Objective To explore the effects of estrogen on the cognitive function,the expression of Bcl-2 and Bax proteins and the neuroal apoptosis in hippocampus in rats with vascular dementia(VD).Methods 60 male Wistar rats were randomly divided into sham-operated group,VD group and estrogen group,20 rats in each group.VD models were made with permanent bilateral common carotid arteries occlusion.The 17-?estrogen (200 ?g/kg,dissolution in peanut oil ) was intraperitoneal injected in estrogen group,once in other day,for 30 times.While,same-dose peanut oil was intraperitoneal injected in the sham-operated group and VD group once in other day,total 30 times.After 60 d the capability of learning and memory were observed by Y-maze test.Neuroal morphology and the expression of Bcl-2 and Bax proteins in hippocampal CA1 subregion were detected by HE and immunohistochemistry stain respectively;neuroal apoptosis was measured by dUTP nick-end labeling(TUNEL).Results Comparing to sham-operated group,cognitive ability in the VD group and estrogen group were significently decreased;both the expressions of Bcl-2 and Bax proteins were increased in hippocampal CA1 subregion;the number of TUNEL positive cells was obviously increased (allP
2.Three different regimens of CF/5-FU,DDP in treatment of patients with advanced gastrointestinal adenocarcinoma

Purpose:To study the efficacies and adverse effects of three different regimens in treatment of patients with gastrointestinal adenocarcinoma, which consisted of two doses of CF(20 mg/m 2, 200 mg/m 2) and two methods of administration of 5-Fu (continuous intravenous infusion and two hours per day intravenously) and DDP 20 mg/m 2. Methods:We selected 36 cases with GI adenocarcinoma and randomizedly divided them into A and B groups, who were hospitalized from January 1999 to November 2000. At the same time, 29 cases hospitalized from April 1997 to December 1998 with the same disease were designated as C group (control) . A group: CF 200 mg/(m 2.d) for 5 days, 2 hours intravenously ; 5-Fu 3 g/m 2 , continuous intravenous infusion during 5 days; DDP 20 mg/(m 2.d) for 5 days, 2 hours IV. B group: CF 20mg/(m 2.d) for 5 days, 2 hours, IV.; the dosage and usage of 5-Fu and DDP as A group. C group: CF 200 mg/m 2, usage as A, B groups; 5-Fu 600 mg/(m 2.d) for 5 days, 2 hours; dosage and usage of DDP as A, B groups. One cycle of these regimens consisted of three weeks and they were evaluated after three cycles. Results:There were 8 cases of gastral cancer and 12 of rectal cancer in A group whose response rates were 50% and 41.7% respectively. B group had 7 cases of gastric cancer and 9 cases of rectal cancer, their response rates were 42.9% and 44.4%. C group consisted of 14 cases of gastric cancer and 15 cases of rectal cancer, whose response rates were 35.7%and 40%, respectively. Adverse effects included hematological, renal and bladder and gastrointestinal toxicity. A group was similar to B group. C group had more renal and bladder toxicities(27.5%) and incidence of phlebitis was 34.5%. Conclusions:Combination regimen of low dose of CF, continuous intravenous infusion of 5-Fu and DDP is of better efficacy, tolerance and relative economy, especially suitable for to the elderly patients.
3.The effect of transplanted bone marrow mononuclear cells under different transplant paths on neovascu-larization in ischemic cerebral infarction rats

Objective To investigate the effect of treatment with transplanted bone marrow mononuclear cells under different transplant paths on neovascularization in ischemic cerebral infarction rats. Methods The MCAO models were established by intraluminal vascular suture method. All the successful models were randomly divided into control group, intravenous transplantation group and subarachnoid transplantation group according to the random number table. The Extraction,isolation and culture of bone marrow mononuclear cells were extracted,i?solated and cultured. The models in the control group were injected 200μl PBS through the femoral vein. The mod?els in the intravenous transplantation group were injected 200μl containing 2×107 bone marrow mononuclear cells through the femoral vein. The models in the subarachnoid transplantation group were injected 200μl containing 2× 107 bone marrow mononuclear cells through the subarachnoid. The VEGF levels in cerebrospinal fluid of rats were detected using quantitative ELISA method. The vascular regenerations in the infarct area were observed by using quantitative immunofluorescence. Results After transplantation of bone marrow mononuclear cells 7 d,14 d and 28 d,the VEGF levels of rat cerebrospinal fluid in subarachnoid transplantation group were (207.4±8.9)pg/ml, (171.2±10.3)pg/ml and (143.8±13.8)pg/ml,respectively,which were all higher than the VEGF levels in intra?venous transplantation group,the differences were all statistically significant (P<0.05). After transplantation 14 d and 28 d,the BrdU?positive cells in subarachnoid transplantation group were ( 2043. 8 ± 514. 2) and ( 1834. 8 ± 307.4) ,respectively,which were higher than the intravenous transplantation group,the differences were all statisti?cally significant (P<0.05). The microvessel counts in subarachnoid transplantation group were (384.6±45.1) and (514.8±51.3),respectively,which were higher than that in the intravenous transplantation group,and the differ?ences were all statistically significant (P<0.05) . Conclusions Bone marrow mononuclear cells transplantation can increase VEGF secretion,and promote angiogenesis infarct. The effect of subarachnoid transplantation is better.
4.Typing of Acinetobacter baumannii isolated from patients in intensive care unit with IRS-PCR

