1.Determination of emodin and berberine hydrochloride in Wangshibaochi Pills by HPLC

AIM: To established a method for determinating emodin and berberine hydrochloride in Wangshi baochi (Pills Radix et Rhizoma Rhei,Rhizoma Coptidis,etc.) by HPLC. METHODS : The separation was performed on Lichrospher C_ 18 column,using methanol-0.1% H_3PO_4(90∶10) and acetonitrile-0.05mol?L -1 KH_2PO_4 (35∶65) as mobile phase,respectively. The detection wavelength was at 290nm and 345nm,respectively. The flow rate was at 1.0mL?min -1 ,the column temperature was room temperature. RESULTS : The linear range of emodin was in the range of 0.1966~0.9830 ?g,r =0.9997, the average recovery was 98.81% and RSD was 1.41%,respectively. The linear range of berberine hydrochloride was 0.1644~0.8220 ?g,r =0.9996,the average recovery was 98.47% and RSD was 2.08%. CONCLUSION : This method is rapid,sensitive and accurate. It can be used for quality control of Wangshi baochi Pills.
2.Study on quality standard of Yupingfeng Oral Liquid

Objective: To established a method for the quality standard of Yupingfeng Oral Liquid(Radix Astragali, Radix Saposhnikoviae and Rhizoma Atractylodis Macrocephalae). Methods:Radix Astragali, Radix Saposhnikoviae and Rhizoma Atractylodis Macrocephalae in oral liquid were identified by TLC. The content of astragaloside IV was determined by HPLC-ELSD. Results:The study showed that spots of samples on TLC can be well separated and the method had strong specificity. The average recovery was 97.75% and RSD was 1.31%. Conclusion:The method is simple, sensitive and accurate. It can be used for quality control of Yupingfeng Oral Liquid.
3.The therapeutic effect of high flow nasal cannula oxygen therapy for the first imported case of Middle East respiratory syndrome to China

ObjectiveTo investigate the value of high flow nasal cannula (HFNC) in treating a patient with Middle East respiratory syndrome (MERS).Methods The effect of HFNC applied in the first imported MERS patient with complication of acute respiratory distress syndrome (ARDS) to China was observed. The patient was admitted to Department of Critical Care Medicine of Huizhou Municipal Central Hospital on May 28th, 2015, and the changes in various clinical parameters and their significance were analyzed.Results A 43-year old male was admitted to negative pressure isolation intensive care unit with the complaint of back ache for 7 days and fever for 2 days. Vital signs and saturation of pulse oximetry (SpO2) were monitored continuously. After admission, ribavirin was given orally for 12 days andα-interferon was administered once on the first day. However, after 2-week anti-virus therapy, the virus test was positive. Ceftriaxone was given on the 4th day, and it was changed to meropenem on the 3rd day for 2 weeks. Immune globulin was given on the 4th day and continued for 1 week. Thymosin-α1 was given on the 8th day and continued for 2 weeks. According to his past history, methimazole had been given continuously for hyperthyroidism and other symptomatic treatment. Oxygen inhalation (6 L/min) was given immediately after admission, but the condition of patient worsened with the following symptoms: frequent cough and obvious shortness of breath. Moreover pleural effusion gradually increased as shown by X-ray. SpO2 was maintained only at about 0.91. Oxygenation index (PaO2/FiO2) decreased to 144 mmHg (1 mmHg = 0.133 kPa). So oxygen inhalation via nasal cannula was changed to HFNC after 2 days. The parameters were set as follows: temperature 34℃, flow rate 20 L/min, fraction of inspired oxygen (FiO2) 0.50. The flow was raised 5 L/min every 10 minutes, and was continued till the target value reached 60 L/min. FiO2 was modified according to SpO2 and PaO2/FiO2. FiO2 was set to 0.80 on the 5th day of admission. Shortness of breath of the patient was improved on the 7th day of admission after the application of HFNC. FiO2 was then decreased to 0.58 as PaO2/FiO2 rose. Then the flow was gradually decreased to 30 L/min. HFNC was reduced with continuous improvement in PaO2/FiO2. HFNC was changed to low flow oxygen inhalation nasal cannula (2-3 L/min) on the 20th day. Oxygen treatment was stopped on the 23rd day, and SpO2 was maintained at 0.98-1.00. Activities on bed were gradually increased. The patient was cured and discharged from hospital on June 26th. The patient showed good tolerance and high compliance during the treatment with HFNC. No nosocomial spread occurred during the treatment.ConclusionsHFNC could improve respiratory function of the patient with MERS obviously, and complication ARDS was prevented. HFNC might reduce nosocomial spread.
4.Immediate breast construction using prosthesi after nipple-areola-sparing mastectomy in 26 cases

