OBJECTIVE:To investigate the pathogen distribution and drug resistance situation in bronchoalveolar lavage fluid (BALF) from patients with refractory pulmonary infection, and provide reference for clinical rational drug use. METHODS:The sputum and BALF specimen of 136 inpatients with refractory pulmonary infection from the First People’s Hospital of Zhenjiang City from Jan. 2012 to Dec. 2015 were cultured and identified, pathogen detection was compared, and results of drug sensitivity test for pathogen in BALF specimen was analyzed. RESULTS:The positive rates of sputum and BALF specimen of 136 inpatients were 22.06%and 47.79%,with statistical significance(P<0.01);a total of 32 and 72 strains were isolated, they mainly were Ste-notrophomonas maltophilia,Pseudomonas aeruginosa,Acinetobacter baumannii and Staphylococcus aureus. S. maltophilia in BALF was sensitive to minocycline and levofloxacin;resistance rate of P. aeruginosa to common antibiotics was lower than 50%;A. baumannii was sensitive to imipenem, ampicillin sodium and sulbactam sodium, amikacin and minocycline;S. aureus was sen-sitive to linezolid, chloramphenicol and vancomycin. CONCLUSIONS:The positive rate of BALF specimen is higher than spu-tum,the pathogens are mainly Gram-negative bacteria. Clinic should rationally select antibiotics based on the drug sensitivity test and clinical symptoms.

Chinese Journal of General Practitioners 2011;10(7):499-500
Both the sleep apnea scale of sleep disorders questionnaire (SA-SDQ) and the Epworth sleepiness scale (ESS) were used to evaluate 25 patients with snoring and 125 patients with obstructive sleep apnea hypoventilation syndrome ( OSAHS), and scores were calculated and compared to the results of polysomnography ( PSG). Results showed that there was significant difference in scores of ESS and SA-SDQ, apnea hyponea index (AHI) and the minimal saturation of blood oxygen (LSpO2) at night between patients with OSAHS and those with snoring (P < 0.05). Score of both ESS and SA-SDQ significantly correlated with AHI ( r = 0. 78 and r = 0. 66, respectively, P < 0. 01 ) , i. e. , more severe of the condition, higher the scores. Score of ESS is highly consistent with clinical presentations in patients with OSAHS and can be used as preliminary screening for its diagnosis, however, use of SA-SDQ score is limited in diagnosing OSAHS due to its lower consistency.

OBJECTIVETo investigate the expression of microRNA-10a-5p (miR-10a) in laryngeal epithelial premalignant lesions (LEPL) and to analyze the correlations of its dysregulation with clinicopathologic parameters of LEPL.
METHODSAccording to the WHO classification (2005), 94 cases of LEPL were grouped into mild dysplasia (MID), moderate dysplasia (MOD), severe dysplasia (SD) and carcinoma in situ ( CIS). The expression of miR-10a in 94 cases of LEPL was detected by quantitative real-time polymerase chain reaction (qRT-PCR), and correlated with the clinical and follow-up data of all LEPL patients.
RESULTSmiR-10a was down-regulated in LEPL with increasing grade of dysplasia. There was significantly statistical difference between low-risk ( MID + MOD) and high-risk ( SD + CIS) lesion groups (P = 0.03). The linear regression analysis showed that miR-10a was correlated with grade and gender of LEPL (P < 0.05).
CONCLUSIONSDown regulation of miR-10a may be a useful molecular marker for grading of LEPL and diagnosis of early laryngeal cancer.

Cystic echinococcosis (CE), a zoonotic disease caused by Echinococcus granulosus at the larval stage, predominantly develops in the liver and lungs of intermediate hosts and eventually results in organ malfunction or even death. The interaction between E. granulosus and human body is incompletely understood. Exosomes are nanosized particles ubiquitously present in human body fluids. Exosomes carry biomolecules that facilitate communication between cells. To the best of our knowledge, the role of exosomes in patients with CE is not reported. Here, we isolated exosomes from the sera of patients with CE (CE-exo) and healthy donors and subjected them to liquid chromatography-tandem mass spectrometry analysis. Proteomic analysis identified 49 proteins specifically expressed in CE-exo, including 4 proteins of parasitic origin. The most valuable parasitic proteins included tubulin alpha-1C chain and histone H4. And 8 proteins were differentially regulated in CE-exo (fold change>1.5), as analyzed with bioinformatic methods such as annotation and functional enrichment analyses. These findings may improve our understanding about the interaction between E. granulosus and human body, and may contribute to the diagnosis and prevention of CE.

OBJECTIVETo explore the differences of the main pharmacological action in wild and cultivated Atractylodes lancea.
METHODStudy the effect of the 70% ethanol extracts from the wild and the culticated A. lancea on the mice with gastriculcer induced by absolute ethyl alcohol or HCl. Stdudy the effect on intestinal propulsive function in mice by measuring the length of intestine and distance of Indian ink. Observe anti-bacteria effect in vitro of the A. lancea by determining the minimum inhibitory concentrations.
RESULTThe pharmacological action, which made a little difference in the sane productive places, and made significant differences in different productive places. Compared to Jurong, it has better actions in Luotian on small intestinal propulsive function and bacteriostatic action. There was not an obvious difference in protective effect on gastric ulcer induced by absolute ethyl alcohol and 0.6 mol x L(-1) HC1 between different locations.
CONCLUSIONTo the main effect, cultvated A. lancea had no significant difference on the whole but having better result in some respects, which can provide the pharmacodynamics evidence of cultivated A. lancea being used instead of the wild.
