1.Bioequivalence of Fluconazole in Health Volunteers

OBJECTIVE:To study the bioequivalence of fluconazole in healthy volunteers.METHODS:By a randomized double crossover design,a total of 20 healthy male volunteers were assigned to receive oral single dose of test fluconazole tablets (300 mg) and reference fluconazole tablets(300 mg).The plasma concentrations of fluconazole at different time after oral ad- ministration were determined by HPLC,with pharmacokinetic parameters computed and the bioequivalence of the two formu- lations of fluconazole evaluated.RESULTS:The pharmacokinetic parameters of test fluconazole tablets vs.the reference flu- conazole tablets were as follows:t_(1~2):(29.98?4.79)hvs.(30.60?4.66)h,t_(max):(1.9?0.3)hvs.(1.9?0.6)h;C _(max):(7.64?0.98)?g?mL~(-1)vs.(7.93?0.78)?g?mL~(-1) AUC_(0~96):(258.41?36.49)?g?h?mL~(-1)vs.(267.93?31.24)?g?h?mL~(-1) AUC_(0~∞):(283.17?45.80)?g?h?mL~(-1)vs.(299.70?38.41)?g?h?mL~(-1) and derived from planimetry AUC_(0~96) and AUC_(0~∞),the relative bioavailability of the test tablet were(94.9?11.3)%and(94.8?11.9)%,respectively.CONCLUSION: The test fluconazole tablets and the reference fluconazole tablets were proved to be bioequivalent.
2.Cloning and Optimized Prokaryotic Expression of a pbmag-1 cDNA Fragment from Plasmodium berghei ANKA

Objective To clone and express a novel gene cDNA fragment,pbmag-1,from Plasmodium berghei ANKA strain. Methods The cDNA sequence of pbmag-1 was obtained from the GenBank of P.berghei ANKA genomic databases,with which a pair of primers was designed and RT-PCR was used to get a cDNA fragment of the gene from the parasite. The expanded cDNA 3′ fragment of the gene was obtained by 3′-RACE using the oligo dT primer and a set of specific primers. The intact cDNA 3′ fragment was cloned into a prokaryotic expressional vector and transformed into the BL21-(DE3)-RIL strain of Escherichia coli. The recombinant protein of PbMAg-1 was expressed with an optimized strategy and used to immunize mice. Results The pbmag-1 cDNA fragment obtained was 1 341 bp in length,A/T rich (73%) and with a correct 3′ end sequence. By Western blot,the anti-serum of mice immunized with the recombinant protein of PbMAg-1/GST,which was expressed as inclusion bodies,specifically recognized a band with Mr 64 000 molecule from the protein extracts of P. berghei-infected mouse erythrocytes. Conclusion The pbmag-1 cDNA sequence with intact 3′ has been obtained,which will be used for further study on its role in the immune response of P. berghei infection.
3.Evaluation of efficacy and safety of different doses Octreotide combined with Lansoprazole in the treatment of hepatocirrhosis with upper gastrointestinal bleeding

Objective To evaluate the efficacy and safety of different doses Octreotide combined with Lansoprazole in the treatment of hepatocirrhosis with upper gastrointestinal bleeding.Methods Eighty hepatocirrhosis patients with upper gastrointestinal bleeding during May.2013 to May.2016 were treated with Octreotide combined with Lansoprazole,and were divided into high dose group (40 cases) and low dose group (40 cases) according to different doses Octreotide.Before and after treatment,portalvenousmaximumvelocity (PVV) and portalveindiameter (PVD) were detected by color Doppler ultrasound respectively,and haemostatic time,blood transfusion cases and volume,rehaemorrhagia cases were recorded,and adverse reactions were observed.Results After treatment,PVV and PVD of the two groups were significantly ameliorative (P < 0.05),and there were no significantly different between two groups.Haemostatic time,blood transfusiorate,blood transfusiorate volume and rehaemorrhagia rate of the high dose group were significantly less than the low dose group (P < 0.05).The clinical effective rate of the high dose group was significantly higher than that of the low dose group (P < 0.05).The rate of adverse reactions of the high dose group was not significantly different from that of the low dose group.Conclusions High doses Octreotide combined with Lansoprazole in the treatment of hepatocirrhosis with upper gastrointestinal bleeding have a gain on hemostatic effect,and cannot increase adverse reactions significantly.

