1.The Clinical Researches into the Laceration of Side- cuts in Perineum and Coalescence about 110 Cases

This paper gives the brief account of the researches in the effection of the side- cuts in perineum carried out in two angles (45°and 25~30°) during the helps in child birth between two groups of eutosia on the similar condition:fetal weight the contradict of womb perineum tissue e. g.. The results show that amnong 116 parturients with side- cut in 45( contrated group), the rate of tearing is 62.06%; on the contrary, among 110 parturients with side - cut in 25 ~ 30°( improved group), the rate of tearing is 35.45 %. According to statistics analysis, the rate of tearing in two groups has a marked difference (P<0.01). It directly shows the parturients in improved group have the advantages:shortening the sewing- up time lessening needling numbers reliving of the after - pains making the parturients get out bed some time later and sulkle the new- born babies in sitting way,reducing the nursing,shortening the time of infection.

Objective To investigate the effects of glucocorticoid(GC) on the expression of CD80 and CD4+CD25+T cells in peripheral blood lymphocyte of patients with multiple sclerosis (MS). Objective To investigate the relationship between serum levels of surfactant protein( SP)-A,SP-C and lung function impairment in coal workers with pneumoconiosis(CWP) in order to provide evidence for the biomarker study of pneumoconiosis.Methods Thirty-two coal workers with pneumoconiosis snd 41 healthy controls were included in this study.Serum levels of SP-A and SP-C were measured.Grading assessment of dyspnea and pulmonary function including predicted percentages of FVC,FEV1,FEV/FVC,MVV,and DLCO were conducted.Results Among the 32 participants with CWP,the severity of dyspnea was rated as level Ⅲ for 14 and level Ⅳ for 18 individuals.The pulmonary function was significantly impaired in CWP patients with level Ⅳ dyspnea compared with the healthy controls (FEV1% predicted:[69.38 ± 15.17 ]% vs.[96.35 ±10.24 ] % ; MVV% predicted:[ 65.89± 8.14 ] % vs.[ 94.13 ± 10.38 ] % ; DLCO% predicted:[ 96.51 ±11.37 ] %.The serum levels of SP-A and SP-C were significantly higher in CWP patients than that in the healthy controls (SP-A:[4.02 ± 1.22] μg/L vs.[2.41 ±0.68 ] μg/L,t =6.480,P =0.001 ;SP-C:[3.58 ±0.67 ] ng/L vs.[ 2.31 ± 0.29] ng/L,t =9.290,P < 0.001 ).Serum SP-A and SP-C levels in CWP patients were found to be significantly correlated with exposure to dust,dyspnea severity,FEV1% predicted and DLCO% predicted.Conclusion Serum SP-A and SP-C levels in CWP patients are closely associated with lung function,suggesting their role as candidate biomarkers for CWP.
4.Application of Data Mining on Selection and Specificity of Acupoints

Objective To explore the feasibility of application of data mining in the study on selection and specificity of acupoints.Methods The ancient and modern prescription data of acupuncture treating migraine and functional dyspepsia were regarded as sample data. Database techniques and natural language processing techniques were introduced to structurize and standardize acupuncture prescription data. Based on association rule data mining, the self-developed Acupuncture Data Mining System was applied to discover the rules of selection of acupoints.Results There were collected 634 and 386 prescriptions of acupuncture treating migraine and functional dyspepsia respectively. In the prescriptions treating migraine, acupoints distributed on Gallbladder meridian and Triple Energizer meridian accounted for 40.5% and 50.6% in ancient time and modern time respectively, and specific points accounted for 84.0% and 82.8% in ancient time and modern time respectively. In the prescriptions treating functional dyspepsia, acupoints distributed on Conception vessel, Bladder meridian and Stomach meridian accounted for 62.1% and 75.7% in ancient time and modern time respectively, and specific points accounted for 70.0% and 83.3% in ancient time and modern time respectively.Conclusion The result of data mining proves that the rule of selection of acupoints is "selecting acupoints along the affected meridian" and "mostly selecting specific points", and the specificity of acupoints is "where the meridian goes, where the qi accumulates".

