1.Relationship between outdoor playing time and moderate-to-vigorous physical activity for Japanese young children and correlates of physical activity

This study aimed to examine the relationship between daily outdoor playing time assessed using questionnaires for parents and daily moderate-to-vigorous physical activity (MVPA) using triaxial accelerometers for young children (213 boys: 5.8±0.6years and 193 girls: 5.9±0.6years). The study also examined the potential relationships between daily MVPA and exercise habits or familial and preschool factors which were assessed using questionnaires for parents. Daily MVPA was assessed using a triaxial accelerometer (ActivTracer, GMS) for 6 consecutive days, including weekdays and weekends. Outdoor playing time and percentage of outdoor playing time more than 60 min/day were 81±62 min/day and 64% for boys and 78±61 min/day and 62% for girls, respectively. Outdoor playing time was not significantly correlated with MVPA. The study found that no factors for MVPA, including exercise habits, were correlated with MVPA when adjusted for age, body height and log-body weight, except that MVPA for girls in kindergartens was higher than that in nursery schools. These findings suggest that the percentage of outdoor playing time more than 60 min/day was about 2/3 in young children. The questionnaire didn't reflect their MVPA. The factors used in the present study except for the preschool type for girls may not contribute to the time spent in MVPA in preschool children.
2.Efforts for Perioperative Care in Children with Congenital Heart Defects

Background : Perioperative care in congenital heart surgery has evolved in recent years, and it was considered a contributive factor to improve surgical outcome and prognosis. Objective : To extract perioperative clinical protocols that have been applied in our hospital, then assess their usefulness for better clinical outcome. Methods : We retrospectively reviewed our patients' records to analyze representative perioperative protocols that might have contributed to surgical outcome, such as intraoperative transesophageal echocardiography (ITEE), extubation in the operating room on patients of atrial septal defect (ASD), ventricular septal defect (VSD), tetralogy of Fallot (TOF), Glenn procedure and Fontan procedure. We also assessed clinical pathway of ASD and VSD, and each protocol was individually explored to calculate achievement ratio in order to show its adequacy. Results : This study included 482 of on-pump surgery patients and 146 of off-pump surgery patients from June 2007 to June 2014. ITEE was performed in 474 of on-pump surgery patients and 102 of off-pump surgery patients. No case had a residual lesion immediately after operation. Extubation in the operating room was performed in cases without severe pulmonary hypertension (PH). The extubation ratio was 94.7% (ASD repair), 60.0% (VSD repair), 50.0% (TOF repair), 42.5% (Glenn procedure), and 45.2% (Fontan procedure), respectively. Clinical pathways of ASD and VSD included patients without severe PH. Achievement ratio of the clinical pathway was 98.2% in ASD and 94.2% in VSD patients, respectively. Four patients were excluded because of high c-reactive protein (CRP), and one patient because of familial circumstance. Conclusion : ITEE was useful in evaluation of cardiac function, residual issue and residual air at weaning of cardiopulmonary bypass. Reintubation did not occur in any clinical course of extubation in the operating room, but the extubation rate was not high because of safety concerns. Achievement ratio of the clinical pathways of both ASD and VSD was more than 90%, therefore, application of the clinical pathway was considered appropriate.
3.Relationship of exercise at preschool and out of school and daily physical activity to physical fitness in preschool children in the Kanto region: a cross-sectional study

This study sought to examine the potential relationships of physical education (PE) participation at preschool and sports club (SC) participation out of school and daily physical activity (PA) measured objectively using a triaxial accelerometer, with physical fitness in preschool children. Physical fitness testing was used to measure both health-related and skill-related parameters of fitness in 191 Japanese preschoolers in the Kanto region (94 boys and 97 girls, 5.6 ± 0.6 years). Daily PA was assessed using a triaxial accelerometer (ActivTracer, GMS) for 6 consecutive days, including weekdays and weekend days. PE and SC participation was assessed using questionnaires for preschool teachers or parents of subjects, respectively. All health-related and skill-related physical fitness and total Z-score were correlated with time spent with physical activity ratio (PAR)≧4 when adjusted for age, sex, body height and weight. In addition, greater grip strength was associated with PE participation at preschool, and 25 m run speed was associated with SC participation, also. However, 25 m run speed was negatively associated with PE participation. These findings suggest that daily PA may contribute to the development of both health-related and skill-related fitness in preschool children, although further research on the cause-effect relationship is needed. Moreover, participation in a SC may contribute to the development of running speed.
4.Current Terminal Care for Cancer Patients andSignificance of Palliative Care Units

