1.The impact of stenting prior to extracorporeal shock wave lithotripsy in non-obstructive upper urinary tract calculi

Objective To assess the efficacy and safety of stenting ureteral stent prior to extracorporeal shock wave lithotrip‐sy(ESWL) for non‐obstructive upper ureter and renal stone .Methods From January 2012 to October 2013 ,a total of 96 patients who met inclusion and exclusion criteria were randomly devided into two groups :a stented group (group B) with a double‐J ureter stent fixed pre‐ESWL and a non‐stented group (group A) treated with ESWL direactly .Outcomes were evaluated in 1 month and then the ureter stent was removed ,another evaluation was taken after 2 months .Results The clearance rate in group A was signifi‐cant higher than that of group B at first month(P=0 .02) ,but group B was significant higher than that of group A at the second month(P=0 .001) .The renal colic rate of group A was significant higher than that of group B(P=0 .001) ,the gross hematuria rate of group B was significant higher than that of group A(P=0 .005) .The comparison of fever ,steinstrasse ,lower urinary tract syn‐drome showed no statistical difference(P>0 .05) .Conclusion Stenting ureter stent may increase the clearance rate of ESWL by 1 session for non‐obstructive upper ureter and renal stone ,but the passage of stones mainly happen after the removing of stent .Stent‐ing can also prevent the morbidity of renal colic without increasing low urinary tract syndrome ,but can increase the morbidity of gross hematuira .
2.Genetic association between interleukin-23 receptor gene polymorphisms and multiple sclerosis in a Southern Han Chinese population

Objective To analyze the association between interleukin-23 receptor (IL23R) gene single nucleotide polymorphisms (SNPs) and multiple sclerosis (MS) in a Southern Han Chinese population.Methods Three SNPs (rs2201841,rs10889677,rs7517847) within the IL23R gene were detected in 178 MS patients and 221 controls using polymerase chain reaction and restriction fragment length polymorphism method (PCR-RFLP).Haplotypes of the IL23R gene were constructed with SHEsis software.Results The frequencies of the genotype or allele in the IL23R gene SNPs(rs2201841:TT,TC,CC in MS patients:5.7% (10/175),45.7% (80/175),48.6% (85/175),in controls:7.4% (16/217),41.0% (89/217),51.6% (112/217),x2 =1.08,P =0.58 ; rs10889677:AA,AC,CC in MS patients:52.0% (89/171),42.7% (73/171),5.3% (9/171),in controls:57.7% (123/213),36.2% (77/213),6.1% (13/213),x2 =1.71,P =0.43 ; rs7517847:GG,GT,TT in MS patients:16.9% (29/172),51.7%(89/172),31.4% (54/172),in controls:14.4% (31/215),49.3% (106/215),36.3%(78/215),x2=1.15,P =0.56)showed no significant difference between MS patients and controls.The genotype did not influence the age of onset,the duration or disease severity in MS patients.The SNPs rs2201841 and rs10889677 had strong linkage disequlibrium (D' =0.614,r2 =0.327).The haplotype frequencies showed no significant difference between the MS patients and controls.Conclusion There is no evidence in our study to support the association between IL23R SNPs and MS in a Southern Han Chinese population.

Objective This paper discussed the implementation effect of medical research integrity and related code of conduct in a general hospital in xinjiang,to provide theoretical basis for the construction of medical research integrity.Methods According to the gap analysis theory,a questionnaire was designed for satisfaction with the implementation effect of medical research integrity and related code of conduct in the hospital.101 researchers in the hospital were surveyed to understand their evaluation on various aspects of the hospital's scientific research integrity construction policies and the space for improvement.Results The gap analysis showed that the hospital has a large gap in the construction of "institutional construction of scientific research integrity management " and "methods and regulations for dealing with academic misconduct",which were urgently need to be improved in the scientific research integrity construction.Conclusions The hospital should further improve the system of scientific research integrity construction,combine the characteristics of medical development and the corresponding national policy development,strengthen the innovation of scientific research management,create a healthy and positive environment for scientific research activities,and promote the comprehensive construction and long-term development of scientific research integrity construction.
4.Clinical effect of transvaginal and laparoscopic operation methods for treating exogenous uterine cesarean scar pregnancy

