1.Strengthening self -management improves treatment compliance and outcome in hepatitis B patients with decompen-sated cirrhosis treated with entecavir

Objective To investigate the effect of strengthening self -management on the treatment compliance and outcome in hepatitis B patients with decompensated cirrhosis treated with entecavir.Methods A total of 120 hepatitis B patients with decompensated cirrhosis, who received antiviral therapy with entecavir in our hospital from January 2010 to January 2012,were randomly divided into control group (n=60)and intervention group (n =60).Both groups received routine nursing care;in addition,regular telephone follow -up was performed for the intervention group to provide guidance for the self -management during the comprehensive prevention and treatment of decompensated cirrhosis.The two groups were compared in terms of withdrawal rate,rehospitalization rate,hepatitis B virus (HBV)DNA negative conver-sion rate,incidence of primary liver cancer,and mortality one year later.Continuous data were analyzed by t test,and categorical data by chi -square test or Fisher′s exact test.Results The withdrawal rate and rehospitalization rate in the intervention group were 0 and 18.3%, respectively,versus 8.3% and 35% in the control group (P <0.05 for both).The HBV DNA negative conversion rate,incidence of prima-ry liver cancer,and mortality in the intervention group were 90%,3.3%,and 5.3%,versus 88.3%,5%,and 6.7% in the control group (P >0.05 for all).Conclusion Strengthening self -management,as well as regular health education,can improve treatment compliance and promote recovery in hepatitis B patients with decompensated cirrhosis treated with entecavir.

An improved method for Ge phenotyping using sulfosalicylic acid staining after IEF is reported. Gc subtyping had been achieved and Gc variants were detected using this method without using anfi-Gc serum for immunofixation. Two new Gc variants Gc~(1c3) and Gc~(2c7) were discovered in Chinese people, and their frequencies were 0.0008 and 0.0004 respectively.
3.A method of plethysmography wave de-noising based on wavelet transform and spline interpolation

Objective: To eliminate the noise of the plethysmography wave and obtain plethysmography wave signal without noise. Methods:Using sym4 and combining the improved wavelet threshold wavelet algorithm to remove high frequency white noise of plethysmography wave, and then using spline differential fitting method to remove baseline drift of pulse wave volume. Results:The experimental results show that the high frequency noise and baseline drift of plethysmography wave have been removed effectively processed by the sym4 wavelet and combining with improved threshold of wavelet algorithm and spline difference after fitting. Conclusion:The method can well remove the noise of the plethysmography wave and retains the effective information of plethysmography wave effectively.
4.Investigation on influence factors of pathogenesis in 143 cases of dangerous placenta previa and analysis on perinatal period outcomes

Objective To explore the influential factors of dangerous placenta previa pathogenesis,and to analyze their peri natal period outcomes.Methods The clinical data in 143 pregnant women and parturients of dangerous placenta previa(research group) and 286 pregnant women and parturients of non-dangerous placenta previa(control group) in our hospital from January 2011 to December 2015 were retrospectively analyzed.Results The average age of the research group was higher than that of the control group (t =2.92,P =0.004);the previous pregnancy frequency,previous parity,previous abortion frequency,previous cesarean delivery frequency and incidence of placenta previa in the research group were higher than those in the control group(P<0.01);the incidence of placenta previa in the research group was higher than that in the control group(x2 =7.60,P=0.006);the prenatal gestational weeks of the research group were lower than those of the control group(t=-12.33,P<0.01).The multi-factor analysis showed that the age,previous abortion frequency and previous cesarean delivery frequency were the influence factors of placenta previa occurrence.Among them,the previous cesarean delivery frequency was the main risk factor.The occurrence rates of antepartum hemorrhage,postpartum hemorrhage,uterine removal and premature infants in the research group were higher than those in the control group(P<0.01);The intraoperative blood loss of the research group was higher than that of the control group(P<0.05);the neonatal birth weight and the 1 min Apgar score of the research group were lower than those in the control group(P<0.05).Conclusion The age,previous abortion frequency and previous cesarean section frequency are related to the occurrence of dangerous placenta previa.
5.Inhibition effect of allitride on Candida albicans biofilm

