1.The Effect of Electrothermal Needle Acupuncture on Oxidative Stress Levels in Knee Osteoarthritis Patients

Objective To compare the efficacies of electrothermal needle acupuncture versus nimesulide in treating knee osteoarthritis and the relieving effects of them on the pain and explore the possible mechanism of electrothermal needle treatment for knee osteoarthritis.Method Eighty patients with knee osteoarthritis were randomly allocated to electrothermal needle and Western medicine groups, 40 cases each.The electrothermal needle group received electrothermal needle therapy and the Western medicine group took nimesulide sustained-release tablets.The therapeutic effects were compared after thecompletionof treatment.Therelieving effects of the two treatments on the pain were evaluated using the Pain Visual Analogue Scale(VAS).Serum SOD and MDA were measured in all the subjects before and after treatment.Result The marked efficacy rate and the total efficacy rate for knee osteoarthritis were higher in the electrothermal needle group than in the Western medicine group.There was no significant post-treatment difference in the VAS pain score between the electrothermal needle and Western medicine groups.After treatment, serum SOD increased and MDA decreased in the electrothermal needle group and they did not change significantly in the Western medicine group.Conclusion Electrothermal needle therapy can well relieve the symptoms of knee osteoarthritis.Its relieving effect on the pain is equal to that of nimesulide.The mechanism of electrothermal needle treatment for knee osteoarthritis may be relatedto its improving bodilyoxidative stress levels.
2.Effect of early off-bed to arthroscopically assisted radiofrequency release of gluteal muscle contracture

[Objective]To evaluate the feasibility of early off-bed exercises after arthroscopically assisted radiofrequency release of gluteal muscle contracture.[Method]Patients of gluteal muscle contracture underwent arthroscopically assisted radiofrequency release of gluteal muscle contracture.Post-operative off-bed time,side effects,duration of urinary catheter drainage,wound healing,early recovery of function,and in-hospital time were studied.[Result]Off-bed time averaged 7 hours(6~8 hours).Two cases complained of being faint and nausea.After allopathic treatment,no serious complications appeared.Averaged duration of urinary catheter drainage was 7.5 hours.Primary healing of wounds were found in 67 patients,while 1 case got delayed wound healing.None of them had any infections.Functional scores averaged 9 at the time of discharging and 10 after 1 month.Post-operative in-hospital time averaged 2.58 days(2~6 days).[Conclusion]Arthroscopically assisted radiofrequency release of gluteal muscle contracture results in early post-operative off-bed time and reduction of suffering,which is propitious to functional recovery,rate of healing,and shorter in-hospital time.
3.A Randomized Controlled Trial of Ozone Injection plus Electrothermal Acupuncture for Treatment of Knee Osteoarthritis

Objective To compare the efficacy of ozone injection plus electrothermal acupuncture versus oral glucosamine and nimesulide in treating knee osteoarthritis and investigate the effects of the two treatments on the WOMAC total score and the pain, stiffness and physical function scores.Method Eighty patients with knee osteoarthritis were randomly allocated to combination and Western medicine groups, 40 cases each. The combination group received a combined treatment with intra-articular ozone injection and electrothermal acupuncture and the Western medicine group took glucosamine hydrochloride and nimesulide sustained-release tablets. The WOMAC score was recorded in the patients before and after treatment. The therapeutic effects were evaluated using the WOMAC score. Result The control and marked efficacy rate of ozone injection plus electrothermal acupuncture for knee osteoarthritis was higher than that of Western medicine. The WOMAC score decreased in both groups after treatment (P<0.05) but decreased more in the combination group than in the Western medicine group (P<0.05). There were no significant post-treatment differences in the pain and stiffness scores between the combination and Western medicine groups (P>0.05). The physicalfunction score decreased more in the combination group than in the Western medicine group (P<0.05).Conclusion Ozone injection plus electrothermal acupuncture is more effective than Western medicine in treating knee osteoarthritis. It is more effective in improving knee function but not in relieving the pain and joint stiffness in comparison with Western medicine.
4.Effect of different analgesia on pain relief during labor

