1.The Effects of Long-Term Training of Aerobic Exercise on Some Risk Factors of Cardiovascular Disease.
Korean Circulation Journal 1990;20(2):226-231
25 males, aged 40-60 years, who participated in aerobic-type exercise on moderate intensity(60~85% of MHR), 3~5 days a week for 6~12 months were selected. Data were obtained at pre- and post-participation of tranining period for height, weight, girths of chest and abdomen, body fat, BP, HR, serum cholesterol, triglyceride, HDL-cholesterol, glucose, uric acid, BP and HR during graded treadmil exercise, and VO2max. After training for average 38.4 weeks, significant reductions occured in percent of body fat, HR and SBP both at rest and exercise, triglyceride, but not in cholesterol, LDL-cholesterol. And DBP, HDL-cholesterol were elevated. VO2max was increased apparantly by 16.4%. From the results was concluded that training of aerobic exercise could reduce the risk of coronary heart disease by 1) decreasing myocaridal oxygen demand, 2) improving on lipid profile, 3) increasing cardiac work capacity.

2.Experimental Study on the pathogenesis of Candida albicans in Swiss mice.
Korean Journal of Dermatology 1989;27(4):371-378
In search for animal model of systemic candidiasis, the authors attempted to infect the mice with Candida albicans (CUMD 23 and CUMD 25) isolated from human subjeets and study the mode of infection and pathogenesis. Swiss mice were inoculated with these organisrns either intravenously or intraperitoneally. After deterrnining the lethal doses of the pathogens at 30th day, another group of mice were inoculated with a sublethal dose. They were sacrificed weekly up to 4 weeks and the organs were examined mycologically and histopathologically. The results show that strain CUMD 23 is more virulent than CUMD 25. The most susceptible organ to C. albicans is the kidney followed by the peritoneum, mesentery and the pancreas. In the early lesion, C. atbicans generally show the spore form in the peritoneum, mesentery and the kidney when injected intraperitonea, lly but as time went by it rather took the hyphal form especially in the kidney. In the case of intravenous injection, the hyphal masses were seen in the kidney more quickly. With this result, it is suggested that the dissemination of C. albicans occurs easily in the latter and the spores reaching the kidney transform into hyphae which are not easily subjected to phagocytosis.

3.Vestibular Adenoma of the Vulva: A case report.
Korean Journal of Pathology 1992;26(2):197-200
A vestibular adenoma arising in mucin-secreting glands of the vulva is described. Grossly, the adenoma was a firm, well-demarcated solid mass with mucoid cut surface, measuring 3.5x3x3 cm, which was associated with an adjacent Bartholin's duct cyst. Microscopically, the solid mass was composed of proliferated mucous acini separted by fibromuscular septa and ducts lined by mucin-secreting columnar epithelium, transitional epithelium or metaplastic squamous epithelium. To our knowledge, less than 20 cases of such cases have been reported in the English literatures. However, whether the nature of proliferation is neoplastic or non-tumorous is still unclear.

Korean Leprosy Bulletin 2002;35(2):29-36
The surgical treatment of long duration lagaphthalmos was traditionally conducted using the Gillies (1934) method of temporal muscle transfer. This method has been effective in cases of simultaneous lagaphthalmos in the upper eyelid and ectropion in the lower eyelid. In the past, we treated 18 leprosy patients who suffered complications of ptosis in the upper eyelid and ectropion in the lower eyelid after the application of the Gillies method. To treat these complications, we removed the fascia strips emplaced during temporal muscle transfer from the upper and lower eyelid. Thereafter, we implanted a 1.0 gram gold plate in the upper eyelid and either a conchal cartilage graft. In the treatment of Lagaphthalmos over the past seven years, we have applied gold plate lid loading in 120 cases. However, in five of these cases, patients suffered from exposure of the gold plate due to paralytic orbicularis oculi muscle tearing. Recently, to correct this, we covered the gold plate with AlloDerm tissue to protect and support the muscle. In the treatment of ectropion over the last six years, we have had over 30 cases of cartilage grafts (over 35mm in length and 5mm in breadth) in the lower eyelid. However, the end of the cartilage graft would warp and create new partial ectropion several months later. Because of this, we shifted from cartilage to AlloDerm. We applied 40mm (length) and 7mm (breadth) to the lower border of the tarsus and fixed at each end with medial and canthal ligament. From 1997 to 2002, in the Institute of Leprosy Research, we operated on 25 patients suffering from Lagaphthalmos and Ectropion using a newer treatment to correct ectropion more effectively. The levator muscle of the upper eyelid loses the funtion of its antagonistic muscle, namely the orbicularis oculi due to facial nerve palsy, and results in retraction of the upper eyelid. To correct upper eyelid retraction, we severed the levator and muller muscle from the superior tarsal border and discontinued the use of gold lid loading which is visually evident. This method does not result in ptosis and the eye is more normal in appearance.

