1.Study of pain heterogeneity in transfusion with different gestures

Objective To investigate pain heterogeneity in transfusion with different gestures in order to find the best gesture.Methods 80 cases were randomly selected and transfused in the back of the hand with three infusion gestures,which including horizontally laid hand with extended fingers for 20 min as control,horizontally laid hand with relaxed fist for 20 min and horizontally laid hand with fist for 20 min in time sequence.Mark-Gail method was used to assess the pain grade.Results Pain was enhanced in horizontally laid hand with relaxed fist group and horizontally laid hand with fist group when comparing with horizontally laid hand with extended fingers group.And pain was more stronger in horizontally laid hand with fist group than that of horizontally laid hand with relaxed fist group.Conclusions Transfusion with horizontally laid hand combining with extended fingers implicated minimum pain and was the best gesture.Horizontally laid hand with fist flat finger displayed the strongest pain and should be banned.

Biological safety is an important international issue. For this reason, WHO published Laboratory Biosafety Manual in 1983, 2003 and 2005. According to the concepts of Biological safety, countries all over the world have developed and established their own national codes of practice for the safe handling of pathogenic microorganism for clinical, studying and epidemiological purposes. In BSL-4 laboratories, individual biosecurity is very important for staffs when they are faced with dangerous level-4 pathogens. In this paper, individual protective equipment and correlative technical requirements for BSL-4 laboratories are introduced.
4.Changes in plasma pro-brain natriuretic peptide concentration before and after interventional therapy in congenital heart diseases

Objective To investigate the relationship between plasma pro-brain natriuretic peptide (N-pro-BNP) level and heart function, as its well as change before and after interventional therapy. Methods Fifty-eight children with left to right shunt congenital heart diseases were enrolled from July, 2004 to Jan, 2005. Clinical heart function were categorized and plasma N-pro-BNP concentration were detected. The ejection fraction (EF), fractional shortening (FS) and left ventricular and diastolic dimension (LVDd) were measured by two-dimensional echocardiography. The relationship between plasma pro-BNP concentration and the echo parameters, and its change before and after interventional therapy were analyzed. 5.Application effect of self- edited doggerel on health education of senile AIDS patients in northern Guangxi region

Objective To study the effects of self- edited doggerel on health education of senile AIDS patients in northern Guangxi region, in order to seek a suitable and effective way for the senile AIDS patients' health education. Methods 200 senile patients who were inpatients in dermatological department and examinees in the center of health examination of our hospital were randomly divided into the experimental group and the control group from August to December in 2009, 100 cases in each group, the experimental group was educated by a method of adopting a self-edited doggerel, the control group was educated by traditional way including manual + video or a series of special lectures, Two groups were carried out questionnaire survey designed by ourselves before and after the education during a period of a week and then evaluated the effects of health education. Results Compared with the rate before adopting self-edited doggerel, the knowing rate of AIDS and regulations of the experimental group was above 90%,there was a significant increase; and compared with the control group, the rate was also higher than the latter. The behaviors and attitudes of the experimental group had a significant positive improvement, but showed no significant difference between the two groups. Conclusions The doggerel is humorous, rhyming, simple and easy-going, it is so user-friendly, easy to understand and to remember that it is a suitable and effective way for the health education of senile AIDS patients.
6.Influence of dietary nutrition intervention on patients with psoriasis

Objective To study on the effect of dietary nutrition intervention in community on nutrition knowledge and dietary compliance in patients with psoriasis.Methods 40 patients with psoriasis were given a balanced diet pagoda and food exchange method,along with psoriasis nutrition knowledge manual to make individual and concise diet prescription.The nutrition knowledge and dietary compliance of patients before and after the intervention were compared.Results In a nutrition knowledge test,before the intervention,31 patients obtained a score below 60 which accounted for 77.5%,and 9 patients got higher score which meant 22.5% patients had a good dietary compliance.After the intervention,18 patients (45.0%) got a score below 60,while 22 (55.5%) patients got a good score,the results after the intervention were statistically higher than those before the intervention.Conclusions Patients with psoriasis have poor cognitions and bad dietary compliance.After dietary nutrition intervention,the nutrition knowledge is enhanced and the dietary compliance gets higher,the dietary structure is improved,and consequently accelerate the recovery of disease.
7.Complications related to transcatheter occlusion of atrial septal defect with Amplatzer septal occluder in children

Objective To probe into the causes,therapeutic and preventive methods of complications associated with transcatheter occlusion of atrial septal defect using the Amplatzer septal occluder in children. Methods Complications occurred in 9 of 211 cases. The complications in procedure of occlusion were those in which there appeared systemic circulatory systemic air embolism in 2,pulmonary air embolism in 1,pericardial tamponade in 1,ASO malposition requiring emergency surgical removal in 1,transient atrial extrasystoles in 1 and sizing balloon rupture in 1 case. Two cases with postoperative complications were found in the follow-up studies. These included perforation of mitral valve and ASO partially dislodged. In this study,5 children with intra-procedure complication received emergency therapy including surgical intervention,and others were needed only in the follow-up,as the complications were transient or asymptomatic. Results There were no children′s deaths in this study. The 5 cases that received treatment completely healed,and the others with intra-procedure complications were also had no sequela. Cases with mitral valve perforation and ASO partially dislodged were still in our follow-up studies,as the 2 patients had no symptoms. Conclusion Air embolisms occurred easily in atrial septal defect cases that received ASO therapy. Complications were mainly caused by inappropriate operative procedure and some complications needed emergency treatments. Follow-up studies were important to cases with transcatheter occlusion therapy.
8.Perioperative protective procedures of myocardium in patients undergoing heart-valve replacement

