1.The comparative effect of capecitabine and tegafur/gimeracil/oteracil(S-1) on advanced colorectal cancer

Objective To test comparative effect of capecitabine and S-1 (Tegafur/gimeracil/oteracil) on advanced colorectal cancer.Methods 106 cases of advanced colorectal cancer in our hospital oncology were collected and randomly divided into 2 groups:capecitabine group (52 cases) and S-1 group (54 cases).The chemotherapy effect and adverse effect of 2 groups were collected.Results After one treatment period,3 cases of complete remission,19 cases of partial remission,15 cases of stable disease,15 cases of disease progression,efficient rate of 42.3%and control rate of 71.2% were observed in capecitabine group.5 cases of complete remission,21cases of partial remission,19 cases of stable disease,9 cases of disease progression and efficient rate of 48.1% and control rate of 83.3% were observed in S-1 group.There were no significant differences in efficient rate,control rate adverse effect rate and survival time between 2 groups(P > 0.05).Conclusion Capecitabine and S-1 are both safe and effective in treatment of advanced colorectal cancer.
2.Comparison of percutaneous suturing with sponge forceps and open suturing for repair of acute closed Achilles ruptures

Objective To compare percutaneous suturing with sponge forceps and open suturing for repair of acute closed Achilles ruptures.Methods From March 2011 to June 2014,40 patients with acute closed Achilles rupture were enrolled in this study and randomized into 2 equal groups (n =20) which were subjected to percutaneous suturing with sponge forceps or open suturing.The operative time,incisive length,blood loss,wound healing,hospitalization time and complications were compared between the 2 groups.Their ankle functions were evaluated at postoperative 3,6,and 12 months according to American Orthopedic Foot and Ankle Society (AOFAS) ankle-hindfoot scores.Results The operative time (median,60.0 min),hospitalization time (median,8.5 d) and incisive length (median,8.0 cm) in the open suturing group were significantly greater than those in the sponge forceps group (32.5 min,7.0 d and 1.5 cm) (P < 0.05).The blood loss (15.1 ± 13.7 mL) in the sponge forceps group was significantly less than in the open suturing group (54.2 ± 33.9 mL) (P < 0.05).The open suturing group received an average follow-up of 16.8 months (from 12 to 23 months),witnessed superficial skin infection in 3 cases,and obtained AOFAS scores of 91.2 ± 5.1,95.2 ± 2.8 and 97.8 ± 1.6 at 3,6,and 12 months postoperatively.The sponge forceps group received an average follow-up of 15.6 months (from 12 to 24 months),witnessed wound healing in all cases,and obtained AOFAS scores of 90.6 ±5.3,94.I ±2.6 and 97.0 ±2.6 at 3,6,and 12 months postoperatively.The sponge forceps group obtained insignificantly lower AOFAS scores than the open suturing group (P > 0.05).No injury to the sural nerve or tendon rerupture happened in either group.Conclusions In treatment of acute closed Achilles ruptures,compared with open suturing,percutaneous suturing with sponge forceps may be relatively easier without using special equipment,and additionally,may lead to lower incidence of incision complications and better short-term efficacy.
3.Study of correlation between polymorphism of HLA-DRB allele and susceptivity of atopic dermatitis

Objective To look for susceptible and resistant HLA gene of Atopic Dermatitis(AD), analyze single strand conformation polymorphism of the susceptible gene and observe whether the base sequence was different from healthy Method Polymerase chain reaction sequence specific primer(PCR SSP)was used to classify the HLA DRB allele of 122 patients and 80 controls PCR single strand conformation polymorphism(SSCP) was used to analyse the susceptible gene Results The DR 3 frequency of extrinic type of “pure” AD(EAD)group and complicated group was higher than that of the control group ( RR =5 27 and 3 59 respectively P 0 05). Conclusion HLA DR 3 was the susceptible gene of AD, while HLA DR 6 was the resistant gene of AD It indicated that people with HLA DR 3 were reliable to AD, while people with HLA DR 6 were not But there was no base difference between these people and the healthy in HLA DR 3
4.Analysis of activated brain areas in uncertain rewarding processing of decision making on healthy volunteers

