1.Clinical study on cognitive function in ischemic stroke patients with cerebral microbleeds

Objective To investigate the impact of cerebral microbleeds (CMBs) on cognitive function in ischemic stroke patients.Methods A total of 268 acute ischemic stroke patients recruited in the Ningbo First Hospital from January 2014 to June 2015 were divided into CMBs group (199 patients) and non-CMBs group (69 patients) according to whether complicated with CMBs by susceptibility-weighted imaging (SWI).According to the microbleed number, CMBs group patients were divided into 2 grades: grade 1 (1-5 CMBs) and grade 2 (≥6 CMBs). Montreal Cognitive Assessment (MoCA) was used to evaluate and compare the global cognitive function and cognitive domains of the patients. Results The total MoCA score and the scores of visuospatial/executive, attention domains in CMBs group were 20.95±4.53, 2.53±1.09, 3.83±0.97, while those in non-CMBs group were 26.82±1.25, 3.16±1.24 and 4.91±0.84. The total MoCA score and the scores of visuospatial/executive, attention domains were significantly lower in CMBs group than those in non-CMBs group (t=16.59, P<0.01;t=3.75, P<0.01;t=8.83, P<0.01). The total MoCA score and the score of attention domain in grade 1 CMBs group were 21.53±4.61 and 4.11±0.91 , which were significantly lower than those in non-CMBs group (t=14.09, P<0.01;t=14.23, P<0.01). Whereas the total MoCA score and the scores of visuospatial/executive, attention, orientation domains in grade 1 CMBs group were 21.53±4.61, 2.88±1.06, 4.11±0.91, 4.96±0.40, which were significantly higher than those in grade 2 CMBs group (18.58±3.08, 2.23±0.95, 3.63±1.01, 3.85±0.39, respectively;t=2.85, P<0.01;t=2.54, P<0.05;t=5.63, P<0.01;t=2.58, P<0.01). Multivariate regression analysis showed that independent risk factors of MoCA scores in ischemic stroke patients included CMBs (OR=3.15, 95% CI 1.28-5.12, P=0.005) and the number of CMBs (OR=1.73,95% CI 1.08-2.32,P=0.031). Conclusions CMBs and the number of CMBs were independently associated with cognitive impairment in ischemic stroke patients. And with the increasing of the microbleed number, the impairments of certain cognitive domains were more obviously.
2.Microarray gene chip analysis and action of yiyiren oil on anti-tumor metastasis reactive gene

BACKGROUND: From yiyiren oil is extracted kanglaite injection that has been extensively used against lung cancer for its function against cancer. It is revealed that kanglaite injection inhibits many strains of cancer cells including LA795 metastatic lung cancer. Genechip is used here for detection of metastasis reactive differentially expressed genes in mice that carry LA795 metastatic lung cancer.OBJECTIVE: To detect the metastasis reactive diferentially expressed genes with genechip technology in metastatic cancer carrying mice that treated with y iyiren oil injection(kanglaite injection).DESIGN: It was a controlled trial with mice as subjects.SETTING: Oncology Department of Xiyuan Hospital Affiliated to China Academy of Traditional Chinese Medicine.MATERIALS: The trial was conducted from February through August 2003. Forty mice were implanted with T739 strain lung adenocarcinoma cells and were randomly and averagely assigned into 2 groups.METHODS: mRNA probe was made from cancer sample of the mice. ScanArray 3000 scanner was used to scan the fluorescence signal in cDNA expression genechip that contained 4096 mouse genes in order to detect the differentially expressed genes.MAIN OUTCOME MEASURES: The differentially expressed genes in the two groups of mice.RESULTS: The mice genes expressed differentially between the kanglaite injection group and control group. After examination being repeated for 3 times, 27 genes presented different expression; 25 genes expression increased in both groups and 2 genes decreased. Among these differentially expressed genes, 12 genes were of the mouse muscle cDNA. It was also observed that 6 genes might be associated with the occurrence and development of tumor.CONCLUSION: Gene expression spectrum genechip is useful in effectively detect the relevent differentially expressed gene induced by kanglaite injection.
3.Situation analysis of medical social support and self-perceived burden in elderly patients with chronic obstructive pulmonary diseases in Chengdu