Objective To investigate the effect of immediate breast construction using prosthesi after nipple-areola-sparing mastectomy.Methods The immediate breast construction using prosthesi after nippleareola-sparing mastectomy was performed in 26 cases with breast cancer from January 2008 to December 2011 in Jiangsu Cancer Hospital.The postoperative cosmetic results and the complications were observed.The therapeutic effects were followed up.Results All operations were successful.The s,uperior rate of cosmetic result after one month according to JCRT was 88.5 % (23/26).No severe complication was found.After a median follow-up of 26 months (range 3-48 months),there was no recurrence and metastasis.Conclusion The immediate breast construction using prosthesi after nipple-areola-sparing mastectomy is maneuverable with satisfactory aesthetic result and the clinical effect,which deserves the further clinical application.
5.The clinical significance of the change of serum HER2/neu concentration during neoadjuvant chemotherapy of the HER2-overexpressed breast cancer

Objective To explore the change of serum HER2/neu concentration during neoadjuvant chemotherapy for HER2-overexpressed breast cancer and its correlation with the response to the neoadjuvant chemotherapy.Methods The concentration of the serum HER2/neu in 78 cases of HER2-overexpressed advanced breast cancer treated with neoadjuvant chemotherapy were detected with enzyme-linked immunosordent assay(ELISA).The relationship between the serum HER2/neu concentration and the response of neoadjuvant chemotherapy was analyzed.Results The serum HER2/neu concentration of befor and after neoadjuvant chemotherapy was 18.6 ± 10.0ng/ml,11.6 ± 6.lng/ml respectively.The serum HER2/neu concentration decreased significantly(P < 0.001).The response of neoadjuvant chemotherapy was correlated with the change of the serum HER2/neu concentration.The pathologic complete response was correlated with the serum HER2/neu concentration of prechemotherapy and the change of the serum HER2/neu concentration.Conclusion The change of serum HER2/neu concentration may serve as a marker predicting the response of neoadjuvant chemotherapy in HER2-overexpressed breast cancer.
6.Expression of HER3 in HER2-positive breast cancer and its relationship with clinical prognosis

Objective To explore the expression of human epidermal growth factor receptor -3( HER3) in human epidermal growth factor receptor-2(HER2)-positive breast cancer and its relationship with the therapeutic effect of trastuzumab and clinical prognosis .Methods Clinicopathological characteristics of 235 HER2-positive breast cancer patients undergoing surgery in Jiangsu Cancer Hospital from Jan .2007 to Jun.2012 were analyzed retrospectively.The expression of HER3 was detected using immunohistochemisty staining .The expression of HER3 and its correlation with the clinicopathological characteristics were analyzed .All patients were followed-up to find out the impact of HER3 on the disease free survival and the therapeutic effect of trastuzumab .Results The positive rate of HER3 in Luminal B ( HER2 +) and HER2-overexpressing breast cancer was 100/135 (74.1%), and 85/100(85%)respectively.The difference had statistical significance (P<0.05).The histolog-ical grading and the lymph node metastasis were significantly different in Luminal B ( HER2+) breast cancer .The tumor volume , histological grading and lymph node metastasis were significantly different in HER 2-overexpressing breast cancer .The 5-year disease free survival of HER 2-positive breast cancer patients with negative HER 3 was higher than that with positive HER3.The non-relapse survival time was not significantly different between the pos-itive and negative HER 3 expression in Luminal B ( HER2+) breast cancer patients receiving trastuzumab treat-ment , but was significantly different in HER 2-overexpressing breast cancer patients .Conclusions HER3 is cor-related with unfavourable prognosis in HER 2-positive breast cancer .The treatment targeting HER3 may improve the clinical prognosis of both HER 2-positive and HER3-positive breast cancer patients .The HER2-overexpressing breast cancer patients with negative HER 3 may benefit more from trastuzumab treatment .
7.Study on Polyploid Induction and Identification of Nervilia fordii