Objective To research the activated brain areas of decision making under uncertainty reward processing on healthy volunteers.Method The E-Prime programs were presented 3 kinds of tasks of the decision under uncertain reward processing.15 right-handed healthy volunteers made a response after receiving the task.At the same time,the GE 3.0T magnetic resonance scanner scanned the brains areas of subjects.Individual analysis and group analysis was done with SPM8 software,then the brain activating regions and the peak intensity were gotten.Results Orbitofrontal cortex was activated in certainty,peak intensity 2.4328 ± 0.1949 (P < 0.05).Prefrontal cortex,occipital lobe,parietal lobe,posterior lobe of cerebellum,limbic lobe and midbrain were activated under the risk reward processing,peak intensity 2.4228 ± 1.3762 (P < 0.05).When under ambiguity reward processing,prefrontal cortex,temporal lobe,occipital lobe,left inferior frontal gyrus and posterior lobe of cerebellum were activated,peak intensity 2.4056 ± 0.4222 (P < 0.05).Compared with the task under certainty,posterior lobe of cerebellum,gyri subtemporalis and gyri fusiformis,inferior parietal lobule,anterior central convolution,orbitofrontal cortex,ventrolateral prefrontal cortex,both frontopolar and supramarginal gyrus were activated in task under risk (P <0.05) ;and both frontopolar were activated in task under ambiguity (P < 0.05).Compared with the task under risk,dorsolateral prefrontal cortex and posterior lobe of cerebellum were activated in task under ambiguity (P < 0.05).Conclusion There are differences in different types of reward processing of decision making.
5.Clinical study on the accuracy of dual-energy computed tomography colonography in colorectal neoplasia detection

[Abstract ] Objective To compare the accuracy between regular computed tomography colonography (CTC)and dual-energy CTC in lesion detection.Methods Twenty-eight patients with clinical suspicious space occupying lesions of the colon were selected.All patients were underwent dual-energy mode contrast-enhanced CT scan and the data were reconstructed with colonography and dual-energy iodine maps methods.The diameter,enhanced computed tomography (CT)value and iodine value were measured.The results of colonoscopy and pathology were taken as gold standard.The sensitivity, specificity,accuracy,positive predictive value and negative predictive value of regular CTC and dual-energy CTC were compared.Variance analysis was performed for measurement data comparison among groups and chi-square test was used for count data analysis.Results Among 28 patients,colorectal lesions were detected in 24 cases by regular CTC,of which four cases were false-positive and one case was false-negative confirmed by colonoscopy and pathology.Colorectal lesions were detected in 20 cases by dual-energy CTC,of which no false-positive and one case was false negative confirmed by colonoscopy and pathology.The contrast enhanced CT value of polyps,adenoma,adenocarcinoma and stool was (38.54± 6.82),(49.16±7.31 ),(52.61 ±5 .93 )and (34.00±1 .41 )Hu,respectively.The enhanced value of adenoma and adenocarcinoma was significantly higher than that of polyps and stool,the differences were statistically significant among groups (F = 10.760,P = 0.001 ).There was no significant difference between polyps and stool (t=1 .44,P =0.188).The sensitivity of regular CTC and dual-energy CTC in lesion detection was 95 .6% (95 %cofidence interval(CI ):77.9%-99.2%)and 95 .6% (95 %CI :77.9%-99.2%),respectively.The specificity was 42.8% (95 %CI :15 .4%-93.5 %)and 100.0% (95 %CI :47.9%-100.0%).Conclusion Compared with traditional CTC,dual-energy CTC would distinguish lesions from stool,help differentiate between benign and malignant tumors and further increase the accuracy of CTC diagnosis.
6.Comparative Study on the Chemical Constitutions of Volatile Oil in Forsythia suspensa and Old F. suspensa

OBJECTIVE:To study and compare the volatile constituents in Forsythia suspensa and old F. suspensa. METH-ODS:Steam distillation method was adopted to extract volatile oil. GC-MS was adopted to determine the content of volatile oil:the chromatographic conditions:the column was HP-5MS,inlet temperature was 200 ℃,the carrier gas was nitrogen,split ratio was 20 to 1,by programmed temperature,flow rate was 1 ml/min,the injection volume was 1 μl;MS conditions:ion source was electric bombardment ionization source,electron energy was 70 eV,the interface temperature was 280 ℃,ion source temperature was 230 ℃,quadrupole temperature was 150 ℃,electron multiplier voltage was 2 300 V,and mass scanning range was 29-350 amu. RESULTS:The extraction yield order of volatile oil was that F. suspensa heart was higher than F. suspensa,followed by F. suspensa shell and old F. suspensa;there were 18 common peaks in the volatile oil,and the content of β-pinene was the maxi-mum,followed by α-pinene,β-phellandrene,terpinen-4-ol and terpinene;in addition,4-methylene-1-cyclohexanone,3-cyclohex-ene-1-methanol,cumene formaldehyde,1,7-trimethyl-4-methylenedecahydro,palmitic acid,nutmeg aldehyde can only be detected in the volatile oil of old F. suspensa;while α-phellandrene,ocimene,α-terpineol,α-pinocarvone,cis-piperitol,bornyl acetate,α-cubebene and germacrene D can only present in F. suspensa. CONCLUSIONS:The main chemical components in the volatile oil of F. suspensa and old F. suspensa are basically the same,but there are also some differences. The discovery can provide some ba-sis for the material basic research in the pharmacodynamic differences of F. suspensa.
7.Screening of genetic markers based on human type 2 diabetes mellitus (T2DM) susceptibility loci in a Cynomolgus T2DM model

