2.Clinical value of drug sensitivity test of ovarian tumor cells in vitro

Objective To evaluate the clinical value of tumor chemosensitivity test in the guidance of the advanced epithelial ovarian carcinoma patients treated with chemotherapy.Methods Women with ovarian cancer were enrolled and fresh tissue samples were collected for chemoresponse testing.Oncologists chose a drug for each patient according to the patient’ s condition by double blind method.Each treatment was classified by the assay as: sensitivity (S); moderate sensitivity (I);resistant (R).Progression-free survival (PFS) and overall survival (OS) were detection.The relationship between treatment response and PFS or OS was analysis.Results 262 patients were enrolled.The PFS and OS were significantly improved in sensitivity patients (S) after chemotherapy,but no significant difference were found in I and R groups.In I+R groups (HR=0.66,P=0.008) , median PFS was 8.9 months, OS was 5.8 months.Chemotherapy response test results was consistent with clinical results cisplatin sensitive and resistant ( HR:0.72 vs.0.66).Multi factor regression analysis showed that tumor chemosensitivity test was independent prognostic factors (HR =0.66, P =0.021).The average OS extended for 14 months (37.6 months for the S group vs.group I +R 23.8 months,HR =0.62, P =0.011).Conclusion Tumor chemosensitivity assay is correlated with the clinical effects and might play an important role in guiding the recurrent epithelial ovarian cancer patients with individualized chemotherapy.
3.Effect of fructose sodium diphosphate and alpha-lipoic acid on diabetic peripheral neuropathy

Objective To assess the effect of fructose sodium diphosphate and alpha-lipoic acid on diabetic peripheral neuropathy. Methods Sixty-six patients with diabetic peripheral neuropathy were randomly divided into two groups, additional to the regular treatment to control blood glucose, 34 patients in the treatment grouptreated with fructse sodium diphosphate 10 g and alpha-lipoic acid 0.45 g daily through intravenous drip for two weeks (3 patients dropped out due to un-tolerance), 32 patients in the control group treated with alpha-lipoic acid 0.45 g dai-ly through intravenous drip for two weeks. All patients were examined on the improvement of symptoms, motor and sensory nerve conduction velocity of the median nerve and nervus peroneus communis before and after the treatment. Results After treatment for two weeks, the overall effect rate in the treatment group was 93.55 % (28/31), which was significantly higher than that in the control group(68.75%,22/32) (P < 0.05). In the treatment group, the motor nerve conduction velocity for the median nerve and nervus peroneus communis was 47.7 (SD:3.8) m/s and 41.6 (SD: 3.3) m/s after treatment, which was significantly higher than that before treatment (42.5 [SD:2.5] and 36.0[SD:2.8] m/s,respectively) (P <0.05). In the control group, the motor nerve conduction velocity for the median nerve and nervus peroneus communis was 43.4 (SD: 2.0) m/s and 39.6 (SD: 3.3) m/s after treat-ment, which was significantly higher than that before treatment (41.9 [SD16.0] and 36.2 [SD: 1.9] m/s, respec-tively) (P < 0.05). In the treatment group,the sensory nerve conduction velocity for the median nerve and nervus peroneus communis was 40.3 (SD: 1.8) m/s and 34.1 (SD: 2.3) m/s after treatment, which was significantly higher than that before treatment (35.2 [SD:2.3] and 28.8 [SD:2.5] m/s,rospectively) (P <0.05). In the control group,the sensory nerve conduction velocity for the median nerve and nervus peroneus communis was 37.7 (SD:1.7) m/s and 30.8 (SD:2.6) m/s after treatment,which was significantly higher than that before treatment (34.8 [SD :3.1] and 27.8 [SD :2.4] m/s, respectively) (P < 0.05). Furthermore, the improvements in the treat-ment group was significantly higher than those of the control group (P < 0.01). Conclusions Fructse sodium si-phosphate combined with alpha-lipoic acid is more effective to treat diabetic peripheral neuropathy than alpha-lipoic acid only.
4.ANTIMICROBIAL EFFICIENCY OF NITROPROPANE DERIVATIVES AGAINST FUSARIUM