Objective To type Acinetobacter baumannii using infrequent restriction site PCR (IRS PCR). Methods Strain specific electrophoretic patterns from PCR products by amplifying DNA sequences flanking infrequent restriction sites of 15 strains of Acinetobacter baumannii were compared with the results of biotyping and antimicrobial susceptibility. Results The 15 bacteria were divided into 5 gene types with IRS PCR, but 3 with biotyping, and 4 with antimicrobial susceptibility. Conclusion IRS PCR method for typing Acinetobacter baumannii is of strong sensitivity, high recognition, good repeatability, convenient operation, and wide range of application.
5.Curative effect observation of intermediate care on patients with after acute stage

Objective To observe the clinical efficacy of intermediate care in patients with after acute stroke.Methods Six hundred cases patients with after acute stroke were divided into experimental group with 301 cases and control groups with 299 cases according to the opinions of patient and family members.The experimental group was given intermediate care for 2 weeks.The control group was given the basic treatment of cerebral infarction.Activities of Daily Living (ADL) score,fall risk score and geriatric depression scale (GDS) were compared between the two groups at the time of entry,2 weeks after treatment,6 months and 12 months of follow-up.Results There were no significant differences in terms of ADL score,fall risk score and GDS between the two groups at the time of entry(P>0.05).Compared with at the time of entry,ADL score and fall risk score were decreased of two group after 2 weeks after treatment,6 months and 12 months of follow-up(P <0.05),and ADL score and fall risk score of the experimental group both were lower than of the control group after 2 weeks after treatment,6 months and 12 months of follow-up(P<0.05).Compared with at the time of entry and 2 weeks after treatment,GDS of two groups both were decreased 6 months and 12 months of follow-up (P <0.05),but there was no significant difference between the two groups in different time periods(P >0.05).Conclusion Intermediate care has a significant effect on improving the activities of daily living and reducing the risk of falls in patients with after acute stroke,and can reduce the GDS score.

The outcome of diffuse.Large B-cell lymphoma (DLBCL) has been improved in the last decades and will be raised further with the development of novel agents in the future.Personalized treatment for DLBCL is based on its molecular heterogeneity.Except for concurrent translocation of myc and bcl-2,neither clinical nor biological factor is sufficiently available to be used to guide the deviation of R-CHOP therapy in DLBCL.Gene expression profiling and whole genomic analytic techniques are expected to be important approaches at present to determine personalized treatment for DLBCL.The advance in the field being reported by Laurie H.Sehn from University of British Columbia in the 54 ASH meeting was summarized in the paper.
7.Clinical analysis of 18 adult patients with hemophagocytic syndrome

Objective To analyze the adult hemophagocytic syndrome' s pathogeny,clinical features,prognostic factors and therapeutic options.Methods 18 cases of adult hemophagocytic syndrome were analyzed,the Kaplan-Meier analysis was used to investigate the total survival rate,and 17 clinical pathological factors and clinical treatment methods which may influence survival were analyzed by Log-rank test in the univariate analysis.Results In this group of patients,EBV infection and malignant lymphoma were the most common initiating diseases.The most common clinical features were peripheral cytopenia in two or three lineages (100 %),fever (83 %),splenomegaly (78 %),swollen lymph nodes (56 %).The mortality rates as high as 66.7 %.The median survival time was 7.4 weeks.One-way ANOVA results showed that the initial symptoms as fever (P =0.039),age > 30 years old (P =0.031),enlargement of the liver (P =0.041),Hb < 100 g/L and Ph < 50 g/L (P =0.039) were relevant prognostic factors.Conclusion Adult hemophagocytic syndrome patients with fever as the initial symptoms,age > 30 years old,liver enlargement,Hb < 100 g/L,Plt < 50 g/L indicates poor prognosis,thus these patients having HPS risk factors should be given active chemotherapy and supportive therapy.
8.Effect of intermediate care on the recurrence and recurrence rate of elderly patients with cerebral infarction