Objective Type 2 diabetes mellitus ( T2DM) as a common disease around the world becomes a great threat to the health of human beings.The cynomolgus T2DM model, which preferably simulates human T2DM onset and progress, can be beneficial to the drug development and clinical treatment.In the present study, 37 of T2DM-susceptibility SNPs and the extended genome sequences were used to obtain corresponding SNPs in the T2DM cynomolgus monkeys. Methods Firstly, DNA pool screening was conducted.Then, using polymerase chain reaction to amplify and to sequence the cynomolgus homologous sequences.Using DNAStar software to analyze the differences between bases.Finally, we used analysis of variance and F test to calculate the frequency of alleles.We also used the GLM models of SAS software to analyze the association of genotype with fasting plasma glucose and glycosylated hemoglobin.Results SNP661A,SNP661B, SNP343A, SNP343B, SNP343C, SNP565A, SNP565B and SNP565C were found to have a significant difference of allele frequencies between spontaneous cases and controls.Conclusions The findings of this study suggest that SNP661A, SNP661B, SNP343A, SNP343B and SNP343C may play an important role in the establishment of cynomolgus T2DM models.
8.Investigation of the professional ability of community nurses and its influencing factors in Beijing

Objective To study the status of professional ability of community nurses and its influencing factors. Methods From November 2013 to March 2014,the questionnaire of community nurses'professional ability,general self-efficacy scale and the questionnaire of job satisfaction were used to investigate 240 community nurses in 12 community medical institutions in Beijing. Results The average score of community nurses' professional ability was(3. 92 ± 0. 51)with the highest score in the communication and coordination ability of(4. 08 ± 0. 54)and the lowest score in the evaluation and analysis ability of(3. 81 ± 0. 59). The ability of community nurses was positively related to self-efficacy and working satisfaction( r =0. 520,0. 336;P < 0. 01). As the result of multiple linear regression,general self-efficacy,job satisfaction,age and the final degree were the influencing factors of the professional ability of community nurses(t = 4. 64,4. 47, 5. 72,5. 01,2. 69,7. 15,2. 94;P < 0. 05 ). Multiple factors analysis showed that self-efficacy,working satisfaction,age,education were the influencing factors of the ability of community nurses(P < 0. 05),within which the self-efficacy was the strongest influencing factor(β = 0. 43). Conclusions It is necessary for the managers and educators to attach more importance to community nurses' self-efficacy and job satisfaction,so as to improve the professional ability of the whole community nurse team.
9.Curative effect of vancomycin-loaded calcium sulfate for one-stage treatment of chronic osteomyelitis

Objective To observe the effect of vancomycin-loaded calcium sulfate for one-stage treatment of chronic osteomyelitis.Methods A retrospective case series study was made on 26 patients with chronic osteomyelitis treated from January 2014 to April 2016.There were 23 males and 3 females,with a mean age of 35.8 years (range,16-90 years).After focus debridement of osteomyelitis,the cavities were filled with vancomycin-loaded calcium sulfate and systemic antibiotics were used.Meanwhile,gentamicin was added to patients showing unclear pathogens.Three patients underwent flap transfer surgery simultaneously due to the inadequate soft tissue coverage.Wound healing,local redness and swelling,sinus recurrence,recovery of osteomyelitis and repair of bone defect were detected after operation.Results All patients were followed up for 6-30 months (mean,19.5 months).Primary healing was achieved in all patients.One patient experienced wound exudate and was effectively managed by dressing change,one patient experienced inadequate local soft tissue coverage and was effectively managed by a second flap transfer surgery,and one patient remained wound unhealing after two surgeries and was cured by infected bone resection using the Masquelet induced membrane technique.Follow-up showed local bone defect was repaired in all patients without bone infection recurrence.Conclusions For patients with chronic osteomyelitis,one-stage surgery with vancomycin-loaded calcium sulfate can control infection with simultaneous repair of bone defect.Therefore,the procedure is a simple and effective method and worthy of clinical practice.
10.Advances of metagenomics in discovering novel biocatalysts.

Microorganisms contain a large number of biocatalysts, which are of great potential in industrial applications. However, the traditional cultural approaches can obtain only less than 1% of microorganisms. As a culture-independent method, metagenomics is an advanced solution by means of extracting all microbial genomic DNAs in certain environmental habitat, constructing and screening metagenomic libraries to seek novel functional genes. It serves as an effective tool for studying these uncultured microorganisms. Therefore, mining novel biocatalysts from metagenome has drawn the attention of researchers in the world. In this paper, environment sample category, genomic DNA extraction, library construction and screening strategies were reviewed. Recent examples of isolated biocatalysts from metagenomic libraries were presented. Future research directions of metagenomics were also discussed.