Objective To investigate the killing effect of allitride on fungus inside Candida albicans biofilm (BF) .Methods The polyascchride components of BF was labeled by FITC-conA .The fluorescence microscopic was adopted to observe the effect of al-litride on the BF forming process of Candida albicans in morphology .The specimens were divided into 3 groups ,the control group (allicin concentration of 0) ,0 .25 mg/mL allicin group ,0 .5 mg/mL allicin group ,1 .0 mg/mL allicin group ,2 .0 mg/mL allicin group .The XTT reduction assay was performed to quantitatively detect the killing effect of allitride on fungus inside Candida albi-cans BF .Results After the treatment by different concentrations of allitride ,the BF structure was observed by fluorescence micros-copy .The fluorescence intensity of high concentration group was weakened compared with the low concentration group and the con-trol group .The ODA450 values of the XTT reduction assay after acting on Candida albicans BF for 5 h by different concentrations of allitride (0 .5 ,1 .0 ,2 .0 mg/mL) were 1 .87 ± 0 .31 ,1 .64 ± 0 .25 and 1 .30 ± 0 .29 ,respectively ,and the OD450 value of the control group was 2 .11 ± 0 .26 ,there was statistically significant difference between the allitride groups and control group (P<0 .05);after acting on Candida albicans BF by 0 .25 mg/mL allitride ,the OD450 value was 1 .93 ± 0 .31 ,the difference had no statistical signifi-cance compared with the control group(P>0 .05) .Conclusion Allitride can kill the fungus inside BF of Candida albicans and has the inhibiting effect on biofilm .Moreover the inhibiting effect on biofilm is increased with its concentration increase within certain concentration range .
6.Interleukin-10 inhibit intercellular adhesion molecule-1 expression in isolated rat hepatic stellate cells

Objective To investigate the effects of interleukin 10(IL 10) on the intercellular adhesion molecule 1(ICAM 1) expression of rat hepatic stellate cells. Method Hepatic stellate cells were isolated and purified from rat liver by collagenase Ⅳ perfusion and density gradient centrifugation with Nycodenz. Passaged hepatic stellate cells were divided randomly into four groups: control group (A group), TNF? 100 U/ml group (B group), TNF? 100 U/ml plus IL 10 2 ng/ml group (C group). TNF? 100 U/ml plus IL 10 20 ng/ml group (D group). 24 h after incubation expression of ICAM 1 protein and mRNA in hepatic stellate cells were detected by using cell ELISA and semi quantitative RT PCR method. Results The yield rate of hepatic stellate cells was 2.5?10 7 per rat. The cell viability was above 98%. The purity of hepatic stellate cells was more then 92%. 7.Correlation of blood transfusion volume to motility rate in the patients with known duration of transfusion-acquired HIV infection in China

Objective To investigate the association of blood transfusion volume with early death among recipients with known duration of infection in rural central China. Methods From January 1, 1999 to December 30, 2003, 76 individuals with duration of blood transfusion who met the study criterion had been found to be infected in the HIV/AIDS notified system in rural of Yuncheng Prefecture of Shanxi Province, China. A retrospective study was conducted to investigate Socio-demographic characteristics, transfusion volume, and mortality. Cox' proportional hazard regression model was used to analyze the data. Results By December 30, 2003, among 76 subjects with transfusion-acquired HIV infection, 24 recipients had died. Mortality rate and median survival was 45.64 and 8.3 per 1000 person years, respectively. Transfusion volume (HR=3.12, 95% CI: 1.18~8.25; P=0.0218) was significantly associated with mortality in recipients in the multiply Cox' proportional hazard regression model. Conclusions This study suggests that more units of transfusion in recipients with known duration of HIV infection can influence time to death in recipients in China.
8.Clinical study of low dose mifepristone with methyltestosterone on the treatment of climacteric dysfunctional uterine bleeding