Objective To observe the effect of intermediate care on recurrence factors and recurrence rate of elderly patients with cerebral infarction.Methods Five Hundred and ninety-eight cases of acute cerebral stroke were divided into two groups,301 cases in the experimental group and 297 cases in the control group.After the acute stage,patients in the experimental group was included into the intermediate care unit.The experimental group was given comprehensive evaluation and multidisciplinary management,including the combination of traditional Chinese and Western medicine treatment,rehabilitation training and physical therapy,medication guidance,nursing and health education.After the acute stage of the disease,the control group patients were in the general ward.The course of treatment was 2 weeks.At baseline(t0),second weeks(t1),sixth months(t2)and twelfth months(t3),the blood pressure(SBP,DBP),fasting blood-glucose(FBG),blood lipid(TC,TG,LDL),serum homocysteine(Hcy)level and the recurrence rate of cerebral infarction in all two groups were observed.Results SBP at t1,t2 and t3 was lower than that at t0 in the experimental group,and the difference was statistically significant(P<0.05).SBP in the experimental group at t2 and t3 was lower than that in the control group.There was significant difference between the experimental group and the control group(P<0.05).DBP at t2 and t3 was lower than that at t0 in the experimental group,and the difference was statistically significant(P<0.05).DP in the experimental group at t2 and t3 was lower than that in the control group.There was significant difference between the experimental group and the control group(P<0.05).FBG at t1((5.34±0.76)mmol/L),t2(5.86±1.05)mmol/L)and t3(5.62±0.89)mmol/L)were lower than that at t0((7.27±2.34)mmol/L),and the difference was statistically significant(P<0.05).FBG in the experimental group at t2 and t3 were lower than that in the control group((6.85±0.92)mmol/L,(6.36±1.03)mmol/L).There was significant difference between two groups(P<0.05).TC at t1((4.86±0.85)mmol/L),t2((4.21±0.76)mmol/L)and t3((3.95±0.64)mmol/L)were lower than that at t0((5.56±1.06)mmol/L),and the difference was statistically significant(P<0.05).TC in the experimental group at t2 and t3 was lower than that in the control group((5.06±0.93)mmol/L,(4.84±0.76)mmol/L).There was significant difference between two groups(P<0.05).TG at t3((1.48±0.26)mmol/L)was lower than that at t0((1.86±1.31)mmol/L),and the difference was statistically significant(P<0.05).TG in the experimental group at t3 was lower than that in the control group((1.71±0.66)mmol/L).There was significant difference between two groups(P<0.05).LDL at t1((3.76±0.92)mmol/L),t2((3.09±0.62)mmol/L)and t3((2.59±0.51)mmol/L)were lower than that at t0((4.59±1.45)mmol/L),and the difference was statistically significant(P<0.05).LDL in the experimental group at t2and t3 was lower than that in the control group((4.09±0.75)mmol/L,(4.12±0.64)mmol/L).There was significant difference between two groups(P<0.05).Hcy at t1((14.16±3.17)μmol/L),t2((10.37±2.59)μmol/L)and t3((10.10±1.86)μmol/L)were lower than that at t0((23.62±5.62)μmol/L),and the difference was statistically significant(P<0.05).Hcy in the experimental group at t2and t3 was lower than that in the control group((18.52±3.64)μmol/L,(16.37±2.75)μmol/L).There was significant difference between two groups(P<0.05).The recurrence rate of cerebral infarction within 1 years was 4.7%(14/301)and 7.1%(21/297)in the experimental group and the control group.There was a decreasing tendency,but no statistical significance(P=0.208).Conclusion The intermediate care may affect the blood pressure,FBG,blood lipids,serum Hcy levels in elderly patients with cerebral infarction,so as to reduce the recurrence rate of cerebral infarction.
9.Clinical observation of four-weekly pirarubicin combined with COP regimen in aggressive non-Hodgkin lymphoma patients with bone marrow involvement

