) and AQP-5 in lung tissues.Results Compared with group C,the concentrations of IL-8 and TNF-α in serum were significantly increased and the contents of AQP-1 and AQP-5 in lung tissues were decreased in group VILI (P<0.05),and no significant change was found in the parameters mentioned above in D and P groups (P>0.05).Compared with group VILI,the concentrations of IL-8 and TNF-α in serum were significantly decreased,the content of AQP-1 in lung tissues was increased (P<0.05),no significant change was found in AQP-1 content (P>0.05),and the pathological changes were significantly attenuated in D and P groups.Compared with group D,no significant change was found in concentrations of IL-8 and TNF-α in serum or contents of AQP-1 and AQP-5 in lung tissues (P>0.05),and the pathological changes were comparable in group P.Conclusion Peiminine preconditioning can reduce VILI in mice.
4.Association of 24hour movement behaviors with emotional and behavioral problems among left-behind children

Objective:
To examine the association of 24 hour movement behaviors with emotional and behavioral problems among left behind children, so as to provide a theoretical reference for the practice of 24 hour activity interventions to promote emotional and behavioral problems in this population.
Methods:
From February to May 2023, 1 117 left behind children in grades 4-6 from 10 primary schools in five cities in Zhejiang Province were selected using a convenient cluster sampling method to conduct a questionnaire survey examining 24 hour movement behaviors, as well as emotional and behavioral problems. The general linear model was adopted to analyze the association between satisfying the 24 hour movement behavior guidelines, and emotional and behavioral problems among left behind children.
Results:
The sleep duration compliance rate was the highest (52.19%), while the moderate-to-vigorous PA (MVPA) compliance rate was the lowest (17.73%). The compliance rate of the three activities accounted for 7.43 %. There was a dose response between the number of guidelines satisfied, and the emotional and behavior of left behind children; that was, satisfaction of a higher number of guidelines was associated with a lower risk of emotional and behavioral problems among left behind children (difficulty factor: β=-0.56, 95%CI =-1.23--0.19; strength factor: β=0.50, 95%CI =-0.48-1.22, P < 0.01). Compared to satisfying none of the guidelines, satisfying the guidelines for screen time ( β=-0.23, 95%CI =-2.18- -0.14 ) and sleep duration ( β=-0.13, 95%CI =-1.66--0.11) was negatively correlated with the difficulty factor, while satisfying the guideline for MVPA ( β=0.13, 95%CI =0.09-1.08) and sleep duration ( β=0.18, 95%CI =0.09-1.40) was positively associated with the strength factor. In addition, satisfying two or all three of the guidelines was more strongly associated with these outcomes than satisfying one of the recommendations ( P <0.01).
Conclusions
Meeting the 24 hour movement behavior guidelines can improve emotional and behavioral problems among left behind children. It is necessary to raise their awareness of the effect of satisfying the 24 hour movement behavior guidelines and formulate comprehensive intervention measures.
5.Analysis on Biomechanical Properties of Anterior Ligament Reinforcement Combined with Screw Fixation for Inferior Tibiofibular Instability

Objective To establish the finite element (FE) model of the anterior tibiofibular ligament injury by ankle fracture fixation, so as to compare the stress and deformation of the finite element model by using anchor-reinforced repair combined with screw fixation. Methods CT images of the ankle joint from a normal young male volunteer was selected to establish the FE model of the anterior tibiofibular joint injury by ankle fracture fixation. The injury models were divided into experimental group and control group, which were fixed by anchors or screws, respectively. The maximum stress distributions and the maximum deformation of the two models under various load conditions were observed through the FE analysis and calculation. Results Under the vertical load, the deformation of the experimental group was 6.8% higher than that of the control group. The deformation increased by 22.6% under external rotation load, while the deformation decreased by 5.1% under internal rotation load. Under the same load, differences in the maximum peak stress on the model between the control group and the experimental group was not significant. Due to the anchor fixation, the maximum stress of the experimental group were borne by the screw that fixed the distal tibial fracture, while the maximum stress of the control group was borne by strong fixation of the steel plate and the inferior tibiofibular screw. The maximum stresses on the anchor were distributed on the screw, and the suture mainly played the role of fixing. Conclusions The lower tibiofibular screw and anchor fixation could effectively treat the tibiofibular instability left by ankle fracture fixation. The anchor fixation dispersed the stress on the steel plate, and it showed greater ankle joint deformation ability while fixing the lower tibiofibula, so as to avoid the risk of broken nails.