5.Hospital Pharmaceutical Care Viewed from Perspective of Rational Combination of Chinese Patent Drugs with Western Medicine for Patients with Cardiovascular and Cerebrovascular Diseases

OBJECTIVE:To explore hospital pharmaceutical care mode to promote clinical rational drug use.METHODS:The consumption data of drugs for patients with cardiovascular and cerebrovascular diseases during 2006 in dispensary of traditional Chinese medicine of our hospital were investigated and analyzed.RESULTS:The ratio of drug combination increased with the rising of patients' age,so did the incidence of adverse drug reactions(ADR).CONCLUSION:Pharmacists' pharmaceutical care plays an important role in promoting patients' rational drug use and treatment.
6.Risk factors that worsened the pathologic course of acute ischemic stroke

Objective To investigate the risk factors that worsened the pathologic course of acute ischemic stroke .Methods The clinical data of 264 patients with acute ischemic stroke treated in Shijitan hospital were analyzed retrospectively .According to the outcome ,the patients were divided into deterioration group (70 cases) and improved group(194 cases) .The clinical data ,labora-tory examination results of the patients were collected through self-made survey and compared between the two groups .The risk factors that worsened the pathologic course of acute ischemic stroke were analyzed by multiple linear regression .Results There were 42 .86% patients with hyperlipidemia history and 70 .00% with diabetes history in the deterioration group ,which were signifi-cantly higher than the improved group(P<0 .05) .The levels of glucose ,glycosylated hemoglobin ,fibrinogen ,serum ferritin ,C reac-tive protein ,low density lipoprotein cholesterol in the deterioration group were significantly higher than those in the improved group (P<0 .05) .Multiple linear regression analysis showed that hyperlipidemia history ,high level of C reactive protein ,and high level of low density lipoprotein cholesterol were the independent risk factors that worsened the pathologic course of acute ischemic stroke . Conclusion Hyperlipidemia history ,high level of C reactive protein ,and high level of low density lipoprotein cholesterol were the independent risk factors that worsened the pathologic course of acute ischemic stroke .According to these factors ,patients were e-valuated and targeted measures were made to improve the prognosis of the patients .
7.Late-course and concurrent radiochemotherapy for advanced esophageal carcinoma

Objective To compare the efficacy and toxicity of late-course concurrent radiochemotherapy and sequential chemoradiotherapy for advanced esophageal carcinoma. Methods Eighty-two patients with advanced esophageal carcinoma were randomized into two groups: 41 cases in late-course concurrent radiochemotherapy (LCRC) group were received two cycles chemotherapy and then underwent concurrent radiochemotherapy;41 cases in sequential chemoradiotherapy (SCR) group were received four cycles chemotherapy and then underwent radiotherapy. The regimen of chemotherapy in all cases:cisplatin 25mg/m2, 1-3 d;calcium folinate (CF) 150 mg/m2, 1-5 d;fluorouracil 375 mg/m2, 1-5 d, 21 d was one cycle. All patients were received the three-dimensional conformal radiation therapy, the total dose of radiation was same as 64 Gy. Results The short-term response rate was 85.4%(35/41) in LCRC group and 65.9%(27/41) in SCR group, they had significant difference ( P<0.05). The rates of acute radiation esophagitis that need treatment was 90.2%(37/41) in LCRC group and 87.8%(36/41) in SCR group, there had no significant difference (P>0.05). The l year, 2 years, 3 years survival rate were 68.3%(28/41) and 65.9%(27/41), 56.1%(23/41) and 51.2%(21/41), 46.3%(19/41) and 36.6%(15/41) respectively,the median survival time were 30.0 months and 26.0 months, there had no significant difference ( P>0.05). Conclusion The short-term efficacy of advanced esophageal carcinoma could be improved by the late-course concurrent radiochemotherapy.
8.Investigation of vitamin B12 deficiency in elderly inpatients in department of neurology

