1.Analysis on the Use of TCM Decoction Pieces of Tonifying Deficiency Medicine in Jiangsu Hospital of TCM During 2011–2015

Objective To provide references for clinical rational use and purchase of TCM decoction pieces of tonifying deficiency medicine through analysis on TCM decoction pieces of tonifying deficiency medicine in Jiangsu Hospital of TCM during 2011–2015.Methods Excel 2010 software was used to analyze the categories, dosage and sales of the TCM decoction pieces of tonifying deficiency medicine in our hospital during 2011–2015.Results The dosage of TCM decoction pieces for tonifying deficiency medicine in our hospital during 2011–2015 increased from 526434 kg to 800022 kg, with annual growth rate of 11.09%; consumption sum increased from 72151700 yuan to 125066300 yuan, with annual growth rate of 20.20%; dosage rate was 25.97%–27.03%. Among subtypes of tonifying deficiency medicine, the quantity of medicine for tonifying qi was the largest (accounting for 44.64%–46.07%), followed by medicine for tonifying yin (accounting for 20.63%–21.62%), medicine for tonifying blood (accounting for 17.23%–18.93%), and medicine for tonifying yang (accounting for 15.00%–15.17%). Top 5 TCM decoction pieces for tonifying deficiency medicine in the list of annual quantity were Astragali Radix, Atractylodix Macrocephalae Rhizoma, Paeoniae Radix Alba, Dioscoreae Rhizoma, and Codonopsis Radix during 2011–2015. The top of TCM decoction pieces for tonifying deficiency medicine in the list of consumption sum was Pseudostellariae Radix during 2011–2015.Conclusion Both dosage and consumption sum of TCM decoction pieces for tonifying deficiency medicine in our hospital have increased year by year, but the ratio of the total quantity to total amount and medication structure keep stable. TCM decoction pieces for tonifying qi is the greatest in dosage, which is in line with rational clinical medication regularities. Purchasing staff can purchase medicine predictably according to the Results .

4.Inhibition of HER-2 expression by RNA interference in osteosarcoma cells

[Objective]To investigate whether RNA interference(RNAi)could suppress the expression of HER-2 in osteosarcoma OS-732 cells.[Method]OS-732 cells were transsfected using chemically synthesized double stranded RNA(dsRNA)formulated with Lipofectamine 2000.The HER-2 gene expression inhibition was assessed by RT-PCR,Western Blot and immunocytochemistry.The chemosensitivity of transfected cells to cisplatin was determined by MTT measurement.[Result]Sequence specific siRNAs targeting HER-2 down-regulated the expression of HER-2 significantly.Currently,a range of effects on cell physiology,such as growth inhibition or apoptosis,was observed,Introduction of the siRNA into HER-2 positive tumor lines in vitro greatly reduced the expression of the HER-2 mRNA,effects lasted for 10 days,On the 9th day of interference,HER-2 protein expression was down-regulated in interference group.After RNAi,HER-2-siRNA increased thesensitivity of OS-732 to cisplatin by 10-fold.[Conclusion]Sequence specific siRNAs targeting HER-2 can suppress the expression of HER-2,and significantly inhibiting cellular proliferation,increasing the sensitivity of OS-732 to eisplatin.The successful application of HER-2 siRNA for inhibition of proliferation in HER-2 overexpressed cells extends the list of available therapeutic modalities in the treatment of human osteosarcoma.
5.Analysis of 42 cases implanting with autoskull stored utra-hypothermia skull bone of oneself

Objective To analyze the superiority of implanting operation with autoskull stored at ultrahypothermia over low temperature's and frozen skull bone of oneself.Methods 42 case were observed and followed up after operation.Results Incision is healing by firct intention.there were many advantages,such as good biological activity,beautiful appearance,no repellent response,no immunoreaction and hard to infection,etc. After storing 1,3,6 and 12 months,the histological structure of frozen skull observed by electronscope was similar to those of fresh skull.Pestroyed skull bone cells were not found.All cases had no complication during the period of 3 to 12 months(mean 6.5 months) follow-up after operation.After 12 months,X ray and CT scan showed that skulls were healed.Conclusions Implanting operation with autoskull stored at ultra-hypothermia is one of the most effective technique for repairing skull defects.
6.Effects of sleep deprivation on depressive behaviors and hippocampus bcl-xl in rats treated with chronic stresses

Objective To explore the changes of Bcl-xl levels and activities in hippocampus resulting from sleep deprivation, and then to reveal the mechanism for rapid antidepressant aroused by sleep deprivation.Methods 40 SD rats were randomly divided into normal control group, chrinic stress group, sleep deprivation group and tank contral group.10 rats in each group.The depression animal model was established by chronic mild unpredictable stress(CUMS) methods.Sleep deprivation was preformed by the modified multiple platform method ( MMPM ).The animal model and the effect of antidepressant were evaluated by the open field test.The expression levels of Bcl-xl were separately observed by immunohistochemical technology in hippocampus CA1 ,CA3 and DG.Results 1.Compared with the normal control rats, ambulation ( 35.30 ± 18.77,81.30 ± 18.41, P < 0.01 ) and rearing (20.50 ±4.84,27.70 ± 8.19, P<0.05 ) increased ,and the stopping time in the center decreased (4.60 ± 1.35,2.20 ± 1.55, P < 0.01 ) in the CUMS depressant animal model.2.The Bcl-xl average values of optical density (OD) in hippocampus CA1 ,CA3 ,DG of the model group was lower than that of the normal control group significantly (0.1356 ±0.0224,0.1389 ±0.0250,0.1457 ±0.0162;0.1725 ±0.0327,0.1734 ±0.0261,0.1768 ±0.0271; P<0.01 ) ,and that of the sleep deprivation group was higher than that of the model group (0.1621 ± 0.0128,0.1603 ± 0.0137,0.1625 ± 0.0192 ;0.1356 ± 0.0224,0.1389 ± 0.0250,0.1457 ± 0.0162; P < 0.05 ).Conclusion Rats showed depressive behaviors after 21 days stresses,while 72 hours sleep deprivation could reverse this effect.The up regulation of the expression and phosphorylation of Bcl-xl by sleep deprivation may participate in the antidepressant-like effect of sleep deprivation.
7.Study on the risk factors of the occurrence of hepato renal syndrome for patients with acute on chronic hepatitis B liver failure