Objective To evaluate the clinic efficacy and adverse effects of four-weekly pirarubicin combined with COP (CTOP-28) regimen in aggressive non-Hodgkin lymphoma patients with bone marrow involvement. Methods 65 aggressive non-Hodgkin lymphoma cases with bone marrow involvement (BMI)from January 2002 to January 2007 were chosen, who were newly diagnosed according to 2001 World Health Organisition (WHO) classification. The response rate and adverse effects of CTOP-28 regimen were retrospectively compared with standard CHOP-21 regimen in these patients. CHOP-21 regimen:eyclophosphamide 750 mg/m2(day 1), vincristine 1.4 mg/m2 (maximum 2 rag, day 1), adrimycin 50 mg/m2 (day1), prednisene 100 nag (day 1 to 5). CTOP-28 regimen: cyclopbosphamide 750 mg/m2 (day 1, 15), vincristine 1.4 mg/m2 (maximum 2 rag, day 1,8,15,22), pirarubicin 25 mg/m2 (day 1,2 and 15,16), prednisone 40 mg/m2(day 1 to 28). Results The complete remission rate (CRR) in CHOP-21 group was less than in CTOP-28group (40 % vs 80 %, P<0.05). The incidence of 3 and 4 degree blood toxicity in CTOP-28 was more than in CHOP-21 (100.0 % vs 50.0 %, P <0.05), but incidence of 1 and 2 degree cardic toxicity was similar as CHOP-21 (8.57 % vs 10.0 %, P>0.05) and 3 and 4 degree cardic toxicity did not occurred in both groups.The gastrointestinal, hepatobiliary and renal toxicities at only 1 and 2 degree took place and no difference wasfound between two groups. No therapy-related mortality occurred in these patients. Conclusion CTOP-28regimen is more effective, safer for aggressive non-Hodgkin lymphoma patients with BMI as induction treatment.
10.Expression of Fbxw7 protein in diffuse large B-cell lymphoma and its clinical significance

Objective:To explore the expression of Fbxw7 protein and its clinical significance in diffuse large B-cell lymphoma (DLBCL), and to provide a basis for prognostic judgement and searching the new therapeutic targets of DLBCL.Methods:A total of 72 patients with newly diagnosed DLBCL who received immunohistochemical detection of c-myc protein from January 2011 to September 2017 in Cancer Hospital Affilicoted to Zhengzhou University were enrolled. The paraffin-embedded specimens after lymph node biopsy and the clinical data of patients were also collected. At the same time, 22 samples of lymph node reactive hyperplasia were selected as the control group. Immunohistochemical method was used to detect the expression of Fbxw7 protein in DLBCL tissues and control tissues. The relationship between the expression of Fbxw7 protein and c-myc protein, the association of Fbxw7 protein expression with DLBCL patients' clinicopathological characteristics, efficacy and prognosis were analyzed.Results:The positive rate of Fbxw7 protein in DLBCL tissues was lower than that in control tissues, and the difference was statistically significant [63.89% (46/72) vs. 86.36% (19/22), χ 2 = 3.990, P = 0.046]. Among DLBCL patients, the positive rate of Fbxw7 protein in non-germinal center B cell (non-GCB) group was lower than that in germinal center B cell (GCB) group, and the difference was statistically significant [48.15% (13/27) vs. 73.33% (33/45), χ 2 = 4.639, P = 0.031]. There were no statistically significant differences in the positive rate of Fbxw7 protein among patients with different age, gender, neoplasm staging, international prognostic index (IPI) scores, B symptom, Eastern Cooperative Oncology Group (ECOG) score, lactate dehydrogenase (LDH) level, β 2 microglobulin level, and therapeutic efficacy after initial treatment (all P > 0.05). In DLBCL tissues, the expression of Fbxw7 and c-myc protein was negatively correlated ( r = -0.255, P = 0.031). The 3-year overall survival (OS) rate and 3-year progression-free survival (PFS) rate (88.3% and 82.0%) of the Fbxw7 positive group were higher than those of the Fbxw7 negative group (70.2% and 60.1%). Cox multivariate analysis showed that the down-regulation of Fbxw7 protein expression was an independent risk factor affecting OS and PFS in DLBCL patients ( HR = 3.656, 95% CI 1.055-12.674, P = 0.041; HR = 2.897, 95% CI 1.092-7.688, P = 0.033). Conclusions:The expression of Fbxw7 protein and c-myc protein in DLBCL patients is negatively correlated. Fbxw7 protein is down-regulated in DLBCL, and it is more obvious in non-GCB subtype. The down-regulated expression of Fbxw7 protein is related to the poor prognosis of DLBCL, and Fbxw7 may become a new therapeutic target of DLBCL.