Objective To investigate the polyploid induction and identification of Nerviliae fordii for harvesting the polyploid plants. Methods The materials and methods for polyploid induction of Nerviliae fordii were screened separately by comparing the induction rates of rhizomes and bulbs under natural conditions and tissue culture environment, and by comparing soaking method with agar method. The effects of colchicine concentration ( 200, 300, 400, 300 mg/L), colchicine action time ( 7, 14, 21, 28 d), DMSO concentration ( 0, 10, 20, 40 mL/L) and KT concentration ( 0, 1.0, 2.0, 4.0 mg/L) on induction rate were observed by orthogonal design method. The polyploid induction in the treated plants was identified by morphology, cytology and chromosome methods. Results After the tissue culture rhizomes were treated with 300 mg/L colchicine, 10 mL/L DMSO, and 2.0 mg/L KT by agar method for 28 days, the polyploid induction rate arrived to 50%, showing better induction effect. The morphology of polyploid plants was characterized by giantism, and the leaf length, leaf width and plant height were respectively 152.17%, 158.67%and 60.90%of those of the diploid plants. The length, width and density of stoma of leaf epidermal cell as well as the number of chloroplast in the treated plants were 138.46%, 153.00%, 59.09% and 109.09% of those of the untreated plants. The results of chromosome identification showed that the amount of the tetraploid ( about 40) was 2 times of the diploid chromosome ( about 20) in the treated plants, proving that the achieved Nerviliae fordii was a tetraploid plant. Conclusion Polyploid plants of Nerviliae Fordii have been successfully obtained, which will supply evidence for improving species, richening seed-breeding resources, and selecting of improved seeds of Nerviliae fordii.
8.Effects of monocyte CD141 and platelet CD36 on peripheral vascular endothelial function in patients with hypothyroidism

Objective To investigate the effects of monocyte CD141 and platelet CD36 on peripheral vascular endothelial function (VEF) in patients with hypothyroidism (PWH). Methods The number of mono-cyte CD141and platelet CD36 from 52 patients wtih PWH and 40 healthy persons (control group) was calculated by flow cytometry. VEF was detected in terms of brachial artery flow-mediated dilatation (FMD) using Doppler ultrasonography. The differences of expressions of monocyte CD141 and platelet CD36 were compared between the two groups, so was FMD. The association of monocyte CD141 and platelet CD36 with FMD (%) was analyzed by Pearson analysis. Results Expression of monocyte CD141 (%) was significantly higher in the PWH group than in the control group (21.79 ± 9.45 vs. 18.84 ± 9.64, P < 0.001), so was expression of platelet CD36 (%) (34.26 ± 10.23 vs. 28.65 ± 9.53, P < 0.001). FMD% was lower in the PWH group than in the control group (8.65 ± 1.97 vs. 11.25 ± 2.72, P < 0.001). CD141 was negatively correlated with FMD% (r = -0.657, P < 0.01), so was CD36 (r = -0.557, P < 0.01). Conclusions Both monocyte CD141 and platelet CD36 are significantly higher in the PWH group than in the control group. CD141 and platelet CD36 are negatively related with VEF.
9.Exploration of cut-off value of TSH during screening of congenital hypothyroidism in the newborns

Positive linear correlation was found between TSH level and detection rate of congenital hypothyroidism (CH). When measured with a cut-off point at 9, 10, 15 and 20 mU/L of TSH, the sensitivity of positive detection rate for CH was 99.77%, 96.80%, 81.25% and 71.88%, respectively. TSH > 9 mU/L seems to be a reasonable cut-off value.