The antimicrobial activity of nitropropane compound 791224 against several phytopachogenic fungi and Fusarium was studied in vitro. Its inhibitory effect on mycelial growth of Fusarium oxysporum f. sp. vasinfectum (Atk.) Snyder et Hansen and Fusarium graminearum Schwabe showed a much stronger than that on the other phytopathogenic fungi tested. The inhibition of 791224 on all tested Fusarium showed excellent. Its inhibitory effect values ranged from 54.2 to 100% at 5 ppm for 12 tested Fusarium whereas the respective values of carbendazim ranged from 0 to 27.3%. The inhibitory effect of 791224 at 5 ppm on Fusarium lateritium Nees reached to 100% whereas for carbendazim it was 0, i.e.100 times lower than that of 791224. In green house, the control efficiency of 791224 on Fusarium solani sothora japonico H. has been tested and showed a promising result.
5.Prevention and Control of Methicillin-resistant Staphylococcus aureus Infection in ICU

OBJECTIVE To investigate and analyze the cause of the cross infection of Methicillin-resistant Staphylococcus aureus(MRSA) in ICU patients,and to apporach its preventive and control measures.METHODS Eiological inspection and sanitary monitoring have been done in ICU where there were two MRSA patients from June 6th,2008 to August 1st,2008.RESULTS MRSA has been detected on the hands of the ICU medical staff,on the bed-desks,on the switches of the vacuum extractor and gas supply unit on the wall,and in the NS for inhaling sputum.The separation rate were 37.5%,20.0%,54.5%,37.5%,87.5%.CONCLUSIONS ICU patients are prone to be infected with MRSA,which has a strong drug resistance and the infection is difficult to be treated.The cross infection can be prevented and controlled by rational administration,sterile operation and insulation.
6.Efficacies of rhGH alone and rhGH combined with stanozolol therapies on growth velocity of girls with Turner syndrome

Objective To compare the different efficacies of recombinant human growth hormone (rhGH) alone and rhGH combined with low-does stanozolol on growth velocity (GV) of girls with Turner syndrome (TS). Methods 51 girls with TS were divided into two groups: Group 1 (n = 23) were treated with rhGH alone and group 2 (n = 28) with rhGH combined with low-does stanozolol both for more than six months. The two groups were compared in terms of GV, height standard deviation score for chronologic age (HtSDSCA), HtSDS for bone age (HtSDSBA), HtSDS (ΔHtSDS) and the ratio of ΔBA/ΔCA. Results In the first year, the GV was (6.29 ± 1.44) and (8.13 ± 1.87) cm/a in Group 1 and Group 2, respectively. HtSDSCA changed from (-3.51 ± 0.99) to (-3.19 ± 1.09) and (-4.21 ± 1.19) to (-3.43 ± 1.06), and ΔBA/ΔCA was (0.60 ± 0.39) and (0.77 ± 0.56) in Group 1 and Group 2, respectively. The GV and ΔHtSDS in Group 2 were significantly better than Group 1 (P < 0.05). The GV was negatively correlated with the age. Conclusion Compared with the therapy with rhGH alone, the one with rhGH combined with low-dose stanazolol is more effective in improving GV without accelerating bone maturation among the girls with Turner syndrome.
7.Effect of gonadotropin-releasing hormone analogue treatment on body mass index in central precocious puberty or early and fast puberty girls

Objective To study the effect on body mass index ( BMI ) caused by treatment with Gonadotropin-releasing hormone analogs ( GnRHa) in girls with central precocious puberty ( CPP) or early and fast puberty ( EFP ) . Methods The BMI in 318 girls ( 227 CPP and 91 EFP ) treated with GnRHa alone was analyzed. Among them 89 were followed up to their final adult height. Results Before GnRHa treatment was started, thegirlswithCPPandEFPhadameanBMIstandarddeviationscore(SDS)forchronologicalage(BMISDSCA)of0.39 ±0.84andforboneage(BMISDSBA)of-0.11±0.69. Attheendoftreatment,themeanBMISDSCAwas0.59±1.01 and BMISDSBA was 0. 24 ± 0. 89, both were significantly higher than those at initiation. The increment in BMISDSBA (0.38±0.50)wasgreaterthanthatinBMISDSCA(0.21±0.56). Moreover,theratioofoverweight(BMI>85%)was significantly elevated. BMISDSCA of 89 girls who were visited at their final adult height was 0. 17 ± 1. 15, which was similar with BMISDS at initiation and in normal population. Conclusion The mean BMISDSCA of CPP and EFP was significantly higher than the general population, while BMISDSBA was significantly lower. During GnRHa treatment, BMISDS tended to be elevated. But it was reversible, for the mean BMISDS was back to normal at final adult height.
8.Clinical characters of childhood adrenocortical tumors

