3.The clinical features of invasive pulmonary aspergillosis in 53 chronic obstructive pulmonary disease patients

Objective To study the clinical features of invasive pulmonary aspergillosis (IPA) with chronic obstructive pulmonary diseases (COPD),so as to provide evidence for early diagnosis and treatment.Methods A retrospective analysis was made upon clinical data,diagnosis,treatment and prognosis of 53 patients with IPA and COPD admitted between January 2005 and February 2011 collected in a respiratory unit of the Second Xiangya Hospital Affiliated Central South University.Results There were 53 cases of diagnosed as IPA with COPD,with history of using broad-spectrum antibiotics.And there were 43 cases using steroids more than 2 weeks,51 with obvious breathlessness,and 20 with fever.Early stage didn't present characteristical changes on CT scan.However,after disease progression,32 cases had maculas shadows and nonspecific consolidations in bilateral lung,14 with solitary or multiple nodules,4 with solitary or multiple air crescent sign,and 2 with halo sign.Four patients of COPD with IPA underwent bronchoscopy examination.In fungi pathogeny,sputum positive rate and galactomannan positive rate were 56.6% and 52.8%,respectively.A total of 53 cases received antifungal treatment.Among 37 cases which underwent mechanical ventilation,24 received noninvasive ventilation and 13 received invasive ventilation.There were 33 cases which were improved and cured,and 20 cases which had no relief after half-a-month treatment or withdrew treatment.Among them,13 cases died because of multiple-organ failure (5/15) or acute renal failure (8/15).Conclusions Early suspected diagnosis,timely examination in allusion to IPA,actively searching for etiological and imaging evidences,early established diagnosis and antifungal treatment would improve prognosis of patients with COPD combined IPA who have history of high doses of cortieosteroids,obvious breathlessness and non-response to antibiotics.

5.Application of the Teratogenic Effects of Embryo Development in Zebrafish to the Determination of Toxicity of Environmental Pollutants

The embryo development technique in Zebrafish, Brachydanio rerio, is a toxicity testing method making use of the high sensitivity of fish embryo development in early stage to study and evaluate the specific effecting mechanism, the most sensitive effecting time, embryo toxicity and teratogenicity of chemicals through observing the development process of zebrafish embryo after chemical exposure to fertilized ova. This technique has been widely used to test toxicity of chemicals with the advantages of low cost, high sensitivity, simple to operate and simultaneous to detect multi-endpoints. The main methodology, technical characteristics and the status of world-wide application of this technique are reviewed in this paper. Based on the urgent environmental problems in China, the prospects to use this method for monitoring toxicity of mixed pollutants in wastewater are put forward.
6.Research on allogeneic mouse spleen T lymphocyte apoptosis induced by FasL transfected dendritic cells

Objective To establish the mouse bone marrow-derived dendritic cells expressing FasL protein and explore the mechanism of inducing allogeneic mouse spleen T lymphocyte apoptosis. MethodsMouse myeloid DCs were cultured in selective medium zontaining essential cytokines for DC growth in vitro. The mouse DCs were transfected with liposome-mediated FasL gene. The levels of FasL mRNA before and after transfection were assayed by real-time quantitative PCR. The expression levels of FasL protein were assayed by flow cytometry(FCM)and Western blot. Non-transfected DC,empty plasmid transfected DC and FasL transfected DC were infused intravenouslY into allogeneic mouse. After 7 days, the apoptosis in spleen T lymphocytes was evaluated by TUNEL (terminal deoxynucleotidyl transferase-mediated dUTP nick-end-labeling)method and FCM. ResultsCultured in vitro, the mature myeloid DCs from mouse could be obtained.The expressions of FasL mRNA and protein in FasL transfected DCs were significantly higher. Through the detection of spleen T lymphocyte apoptosis with TUNEL,the apoptosis index(AI)was higher in FasL transfected DC(11.67±1.53),compared with non-transfected DC(2.67±0.58)and empty plasmid transfected DC(3.33±0.58),P<0.01. ConclusionA large quantity of myeloid DCs can be obtained through in vitro culture in selective medium. The liposome-mediated FasL gene transfected DCs could successfully express high levels of FasL protein. Intravenous infusion of FasL gene transfected DCs could induce apoptosis of allogeneic mouse spleen T lymphocytes.
7.Study on osteogenic/chondrogenic differentiation potential of cartilage endplate derived stem cells in vivo

