1.Aesthetic correction of secondary nasal deformity following treatment of bilateral cleft lip

objective: To improve the repair technique of secondary nasal deformity following surgical treatment in the patients with bilateral cleft lip. Methods: 12 cases were treated with the operation technique: bilateral vermilion flaps were inserted into prolabial for the enlongation of upper lip and "V Y incision suture" principle was employed to repaire the nasal deformity. Results: Satisfactory results were achieved in all the 12 cases. Conclusion: The technique is feasible in the reconstruction of Cupid's bow, philtrum and correction of nasal and labia deformities.
2.Analysis of 1262 cases with fractures caused by earthquake injury

Objective To investigate the epidemiological features of fractures caused by earthquake to elevate the quality of prevention and treatment and reduce the mortality and disability rate.Methods A retrospectire study was performed on the data of 1 262 patients with fractures caused by earthquake from May 12,2008 to Aug 31,2008.The following data was statistically analyzed:cause of injury,injured part,associated injuries and treatment.Resultsof the 1 262 cases,11 resulted in deaths;bruise and crush trauma was the major cause (93.98%)of casualties;fractures oflimbs were the main types(55.7%)of fracture;operation was the major treatment.Concluslon Correct treatment is important for fractures caused by earthquake to reduce mortality and disability rate.
3.AmpC Enzyme,ESBLs Production and Antibiotic Resistance in Gram-negative Bacilli Isolated in Intensive Care Unit

OBJECTIVE To investigate the production of AmpC enzyme and ESBLs in Gram-negative bacilli and their drug-resistance to provide basis for reasonable use of antibiotics in clinical practice.METHODS All bacteria were identified by VITEK-32 system and disk test.RESULTS The main five strains of all 184 Gram-negative bacilli were Pseudomonas aeruginosa(24.4%),Acinetobacter baumannii(19.3%),Enterobacter cloacae(14.8%),Escherichia coli(10.2%) and Klebsiella pneumoniae(9.7%).The AmpC enzyme-producing rate was 53.8% and 14.7% in E.cloacae and A.baumannii the ESBLs-producing rate was 35.3%(12/34) and 15.4%(4/26) in A.baumannii and E.cloacae.P.aeruginosa was sensitive to imipenem and the drug-resistance rate of K.pneumoniae was 0%.CONCLUSIONS ESBLs and AmpC enzymes are important mechanisms of the drug-resistance of Gram-negative bacilli.
4.Effect of Telbivudine on CD4 + CD25 + CD127low/- Regulatory T cell in Patients with HBeAg-positive ;Chronic Hepatitis B

Objective To study the effect of telbivudine (TBV) on the ratio of CD4+ CD25+CD127low/-regulatory T cell (Treg) of HBeAg-positive chronic hepatitis B (CHB). Methods The ratios of Treg before and after treatment for 12, 24, 36 and 48 weeks with TBV of thirty-five patients with HBeAg-positive CHB were detected. Results Ratios of Treg were (12.55 ± 1.78)%, (10.42 ± 1.04)%, (9.55 ± 0.74)%, (8.78 ± 0.67)%, (8.74 ± 0.71)%respectively before and after treatment of 12, 24, 36 and 48 weeks and all the values were lower than its former one (P < 0.05), except the 48 weeks′. Conclusions The ratio of Treg of HBeAg (+) CHB decreases after treatment with TBV.
5.Micro-traumatic surgical procedure of loose seton and cutting seton in the treatment of high anorectal fistulae of 136 cases

To explore the therapeutic effect of micro-traumatic surgery of loose seton and cutting seton by rubber bands in the treatment of high anorectal fistulae.Application of cutting seton (truss rubber bands) implemented the high part of fistulae and loose seton (ligation rubber band but non-fastened) for the low part of fistulae.133/136 patients undergoing micro-traumatic surgery were cured by one operation,2 cases had pseudo-healing and there was 1 recurrent case.And the curative rate was 97.8%.The microtraumatic surgery of loose seton and cutting seton by rubber bands in the treatment of high anorectal fistula has such multiple advantages as small incision,minor trauma,lesser pain,faster healing and a shorter course of treatment.And it may preserve the proper anal function and the integrity of anal skin.And its clinical efficacy is satisfactory.
6.Effects of monocyte CD141 and platelet CD36 on peripheral vascular endothelial function in patients with hypothyroidism

Objective To investigate the effects of monocyte CD141 and platelet CD36 on peripheral vascular endothelial function (VEF) in patients with hypothyroidism (PWH). Methods The number of mono-cyte CD141and platelet CD36 from 52 patients wtih PWH and 40 healthy persons (control group) was calculated by flow cytometry. VEF was detected in terms of brachial artery flow-mediated dilatation (FMD) using Doppler ultrasonography. The differences of expressions of monocyte CD141 and platelet CD36 were compared between the two groups, so was FMD. The association of monocyte CD141 and platelet CD36 with FMD (%) was analyzed by Pearson analysis. Results Expression of monocyte CD141 (%) was significantly higher in the PWH group than in the control group (21.79 ± 9.45 vs. 18.84 ± 9.64, P < 0.001), so was expression of platelet CD36 (%) (34.26 ± 10.23 vs. 28.65 ± 9.53, P < 0.001). FMD% was lower in the PWH group than in the control group (8.65 ± 1.97 vs. 11.25 ± 2.72, P < 0.001). CD141 was negatively correlated with FMD% (r = -0.657, P < 0.01), so was CD36 (r = -0.557, P < 0.01). Conclusions Both monocyte CD141 and platelet CD36 are significantly higher in the PWH group than in the control group. CD141 and platelet CD36 are negatively related with VEF.
7.Impact of psychological intervention on response and defense style of AIDS patients