Objective To evaluate the effect of spinal-epidural and epidural anesthesia for pain relief in labor. Methods Totally 6671 cases selected from pregnant women delivered from Aug. 2001 to Oct. 2004 in our hospital were reviewed retrospectively. All cases were divided into three groups,1482 cases in spinal-epidural group (combined epidural) and 1111 in epidural group (epidural) who received pain relief during labor; 4078 as control group without any pain relief during labor. Delivery method and maternal, fetal complications among three groups were compared. Results (1) Delivery methods were significantly different (P0.05) in postpartum hemorrhage and neonatal asphyxia between spinal epidural 4.3%(63/1482) 1.0%(15/1482), epidural 4.1%(45/1111),0.8%(9/1111), and control 3.9%(159/4078), 1.4%(56/4078). Conclusions Anesthetic pain relief in labor may reduce the CS rate, but increase the rate of forceps delivery. Pain relief is associated with arrested and prolonged active phase, prolonged second stage. However, pain relief in labor doesnot enhance the rate of postpartum hemorrhage and neonatal asphyxia.
5.Improve reconstructed metacarpophalangeal joint function with second toe transplantation after curved osteotomy under the metatarsal head cartilage

ObjectiveTo investigate the technique and clinical results of curved osteotomy under the metatarsal head's cartilage for improving reconstructed metacarpophalangeal joint function of fingers with second toe transplantation. MethodsThere were total 21 cases with 21 digits. During second toe transplantation with the metatarsophalangeal joint, the bottom of the second metatarsal head was incised. Then a curved osteotomy were carried out on about 5.0 mm under the metatarsal head's cartilage until the passive range of motion could be 90°.Longitudinal or cross-Kirschner wires were used to fix the joint.Finally,the conventional methods were used to reconstruct the blood supply, movement and nerves. ResultsAll 21 digits of the 21 cases survived uneventfully.The follow-up time was ranged from 6 to 24 months.Their average passive range of motion of the reconstructed metacarpophalangeal joint was 75°,ranging from 65° to 85°.The average active range of motion was 65°, ranging from 45° to 80°. Postoperative X-ray revealed fracture healed well without joint degeneration. ConclusionCurved osteotomy under the metatarsal head's cartilage is an effective way to improve active and passive activities function of the reconstructed metacarpophalangeal joint with second toe transplantation.
6.Replantation methods of mini tissue mass of amputated finger

Objective To investigate the replantation methods and outcomes of mini tissue mass of amputated finger. Methods Twenty-six fingers of 20 patients were replanted with multiple vessel anastomosis methods to restore blood supply.The methods include vascular anastomosis,vascular bridge,arteriovenolization,veno-arteriolization,et al. Results Twenty-five replanted tissues were survived completely.Partly necrosis occur in 1 case,and rehabilitation by change dressings.The patients were follow up from 6 to 12 months.The contour and function of the replantation fingers recovered satisfactory. Conclusion By using the rational anastomosis according to the traumatic condition,it could be obtain good outcome of mini tissue mass replantation

Objective To investigate the multi factor for type 2 diabetes with hyperuricemia and metabolic syndrome for preven‐tion cardiovascular disease .Methods 672 cases diagnosed type 2 diabetes were selected in Beijing Anzhen and Cuigezhuang com‐munities to measure height ,weight ,neck circumference ,waist ,BMI ,blood pressure ,fasting plasma glucose ,HbA1c and lipid pro‐file .Results The prevalence of HUA in 672 cases is 8 .04% .It was significantly lower than coastal area population in Shandong Province .BMI ,neck circumference ,waist ,DBP ,FBG ,2 hPG ,HbA1c ,GLT ,BUN ,blood uric were significantly higher than NUA group .The prevalence of MS and fatty in HUA group is 75 .93% ,77 .78% .While in NUA group is 58 .9% ,56 .8% respectively P=0 .014 ,0 .003 .Logistic regression analysis showed that BMI ,HbA1c ,ALT ,BUN were associated with hyperuricemia in T2DM . Conclusion The prevalence of HUA in Beijing diabetic patients was lower than coastal area population ,but complicated MS and cardiovascular risk factor were high and seriously .
8.B7-H1 and PD-1 expressions on peripheral leukocytes from patients with Henoch-Sch(o)nlein purpura