Objective To analyze the clinical characters of childhood adrenocortical tumors, and to enhance the knowledge of diagnosis of this disease. Methods A retrospective analysis of clinical characters,laboratory tests,and imaging findings in 31 cases of childhood adrenocortical tumors was carried out. Results 16 cases of adenoma and 15 cases of carcinoma were included. The average age was (4.49±3.51) years old, and 67.7% of the patients were younger than 5 years old. The ratio of male to female was 1.0: 1.4. 12 patients presented only precocious sexual development, 4 patients presented only Cushing syndrome, 10 patients showed sexual precocity combined with Cushing syndrome, and 5 patients did not have any endocrine abnormalities. Raised testosterone level in 92.3% of these patients was the most common finding in laboratory tests. Only 12.5% of ultrasound images and 20.8% of CT images were consistent with pathologic diagnosis. Conclusions The clinical manifestations of adrenocortical tumors in childhood are precocious sexual development, Cushing's syndrome, or nonfunctional. The common laboratory findings are elevation of sex hormone and disorder of cortisol circadian rhythm. Precocious sexual development and elevation of androgens are more common in childhood adrenocortical tumor than those in adults. Imaging usually cannot give proper diagnosis. Final diagnosis should be established by clinical features, laboratory tests, imaging, and pathologic results.
9.Clinical research of oral compound polyethylene glycol electrolyte powder in different ways before colonoscopy

Objective To study the different oral compound polyethylene glycol electrolyte powder on the effect of bowel preparation before colonoscopy.Methods A total of 602 inpatients or outpatients were selected from department of gastroenterology,and they were randomly divided into tranditional group(n =288)and improvement group(n =314).All patients took a dose of two box of compound polyethylene glycol electrolyte powder 4 hours before colonoscopy.Endoscopic doctor investigated the antibiotic tolerance,adverse reactions,heart rate,blood pressure,blood electrolytes and blood glucose during treatment in form of questionnaire with single blind Bosdon bowel preparation scale (BBPS)score.Results The BBPS scores of the tranditional group and improvement group were (7.03 ± 1.24)points,(7.82 ±1.12)points,the score of the improvement group was significantly higher than the traditional group(t =2.259,P <0.05).The taste scores were (1.89 ±0.07)points,(1.25 ±0.09)points (t =2.624,P <0.05),the fully taking rates were 90.28%,95.86%(χ2 =7.375,P <0.05 ),and the second taking rates were 88.54%,93.95%(χ2 =5.566,P <0.05).In contract,the score of adverse reactions of the improvement group was significantly lower than that of the traditional group [(2.58 ±0.07)points vs(2.22 ±0.09)points,t =2.068,P <0.05].There was no significant difference of colonoscopy time between the two groups[(10.78 ±4.34)min vs (9.89 ± 1.77)min,t =1.766,P >0.05 ],althought the improvement group was shorter than the tranditional group. Conclusion The modified way of oral compound polyethylene glycol electrolyte powder not only can improve the qulity of intestinal cleaning,but also acts as a more effective method in preparation of colonoscopy due to its high tolerance, good comfort,less pain and high acceptance of patients.Therefore,it is worthy of clinical promotion and application.
10.Comparison of transcatheter intervention and surgical operation in pulmonary atresia with intact ventricular septum

Objective To compare the results of transcatheter intervention (TI) and surgical operation (SO) in the pri-mary treatment for pulmonary atrcsia with intact ventricular septum (PA/IVS). Methods From January 2006 to May 2009, 25 patients (20 male,5 female) with PA/IVS were treated. The age at treatment was from 2 days to 8 months. The body weight was from 2.1 kg to 6.7 kg. All patients had mild to moderate hypoplasia of the right ventricle (the Z-valvue of the tricuspid valve: from -2 to 1.5) with tripatite right ventricle and without coronary artery-right ventricular fistula. Eight patients under-went TI and 17 patients underwent SO. Results The primary procedure was successful in 7 patients (88%) in TI group and in 16 patients (94%) in SO group. There were 2 deaths (1 in each group). There was no significant difference regarding re-sidual pulmonary stenosis between two groups. The ventilation time and the days of hospital stay were shorter in TI group than in SO group. All the survivors were followed up for 3-36 months. One patient required repeat balloon dilation in TI group. While in SO group, one required reoperation and one required balloon dilation for PS, and 2 patients were waiting for balloon dilation. Twenty-one patients have achieved complete biventricular circulation. Two patients were scheduled for Glenn shunt operation later (1 in each group). Conclusion For PA/IVS patients with mild or moderate right ventricle hypoplasia, tran-scatheter intervention is a better alternative than surgical operation in the primary treatment.