In order to understand the current status of terminal care for cancer patients and to investigate the significance of palliative care units (PCUs), we reviewed 414 cancer patients who died at our hospital during the 2-year period since October 2013 (PCU: 219 patients, general wards: 195 patients) based on their history of anticancer therapy and use of palliative care. Compared with PCU patients, those in the general wards were older, the diagnosis was delayed, and disease progression was more rapid. It was suggested that these factors had a negative impact on the opportunity to receive standard anticancer therapy and palliative care. Among the patients who received chemotherapy, the median interval from the final treatment to death was 110 days for those in the PCU while it was significantly shorter (55 days) for those in the general wards. Chemotherapy was administered within 1 month before death to 2% of patients in the PCU versus 32% of patients in the general wards, so the rate was much higher among the latter patients. In order to provide appropriate terminal care for cancer patients, the PCU seems to be important. Irrespective of the timing of cancer diagnosis and progression, it is important to increase general social awareness of palliative care and advanced care planning in order to promote the use of palliative care strategies.
5.Effects of voluntary exercise training on liver fat accumulation - Measurement of over time CT imaging -

High fat diet consumption induces fat accumulation in the liver. An understanding of when liver fat accumulation begins is important for exploring the mechanisms underlying fatty liver. The aim of this study was to investigate the processes of fat accumulation in the liver during high fat diet consumption with or without exercise using computed tomography (CT). Male 6 week old C57BL/6J mice were randomly assigned to the normal diet or high fat diet group. After 6 weeks, mice in the high-fat diet group were further divided into voluntary wheel exercise (HFD+Ex) and no exercise (HFD) groups. We measured body weight, food intake and locomotor activity in all mice. Liver fat accumulation was measured by CT scan weekly. Blood and tissue sampling was performed at the end of the experimental period. Following the 6 week exercise period, total body, mesenteric fat and liver weight in the HFD+Ex group were significantly lower than those in the HFD group. Alanine aminotransferase levels improved in HFD+Ex mice compared with those of HFD mice. The hounsfield unit value in HFD mice decreased between 3 and 8 weeks, suggesting that liver fat accumulation accelerated during this period. In contrast this decrease was not observed one week after exercise in HFD+Ex mice. These results suggest that liver fat accumulation estimated by CT was not observed until the 3rd week of high fat feeding while the effects of voluntary wheel exercise appeared immediately.
6.A case in which sodium valproate through a percutaneous endoscopic gastrostomy tube was effective for delirium in a terminal cancer patient

Purpose: We report a case in which oral sodium valproate through a percutaneous endoscopic gastrostomy (PEG) tube for palliative decompression drainage in inoperable ileus achieved an effective drug concentration and ameliorated delirium and extrapyramidal side effects. Case Report: The patient was a male in his seventies who suffered from bladder cancer. He underwent PEG because of paralytic ileus with cancerous peritonitis. He had been receiving continuous intravenous infusion of haloperidol for the management of delirium, but needed to discontinue treatment with this agent due to tremor, an extrapyramidal side effect of haloperidol. Oral sodium valproate was therefore administered through the PEG tube for palliative decompression drainage. Plasma valproate concentrations showed effective levels, and his irritability was relieved. A good combination of sodium valproate and quetiapine fumarate allowed successful reduction of and eventual withdrawal from haloperidol, and tremor then improved. Conclusion: We often encounter difficulty with symptom control for patients who cannot use oral medications. When medication by other routes is difficult, this case suggests that sodium valproate through a PEG tube for palliative decompression drainage may be useful as a last resort for improving delirium.
7.Serum albumin levels and their correlates among individuals with motor disorders at five institutions in Japan.

BACKGROUND/OBJECTIVES: The level of serum albumin is an index of nourishment care and management. However, the distribution and correlates of serum albumin levels among individuals with motor disorders have not been reported until now. Therefore, we examined the distribution and correlates of serum albumin levels among individuals with motor disorders. SUBJECTS/METHODS: A cross-sectional study on 249 individuals with motor disabilities (144 men, mean age: 51.4 years; 105 women, mean age: 51.4 years) was conducted at five institutions in Ibaraki Prefecture, Japan in 2008. The results were compared with data from the National Health and Nutrition Survey. RESULTS: The mean serum albumin levels were 4.0 ± 0.4 g/dL for men and 3.8 ± 0.5 g/dL for women. Overall, 17 (11.8%) men and 25 (23.8%) women had hypoalbuminemia (serum albumin level ≤ 3.5 g/dL); these proportions were greater than those among healthy Japanese adults (≤ 1%). Low serum albumin level was related with female sex, older age, low calf circumference, low relative daily energy intake, low hemoglobin (Hb), low blood platelet count, low high-density lipoprotein cholesterol (HDL-C), low HbA1c, and high C-reactive protein (CRP) levels. The strongest correlates, based on standardized betas, were Hb (0.321), CRP (-0.279), and HDL-C (0.279) levels. CONCLUSIONS: These results indicate that the prevalence of hypoalbuminemia is higher in individuals with motor disabilities than in healthy individuals and that inflammation is a strong negative correlate of serum albumin levels. Therefore, inflammation should be examined for the assessment of hypoalbuminemia among institutionalized individuals with motor disabilities.