Journal of the Korean Society of Emergency Medicine 1998;9(2):257-263
Dissecting aortic aneurysm is an emergency condition which requires prompt diagnosis and management. From January 1992 to December 1996, 54 patients were admitted to our department. Patients were categorized according to the Stanford classification of aortic dissection and investigation of the clinical records utilized the retrospective method. Male constituted the majority, according for 38(70.4%) of the total number of patients, whereas females accounted for 16(29.6%). Mean age was 52.1+/-2.2 years old (range:25~82). Clinical findings included chest pain in 46 cases(85.2%), renal failure in 11(20.4%), aortic insufficiency in 10(18.5%) and stroke in 9(16.7%). Predisposing factors were hypertension in 48 cases(88.9%), Marfan's syndrome in 5(9.3%) and diabetes mellitus in 1(1.8%). 21 patients(type A:12, type B:9) underwent surgical treatment. There were three(2 in type A and 1 in type B) deaths in the surgical treatment group and nine(5 in type A and 4 in type B) deaths in the medical treatment group. These results reflect our current policy in the management of dissecting aortic aneurysm.

A follow-up study was made of 26 knees of 21 patients who underwent arthroscopic synovectomy between October 1982 and August 1983 in Catholic Medical College and Center. The patient's ages ranged from 3 to 66 years of age. There were 14 female patients and 7 male patients in this series. The follow-up period of all patients was a minimum of twelve months to a maximum of 20 months. The average follow-up of the whole group was 16.8 months. The abnormal findings were rated from 0 to 4 according to Marmors rating system. Final clinical evaluations of the patients were done according to the modified criteria of Sledge et al. The results were as follows; 1. Rheumatoid Arthritis Of 14 knees, 3 joints(21.4%) were rated “excellent”, 6(42.9%) were “good”, and 5(35.7%) were “fair”. There was no case of “unimproved”. 2. Juvenile Rheumatoid Arthritis The results of all 5 knees were satisfactory. Of 5 knees, 4 joints(80%) were rated excellent and one joint (20%) was good. 3. There were 3 knees of osteoarthritis, 3 knees of chronic synovitis and one knee of pigmented villonodular synovitis. Their final results were all satisfactory. Three joints were rated excellent and the other 3 joints were good. 4. Of 26 knees, as a whole, 10 joints (38.5%) were rated “excellent”, 11 joints (42.3%) were “good”, 5 joints (19.2%) were “fair” and no joint was “unimproved”. There were 5 recurrences of active synovial disease in this series, and these patients all were “classic” rheumatoid arthritis. But surprisingly the average patient in this group of recurrence did well even after recurrence. These patients all had less pain and effusion than before operation. 5. The major cause of unsatisfactory results other than recurrence of synovitis were articular degeneration (joint space narrowing) and patellofemoral osteoarthritis (4 knees). 6. A good range of motion with the shortest rehabilitation period can be expected if alternate flexion and extension splints are applied on alternate days for less than a week following the surgery. In conclusion, one may state that the arthroscopic synovectomy is worth considering for the treatment of various kinds of synovitis of the knee joint and when the rheumatoid process follows a favourahle course.

10.The Influence of Negative Emotion to Cortical Activity Induced by Auditory Verbal Imagery in Patients with Schizophrenia.

Journal of the Korean Society of Biological Psychiatry 2000;7(2):174-179
OBJECTIVES: Cognitive psychological models propose that auditory hallucinations arise from a problem with monitoring one's auditory verbal imagery. Most auditory hallucinations are derogatory in content and accompany negative emotions. If auditory verbal imagery plays an critical role in the pathogenesis of auditory hallucination, it must be influenced by negative emotions. This study was aimed at understanding the influence of negative emotions on the development of hallucinations by investigating the way by which negative emotions have influence on cortical activity induced by auditory verbal imagery. METHODS: For both normal subjects and patients with schizophrenia, quantitative electroencephalography(Q-EEG) was applied during the auditory verbal imagery tasks using a two word list. The one word list accompanied negative emotion and the other accompanied neutral emotions. The difference of EEG activity between two tasks was compared by paired t-test. WE also compare the difference of the influence of negative emotions between normal subjects and patients with schizophrenia. RESULTS: In normal subjects, amplitude of beta wave was increased in temporal area such as TCP1, and, the amplitude of theta frequency wave was decreased in right hemisphere such as FP2, F4, C4, CP2, P4, But, in the schizophrenia group, there were no significant differences. CONCLUSION: These results may suggest that auditory verbal imagery with negative emotion requires more activation in left temporal area, but, appropriate activation may not achieved in schizophenia patients. So, the possibility that the resultant disturbance of verbal self monitoring may be related to auditory hallucination is suggested in this study.