Objective To study the clinical curative effect of transvaginal and laparoscopic operation methods for treating exogenous uterine cesarean scar pregnancy.Methods Sixty patients with exogenous uterine CSP in our hospital from August 2013 to August 2016 were selected and divided into the two groups with 30 cases in each group.The control group adopted laparoscopic lesion resection,while the observation group adopted transvaginal lesion excision of lower uterine segment.Then the operation situation,postoperative VAS score,β-HCG levels and traumatic response indexes were compared between the two groups.Results The hospitalization time had no statistical difference between the two groups(P>0.05).The hospitalization cost,blood loss volume and anal exhaustion time in the observation group were less than those in the control group(P<0.05).The β-HCG level before and after operation had no statistical difference between the two groups (P>0.05).The VAS score and traumatic response indexes in the observation group were better than those in the control group(P<0.05).The complication occurrence rate in the observation group was lower than that in the control group(P<0.05).Conclusion Transvaginal lesion excision of lower uterine segment in the treatment of exogenous uterine CSP has better curative effect and safety.
5.Investigation of the status of disaster preparedness and the influence path of psychological capital and perceived organizational support on it among pediatric nurses in Henan province

Objective:To investigate the status of disaster preparedness of pediatric nurses and its influencing factors, as well as the impact path of psychological capital and perceived organizational support on disaster preparedness.Methods:This was a cross-sectional study. A total of 361 pediatric nurses from Henan Provincial People ′s Hospital, the Third Affiliated Hospital of Zhengzhou University, Fuwai Central China Cardiovascular Hospital were sampled from August to October 2021. They were investigated by the general information questionnaire, psychological capital questionnaire, perceived organizational support questionnaire and disaster preparedness questionnaire. SPSS25.0 was used to analyze the questionnaire and scale data, and AMOS23.0 was used to construct a structural equation model about the disaster preparedness of pediatric nurses. Results:The scores of disaster preparedness from pediatric nurses was 4.65 ± 0.92. The level of disaster preparedness was significantly positively correlated with psychological capital and perceived organizational support ( r=0.690, 0.525, both P<0.05). Disaster training and emergency drill 2 dimensions of psychological capital questionnaire (hope, resilience), perceived organizational support were independent contributing factors ( P<0.05). Psychological capital had direct effect on disaster preparedness. And the direct effect was 0.77, the indirect effect was 0.11, the total effect was 0.88, and the direct effect of perceived organizational support on disaster preparedness was 0.21. Conclusions:The score of disaster preparedness of pediatric nurses was in the middle level. Hospital managers should strengthen the training of pediatric nurses in disaster knowledge, operational and simulation exercise, pay attention to the level of nurses' psychological capital and perceived organizational support, so as to provide reference for the follow-up clinical pediatric disaster nursing education and management.
6.Improving the production of plant-based recombinant protein: a review.

Recombinant proteins provide new means for disease treatment, while creating considerable economic benefits. Using commercial crops (mainly tobacco), cereal crops, legumes, and vegetable crops to produce recombinant proteins with medicinal value is a hot-spot for research in "molecular farming". Although many recombinant proteins have been expressed in plants, only a small number have been successfully put into use. To overcome the problems that greatly hamper the development of recombinant protein production in plants, researchers have improved expression systems to increase the yield of recombinant proteins. Starting from analyzing the problems of low yield and/or low biological activity of recombinant proteins produced by plants, the optimization strategies to solve these problems were reviewed, and future research directions for improving the yield of recombinant proteins produced by plants were proposed.

7.LINC00926 promotes pyroptosis of hypoxia-induced human umbilical vein vascular endothelial cells by recruiting ELAVL1.

OBJECTIVE:
To investigate the regulatory role of the long non-coding RNA LINC00926 in pyroptosis of hypoxia-induced human umbilical vein vascular endothelial cells (HUVECs) and explore the molecular mechanism.
METHODS:
HUVECs were transfected with a LINC00926-overexpressing plasmid (OE-LINC00926), a siRNA targeting ELAVL1, or both, followed by exposure to hypoxia (5% O2) or normoxia. The expression of LINC00926 and ELAVL1 in hypoxia-treated HUVECs was detected using real-time quantitative PCR (RT-qPCR) and Western blotting. Cell proliferation was detected using Cell Counting Kit-8 (CCK-8), and the levels of IL-1β in the cell cultures was determined with ELISA. The protein expression levels of pyroptosis-related proteins (caspase-1, cleaved caspase-1 and NLRP3) in the treated cells were analyzed using Western blotting, and the binding between LINC00926 and ELAVL1 was verified with RNA immunoprecipitation (RIP) assay.
RESULTS:
Exposure to hypoxia obviously up-regulated the mRNA expression of LINC00926 and the protein expression of ELAVL1 in HUVECs, but did not affect the mRNA expression of ELAVL1. LINC00926 overexpression in the cells significantly inhibited cell proliferation, increased IL-1β level and enhanced the expressions of pyroptosis-related proteins (all P < 0.05). LINC00926 overexpression further up-regulated the protein expression of ELAVL1 in hypoxia-exposed HUVECs. The results of RIP assay confirmed the binding between LINC00926 and ELAVL1. ELAVL1 knockdown significantly decreased IL-1β level and the expressions of pyroptosis-related proteins in hypoxia-exposed HUVECs (P < 0.05), while LINC00926 overexpression partially reversed the effects of ELAVL1 knockdown.
CONCLUSION
LINC00926 promotes pyroptosis of hypoxia-induced HUVECs by recruiting ELAVL1.