The objective of this retrospective study was to evaluate the myocardial protection of different periop erative procedures in 100 cases with heart-valve replacement (HVR) between 1980 and 1992. Before Feburary. 1989. HVRs of 48 cases (group Ⅰ) were undertaken with hypothermia (28C=30C) and cold cardioplegic solution (4C. 500ml). under morphine-diazepam-pancuronium anesthesia: following cardiac resuscitation, the procedures were performed including blood transfusion, diuresis, supplemental KCI. inotropic support with dopomine, norepincphrine and aramine. and continuous mechnical ventilation. Since 1989, the procedures were increased to be conducted in 52 cases (group Ⅱ) as followed: A. preoperative managements for achiving the optimum status of cardiac function: 1. oxygen therapy by mask. 2. antirhenmatic treatment for patients aged less than 35 years. 3. oral isosorbide and captopril for pulmonary hypertension, 4, infusion with fructose 1.6-diphusphatc and glucose-insulin-KC1 mixture. 5. clinical dose of digitora and diuretics for cardiac failure; B. methods during surgery: 1. anesthesia was induced with fentanyl, midazolam and pancuronium. and maintained with inhalation of nitrous oxide and infusion of fentanyl at 30-35?g/kg/min, 2, hy pothermia (24C-27C) and cold cardioplegic solution(600-1000ml) 3. immediately before the blockage of aorta was overcome. the artificial perfusion blood pressure was reduced; afterwards, it was gradually recovered, in order to prevent myocardium from injury of reperfusion. 4. intra-and extracoporal circulations worked together over half of blokage time of arota, to improve myoardial oxygen delivery; C. treatments in early postoperative stage: 1. continuous positive end-expiratory pressure ventilation, 2. infusion with magnesium-potassium aspartyl: and the others were similar to those before 1989. 9.Evaluation of effects of cluster-based strategies for prevention of the fibrosclerosis complications of patients with systemic scleroderma

objective To explore the effect of application of cluster-based strategies on preventing the complication of fibrosclerosis of patients with systemic scleroderma.Methods A total of 59 patients with systemic scleroderma were randomly assigned to the experimental group (34 cases) and the control group (25 cases) during the period of hospitalization in our department.The experimental group took the cluster-based strategies after admission,including a gymnastics,two kinds of respiratory breathing,three kinds of habits,four kinds of protection to prevent the complication of fibrosclerosis.The control group received the conventional care procedures of dermatology.The skin stiffness,joint function,Raynaud's phenomenon and swallowing function were evaluated when patients were admitted and before discharge in two groups.The length of stay between the two groups was also compared.Results The skin stiffness,joint function,Raynaud's phenomenon and swallowing function scored (14.74±1.33),(7.06±0.69),(0.88±0.33),(0.54±0.21) in the experimental group,significantly better than those of the control group,(24.08±1.12),(13.48±0.77),(1.24±0.60),(0.96±0.34),P < 0.01.The length of stay in the experimental group and the control group was (23.65±5.45),(32.00±6.56) days,t=-5.340,P < 0.01.Conclusions Application of cluster-based strategies can be effective against the fibrosclerosis damage of patients with systemic scleroderma and reduce the occurrence of complications.
10.Negative immune regulatory molecule TIPE2 for treating SLE mice through regulating macrophage subtype

Objective To investigate the role of tumor necrosis factor (TNF)-alpha-induced protein 8-like 2 (TIPE2) for regulating the macrophage polarization in systemic lupus erythematosus and its curative effects on experimental SLE mice.Methods The mice were treated with activated lymphocytes derived DNA (ALD-DNA) for inducing mice model,randomly divided into AAV-scr control group and AAV-TIPE2 experimental group,and injected with AAV-TIPE2 or AAV-scr virus solution from the tail vein of mice.The expression of TIPE2 mRNA and protein in polarized macrophages,serum dsDNA antibody titer,urine protein and renal pathological index were detected.Results (1) The TIPE2 expression level of TIPE2 mRNA and protein in AAV-TIPE2-transfected cells was 13.5±1.6 times and 10.8±1.6 times of AAV-scr control group respectively.(2) M2 macrophage specific molecule MGL+ was 59.6% in AAV-TIPE2 group and MGL + cells in the AAV-scr group was 8.4%.M2/M1 odds ratio of AAV-TIPE2 experimental group to AAV-scr control group was 16.(3) The recombinant TIPE2 adenovirus related vector could stably expressed in transfected HEK-293.In vitro and in vivo experiments confirmed that AAV-TIPE2 was able to induce M2 polarization of macrophages in ALD-DNA-induced lupus mice.(4) The serum anti-dsDNA antibody,urinary protein and renal pathology in the AAV-TIPE2 group were significantly lower than those in the AAV-scr group(P<0.01).Conclusion TIPE2 alleviates the disease condition of ALD-DNA induced SLE mice through induction of macrophage polarization to M2 phenotype,which may be used as a promising therapeutic method for ALD-DNA induced SLE mice.
