1.Evaluation on the virtual CT endoscopy of the simple congenital malformation and its clinical application.

OBJECTIVE:
To evaluate the preoperative virtual CT endoscopy about the simple congenital middle ear malformations and its clinical application.
METHOD:
Thirteen cases (16 ears) with simple congenital middle ear malformations were undergone high resolution CT scannig to collect the original image data, followed by 3-dimensional reconstruction to process the images to evaluate the possible lesions of the ossicular chain. Then, a comparative analysis was made between the finding of virtual CT endotoscopic and the finding that actually found during the operation of the ossicular chain lesions.
RESULT:
We found malleus and upper strcture of incus abnormity or embeded in upper inner wall of atticus in 5 ears; malleus, incus and stape all abnormity or absence in 3 ears, in the former two conditions we both selected auricular incision through attic pathway remaining intact canal wall; we found crus longum incudis hypoplasia orstructure suprastapedialis absence,stapes aberrance or absence in 8 ears, then we selected vestibular window fenestration and artificial stapes surgery. Postoperative external auditory canals and tympanic membranes were intact in 16 ears,and speech frequency hearing improved by average of 25-45 dB. The coincidence between virtual CT endoscopy and the ossicular chain abnormity under operating views were 100% in cases with congenital ear deformity.
CONCLUSION
Virtual CT endoscopy is a reliable reference for preoperative evaluation on congenital middle ear malformations.

2.Advantage of intravascular ultrasound guiding left main lesion intervention

Objective:To explore advantage of intravascular ultrasound (IVUS) guiding coronary intervention .Meth‐ods:A total of 100 patients with left main (LM) lesion were selected and equally divided into IVUS group (n=50 , received stent implantation under IVUS guidance ) and control group [n=50 ,received stent implantation after coro‐nary angiography (CAG) evaluation without IVUS examination ) .Instant condition after stent implantation and fol‐low-up results after 12 months were compared between two groups .Results:There were no significant difference in general data ,percentages of LM opening ,body part and tail lesions ,different types of plaques between two groups , P>0.05 all .During hospitalization ,there were no significant difference in percentage of emergency coro‐nary artery bypass grafting (0% vs .4% ) and mortality (0% vs .4% ) between IVUS group and control group (P>0.05) .However ,after 12 months ,compared with control group ,there were significant reductions in incidence rates of in-stent restenosis (16% vs .4% ) and late stent malapposition (34% vs .6% ) in IVUS group , P< 0.05 ,<0.01. Conclusion:IVUS guiding left main lesion intervention is helps to select more appropriate therapeutic plan , reduce tissue injury and improve long -term therapeutic effect .
3.Logistic regression analysis of risk factors for ischemic cerebral stroke

Objective:To explore influence of risk factors of ischemic cerebral stroke (ICS)on its onset.Methods:A total of 350 patients diagnosed as ICS were selected as stroke group among 12241 >40 years people from Dongli dis- trict of Harbin city,another 350 >40 years non-ICS people,who were randomly selected from the same district, were enrolled as control group.ICS risk factors were compared between two groups,multi-factor Logistic regres- sion analysis was used to analyze independent risk factors for ICS.Results:(1)There were no significant difference in age,nationality,education,marriage and professions between two groups except food taste (P<0.01); (2) Multifactor Logistic regression analysis indicated that hypertension (OR=5.853,95%CI:3.188~10.673),hyper- lipidemia (OR=5.601,95%CI:2.378~ 9.635),carotid artery plaque (OR=4.963,95%CI:2.350~9.843), heart disease (OR=4.568,95%CI:2.535~7.745),hyperhomocysteinemia (OR=4.076,95%CI:3.001~8.270), apparent overweight (OR=3.421,95%CI:1.598~6.476)and family history of cerebrovascular diseases (OR=3.065,95%CI:2.543~4.236)were independent risk factors for ICS in both groups (P<0.01 all).According to OR value from big to small,risk factors for ICS were hypertension,hyperlipidemia,carotid artery plaque,heart disease,hyperhomocysteinemia,apparent weight and family history of cerebrovascular diseases respectively.Conclu-sion:Besides traditional risk factors like hypertension,dyslipidemia and family history of cerebrovascular diseases, other risk factors such as carotid artery plaque,heart disease,family history of cerebrovascular disease and apparent overweight are also important risk factors for stroke.
4.Study on differential genes expression in HBV-related heptocellular carcinoma

Objective To investigate the expression of oncogenes and tumor suppressor genes in hepatitis B virus (HBV) related hepatocellular carcinoma (HCC). Methods mRNA was extracted from cancerous and paracancerous tissues of 22 patients with HBV related HCC and synthesized into cDNA. The cDNA labeled with 33p dATP was hybridized for cDNA microarray each consisting of 3170 genes or expressed sequence tags (EST). The differential expression genes were searched in gene data base and verified using RT PCR. Results The differential expression of 1369 genes or EST was identified including 121 genes or EST altered 2 times or more in cancerous tissues compared with paracancerous tissues. Compared with paracancerous tissues, 88 showed overexpression and 33 genes were down regulated. Objective To evaluate the in vivo behaviour of the pulsed tablet in six scintigraphic studies.Methods The lag time and the anatomical position at the time of release were detected by scintigraphic evaluation.Results The different types pulsed release tablets all can achieve the lag time in vivo.Conclusion Scintigraphic study is the first considered method to evaluate the in vivo behaviour of the new formulation.
7.Related variance analysis on auditory effects of tympanoplasty

