1.Ultrasonography diagnosis of cystic renal cell carcinoma

Objective To observe the ultrasonic features of cystic renal cell carcinoma. Methods Ultrasonic features of 18 patients of cystic renal cell carcinoma confirmed with operation and pathology were analyzed retrospectively.Results Among 18 patients, 7 were clear-cell carcinoma of cystis degeneration, 5 were cystic renal cell carcinoma (clear cell type), 3 were multilocular renal cell carcinoma, 1 was corpora mammillaria renal cell carcinoma, and one clear-cell carcinoma was on capsule wall of cyst and 1 was nephroblastoma. Ultrasound diagnosed 15 patients and misdiagnosed 3 with a diagnostic rate of 83.33%.Conclusion Cystic renal cell carcinoma has specific ultrasonic characteristics, and having an intimate knowledge of these characteristics may be helpful to diagnosis.
2.Effects of military stress on memory function in battlefield environment confined intensive training of peace-keeping soldiers

Objective To explore the effects of military stress on memory function in battlefield environment confined intensive training of peace-keeping soldiers.Methods 41 peace-keeping soldiers were chosen as the research objects.They were tested the memory function,respectively after they trained intensely in the domestic and in Sudan for five months.The memory function included verbal memory:WHO-UCLA auditory verbal learning test (immediate recall,the insert test,the short time delay memories,long delayed recall,long delayed recognition,visual memory) using the Rey Ostereitb Complex Figure (graphics copy and graphic delayed recall).Results The peace-keeping soldiers got lower scores in the insert tested(4.59±2.03) and graphics copy(35.85± 0.36) than in the domestic (4.85 ± 2.21,35.71 ± 0.46,P> 0.05),but there was no significant difference.They got lower scores in the immediate recall (41.68±9.87),the short time delay memories (9.27±2.77),long delayed recall (9.12±2.99),long delayed recognition (27.56± 1.70),and graphic delayed recall (16.90± 6.16) than those in the domestic(51.68±8.63,11.73±2.15,12.24± 1.96,29.00± 1.43,23.15±7.16,P<0.01),and the difference was statistically significant.Conclusion The peace-keeping soldiers battlefield environment stress has a certain effect on the soldiers' memory function.
3.Validation for access recirculation and access flow rate measurement by contrast-enhanced ultrasonography during hemodialysis.

To evaluate the feasibility and accuracy of contrast-enhanced ultrasonography (CEUS) for the measurement of hemodialysis access recirculation (AR) and access flow rate (Qa), a two pump system was used to simulate access and dialyzer flow. AR and Qa under different conditions, such as reversal connection of dialysis lines and the needle orientation, were compared with each other. The value of access flow and recirculation flow were calculated based on the formulas introduced in this paper, and the correlation and consistency between true flow rate and calculated values were analyzed. The measured R correlated well with true value of flow rate (r = 0.57, P = 0.038, Qa > Qb; r = 0.95, P = 0.001, Qa < Qb). The Bland-altman test showed good agreement between the calculated value based on CEUS and true values. The CEUS can be used as a new advanced technology for AR and Qa measurement.

4.Anti-inflammatory and anti-nociceptive effects in mice of water and ethanol extracts of roots and rhizomes of Asarum heterotropoides var. mandshuricum.

OBJECTIVETo compare the anti-inflammatory and anti-nociceptive effects of the different water extracts which were prepared by regular decoction time or removing volatie oil and ethanol extracts which were prepared in different concentration ethanol of Xixin (the roots and rhizomes of Asarum heterotropoides var. mandshuricum), and then to investigate the anti-inflammatory and antinociceptive mechanisms.
METHODXylene-induced ICR mouse ear edema and hot plate test were utilized to evaluate the anti-inflammatory and anti-nociceptive effects of Xixin at different dose level (water extracts of regular decoction time or removing volatie oil at the dose of 0.8 g x kg(-1) and 1.6 g x kg(-1); 95% ethanol extracts at the dose of 0.91 g x kg(-1) and 1.82 g x kg(-1); 50% ethanol extracts at the dose of 0.76 g x kg(-1) and 1.52 g x kg(-1)). Xylene-induced adrenalectomized mouse ear edema model was used to study the anti-inflammatory mechanisms. To reveal the anti-nociceptive mechanisms, mice were pretreated with naloxone in the hot plate test.
RESULTThe water extracts significantly reduced the weight of ear edema as compared with control group. Inhibition ratios were 43.20% and 63.69% at the higher dose, respectively. The ethanol extracts also significantly reduced the weight of ear edema and the inhibition ratios were 61.86% and 52. 56% at the higher dose, respectively. Mice administered water extracts delayed the latency times in the hot plate test. The anti-nociceptive effects of water extracts peaked at 2.0 h after i.g. administration. The hot plate latency times were increased by 51.27%, 62.78%, 60.08% and 68.00% at peak time, respectively. Regular decoction time group showed more significant effects in both models. The anti-inflammatory effect of 95% ethanol extracts was similar to 50% ethanol extracts. The water extracts were not effective in reducing xylene-induced adrenalectomized mouse ear edema. The anti-nociceptive effect of water extracts was blocked by naloxone.
CONCLUSIONBoth the water extracts and ethanol extracts of Xixin showed considerable anti-inflammatory potency against xylene induced inflammation. The water extracts produced anti-nociception in thermal model. The water extracts prepared in regular decoction time showed better anti-inflammatory and anti-nociceptive effects. Both the 95% ethanol and 50% ethanol extracts showed similarly anti-inflammatory effects. The anti-inflammatory effect of water extracts related to adrenal gland. The anti-nociceptive effect of water extracts was involved in activating opioid receptor.