Objective To investigate the status of vitamin(Vit)B12 deficiency in elderly inpatients in department of neurology.Methods 827 patients admitted to the department of neurology of Shanghai Punan hospital from March 2007 to July 2008,aged 60 and older,with no severe hepatosis and renal disfunction,and without using VitB12 preparation for 3 months,were included in the study.The levels of serum VitB12,folate,homocysteine(Hcy) were tested to screened patients with VitB12 deficiency.The results of symptoms,positive signs of neurological system and the laboratory examination were analysed and compared between patients with or without VitB12 deficiency.Results There were 827 patients,with the mean age of(77.1?7.5)years old,fulfilled the enrolling criteria.VitB12 deficiency was found in 163 patients(19.7%).The prevalence was higher in female(21.9%)than male(17.4%),which had a tendency of increased with age.In the patients with VitB12 deficiency,the rate of complicating gastrointestinal diseases was higher.Compared with patients with normal serum VitB12 level,there were more cases suffered from unstable gait(27.0%),hypopallesthesia(34.4%),and more cases combined with some chronic diseases such as cerebral infarction(62.6%),hypertension(68.7%),coronary heart disease(39.9%).77% cases had neuroelectricphysiological abnormalities,but only 9.82% cases had megaloblastic anemia.Conclusions VitB12 deficiency is common in elderly inpatients in neurology department,whose clinical manifestations are various and atypical,and the neurological symptoms are more common than megaloblastic anemia.
9.Caspase-3 expression on MCA ischemia-reperfusion in rat models

Objective To study the role of caspase 3 in ischemic brain damage of rats, and further understand the molecular mechanisms of ischemic cerebral vascular disease.Methods Rat models of the left middle cerebral artery (MCA) occlusion/reperfusion were made using a modification of the intraluminal sature method of Longa established by Belayev, infarct zones were confirmed by 2,3,5 triphenyltetrazolium chloride (TTC) staining, and caspase 3 expression on brain sections at the mRNA and active protein level was detected with in situ hybridization and immunohistochemistry technique, respectively.Results After 2 hours of left MCA ischemia followed by 24 hours of reperfusion, obvious infarct in the MCA dominate regions was confirmed by TTC staining; low levels of caspase 3 mRNA, and fewer of its active protein expression was found in normal brains, sham brains and contralateral brains of MCAO rats; both caspase 3 mRNA and activated protein expression in ipsilateral region were increased after 24 hours of recirculation, and even higher levels were detected at 48 hours of reperfusion.Conclusion Apoptotic mechanism might involve in delayed neuronal death after cerebral ischemia, and caspase 3 might play an important role in ischemic neuronal injury.
10.Investigation and Analysis on Etiology of Emotional Diseases of Hospital Case History

Objective To find out the relativity of diseases and emotional factors,provide foundation for the clinical epidemiologic survey,and to know about the doctors understanding of emotional etiology.Method Consulting to the diagnostic criterion of emotional diseases,whole-team-delamination method was adopted to research 54 703 registered case history of retrospective etiology in hospitals(Established Hospital of Shandong Province,Jinan City Central Hospital,TCM Hospital of Shandong Province) within 3 years.Result The cases that was related to emotions were totally 506,0.92% of all the cases.Of all the positive cases,62.5% of them were cardicvascular diseases,12.06% were digestive diseases,3.56% were gynecologic diseases.Of all the emotional etiology,agitation was 43.87%,anger was 25.69%.Female patients took up 56.72% of all the positive cases.Conclusion Cardiovascular diseases are more related to negative emotions,the basic emotion"anger"is more related to diseases,female is prior to male patients,age is of positive correlation of the occurrence of emotional diseases.