Objective To analyze the situation of medical social support and self-perceived burden in elderly patients with chronic obstructive pulmonary diseases (COPD) in Chengdu,explore the influence of medical social support for patients with family oxygen cure.Methods A convenient sampling was conducted in 4 hospitals in Chengdu,and selected totally 124 elderly patients with COPD in January 2013 as the research object.Results Those patients' average daily life ability score was (20.19 ± 7.10) points,42 cases' daily life ability was normal,44 had deficiency in daily life,38 had obvious flaws;16 cases received medical social support condition at a critical level,66 at a general level,42 in good condition;the difference of the ability of daily life between actual support and other supports was statistically significant (P< 0.05);the difference in patients with self-perceived burden and medical social support was not statistically significant (P >0.05).Patients with long term oxygen therapy had higher self-pereeived burden than patients without oxygen therapy at home [(18.65±3.97) points vs.(17.69±6.24) points,t=6.015,P<0.05],but patients with long term oxygen therapy had better medical and social support condition than those without oxygen therapy at home [(282.11±37.47) points vs.(262.26±61.14) points,t=6.624,P<0.05].Conclusions The level of medical and social support in elderly patients with COPD need improve.Nurses need to provide better guidance for patients with long term oxygen therapy,ensure them obtain adequate information,improve their emotional support and social interaction,enhance their medical social support condition.
4.Randomized Controlled Trial on Effectiveness of Traditional Chinese Medicine Five Elements Music Therapy in Improving Quality of Life for Senior and Non-senior Advanced Cancer Patients

This study was aimed to evaluate effects of traditional Chinese medicine ( TCM ) five elements music therapy on the quality of life for both senior and non-senior advanced cancer patients , and improve TCM com-prehensive treatment mode . A total of 170 patients were stratified according to 70-year-old in a randomized controlled trial ( RCT ) with a parallel single-blind design . The ratio of patients among three groups was 2:2:1 . In the experimental group , TCM five elements music was given in the treatment . In the positive control group , western music was used in the treatment . And in the negative control group , no music was given in the treat-ment . The treatment was given for 30 min , 5 times per week . And the observation duration was 3 weeks . The Hospice Quality of Life Index-Revised ( HQOLI-R ) , Karnofsky Performance Scale ( KPS ) and Symptom Diary Score recorded by patients were used in the evaluation . The results showed that when comparing HQOLI-R , KPS and Symptom Diary Score before and after treatment among all advanced cancer patients in each group , there was a significant difference before and after treatment in the TCM five elements music therapy group ( P< 0 . 05 ) . But there was no significant difference before and after treatment in the western music therapy group or the negative control group ( P > 0 . 05 ) . It was concluded that TCM five elements music therapy is effective in improving quality of life and subjective symptoms for both senior and non-senior advanced cancer patients . However , the sensitivity of using the Symptom Diary Score as an evaluation index among senior patients still needs to be further proved .
5.The application of traditional Chinese medicine synthetic effect criteria in the late stage lung cancer

Objective To inspect the correlation of synthetic effect criteria to sensitivity,reliability of late stage lung cancer curative effect verdict and median survival time. Method Using size change of tumor,symptom change related to tumor,Karnofsky score,body weight etc.as synthetic effect criteria and nstituting clinical synthetic effect criteria,to assess the curative effect of late stage lung cancer patients who received different treatment in department of tumor of Xiyuan Hospital between March 1997 and March 2000, to analysis medium survival time, and being compared with RR of tumor.Result 52 patients entered this experiment, in which 42 patients were assessable (follow up), 13 patients were effective(30.95% ),17 were patients stable(40.48% ),12 patients deteriorated (28.57% ).The result of tumor objective remission rate: CR is 0,PR is 16.67% (7 patients),NC is 64.29% (27 patients) and PD is 19.05% (8 patients).There was apparent difference between two effect criteria(P< 0.05).The mean survival time of this group is (9.3± 1.6)months,median survival time is 10.5 months(2~ 8months),survival rate of one year is 33.33% (14 patients).In the patients who live longer than median survival time, most of them survive associated with tumor existence who are in NC of tumor objective remission rate, but also in the effective column of synthetic effect criteria. Compared with tumor objective remission rate,the synthetic effect criteria have more apparent correlation to survival time. Conclusion In the assessment of late stage lung cancer,synthetic effect criteria has better sensitivity and reliability,and emphasize survival quality,at same time it reflect the reaction of tumor and host to treatment,it reflect prognosis of tumor preferably and have better correlation to the survival time. It can be used as clinical effect criteria of late stage lung cancer after further consummation.
6.Effect of pentoxifylline on Fas and FasL expressions in pancreas and spleen of NOD mice