6.Value of echocardiography in the diagnosis of aortico-left ventricular tunnel

Objective To evaluate the clinical value of echocardiography in the diagnosis of aorticoleft ventricular tunnel.Methods The echocardiographic and clinical characteristics were retrospectively analyzed in 6 patients with aortico-left ventricular tunnel we reported and 147 cases that documents reported.Results Six patients were confirmed by operation.The main echocardiographic characteristics of aortico-left ventricular tunnel were as follows:An abnormal communication began in the ascending aorta above the level of the coronary arteries,bypassed the aortic valve,and terminated in the left ventricle.The color Doppler flow imaging demonstrated to-and-fro flow,with or without aortic valve involvement.According Hovaguimian and his colleagues,2 cases were type Ⅰ,2 cases were type Ⅲ,1 case was type Ⅳ,1 case was type Ⅱ (showed as recanalization).Base on all of the 153 patients that we and documents reported,the accuracy rate of ultrasound diagnosis was 79.6%,the misdiagnosis rate was 17.1%,and missed diagnosis rate was 3.3 %.Conclusions Echocardiography is the non-invasive and sensitive tool in the diagnosis of the type of aortico-left ventricular tunnel and structure of cardiac involvement,and is the preferred method for preoperative.
7.Application of modified Z-stent in interventional therapy of Budd-Chiari syndrome

Objective To evaluate the utility of modified Z-stent in treatment for Budd-Chiarisyndrome (BCS). Methods A retrospective study was used in twelve BCS patients treated in twohospitals. The Doppler examination was carried out in all patients preoperatively, so as to confirm thenature of the lesion and chose correct type of endovascular modified Z-stent. Under DSA monitoring formembranotomy and dilation of the inferior vena cava(IVC), after that, insert the marked modified Z-stent to IVC correctly, and put the non-stent part to hepatic vein orifice. The cavography and hepaticvenography should confirm the position of the Z-stent. All of 12 patients with membranous obstructionof the IVC(MOVC) or segmental obstruction of the IVC (SOVC)were underwent modified Z-stentplacement. Results The IVC pressure (IVCP) before smd after membranotomy (dilation) were 27.33± 4.12cmH2O and 18.67 ± 5.07cmH2O (P<0.01). Comparing with dilation and putting stent group,the IVCP decreased from 18.67 ± 5.07cmH2O to 11.42 ± 2.11 cmH2O ( P < 0.01 ). The modffied Z-stent could avoid hepatic vein orifice getting compression and resist the retraction of IVC throughly infollowing-up period of 2.5 years. Conclusion s The endovascular treatment of BCS with modified Z-stent is more effective and safer to prevent thrombosis. Further study will be required to observe theirlong term effects.
8.Evaluation of therapeutic effects of flupentixol and melitracen combined with Saccharomyces boulardii on patients with diarrhea-predominant irritable bowel syndrome accompanied with anxiety and depression