8.Daily steps corresponding to the reference quantity of physical activity of Exercise and Physical Activity Reference for Health Promotion 2006 (EPAR2006) assessed by accelerometer

The purpose of this study was to determine daily steps corresponding to the reference value for the quantity of Physical Activity and Exercise for Health Promotion 2006 (23 METs·h/wk) considering non-locomotive activities. Two hundred and thirty one men and 224 women wore a tri-axial accelerometer for two weeks. We analyzed the data in each age group (young (less than 40 years), middle-aged (40 to 59 years), and elderly (60 years or more) groups), also. There were significant relationships between daily steps and locomotive activity (r = 0.762 to 0.820, p < 0.001) and total (locomotive and non-locomotive) physical activity (r = 0.706 to 0.824, p < 0.001) with intensity of 3 METs or more in all groups. The daily steps corresponding to 23 METs·h/wk, calculated using regression lines between the daily steps and total physical activities with intensity of 3 METs or more in men and women were 6,534 steps/d and 6,119 steps/d. On the other hand, the daily steps corresponding to 23 METs·h/wk, calculated using regression lines between the daily steps and locomotive activities with intensity of 3 METs or more in men and women were 7,888 steps/d and 8,584 steps/d. These results suggest that non-locomotive activity should also be taken into consideration in the case of assessment of a daily physical activity.
9.Effects of a physical activity support program based on bench-stepping exercise on physical fitness, mental health and health-related quality of life in Japanese returnees from China

This study aimed to examine the effects of an 8-week physical activity program, which mainly comprised home-based bench-stepping exercise training at the intensity of lactate threshold (LT), on mental health (MH), health-related quality of life (HRQOL), and physical fitness in Japanese returnees from China. Thirty Japanese returnees (63 ± 9 y) participated in the exercise program. Another six subjects were enrolled as the control group. The subjects performed 212 ± 57 min of training, and their daily step counts were increased. Aerobic capacity (LT: 4.5 ± 0.8 vs. 5.5 ± 1.1 METs), lower limb strength (30-s chair stand test [CS-30]: 19.1 ± 5.5 vs. 21.3 ± 5.1 times), and sit-and-reach flexibility (sitting-posture body anteflexion: 36.1 ± 9.4 vs. 39.0 ± 8.4 cm) were significantly increased after the intervention compared with before the intervention. Furthermore, MH, as assessed by the total score of the GHQ-28 (3.4 ± 4.4 vs. 0.3 ± 0.8 points), and the mental component score (MCS) of HRQOL, as evaluated by the SF-36v2 (55.1 ± 11.4 vs. 58.5 ± 10.0), were significantly changed in a positive manner. However, a two-way repeated measures ANOVA (group × period) showed significant interactions for LT and MCS (p<0.05), and a tendency for interactions of CS-30 (p=0.063) and the total score of the GHQ-28 (p=0.098). These results indicate that this bench-stepping exercise program could become a useful health support program for improving physical fitness, as well as MH and HRQOL, in Japanese returnees.
10.Identification of Acupuncture Indications and Evidence-based Medicine

To identify indications for acupuncture treatment, we examined certain evidence-based medicine (EBM) practices by acupuncture/moxibustion clinicians who are novices in this field and identified problems and solutions from the perspective of an acupuncture/moxibustion clinician.
We collected cases of lumbago, shoulder disorders and strokes from the literature, and critically assessed these reports. We also investigated Cochrane Library's acupuncture/moxibustion reviews and evaluated the quality of domestic studies using randomized controlled trials and controlled clinical trials (RCT/CCT). Simultaneously, one of the authors evaluated her own clinical reports in light of EBM.
As a result, acupuncture/moxibustion treatments showed promise for treating lumbago and shoulder disorders, but the higher the quality of studies, the lower the effect of treatment after stroke. According to research by the Cochrane Library (as of issue 1, 2003), the effects of acupuncture/moxibustion was measured only for cases of idiopathic headache but those for other disorders were measured more severely. In Japan, RCT started as early as the 1960 s, but the studies were sluggish and high quality studies were not produced until the late 1990 s.
We presented problems of RCT assessment score, the gap between RCT designs and actual clinical scenes, and the difficulty of masking at the bed-side. To contribute to the accumulation of more evidence-based data, it would be desirable for clinicians to acquire EBM methods, consider clinical problems and collaborate positively with investigators in the field.