Objective To investigate and analyze the risk factors of the occurrence of hepato renal syndrome (Hepatorenal syndrome,HRS) for patients with acute on chronic hepatitis B liver failure.Methods Sixty cases of patients with acute on chronic hepatitis B liver failure from January 2009 to December 2013 in our hospital were selected as the research objects.The single factor and multi-factor regression analyses were in patients with the basic clinical data,and the complications and the baseline clinical testing index of patients.The independent risk factors of the occurrence of HRS for patients with acute on chronic hepatitis B liver failure were screened.Results The cases of the occurrence of HRS for patients with acute on chronic hepatitis B liver failure was 17 among 60 cases with a incidence of 28.3 % ; The results of multivariable logistic regression analysis showed that serum albumin,serum sodium,liver function grade (Child-Pugh score),model for end-stage liver disease (MELD) index,primary bacterial peritonitis,upper gastrointestinal hemorrhage,ascites,and hepatic encephalopathy were the risk factors of the occurrence of HRS for patients with acute on chronic hepatitis B liver failure (P < 0.05).Conclusions The occurrence of HRS for patients with acute on chronic hepatitis B liver failure is higher.The various sensitive indicators should be monitored dynanically,and the relevant prevention and treatment measures should be taken in time.It has a significantly scientific merit to improve the prognosis of patients.
8.Cloning of acidic mammalian chitinase gene mutant and its sequence analysis

Objective To clone acidic mammalian chitinase (AMCase) gene and analyze the sequences of the nucleotides and encoded amino acids. Methods Total RNA was isolated from the stomach of BALB/c mouse and the mRNA was reversely transcribed into cDNA. AMCase gene was amplified by polymerase chain reaction and inserted into recombinant clone vector of pMD18-T. Sequence alignment was performed with DNAMAN 4.0 software. Isoelectric point, amino acid composition, signal peptide and transmembrane helices were analyzed with SignalP 3.0 Server and TMHMM Server v.2.0. Results The cloned gene encodes a protein of 473 amino acids with molecular weight of 51.93 kDa and isoelectric point 4.73. Extensive amino acid sequence homology (up to 99.57%) was found when compared with AMCase. Both protein encoded by this cloned gene and AMCase belonged to family 18 chitinase, which was composed of signal peptide (amino acid position 1-21), the catalytic domain (amino acid position 22-391), hinge region (amino acid position 392-425) and chitin binding domain (amino acid position 426-473). There were two mutations of amino acid in the catalytic domain and hinge region with this AMCase mutant. The amino acid of Ala(position 292)was mutated into amino acid of Pro, and the amino acid of Thr(position 404)was mutated into amino acid of Ser. Conclusion This sequence is a mutant of AMCase gene. Identification of the biological characteristics will play important role in the further study of its biological function by molecular biology technique.
9.Analysis on Application of TCM Decoction Pieces for Blood-activating and Stasis-removing in TCM Hospital of Jiangsu Province During 2011-2015

Objective To provide references for rational clinical usage and purchase of TCM decoction pieces for blood-activating and stasis-removing. Methods Excel 2013 software was used to conduct statistical analysis on categories, consumption quantity, consumption sum and annual average growth rate of TCM decoction pieces for blood-activating and stasis-removing during 2011-2015 in TCM Hospital of Jiangsu Province (hereinafter refered to as our hospital). Results The consumption quantity and consumption sum of TCM decoction pieces for blood-activating and stasis-removing in our hospital increased year by year. Among all types of TCM decoction pieces for blood-activating and stasis-removing, TCM decoction pieces for activating blood and adjusting maridians occupied the largest amount, followed by TCM decoction pieces for activating blood and relieving pains, breaking up the static blood and eliminating masses, and blood-activating and wound - curing. The TCM decoction pieces with the biggest annual consumption quantity were Salviae Miltiorrhizae Radix et Rhizoma, Chuanxiong Rhizoma, and Curcumae Radix, and the TCM decoction pieces with the biggest annual consumption sum were Notoginseng Radix et Rhizoma, Salviae Miltiorrhizae Radix et Rhizoma, and Carthami Flos. Conclusion Both the consumption quantity and the consumption sum of the TCM decoction pieces for blood-activating and stasis-removing in our hospital have been increasing year by year. However, the proportion consumption quantity and the consumption sum of TCM decoction pieces and medicine utilization structure have kept stable, and in basically rational use.
10.Plasma Concentration Monitoring of Vancomycin in ICU Patients and Individualized Medication Adjustment

Objective: To monitor vancomycin plasma concentration results by clinical pharmacists in order to participate in individualized medication adjustment.Methods: Clinical pharmacist monitored the blood concentration of vancomycin in special patients, and then designed the individualized medication according to the monitoring results and clinical manifestations of patients.Results: Through the individualized medication adjustment, clinical pharmacist enhanced the efficacy and reduced the incidence of adverse reactions, and improved the level of clinical rational drug use as well.Conclusion: Clinical pharmacist can provide better service for clinics through the practice of individualized precision pharmacy, and reasonable and safe pharmacy service can be provided for patients.
