1.Detection of p53 Gene Mutation in Endemic Arsenism Patients by PCR-SSCP

Objective To study whether arsenic can induce p53 gene mutation in the lymphocytes in the peripheral blood and the skin cancer tissue of the patients with endemic arsenism in Xinjiang. Methods PCR-SSCP silver staining technique was employed to detect the mutation of exon E5-9 of p53 gene. Results Gel electrophoresis imaging demonstrated Exon E6, 7 of p53 gene appeared abnormal strips, strip absence and strip shift. Conclusion Arsenic can induce p53 gene mutation, the mutation sites located on exon E6,7.
2.Cloning and bioinformatics analysis of gene AsTP2 transactivated by arsenic trioxide with suppression subtractive hybridization

Objective To study cloning and the primary function of a new gene AsTP2 transactivated by arsenic trioxide. Methods Using suppression subtractive hybridization (SSH) technique, the mRNA was isolated from HepG2 cells treated with arsenic trioxide (5?mol/L) and 0.9 percent sodium chloride, respectively, then cDNA was synthesized. SSH method was employed to analyze the differentially expressed DNA sequences between the two groups. From the subtractive cDNA library of genes transactivated by arsenic trioxide, the coding sequence of a new gene was obtained by bioinformatics method, and amplified by the method of reverse transcription polymerase chain reaction (RT-PCR). Results The novel gene was named as AsTP2, which was logged in the GenBank with the accession number AY744366. AsTP2 of 1119 nucleotides (nt), coding a protein of 372 amino acid residues (aa). Conclusion A new gene has been recognized as the new target transactivated by arsenic trioxide. The results will give a new clue to explore the molecular carcinogenic mechanism of inorganic arsenic.
3.Comparison of HC visual laryngoscopy and fiberoptic bronchoscope guided endotracheal intubation in patients undergoing cervical surgery

Objective To compare the clinical effects of HC video laryngoscope and fiberoptic bronchoscope (FOB)in guidance of endotracheal intubation for patients undergoing cervical surgery.Meth-ods A total of 50 patients (ASA I or II)with cervical vertebra injury,nerve root cervical spondylopathy or cervical spondylotic myelopathy in the first affiliated hospital of Wenzhou medical university were selected, all of whom were undergone selective cervical operations between March 2014 and June 2015.The patients were randomly divided into two groups (n =25):HC video laryngoscope group (group H)and FOB group (group F).After induction of anesthesia,HC video laryngoscope and FOB were used for tracheal intuba-tion.⑴ Intubation time and success rates of intubation were recorded,and visual analogue scale (VAS) was used to evaluate the difficulty of intubation;⑵ Mean arterial pressure (MAP),heart rate (HR),and rate-pressure product (RPP)before incubation (Tb),immediate after intubation (T0),and 1min after in-cubation (T1)were recorded;(3)Intubation-related complications were recorded.Results ⑴ The dura-tion of intubation (19.7 ±7.1 )s in the group H was significantly shorter than that of group F (51.9 ± 19.2)s (P <0.05).The one-time success rate of intubation in group H (92%)was higher than that of group F (64%),with statistically significant differences (P <0.05).Intubation was easier in group H (P <0.05).⑵ MAP,HR and RPP of group H at T0 were higher than those of group F.And at T1,there were no statistical differences between groups in MAP,HR and RPP (P >0.05).⑶ The incidence of complications (sore throat)was lower in group H (P <0.05).Conclusions Compared to FOB,HC vid-eo laryngoscope-guided selective cervical operation has the characteristics of higher success rate of intubat-ion,shorter operation time,and easier operation,more stable hemodynamics and lower incidence of intuba-tion complications.Therefore,HC video laryngoscope is a safe and effective method in the intubation for pa-tients undergoing cervical surgery.
4.Study on the expression and mechanism of monocyte chemoattractant protein-1 in interstitial cystitis rats