Objective To assess the therapeutic effects of flupentixol and melitracen tablets combined with Saccharomyces boulardii on patients with diarrhea-predominant irritable bowel syndrome (IBS) accompanied with anxiety and depression.Methods This multi-center,randomized,prospective study enrolled 84 patients with diarrhea-predominant IBS who were divided into combined treatment group (42 patients) and control group (42 patients).Saccharomyces boulardii was administrated in both of the groups,and flupentixol and melitracen was added in combined treatment group.The treatment course was four weeks.The gastrointestinal symptoms and mood disorders were evaluated before treatment,one week and four weeks after treatment.Adverse reactions were also observed.Chi-square test was performed for statistical analysis.Results At the end of one week after treatment,the efficacy rates of gastrointestinal symptoms improvement of combined treatment group and control group were 31.0% (13/42) and 23.8% (10/42),and there was no statistically significant difference (P>0.05).At the end of four weeks after treatment,the efficacy rate of gastrointestinal symptoms improvement of combined treatment group was 92.5% (37/40),which was higher than that of control group (73.2%,30/41),and the difference was statistically significant (x2 =5.291,P =0.037).At the end of one week after treatment,the efficacy rates of Hamilton Depression Scale score improvement of combined treatment group and control group were 31.6% (12/38) and 12.1% (4/33),and there was no statistically significant difference (P>0.05).At the end of four weeks after treatment,the efficacy rates of Hamilton Depression Scale score improvement of combined treatment group was 63.9% (23/36),which was higher than that of control group (34.4%,11/42),and the difference was statistically significant (x2 =6.433,P=0.043).At the end of one week and four weeks after treatment,the efficacy rates of Hamilton Depression Scale score improvement of combined treatment group were 35.7% (15/42) and 80.0% (32/40),which were higher than those of control group (15.4%,6/39 and 34.2%,13/38),and the differences were statistically significant (x2 =9.759,P=0.007; x2 =17.105,P<0.01).One week after treatment,the adverse events rates of combined treatment group and control group were 4.8% (2/42) and 4.8% (2/42) ; four weeks after treatment,the adverse events rates of combined treatment group and control group were 2.5% (1/40) and 2.4% (1/41).There was no statistically significant difference in adverse events rates between two groups (both P>0.05).Conclusions Flupentixol and melitracen combined with Saccharomyces boulardii treatment could not only improve the anxiety and depression symptoms of patients with diarrhea-predominant IBS,but also effectively improve gastrointestinal symptoms.The efficacy of combined treatment is better than monotherapy Saccharomyces boulardii alone treatment.
9.New advances in phenylketonuria therapy research

Phenylketonuria is a most common group of genetic metabolic diseases.Phenylketonuria is caused by enzymatic defects in the metabolic pathway,which is characterized by high blood phenylalanine concentration.Patients need early,reasonable treatment once diagnosis,otherwise there will be serious nervous system sequelae.Available treatments aim to decrease the blood phenylalanine concentration,reduce nervous system symptoms.The current primary treatment of phenylketonuria is the limitation of dietary phenylalanine intake.Considering the poor compliance with long-term eating restrictions and the heavy family burden,the application of new medicine such as trahydropterina cofactor,glycomacropeptide,large neutral amino acids can improve the therapeutic effect and living condition of phenylketonuria patients.In addition,recombinant phenylalanine ammonia lyase,hepatocyte transplantation,gene therapy,probiotics and other new treatments also seem to be a promising approach in the near future.
10.The path analysis on the predisposing factors of suicidal ideation of network addicts

Objective To explore the effect of security,self-acceptance,self-concept,intemet addiction,anxiety,depression on the suicidal ideation of internet addiction patients.Methods The safety,self-acceptance,self-concept,internet addiction,anxiety and depression of 220 internet addiction patients were assessed using the corresponding scale tools.Suicidal ideation was used as the dependent variable,and each influencing factor was used as the independent variable.Correlation analysis,multiple stepwise regression analysis and path analysis were utilized to analyze the predisposing factors.Results The suicidal ideation (12.65 ±5.72) was positively correlated with internet addiction assessment (73.35± 15.83),depression self-evaluation (56.33± 13.75) and anxiety self-assessment (48.93± 17.20) (r=0.250,0.636,0.531,P< 0.01),while negatively correlated with security (50.26± 12.33),self-acceptance (38.29±7.15),and self-concept (56.47± 17.92) (r=-0.390,-0.297,-0.298,P< 0.01).Sequentially depression (β =0.571,P< 0.01),anxiety (β =0.173,P=0.003) were entered into the multiple regression equation.Path analysis based on the structural equation model showed that suicidal ideation had a direct effect with depression and anxiety,with effect values of 0.583 and 0.176.There were indirect effects of self-concept,self-acceptance,sense of security,and internet addiction assessment.The effect values were-0.137,-0.117,-0.281,and 0.034.The total effect of suicidal ideation and self-concept,self-acceptance,sense of security,internet addiction,depression self-assessment,and anxiety self-assessment were-0.137,-0.117,-0.281,0.034,0.583 and 0.176 respectively.Conclusions The influence of security,self-acceptance,and self-concept on suicidal ideation is produced through the mediating effects of depression and anxiety,and it has a negative predictive effect.There is no direct causal relationship between internet addiction and suicidal ideation.