OBJECTIVE To study the relative factors affecting auditory effects after tympanoplasty. METHODS 118 cases (126 ears) of tympanoplasty were reported,16 factors that might influence auditory effects were analysed. RESULTS There were 10 in 16 factors affecting auditory effects after tympanoplasty, including course, the degree of pathological changes, air-bone gap, function of eustanchian tube, remaining handle of malleus, the type of surgery, condition of tympanic mambrane after operation and so on. The results of single-variance analysis had statistical differences (P<0.05). CONCLUSION Auditory effects after tympanoplasty were influenced by a series of factors, the major factors were the degree of pathological change and type of surgery.
8.Evaluation of virtual endoscopy in tympanoplast

OBJECTIVE To study the clinical usability of virtual endoscopy(VE) in tympanoplasty.METHODS A total of 102 patients (204 ears) were observed by virtual endoscopy. Tympanoplasty was performed in 72 cases (75 ears) including 53 cases (55 ears) with chronic otitis media and 19 cases (20 ears) with congenital microtia and middle ear dysmorphia. Ossicular chain reconstruction was conducteded in 65 ears at the same time. RESULTS The ossicular chain was showed eroded in 19 ears of 23 patients with cholesteatoma otitis media pre-operatively by VE, but was found eroded in all of 23 ears during operation. The ossicular chain was showed eroded in 29 ears of 32 patients with osteitis otitis media pre-operatively by VE, but during operation it was found 23 ears with malleus and incus eroded, 11 ears without head of stapes or up-structure of stapes. VE showed 18 ears with congenital microtia and middle ear malformation and 2 ears with small tympanic cavity and no ossicular chain preoperatively, and operation proved 17 ears with severe ossicular malformation, 2 ears without stapes, 1 ear with vestibular window atresia. There were 2 ears with sudden hearing loss after tympanoplasty, the VE showed ossicular displacement. The coincidence rates between VE and operation view were 92 % in patients with otitis media and 100 % in patients with congenital microtia and middle ear malformation. CONCLUSION VE can supply reliability data for evaluation of the damaged ossicular chain and efficacy of tympanoplasty.
9.Regulatory effect of tripterygium wilfordii polyglycoside on expression of epidermal growth factor receptor family in collagen induced arthritis rats

ObjectiveTo study the regulatory effect of Tripterygium wilfprdii polyglycoside (TWP) on the expression of EGFR and ErbB-2 induced arthritis rats.The effect of TWP on arthritis was also explored.MethodsAfter the model of CIA rats were established,the expression of EGFR and ErbB-2 in the synovium and articular cartilage were tested by immunohistochemical stain and real time PCR.ANOVA was used for statistical analysis.ResultsThe protein and mRNA expression of EGFR and ErbB-2 in the synovium (EGFR 0.268±0.059,ErbB-2 0.25±0.04,EGFR mRNA:14.2±0.55,ErbB-2 mRNA 23.46±3.64) and articular cartilage (EGFR 0.193±0.018,ErbB-2 0.217±0.033,EGFR mRNA:4.16±0.50,ErbB-2 mRNA 9.23±0.66) of the model group were significantly higher than those of the control group(P<0.01).After being treated with TWP and MTX,the protein and mRNA expression of the EGFR and ErbB-2 decreased markedly (P<0.01).Conclusion EGFR and ErbB-2 may play an important role in the pathogenesis of arthritis development.The molecular mechanism that TWP can treat synovitis and bone destruction of RA is related to the inhibition of EGFR and ErbB-2.
10.Lengthening over retrograde nail technique of the femurs in children

Objective:In this study, the feasibility of this method was evaluated in children with large distance femoral lengthening cases, and the acute correction and staging surgeries were compared at healing speed. At the same time the key points of surgical techniques would be summarized, so as to clarify the application value of this technology in children patients.Methods:Since July 7 of 2014 to January 16 of 2018, a total of 10 cases of Lengthening Over Retrograde Nail (LORN) surgery have been formulated and carried out, including 9 males and 1 females.The average age of the operation was 13.10±2.18 y. In all cases, the osteotomy point was located in the metaphysis of the distal femur, and the records from the first meeting to the end of follow-up included gender, date of birth, reason of discrepancy and treatment history, age at surgery, lengthing distance, the length of duration with frame, healing time, ROM of the knee joint, complications, et al. To compare the healing time of acute correction and staging surgery. Statistical analysis was made by SPSS 22.0 software, and then reasonable treatment strategies were proposed.Results:The average lengthing distance of 7.07±1.01 cm. The median time of the duration with frame was 8.5 (4,16) months, and the median follow-up time was 39 (34, 54) months. All cases obtained good clinical and imaging healing (1.70±1.10 month/cm vs. 1.16±0.54 month/cm), and all regained to normal walking function (136.67°±20.82° vs. 125.71°±26.37°), without the occurrence of fracture after therapy. There were no statistically significant differences in the speed of healing, range of joint motion or complication rate between acute correction and staging treatment. Conclusion:It is feasible for femoral lengthening surgery assisted by retrograde intramedullary nail in the treatment of femoral shortening deformityin children. The correction of mild angular deformity and lengthening surgery can be performed simultaneously. The osteotomy site was proposed at distal metaphysealof the femur for better quality of osteogenesis and shorter duration for fixator. It is safe to osteotomy and lengthening in the area of enchondroma.