Objective To observe the osteogenic and/or chondrogenic differentiation potential of cartilage endplate derived stem cells ( CESCs) .Methods The cartilage endplate ( CEP) was obtained from 5 patients who underwent posterior discectomy procedure for the lumber degenerative disease.The agarose culture was used to select CESCs.The expanded CESCs were injected into the hydroxyapatite ceram-ic(HA) and incubated for 7 days.After that,the HA contained CESCs were implanted into the subcutaneous of nude mouse for 7 weeks.Then the HA was obtained and sliced for the specific stain and immunohistochemisty.The expanded CESCs were injected into the degenerated in-tervertebral disk of the rabbit for 16 weeks.Then MRI was used to detect the repair of the degenerated disc.Results The metachromasia of toluidin blue indicated that cartilage matrix was formed in the vacant space of HA.Mallory stain indicated that there was bone matrix was formed.The results of immunohistochemistry indicated that there was more collageⅠand collageⅩ,and a small quantity of collage Ⅱ.MRI results indicated that the CESCs could be induced into the chondrocyte-like cells in the rabbit intervertebral disk.Conclusion The CESCs could be induced into the bone/cartilage in vivo.
8.Bacteriological Comparison Between Shaving and No-Shaving Before Knee Arthroscopy Surgery

OBJECTIVE To study the feasibility of no-hair-shaving before knee arthroscopy surgery.METHODS Totally 160 patients preparing for knee arthroscopy surgery were divided into both test and control groups randomly.The test group was no-hair-shaving before operation,and the control group was hair-shaving the day before their operation.Patients of the test group washed their skin with soap water or bathed at the operation day and patients in control group did this the day before operation.Bacterial culture was done in the operative site both before and after sterilization,and observed whether infection occurred in the incision after operation.RESULTS Bacterial culture in skin before sterilization was statistical significance between the two groups and no statistical significance after sterilization.All the incision healed well and no infection was found.CONCLUSIONS No-hair-shaving preoperatively can decrease patients′ discomfort and reduce colonies on skin surface effectively.The method is easy and safety manipulated.

Objective To provide clinical experience in treatment of cook′s fungus infection of evidence‐based medical evidence , separation of medical environment of cook′s bacteria identification and antimicrobial susceptibility test .Methods Collected in the hospital infection of hygiene monitoring ,collection and medical staff hands ,skin and mucous membrane ,air and object surface sam‐ples from 86 strains of cook ,application ATB Expression semi‐automatic bacteria identification of susceptibility analyzer supporting article try ID32 Staph identification and drug susceptibility test Staph 5 ,strain identification and drug sensitive test .Results 86 strains bacteria identification cook 62 strains bacteria ,mutation detection rose cook bacteria strains of 16 ,cook eight strains of bac‐teria .Cook bacterium of norfloxacin(16 .3% ) ,and with nitrofurantoin low because of the sensitive rate(20 .9% ) ,of erythromycin (69 .3% ) and gentamycin(67 .4% ) ,the sensitive rate at nearly 70 .0% ,sensitivity to penicillin and other 12 kinds of antimicrobial a‐gents more than 80 .0% ,even 100 .0% .Conclusion Cook for most still sensitive ,clinicians and microorganism inspection personnel should strengthen the cook .
10.Effect of Robot-Assisted Training on Lower Extremities of Patients with Stroke: A Systematic Review of Randomized Controlled Trials

Objective To evaluate the effectiveness of robot-assisted training on lower extremities of patients with stroke. Methods PubMed, Medline, EMbase, Cochrane Central Register of Controlled Trials (CENTRAL), ISI Web of Knowledge, Ovid, China National Knowledge Infrastructure (CNKI), Wanfang Data and VIP Database for Chinese Technical Periodicals were searched for the randomized controlled trials (RCTs) about robot-assisted training for lower extremities of patients with stroke from the date of establishment to March 2013. The bibliographies of the retrieved studies were also searched. 2 independent researchers evaluated the included studies using the risk of bias provided by Cochrane Library. The extracted data were analyzed by RevMan 5.1 software. Results A total of 12 trials were discovered.Compared with conventional training, the robot-assisted training increased the score of Fugl-Meyer assessment (lower extremity)(WED=3.97, 95%CI: 2.84~5.10, Z=6.89, P<0.00001) and Berg Balance Scale (WED=3.13, 95%CI: 0.80~5.45, Z=2.63, P=0.008). However,the results of stride frequency (WED=8.40, 95%CI: -0.95~17.75, Z=1.76, P=0.08) and speed (WED=0.03, 95%CI: -0.02~0.07, Z=1.18, P=0.24) were not statistically significant. According to the Jadad Score of included studies, 4 of them were of low quality and 8 of them were of high quality. Conclusion The current evidence shows that to some extent, robot-assisted training could improve the lower extremities function.