Objective To investigate the expressions of B7-H1 and PD-1 on peripheral leukocytes from patients with Henoch-Schtmlein purpura(HSP)and their significance.Methods Peripheral leukocytes were obtained from 36 patients with HSP(HSP group)and 24 healthy human controls(control group).Immunofluorescent staining and flow cytometry were used to measure the expressions of B7-H1 and PD-1 on peripheral leukocytes.The expression of both two molecules was compared between the HSP group and control group as well as between patients with Henoch-Sch(o)nlein purpura nephritis(HSPN)(n=9)and those without(n=27).SPSS 12.0 for Windows was used for data analysis.Results The expression rate of B7-H1 on monocytes significantly increased(24.43%±25.79%vs 7.69%±8.31%,t=3.62,P<0.011),while that of PD-1 decreased(0.84%±1.96%vs 2.28%±1.95%,t=2.78,P<0.01)in HSP group compared with those in the control group.No significant difference was revealed in the expression of B7-H1 or PD-1 on lymphocytes between HSP group and control group(both P>0.05).There was a significant increase in the expression of B7-H1 on monocytes(44.81%±12.36%vs 17.63%±25.63%,t=3.05,P<0.01)and lymphocytes(8.78%±2.10%vs 5.65%±3.96%,t=2.25,P<0.05)in patients with HSPN compared with those without.Conclusion There is a high expression of B7-H1.but low expression of PD-1 on peripheral blood monocytes from patients with HSP.suggesting that B7-H1 and PD-1 may play a certain role in the Dathogenesis of HSP.
9.Effect of family support nursing on self-perception and outcome events of elderly patients with chronic diseases

Objective:To explore the effect of family supportive nursing on self-perception and outcome events in elderly patients with chronic diseases.Methods:A simple random sampling method was used to select 115 elderly patients with chronic diseases discharged from Henan Provincial People's Hospital from January 2019 to January 2020. They were divided into the observation group (58 cases) and the control group (57 cases) by random number table method. The control group received routine nursing, while the observation group received family support nursing. The self-perception and family function scores of the two groups were compared before the intervention and after 3 and 6 months of the intervention. The medication compliance and readmission rate were compared between the two groups.Results:After 3 months of intervention, scores of Self-Perceived Burden (SPBS) of the observation group and the control group were respectively (25.73±3.85) and (28.14±3.90) and scores of Family Adaptability Cohesion Environment Scale (FACES) were respectively (105.62±12.41) and (96.35±10.05) . After 6 months of intervention, the SPBS scores of the observation group and the control group were respectively (21.89±4.33) and (25.41±4.20) , and the FACES scores were (122.60±14.70) and (112.35±16.32) respectively. Repeated-measures analysis of variance showed that there were statistically significant differences in interaction effect, time effect and between-group effect in the comparison of SPBS and FACES total scores between the two groups (all P<0.01) . After 6 months of intervention, the rate of good medication compliance in the observation group and the control group was respectively 70.69% (41/58) and 50.88% (29/57) , and that of the observation group was higher than the control group, and the difference was statistically significant (χ 2=4.738, P=0.030) . After one-year follow-up after intervention, the readmission rates of the observation group and the control group were respectively 18.87% (10/53) and 39.22% (20/51) , and the observation group was lower than the control group, and the difference was statistically significant (χ 2=5.243, P=0.022) . Conclusions:Family support nursing can reduce the self-perceived burden of elderly patients with chronic diseases, improve their family function, improve medication compliance and reduce readmission.
10.A preliminary clinical report on bridging digital nerve defect with human acellular nerve graft

Objective To evaluate the safety and efficacy of the human acellular nerve allograft (hANG)for nerve repair in the clinical setting,and report the early outcomes of bridging digital nerve defect with the hANG. Methods Four patients with 5 digital nerve injuries were included in this pilot study.The nerves defect ranged from 10-20 mm and were bridged with the hANG(manufactured by Zhongda Medical Equipment Co.,Ltd,Guangzhou,China).Four digital nerve acute injuries in 3 patients were repaired with hANG primarily,while the nerve in another patient was reconstructed secondarily.The procedure was performed under a 10-manifying operating microscope.The nerve stumps were debrided until the normal fascicles could be seen.hANG was inserted between the proximal and distal stumps and end-to-end neurorrhaphy was performed with 9-0 sutures.Postoperative cares included dressing change and administration of antibiotics.No immunosuppressants had been used.The follow-up time ranged from 1 to 3 months.The wound and blood sample were examined for the safety of hANG.The nerve function Wag evaluated according to the scoring system proposed by the Nerve Injuries Committee of the British Medical Research Council. Results All wounds healed primarily.The adverse effects,such as rejection,allergy,infection,and toxicity to the liver and kidney were absent.The results of blood biochemistry test were within the normal range.The injured nerve achieved good functional recovery.In 2 cages,the 2 point discrimination(2PD)was 8mm(S3~+,excellent). Conclusion Based on the short term follow-up,using hANG to repair digital nerve defect as long as 20mm was safe,and the nerve functional recovery is pretty good.
