1.Association of T cell receptor diversity of CD4+ T lymphocytes with viral load in individuals with HIV-1 infection

Objective To assess the impact of the virus on the complementary determining region 3 (CDR3) length diversity of T cell receptor(TCR) Vβ repertoires of CD4+ T lymphocytes and to explore its association with viral load in individuals with HIV-1 infection. Methods The TCR repertoire was examined using spectratyping of CDR3 length diversity within CD4+ T cells in HIV infected and healthy adults. Separation of CD4+ T cells from peripheral blood mononuclear cells ( PBMCs) was carried out by using immunomagnetic beads coated with anti-CD4 antibody. Total RNAs from the purified CD4 + T lymphocytes were isolated and used to perform nested-PCR amplifications in CDR3 of 22 TCR gene families. CDR3 diversity and its association with viral load in individuals with HIV-1 infection were analyzed. Results An average diversity for all CDR3 profiles in CD4+ T cells from 25 HIV-infected individuals was significantly different as compared to 10 age-matched healthy donors (P<0.05) with the HIV-infected individuals losing diversity in the CDR3 profiles. There was positive correlation between changes in TCR CDR3 diversity and viral load (r = 0. 494, P < 0. 05). The changes in CDR3 length diversity of Vβ families in HIV-infected individuals, particular in Vβ8, Vβ22, Vβ23 were statistically different from the healthy controls. Conclusion HIV-1 infection might induce the loss of TCR Vp repertoire diversity and disrupt the CDR3 Gaussian distributions within CD4 + T cells. There should be positive correlation between changes in TCR CDR3 diversity and the viral load in HIV-1 infected patients.

The clinical data of 1 case of oral hydrochloric acid poisoning successfully treated was retrospectively analyzed to provide reference for diagnosis and treatment of similar clinical cases.

4.Compare therapeutic effects and adverse effects between TEC-NX regime and TEC regime in adjuvant chemotherapy of local advanced breast cancer patients

Objective To compare the difference of efficacy and adverse effects between TEC-NX regimen (Docetaxel + Epirubicin + Cyclophosphamide sequential Vinorelbine + Capecitabine) with TEC regimen (Docetaxel + Epirubicin + Cyclophosphamide) in the adjuvant chemotherapy of locally advanced breast cancer.Methods A retrospective study was conducted to select 58 patients with locally advanced breast cancer who underwent ≥4 axillary lymph node metastasis from April 2008 to April 2015 at the International Peace Maternity and Child Health Hospital,Shanghai Jiao Tong University School of Medicine.All patients were female,average age was 48 years old,rang from 29 to 63 years old.Patients were divided into TEC-NX group (n =23) and TEC group (n =35) according to different treatment methods.TEC-NX group patients received chemotherapy with TEC-NX regimen and TEC group patients received chemotherapy with TEC regimen.The 3-year overall survival rate and 3-year disease-free survival rate were compared between the two groups.The differences in adverse reactions such as myelosuppression,liver dysfunction and fever were compared between the two groups.The Chi-square test was used to compare the count data between the two groups;the survival analysis was performed by Kaplan-Meier method.Results The 3-year overall survival rate was 91.3% in the TEC-NX group and 91.4% in the TEC group,and the difference was not statistically significant (P =0.995).The 3-year disease-free survival rate was 73.9% in the TEC-NX group and 85.7% in the TEC group,and the difference was not statistically significant (P=0.289).The incidence of grade Ⅲ or Ⅳ myelosuppression in the TEC-NX group was 26.0%,which was significantly lower than that in the TEC group (65.7%),and the difference was statistically significant (P =0.009).The incidence of liver dysfunction was 91.3% in the TEC-NX group,and there was not statistically significant compared with 93.5% in the TEC group (P =0.523).The incidence of fever in the TEC-NX group was 17.4%,which was not statistically significant compared with the TEC group (14.3%) (P =0.749).Conclusion In adjuvant chemotherapy for locally advanced breast cancer,the TEC-NX regimen does not improve 3-year overall survival and disease-free survival compared with the TEC regimen,but the TEC-NX regimen significantly reduced the incidence of grade Ⅲ or Ⅳ myelosuppression.
5.Clinical application study on capillary electrophoresis-based gene diagnosis of hyperphenylalaninemia

Objective:To evaluate the clinical value of a capillary electrophoresis-based method for gene diagnosis of hyperphenylalaninemia.Methods:In this single-center prospective study, 40 newborns with suspected hyperphenylalaninemia detected by neonatal liquid chromatography-tandem mass spectrometry screening at Nanjing Maternity and Child Health Care Hospital from February 2021 to February 2023 were included, with 22 males, 18 females and a mean age at diagnosis of 21.93 days.Capillary electrophoresis was used to detect 85 variants of the phenylalanine hydroxylase ( PAH) gene in 40 newborns with suspected hyperphenylalaninemia.The PAH gene of undiagnosed patients was further analyzed by Sanger sequencing.The detection rate, sensitivity and specificity of capillary electrophoresis were calculated. Results:Among these 40 newborns with suspected hyperphenylalaninemia, 71 PAH variants were detected by capillary electrophoresis, 32 patients were clearly diagnosed, only 1 pathogenic variant was found in 5 patients, and no pathogenic variant was found in the last 3 patients.Therefore, the detection rate, sensitivity and specificity of capillary electrophoresis for analysis of the PAH gene were 80.00%, 88.75% and 100%, respectively. Conclusions:The capillary electrophoresis-based method can rapidly, efficiently and accurately detect PAH gene variants at lower cost and is a promising gene detection method for hyperphenylalaninemia in clinical practice.