NOD mice were treated with pentoxifylline (FTX) to investigate the incidence of cyclophosphamide-accelerated diabetes, the apoptosis and the insulin expression of β-cells and expressions of Fas or FasL mRNA in both pancreas and spleen. The results showed that incidence of diabetes in PTX group was significantly lower compared with control group (P<0.05). The apoptosis of β-cells was decreased in PTX group with higher insulin expression level in islet cells. The expression of FasL mRNA in pancreas of PTX group was lower than that of control group (P<0.05), and there was no significant difference in Fas mRNA expression between two groups. Both Fas and FasL mRNA levels in spleen of PTX group were much higher than those of control group (P<0.05 or P<0.01).
7.Effects of Basic Fibroblast Growth Factor on Proliferation as well as Collagen and Fibronectin Expression on Fibroblasts

Objective To explore the mechanism and effects of basic fibroblast growth factor( bFGF)on skin wound healing. Methods Fibroblasts( FB)were isolated from normal skin and hypertrophic scar and cultivated by direct adherence method. FB were then treated with different concentrations of bFGF(0,0. 1,1,10,100,1 000 ng·mL-1 )and cultivated with serum-free medium for 72 hours. The proliferation and apoptosis of FB in each group were detected by cell counting and trypan blue staining. Content and gene expression of typeⅠand type Ⅲ collagen and fibronectin were determined by ELISA and RT-PCR,respectively. Results bFGF promoted the proliferation of FB at low concentrations promoted apoptosis of FB at higher concentrations. The proliferation of FB from hypertrophic scar was slower than that from the normal skin. bFGF significantly inhibited type Ⅰ collagen production from hypertrophic scar FB but not from the normal skin. Moreover,bFGF up-regulated fibronectin expression in the normal fibroblasts,but not in the hypertrophic scar. No change in type Ⅲ collagen expression and production was observed in FB from either source. Conclusion bFGF has differential effects and mechanisms on FB of the normal skin and hypertrophic scar,suggesting that bFGF may play a role in early phase of skin wound healing and scar formation.

In recent years,it has been found high myopia is significantly related with primary open-angle glaucoma.Myopic retinopathy is high risk factors of primary open-angle glaucoma,and the retinal degeneration of high myopia is similar to the feature of primary open-angle glaucoma,which cause difficult to early diagnosis.How to identify early-stage glaucoma from high myopia patients,and give early intervention treatment has become a difficult problem.This article reviews research literature about the relationship between high myopia and between primary open-angle glaucoma.

10.Bone graft to repair isthmus defect plus temporary single segmental screw rod fixation for adolescent lumbar spondylolysis

BACKGROUND: Lumbar spondylolysis is the common cause of teenagers' low back pain. It should apply internal fixation if conservative treatment is invalid. There are a variety of surgeries, which aims to alleviate pain and bony fusion of pars defect.OBJECTIVE: To observe clinical outcome of bone graft of pars defect plus temporary single segmental pedicle screw rod fixation for adolescent lumbar spondylolysis.METHODS: A total of 32 adolescent patients of lumbar spondylolysis were treated by bone graft of pars defect plus temporary single segmental pedicle screw rod fixation. All the patients had bilateral spondylolysis. 20 patients had no lumbar spondylolisthesis, while the others had I° spondylolisthesis. All the patients received lumbar radiograph, CT and MRI. Visual analogue scale and Oswestry disability index were utilized to evaluate pain improvement before and after operation. MacNab was used to assess efficacy. Bone graft healing at isthmus was observed with lumbar CT after fixation. The internal fixation was removed after bone fusion, then the motion of the fixed segment and the degeneration of adjacent intervertebral disc occurred were recorded.RESULTS AND CONCLUSION: (1) Completed bone fusion of pars defect was achieved in all the patients. The average period of bone union was 7.4 months. (2) The visual analogue scale and Oswestry disability index scores were significantly improved after surgery in all patients (P < 0.05). (3) The fixed segment reserved the motion after internal fixation removal. The signal of adjacent intervertebral disc of fixed segment had no changes compared to preoperative MRI. (4) Bone graft to repair isthmus defect plus temporary single segmental screw rod fixation for adolescent lumbar spondylolysis is very effective, the bone fusion is completed, and temporary fixation may effectively alleviate the degeneration of adjacent disc.