8.Treatment patterns of patients with multiple sclerosis in Guangzhou, China

Background & Objective: Disease-modifying treatments (DMTs) for multiple sclerosis (MS) are widely
used in Western countries. In China, however, the current treatment patterns of MS patients are not well characterized. This is to explore the gap between the current treatments in Guangzhou, Southern China and those given in Western countries. Methods: We performed a survey of MS patients at department of neurology, a tertiary MS referral centre in Guangzhou, concerning treatments of MS in Southern China. The clinical data in patients were collected. The initial treatment, drug withdrawal or switching profile, and therapeutic effect of existing treatments in MS patients were analyzed. Results: The ratio of MS patients who receive DMTs in Guangzhou China is extremely low. Among the 178 patients studied, only 28.09% received initial treatment with DMTs. MS patients who receive initial treatment with first-line DMTs have higher drug withdrawal rates (32.6%) and drug switching rates (30.43%) than those of western populations.
The main reasons for withdrawal of first-line DMTs were doctor’s advice (maintenance of remission)
(40.00%), economic burden(20.00%), and no channels to buy drugs(13.33%). In MS patients initially
treated with first-line DMTs who switched to other drugs, a gap between treatments was common (8/14;57.14%). There were 18 patients with highly active MS receiving treatment with rituximab. Annual
relapse rate after treatment significantly decreased than that before treatment (0.74 vs. 1.50 , P < 0.001).
Conclusions: DMTs for MS in Guangzhou, Southern China appear to lag behind those in Western
countries. Much work is needed to improve drug accessibility and affordability of DMTs in China.
Rituximab is an option for highly active MS in limited medical-resource countries.
9.Study on the correlation between positive antinuclear antibody and intervention therapy and assisted reproductive outcome of repeated implantation failure

Objective :
To investigate the impact of positive antinuclear antibody (ANA) and subsequent intervention therapy on the assisted reproductive technology outcomes among the patients experiencing recurrent implantation failure (RIF) .
Methods :
A retrospective study was conducted on 344 RIF patients . Based on ANA test results , the patients were divided into ANA⁃positive group (294 cases) and negative control group (50 cases) . The ANA⁃positive group was further divided into a low titer group (214 cases) and a high titer group (80 cases) . Comparative statistical analyses such as the Wilcoxon rank⁃sum test , Mann⁃Whitney U test , Kruskal - Wallis test and chi⁃square test , etc . were employed to evaluate differences in general clinical data , embryo⁃related parameters , and pregnancy outcomes between the positive and negative groups . The impact of ANA on the assisted reproductive outcomes of patients with recurrent implantation failure was analyzed , and the outcomes of ANA⁃positive patients after intervention therapy were also analyzed .
Results :
Notably , the clinical pregnancy rates of patients in the ANA⁃positive low titer group and high titer group were significantly lower than those in the negative control group ( P < 0. 001) . Similarly , the rates of fertilization and cleavage of oocytes in ANA⁃positive patients were also significantly lower than those in the negative control group (P < 0. 05) . For patients who did not achieve pregnancy after embryo transfer due to ANA positivity , immunomodulatory therapy significantly improved both the clinical pregnancy rate and cumulative clinical pregnancy rate (P < 0. 05) .
Conclusion
Compared with the negative control group , the clinical pregnancy rates decrease in both ANA⁃positive low titer subgroup and high titer subgroup . However , clinical intervention therapy enhances the single⁃cycle clinical pregnancy and cumulative pregnancy rates among ANApositive patients , indicating that ANA positivity is an important factor in RIF . Immunomodulatory therapy is an effective measure to improve recurrent implantation failure among ANA⁃positive patients .