Objective To explore the impact of psychological intervention on response and defense style of AIDS patients.Methods 88 AIDS patients were selected and investigated using medical response (MCMQ) and defense style (DSQ) survey,the scores before and after psychological intervention were compared.Results Mental health problems included fear,afraid,pessimism and worry,the response style included face,avoid,yield.Scores showed significant difference before and after intervention among patients choosing immature-type,mature-type and cover-up defense styles.Conclusions Psychological intervention can promote rehabilitation and health of AIDS patients as well as improve their life quality and prevent disease relapse.
8.Clinical Analysis of Spinal Cord Injury in Sichuan Earthquake: 198 case report

Objective To analyze the constituent ratio and clinical characteristics of spine trauma and spinal cord injury of the survivors in the Sichuan earthquake.Methods198 cases wounded in the Sichuan earthquake were divided into four groups according to age for collecting the type of fracture, nerve damage, spinal fractures combined injuries of data from multiple sites, analyzing their clinical characteristics.ResultsFracture types are most common compression fractures (49.3%), followed by the burst fractures (45.9%). 105 of them had spinal cord injury, ASIA grade: A grade: 20 cases, B Grade: 18 cases, C grade: 25 cases, D Grade: 42 cases. 41 cases had combined injury. T12, L1, and L2 vertebral fractures were common types.ConclusionCompression fractures and burst fractures as spinal injuries occurred commonly in Sichuan earthquake mainly in the 18~65 years old, and spinal cord injury accounted for 53.03% spinal injury.
9.UV-inactivated Epstein-Barr Virus Protein Regulating the Production of Antikeratin Autoantibodies

Objective To investigate UV- or heat-inactivated Epstein-Barr virus(EBV)stimulating the production of anti-keratin autoantibody(AK auto Ab)in cultured human umbilical cord blood B cells. Methods Mononuclear cells were isolated routinely from umbilical cord blood, in which monocytes, NK cells and cytotoxicity T cells were eliminated by L-leucine methyl ester method, and T cells were removed by sheep red blood cells(SRBCs)treated with 2-amino ethyl-isothiouronium bromide (AET). The purified B cells were treated with UV- or heat-inactivated EBV respectively and then cultured in complete IMDM. CD5, CD3, CD4 and CD8 cells were detected by flow cytometry. IgG and IgM of AK auto Ab were measured by ELISA in the supernatant which came from the B cells treated by UV-inactivated EBV or EBV-transformed B cells respectively. Results In UV-inactivated EBV group CD5+B cells accounted for 43% and 47% of all cells detected on the 14th and 28th day, respectively. No CD3, CD4 and CD8 cells were detected during this period. In UV-inactivated EBV group the AK auto Ab of IgG and IgM increased significantly on the 18th and 26th day, respectively (P 0.05). On the 40th day the AK auto Ab of IgG and IgM were significantly higher in EBV-transformed B cell group than those in UV-inactivated EBV group. Conclusions UV-inactivated EBV is able to induce AK auto Ab production but heat-inactivated EBV does not, which suggests that EBV protein might be the effective agent in inducing the production of AK auto Ab.
10.Expression of high mobility group box 1 protein and the receptor for advanced glycation end products in patients with primary gouty arthritis

Objective To investigate the role of high mobility group box 1 protein(HMGB1) and the receptor for advanced glycation end products (RAGE) in the pathogenesis of primary gouty arthritis (GA).Methods Enzyme-linked immunosorbent assay(ELISA) was used to determine the level of plasma HMGB1 in 68 acute gout (AG),48 quiescent gout (QG) and 45 healthy control(HC).Real-time quantitative polymerase chain reaction (RT-qPCR) was employed to measure the expression of HMGB1 and RAGE mRNA in the peripheral blood mononuclear cells (PBMCs) in 68 AG,48 QG and 94 HC.One way ANOVA or Wilcoxon test and Spearman's correlations were used for statistical analysis.Results The level of plasma HMGB1,PBMCs HMGB1 and RAGE mRNA were significantly higher in GA than that in HC [(24±34) ng/ml,0.019±0.029,0.000 5±0.000 3] (P<0.05),while the level of plasma HMGB1 and PBMCs HMGB1 mRNA were significantly higher in AG [(222±178) ng/ml,0.235±0.954,0.001 5±0.003 5] than that in QG [(107±176) ng/ml,0.044±0.117,0.001 3±0.000 9] (P<0.05),and the level of PBMCs RAGE mRNA was higher in AG than that in QG (P>0.05).In the GA patients,the level of plasma HMGB1 was positively correlated with white blood cell count,neutrophile granulocytes count,mononuclear cells and erythrocyte sedimentation rate (r=0.34,0.44,0.39,0.33; P<0.05),while negatively correlated with apolipoprotein A1 (r=-0.28,P<0.05); the level of PBMCs HMGB1 mRNA was positively correlated with RAGE mRNA,white blood cell counts,neutrophil counts,lymphocyte counts,serum total cholesterol level,low density lipoprotein level and apolipoprotein B100 level (r=0.29,0.36,0.26,0.28,0.29; P<0.05),while negatively correlated with high density lipoprotein (r=-0.30,P<0.01); the level of PBMCs RAGE mRNA was positively correlated with lymphocyte counts,total cholesterol and apolipoprotein B100 (r=0.35,0.35,0.44; P<0.05).Conclusion HMGB1 and its signaling pathway may play important role in the pathogenesis of gouty arthritis,which may also be involved in the regulation of the lipid metabolism of gout.