Objective To observe the expression and mechanism of monocyte chemoattractant protein-1 (MCP-1) in interstitial cystitis (IC) rats.Methods Twenty weight of 250-300 g of female SD rats were divided into IC group (n =10) and control group (n =10).IC group were treated by transurethral instillation with 10 mg/ml protamine sulfate (PS) 1 ml reserved for 45 min,and then instillation with 750 ug/ml of lipopolysaccharide (LPS) 1 ml reserved for 30 min.The same operations were repeated after 24 hours,and the rats were killed obtaining the bladder tissue and urine after three days.Control group was given PBS solution perfusion.MCP-1 and histamine (HA) expression levels in the rat bladder tissue and urine were detected by ELISA.The inflammation of bladder tissue was observed and inflammatory score was used by HE staining.MC degranulation count was used by MC special staining.MCP-1 expression and distribution in bladder tissue was observed by immunohistochemical method.The relationship between the MCP-1 and MC was detected by immunofluorescence method.Results By ELISA,the expression levels of MCP-1 and HA in the bladder tissue and urine in IC group were significantly increased compared with control group (P <0.01 ).More inflammatory cell infiltration in the bladder mucosa,edema mucosa,congestion and hemorrhage were seen by HE staining.The inflammatory score in IC and control group were (76.5 ±9.8) and (18.5± 9.8)/field (P < 0.01 ).With MC special staining,degranulation MC count in IC and control group were (6.4±3.1 ) and (0.7 ±0.3)/field (P <0.01 ),and the degranulation in the bladder tissue of IC group was significantly higher than control group (P < 0.01 ).MCP-1 has a higher expression in the bladder epithelium,and more MCP-1 were found gathering round MC surface by immunofluorescence.Conclusions MCP-1 is highly expressed in IC rats,and could induced activation of MC,which could release HA,aggravating the pathological process of inflammatory and fibrosis in IC.
5.Etiological analysis and treatment of jaundice after liver transplantation during mid-later stage

ObjectiveTo analyze the etiologies and diagnosis of jaundice after liver transplantation during mid later stage and summarize the experience of clinical practice.Methods822 liver transplantation patients were collected from the Department of Internal Medicine, Organ Transplantation Institute in General Hospital of Chinese People's Armed Police Forces between June 2005 and Dec.2008.The clinical data of these cases were analyzed retrospectively.ResultsAmongst 822 patients,129 experienced jaundice after liver transplantation and the occurrence of jaundice ranged from 43 days to 39 months postoperation.Total bilirubin levels were from 27.4 to 503 μmol/L.The etiologies of jaundice and its percentage were as follow:59 cases of biliary complications (45.7%),36 of rejection (27.8% ),11 cases of virus infection (8.5%),5 cases of drug-induced hepatic injury (3.9%),4 cases of tumor recurrence (3.1%),4 cases of Gilbert syndrome (3.1%),3.cases of dysfunction of papillary muscle (2.3%),2 cases of vascellum complication (1.6%),2 cases of radiation hepatitis (1.6%),1 case of hepatapostema (0.8%),2 cases of unknown reasons (1.6%),respectively.The jaundice of most patients (93%) got released while small part of patients (7%)failed to treatment,of whom 5 cases received liver re-transplantation and 4 cases died of disease progression.ConclusionThe etiologies of jaundice after liver transplantation during mid-later stage are diversiform and complex.And the etiological diagnosis is the premise to treat effectively.
6.Liver transplantation for treating idiopathic adulthood ductopenia One case report and literature review

To summarize and explore the etiopathogenisis, clinical 'characteristics, diagnosis and treatment and prognosis of idiopathic adulthood ductopenia. We described a 27-year-old Chinese female initially manifested with jaundice, pruritus, dark urine, and pallor of the stools in October, 2002. Symptoms occurred repetitively and severed progressively, and she received therapy at the General Hospital of Chinese People's Armed Police Forces. B-ultrasound demonstrated that hepatic lesion and slightly swelling spleen. Pathological diagnosis of liver biopsy showed that she affected idiopathic adulthood ductopenia. The outcome of treatment of ursodeoxycholic acid and adrenal cortical hormone was not significant. She received orthotopic liver transplantation on June 24~(th), 2005. After liver transplantation, pruritus and jaundice were promptly disappeared. Each index of hepatic function recovered to a normal level. In March 2007, decreased dose of hormone induced an abnormal hepatic function, with mildly acute rejection. Drug dose was regulated, and hepatic function gradually returned to normal. During follow-up 51 months after the operation, she was doing well and was free of symptoms with normal liver function and no evidence of allograft dysfunction. The pathogenesis of idiopathic adulthood ductopenia is still unknown. There are still no precise and effective drugs for treatment of idiopathic adulthood ductopenia. Liver transplantation offers an effective therapy for end-stage patients with idiopathic adulthood ductopenia.
7.Comparison of efficacies of hepatectomy and liver transplantation for patients with hepatocellular carcinoma fulfilling the Milan criteria