Objective To study the clinical effects of low dose mifepristone together with methyltestosterone on the treatment of climacteric dysfunctional uterine bleeding(DUB).Methods Seventy patients with DUB were underwent dilatation and curettage of the uterus and divided into treatment group(n=35) and control group(n=35).The patients in the treatment group were treated with mifepristone and methyltestosterone,and those of the control group were treated with norlutin.The amenorrhea,the changes of endocrine and the pathology of endometrium were observed.Results All the patients were amenorrhea during the period of taking medicine and the regular menses was recovered after the treatment.Eight patients were menopause after the treatment.The levels of FSH,LH and P were decreased.The endometrium were atrophied and the anemia was improved.Conclusion Low dosage of mifepristone with methyltestosterone can prevent endometrial hyperplasis and it is safe and effective for the treatment of climacteric dysfunctional uterine bleeding.
9.Preparation of sgp130 ELISA kit and its application

Objective:To establish the sensitive,specific,stable and convenient sgp130 ELISA kit.Methods:The mAb T2 against human gp130 was used as coating antibody;the other mAb T12 recognized different epitope with T2 was labeled by biotin,then a ELISA kit for detecting sgp130 was set up.Results:sgp130 ELISA kit is successfully established and its sensitivity is 10 ng/ml.After the kit is placed in 4℃ for 3 months,the kit's CV is less than ?7.6% and the retrievable rate is 95%~111%.These indicate that it has highly sensitivity,stability and accuracy.The normal serum concentration of sgp130 in healthy donors is 536.92~287.88(ng/ml),but there is higher in patients with hyperthyroidism(937.16?217.5) and chronic nephritis(806.45?138.47).Significant difference is found comparing with normal control(P

Objective To observe the different changes of macular microstructure in patients with large idiopathic macular hole (IMH) treated with vitrectomy combined with internal limiting membrane (ILM) transplantation or not.Methods Forty eyes in 40 consecutive patients with giant IMH (≥500 μm) were included in the study.Twenty eyes received vitrectomy with ILM transplantation (ILM transplantation group) and others with ILM peel off (ILM removal group).During the operation,a proper size of the ILM was removed and filled in the bottom of the macular hole.The age,duration of disease and the ocular laterality of the two groups of patients were not statistically significant (P>0.05).Minimum resolution angle in logarithmic (logMAR) best corrected visual acuity (BCVA) and frequency domain optical coherence tomography (SD-OCT) scan were examined.There was no statistically significant difference in logMAR BCVA,average defect diameter of photoreceptor ellipsoid (IS/OS) and average defect diameter of external limiting membrane (ELM) between two groups (t=0.128,1.452,1.321;P>0.05).The logMAR BCVA and SD-OCT were examined on 1,3,6,12 months postoperatively.Results On 1 month after the surgery,there was no statistically significant difference in logMAR BCVA,average defect diameter of IS/OS and average defect diameter of ELM between two groups (t=1.226,1.435,1.018;P>0.05).On 3,6,12 months after the surgery,compared with ILM removal group,the logMAR BCVA (t=2.059,2.871,2.415) increased and the average defect diameter of IS/OS (t =2.070,2.110,2.121) and ELM (t =2.034,3.647,3.556) significantly reduced in ILM transplantation group (P<0.05).On 1 month after the surgery,there was statistically significant difference in CRT between two groups (t=2.113,P<0.05).On 3,6,12 months after the surgery,there was no statistically significant difference in CRT between two groups (t=0.428,0.847,0.849;P>0.05).Conclusion Compared with vitrectomy combined with ILM peeling surgery,the diameter of IS/OS and ELM defect were significantly decreased after vitrectomy combined with ILM transplantation in the patients with large IMH.
