China Oncology 2016;(1):73-79
Background and purpose:Pathological lymph node metastasis (LNM) is not rare in clinical lymph node negative (cN0) papillary thyroid carcinoma (PTC). The aim of this study was to investigate the risk factors of LNM, especially of high volume LNM (more than 5 metastatic lymph nodes) and contralateral central compartment LNM, in cN0 PTC.Methods:Medical records of 350 PTC patients (265 female, 85 male, 212 patients with solitary lesion in unilateral lobe) were reviewed. All operations of these patients were performed by one surgical team. The clinical pathological data were collected, and univariate and multivariate analysis was performed. Results:LNM was conifrmed in 138 patients (39.4%) and 20 patients had high volume LNM. In 169 patients with solitary lesion in unilateral lobe with total thyroidectomy and bilateral central neck dissection, 24 patients had contralateral metastasis (14.2%). In univariate analysis, tumor size (58.5% in >1 cmvs 33.6% in≤1 cm) and tumor with calcification in preoperational ultrasonography (43.7% withvs 31.7% without) showed signiifcant difference in prevelance of LNM. In multivariate analysis, tumor size >1 cm (OR=2.792) was the independent risk factor of LNM. Gender (3.8% in male vs 11.8% in female), age (10.7% <40 yearsvs 3.4%≥40 years ), tumor size(13.4% in >1 cmvs 3.4% in≤1 cm) and tumor with low echo in preoperational ultrasonography (13.9% withvs 4.8% without) showed signiifcant difference in univariate analysis of high volume LNM. Male (OR=5.152), tumor size >1 cm (OR=5.712) and age <40 years (OR=3.959) were conifrmed as independent risk factors of high volume LNM. Male (OR=3.105) and tumor size >1 cm (OR=3.863) were also demonstrated as independent risk factors of contralateral LNM in patients with solitary lesion in unilateral lobe, the prevalence of LNM were 26.5% in male and 26.1% in tumor size >1 cm, respectively. Conclusion:LNM was not “rare” in cN0 PTC patients. Prophylactic central neck dissection should be performed in cN0 patients with tumor size >1 cm. For cN0 microcarcinoma, more active surgical treatment may be considered in male and young patients.

Objective To observe the effect of γ-ray irradiation on CD4 + CD25 + regulatory T cells (Tregs),and to investigate the mechanism of immune injury induced by irradiation. Methods The thymus and spleen of C57BL/6 mice were taken and weighted 1-28 d after γ-ray irradiation, and the organ coefficients were calculated. The amount of mouse peripheral WBC measured,CD4 + T cells and Tregs in peripheral and splenic were analyzed by flow cytometry. Results Coefficients of mouse thymus and spleen decreased significantly 1 d post irradiation, and reached to the bottom at 7 d.Coefficients did not recover to control level 28 d after radiation.Peripheral WBC continuously decreased and reached the bottom at 7 d,and did not recover to control level up to 28 d postirradiation.Peripheral CD4 + T lymphocyte temporally reduced at 1 d,while it increased at 7 d,and it approached to control level at 28 d after radiation.Splenic CD4 + T cells slightly reduced at 7 d however, they basically maintained as the same level as control 14 d and 28 d after radiation.Peripheral Tregs ascended at 1 d and reached the peak at 7 d,and reduced at 14 d and 28 d postirradiation, although they still were significantly higher than those of control group. At the same time, splenic Tregs increased significantly and achieved peak value at 1 d,and then gradually decreased and reached the minimum at 28 d after irradiation, which were significantly lower than those of control group( t =2.731,P < 0.05).Conclusions Mouse thymus and spleen were injured severely, and the number of immunocytes decreased after 6 Gy whole body γ-ray irradiation. However,Tregs with immunosuppressive action increased significantly postirradiation, revealing that Tregs were closely correlated with immune function depression and immunomodulation imbalance induced by ionizing radiation.

Objective To explore the effect of the model of comprehensive nursing interventions based on time nursing in improving therapeutic effects and prognosis of patients with schizophrenia. Methods A total of 84 cases of patients with schizophrenia were selected and were divide into two groupsby the random number table—an experimental group and a control group , both of which were 42 cases. The model of comprehensive nursing interventions based on time nursing was adopted in the experimental group and routine nursing was given to the control group until their discharge. Before and after the interventions, the Brief Psychiatric Rating Scale(BPRS) was used to evaluate the severity of the disease. The Activities of Daily Living(ADL) and the Social Disability Screening Schedule(SDSS) were used to evaluate the patients′ abilities of daily living and social skills. After the interventions, a satisfaction survey was conducted. Hospitalization time and disease recurrence of the two groups were observed. Results After the interventions,BPRS scales, ADL scales and SDSS scales were (31.50±4.23), (89.37 ± 8.16)、(6.65 ± 2.02) points in experimental group, and (45.43 ± 5.62), (81.03 ± 5.92), (8.96 ± 2.26) points in control group, the differences are statistically significant (t =2.361, 2.047, 2.206,P<0.05).The rates of satisfaction of the two groups was 95.24%(40/42) and 80.95%(34/42), and there was significant difference(χ2=6.892,P<0.05). Hospitalization time was respectively (42.86 ± 6.21)d and (67.49 ± 7.65)d in experimental group and control group, and and there was significant difference(t=2.253,P<0.05). Disease recurrence was respectively4.76%(2/42) and 23.81%(10/42),and there was significant difference(χ2=7.437, P=0.024). Conclusions The model of time nursing, with higher satisfaction level, shorter hospitalization time, and lower recurrence rate, can significantly improve the symptoms of patients with schizophrenia and enhance their self-care and social life abilities. It is worth popularizing in clinical nursing.

OBJECTIVE To investigate the effect of diallyl sulfide (DAS) in protection against acute lung injury in rats induced by paraquat(PQ). METHODS A total of 100 male Wistar rats were randomly divided into normal control group,PQ 70 mg·kg-1 model group, and DAS 25,50 and 100 mg·kg-1 treatment groups, with 20 rats in each group. A poisoning model was estalolished after administration ig at a single dose of PQ 70 mg·kg-1,while the normal control group was ip given the same volume of normal saline. DAS 25,50 and 100 mg · kg-1 was intraperitoneally injected 30 min before and after PQ exposure. Five rats in each group were sacrificed at 1,3,6 and 12 h,respectively. The inferior lobe of the right lung was observed by HE staining under an optical microscope. Tissue of the upper lobe of the right lung was used to detect the content of nitric oxide (NO). Alveolar macrophages (AMs) were collected and cultured for 24 h,and the content of nitric oxide synthase(iNOS)in the supernatant was detected. AMs were cultured for 72 h and the expression of iNOS protein in AMs was detected by immunocytochemistry method. RESULTS Compared with normal control group, the alveolar structure of PQ group was severely damaged and the pathological score was significantly increased(P<0.01). The NO content of PQ group was significantly higher than in normal control group(P<0.01). The content and protein expression of iNOS were significantly increased in PQ group(P<0.01). Compared with PQ group, the lung injury score of rats in DAS 50 mg·kg-1 group at 3,6 and 12 h and in the DAS 100 mg·kg-1 group at each time point was decreased(P<0.05). Compared with PQ group, the NO content of DAS 25 and 50 mg · kg-1 group was decreased(P<0.05),and the NO content of DAS 100 mg · kg-1 group was significantly reduced(P<0.01). The content of iNOS was reduced in DAS 100 mg · kg-1 group(P<0.05). Compared with PQ group, the expression of iNOS protein in DAS groups was decreased(P<0.05). CONCLUSION DAS can inhibit the oxidative damage in rats induced by PQ.