5.Study on health management service specification and support systems for chronic patients

This article introduces the health management service specification for a chronic patient population. The concept of this specification comes from the whole process of people-centered and integrated health management of the chronic patient population. It is also based on the World Health Organization′s integrated chronic patient population management model. The completion of the specification has gone through several rounds of expert discussion in related fields in China. The contents include seven key links: suspect selection, clinical diagnosis, health assessment, intervention plan development, intervention plan implementation, follow-up, and health reassessment. It also includes establishing a professional team and developing a support system and mechanism of financing, compensation, and incentives. The specification aims at integrating regional health resources through standardizing service technology and scientific management concepts. It would help to improve the efficiency and quality of health management of chronic patient population in China, thereby reducing the economic burden of chronic diseases for patients and their families, and achieving the goal of improving residents′ health.
6.Effects of calcitriol on PI3K/AKT signaling pathway and wound healing in diabetic foot ulcer rats

Objective To investigate the effects of calcitriol intervention on PI3K/AKT signaling pathway and wound healing in rats with diabetic foot ulcer(DFU).Methods Thirty-two male SD rats were divided into normal control(Con)group,DFU group,calcitriol low dose(L)group and calcitriol high dose(H)group.A circular wound of 5 mm in diameter and deep to the fascia was made on the dorsum of the left foot of rats in each group.HE staining was used to evaluate the histopathological changes of the wounds.Immunohistochemical method was selected to compare the distribution of CD34-positive cells and the expression of p-PI3K and p-AKT in traumatic tissues of each group.ELISA was adopted in the detection of serum IL-6,IFN-γ,TNF-α and IL-7.RT-PCR was used to detect the mRNA expression of PI3K and AKT in each group,and western blot was used to detect the protein expression of PI3K,p-PI3K,AKT,p-AKT and VEGF.Results Compared with Con group,the expressions of IL-6,IFN-γ,TNF-α,IL-7,CD34,PI3K mRNA,AKT mRNA,p-AKT protein,p-PI3K protein,p-PI3K/PI3K,p-AKT/AKT increased,while PI3K protein expression decreased in DFU,L and H groups(P<0.05),VEGF and AKT protein expression decreased in DFV and L groups(P<0.05).Compared with DFU group,the expressions of VEGF,AKT and PI3K protein increased(P<0.05),while the expressions of p-PI3K/PI3K,p-AKT/AKT decreased in L and H groups(P<0.05),IL-6 decreased in L group(P<0.05),and CD34 expression increased in H group(P<0.05),while IL-6,IFN-γ,TNF-α and IL-7,PI3K mRNA,AKT mRNA,p-AKT protein and p-PI3K protein expression decreased(P<0.05).Compared with L group,the expressions of CD34,VEGF,AKT and PI3K protein increased(P<0.05),while IL-6,PI3K mRNA,AKT mRNA,p-AKT protein,p-PI3K protein,p-PI3K/PI3K and p-AKT/AKT decreased in H group(P<0.05).Conclusions Calcitriol intervention may reduce the activity of the PI3K/AKT signaling pathway,inhibit inflammation,promote neovascularization,and facilitate wound healing in rats with DFU.
7.Acupuncture for myocardial ischemia based on purinergic signaling:current status and perspectives.

Coronary heart disease (CHD) is believed to be one of the major diseases with myocardial ischemia (MI) as basic pathological change. The clinical trials and experimental researches in the last 15 years were analyzed in this paper, and it was found that purinergic signaling could transduce the selective expression of molecular adenosine receptors A, A, A, Ain cardiovascular system, which had the potential to be a novel target for treating cardiovascular disease. Using purinergic receptors to explore the actions of acupuncture for myocardial ischemia provides new perspectives to elucidate the mechanism of acupuncture for MI.
8.Potential risk factors for mild cognitive impairment in the elderly population in communities of Shanghai

Objective To investigate the potential risk factors for mild cognitive impairment (MCI) in the elderly population in the community, and to provide a basis for the primary prevention of MCI. Methods A cross-sectional study of elderly population in communities of Shanghai, China was conducted. A total of 368 subjects including both males and females, aged 65-80 years old, were selected to complete the mini-mental state examination (MMSE), basic information questionnaires, and physical examinations. Logistic regression analysis was used to analyze the potential risk factors of MCI. Results Of the 368 subjects participating in the study, 53 were found to have MCI and the prevalence rate was 14.4%. Univariate analysis found that older age, low education, no folic acid supplementation, stroke, osteoporosis and hyperlipidemia were risk factors of MCI. Multiple logistic regression analysis showed that advanced age [OR=1.146 (95%CI: 1.052-1.249)] and osteoporosis [OR=2.371 (95%CI: 1.042-5.396)] were the independent risk factors for MCI, while higher education [OR=0.073 (95%CI: 0.011-0.478)] was a protective factor. Age influenced all the aspects of MMSE scores (all P values <0.05). In addition, the analysis of the results suggested that subjects with regular folic acid supplementation got higher MMSE scores, especially in the aspect of language and praxis (P=0.002). On the contrary, patients with osteoporosis had lower attention and computing power scores (P=0.022). Conclusion The prevalence of MCI increased with age. Low education and osteoporosis may be the independent risk factors for MCI in the elderly population. Although no association was observed between folic acid supplementation and MCI, folic acid supplementation could improve the performance of language and praxis.