Objective To compare the efficacies of hepatectomy and liver transplantation for patients with hepatocellular carcinoma (HCC) fulfilling the Milan criteria.Methods From July 2002 to February 2009,121 patients with HCC combined with hepatic cirrhosis fulfilling the Milan criteria were admitted to the Fuzhou General Hospital.Eighty-nine patients who received hepatectomy were in the hepatectomy group,and 32 patients who received liver transplantation were in the liver transplantation group.There were no significant difference in the age,gender,etiology of liver disease,the size of the largest tumor,number of tumors,microscopic venous invasion,microsatellite lesion and tumor differentiation between the 2 groups.The clinical data of the patients in the 2 groups were retrospectively analyzed.The overall survival and disease-free survival were evaluated by Kaplan-Meier method,and differences in survival rates between the 2 groups were determined by Log-rank test.COX proportional hazard was used for univariate and multivariate analysis to evaluate the risk factors for prognosis.Results The median period of follow-up was 37 months.The 1-,3-,5-year survival rates were 86%,63% and 44% in the hepatectomy group,and 87%,70% and 62% in the liver transplantation group.There was no significant difference in the overall survival rate between the 2 groups (x2 =1.092,P > 0.05).The 1-,3-,5-year disease-free survival rates were 68%,44% and 26% in the hepatectomy group,and 80%,65% and 52% in the liver transplantation group.There was a significant difference in the disease-free survival rate between the 2 groups (x2 =4.712,P < 0.05).The result of univariate analysis revealed that microscopic venous invasion and microsatellite lesion were significantly correlated with the survival (Wald =9.625,7.340,P < 0.05),and the result of multivariate analysis indicated that microscopic venous invasion was the independent risk factor influencing the survival (Wald =5.008,P < 0.05).Conclusions As for patients with HCC fulfilling the Milan criteria,the overall survival rate of patients who received hepatectomy is not different from those who received liver transplantation,but the disease-free survival rate of patients who received liver transplantation is higher than those who received hepatectomy.Microscopic venous invasion is an independent risk factor influencing the survival.

9.Recognition of abdominal compartment syndrome among pediatric healthcare providers

Objective To perform the sample survey on the recognition degree of abdominal compartment syndrome (ACS) among domestic pediatric healthcare providers (PHCP) Methods Three hundred self‐designed questionnaires were distributed to the participants at the twelfth Chinese Medical Association Congress of Pediatric Critical Care Medicine in November 2011 .Results A total of 194 effective questionnaires were reclaimed with the recovery rate of 64 .7% .28 .9% (56/194)of respondents did not heard of ACS .49 .5% (96/194)of them heard of ACS ,but did not contact ACS .Only 21 .6% (42/194)of respondents were well fa‐miliar with ACS .Among the medical staffs who were aware of ACS (familiar or just heard of ) ,only 7 .2% (10/138)knew the real definition of ACS .83 .3% (35/42) of respondents who were familiar with ACS used the intravesical route to measure the intra -ab‐dominal pressure(IAP) .However ,only 57 .1% (20/35)of respondents knew the correct saline volume for measuring IAP .Conclusion The recognition degree of ACS is low among domestic PHCP .It is necessary to strengthen the ACS related education among do‐mestic PHCP for increasing the awareness of ACS and promoting its treatment .
10.NLRP3 inflammasome and eye diseases

Nucleotide binding oligomerization domain-like receptor family pyrin domain containing 3 (NLRP3) inflammasome is a set of intracellular protein complexes.NLRP3 inflammasome can trigger the activation of caspase-1 and contribute to the maturation of interleukin, and finally result in a series of inflammation responses in host.Inflammasomes are activated in a variety of cases, including multiple inflammation-related eye diseases, such as xerophthalmia, glaucoma, age-related macular degeneration and so on.This article aimed to interpret the effect and mechanism of the activation of NLRP3 inflammasome, and reviewed how the mutation of gene or the structure disturbance of protein related to NLRP3 inflammasome affects the occurrence and development of inflammation-related eye diseases.
