1.Determination of Arctiin in Pifu Zhiyang Tablets by SPE-HPLC

Objective To establish a method for the determination of arctiin in Pifu Zhiyang Tablets by SPE-HPLC . Methods SPE-HPLC method was performed on a Kromasil C18 column(150 mm?4.6 mm ,5 ?m),the mobile phase consisted of methanol -1 %acetic acid (35 ∶65) ,the wavelength was 283nm . Results The linear range of arctiin was 1.5~4.5 ?g(r= 0. 999 8) ,the average recovery was 98.18 %with RSD of 2.08 %. Conclusion This method is accurate ,reliable and reproducible. It can be used for the determination of arctiin in Pifu Zhiyang Tablets .
2.Analysis on the System of Performance Audit Indicators under New Rural Cooperative Medical System

Objective: To propose a reasonable and feasible system of performance audit indicators for New Rural Cooperative Medical System(NCMS). Methods:Using the balanced score card as amended to perform the evaluation. Results:From 5 dimensions, 37 indicators were used to perform comprehensive evaluation for the NCMS. Conclusion: The indicator system is suit for the current situation, as well as audit evidence through questionnaires to obtain the required data of the system;it provides a better evaluation of the NCMS performance.
3.Determination of Chrysophanol in Diedazhitong Cataplasm by HPLC

OBJECTIVE:To determine the content of chrysophanol in Diedazhitong cataplsam by HPLC.METHODS:The samples were separated on a Kromasasil C 18 ,methol-0.1%phosphoric acid(85∶15)was taken as the mobile phase with a flow rate at1.0ml/min and detection wavelength at254nm,the sample size was10?l.RESULTS:Good linear relationship was achieved when the concentration range of chrysophanol was0.6?g~4?g(r=0.9994);The average recovery of chrysophanol was99.23%(n=5,RSD=2.45%).CONCLUSION:This method is accurate,reliable,and reproducible.
4.The role and significance of serum B factor in the pathogenesis of systemic lupus erythematosus

Objective To investigate the expression of serum B factor in the peripheral blood of patients with systemic lupus erythematosus (SLE),and explore its role in the pathogenesis.Methods Seventy eight patients with SLE in our hospital and 46 healthy persons were eligible to participate in this study.Rate nphelometyr was used to test serum B factor for 78 patients with SLE and 46 healthy controls.According to systemic lupus erythematosus disease activity index (SLEDAI),participants were divided into steady SLE group (SLEDAI < 5) and active SLE group (SLEDAI ≥5),which was further divided into mild,moderate,and serious subgroups.The differences in serum B factor between SLE patients and healthy controls,including SLE patients with different severity,were all compared.Then we analyzed the differences in serum B factor and other laboratory and clinical indexes between active and steady SLE patients.The correction of serum B factor and other laboratory and clinical indexes were also analyzed.Results Compared to healthy controls,patients with SLE had significantly lower value of serum B factor [(27.13 ± 8.98) mg/dl vs (36.73 ± 5.47) mg/dl,t =7.4,P < 0.01].Compared to steady SLE group,SLE active group had significantly lower level of serum factor B,C3 and C4,and also had significant higher level of C-reactive protein (CRP) and erythrocyte sedimentation rate (ESR) (all P < 0.05).Moreover,There were significant differences in the lower level of serum B factor between subgroups.Correlation analysis showed that the level of serum B factor was negatively associated with the levels of CRP and SLEDAI scores,whereas serum B factor was positively associated with the levels of C3 and C4 (all P < 0.05).Conclusions Serum B factor is related to SLE.Serum B factor might be involved in the pathogenesis of SLE.Detection of serum B factor is helpful for diagnosis and evaluation of SLE disease activity.
5.Dynamic observing the effect of different calcium concentration peritoneal dialysate on calcium-phosphorus metabolism in uremia patients under peritoneal dialysis

Objective To observe the effect of different calcium concentration peritoneal dialysate on calcium-phosphorus metabohsm in uremia patients under peritoneal dialysis. Methods Forty uremia patients who the first entered peritoneal dialysis were divided into two groups: standard calcium concentration group (20 cases), low calcium concentration group (20 cases). Treated with continuous ambulatory peritoneal dialysis (CAPD), combining with oral calcium carbonate, observing 12 months. Results When the first entering CAPD, serum calcium, phosphorus, calcium-phosphorus product, immunoreactive parathyroid hormone (iPTH) of the two groups had no differences. Hyperphosphatemia, lower iPTH were universal. After 6 months' treatment, hypercalcemia and hyperphosphatemia in standard calcium concentration group got increasingly serious.While in low calcium concentration group, serum calcium, phosphorus, calcium-phosphorus product all decreased significantly, moreover, they were also lower than post-treatment standard calcium concentration group(P<0.01). In the following 6 months, in low calcium concentration group,serum calcium, calcium-phosphorus product maintained at the normal level serum phosphorus also decreased to the normal level iPTH maintained about 150 ng/L. During the treatment, all patients had no hypotension and hyperspasmia. Conclusion Low calcium concentration peritoneal dialysate can ameliorate disorder of calcium-phosphorus metabohsm, delay the appearance of its comphcations.
6.Atypical vascular lesions after conservative surgery and radiation of breast cancer: report of a case and review of the literature

Purpose To describe the clinical and pathological features of atypical vascular lesions (AVLs) after conservative surgery and radiation of the breast cancer,and to discuss the association with post-radiation angiosarcoma.Methods The clinical and pathological features in one case of AVLs was evaluated.The literatures were reviewed.Results The patient is a 57-yeAR~-old female who underwent a conservative surgery of the right breast because of a carcinoma.She received standard dose of radiation as adjuvant therapy.Three years later, multiple erythematous plaques developed around the former scar, which radiated to the nipple.Clinically,the plaques were considered as relapses of the carcinoma.However, fine needle aspiration gave negative results.Biopsy of one large plague revealed a circumscribed vascular lesion confined to the superficial dermis.It was composed of thin-walled anastomosing lymphatic vessels lined by attenuated endothelial cells.In focal areas, the vessels extended to the mid-dermis.Immunohistochemically, the endothelial cells were positive for CD31, CD34 and D2-40,with absent of α-SMA positive pericytes.Review of the breast tumor sections showed an invasive micropapillary carcinoma.Conclusions AVLs is a rare vascular lesion related with conservative surgery and post-radiation therapy of the breast.AVLs may represent as a precursor of breast angiosarcoma.Being familiar with the clinicopathologic characteristics of AVLs is important not only for the pathologists but also for the clinicians.
7.Bone regenerationvia a novel honeycomb-like polycaprolactone-calcium silicate crystal compound scaffold in extreme-sized cranial defects

BACKGROUND:Polycaprolactone as a polymer material has poor biocompatibility, and needs to be combined with other natural biological materials to increase biocompatibility, thereby promoting tissue regeneration.OBJECTIVE:To develop a novel honeycomb-like polycaprolactone-calcium silicate crystal compound scaffold, and observe its osteogenic effects in Sprague-Dawley rats with skull defects.METHODS: Eighteen Sprague-Dawley rats were used to make skull defect models and randomized into three groups: blank control group with no implantation, control group with implantation of normal polycaprolactone-calcium silicate crystal compound scaffold, and experimental group with implantation of the novel honeycomb-like polycaprolactone- calcium silicate crystal compound scaffold. Six weeks after implantation, bone regeneration effect in the defect region measuredvia X-ray scanning, Micro-CT three-dimensional reconstruction, and histological analysis.RESULTS AND CONCLUSION: (1) X-ray scan: in all the rats, the size of bone defect was reduced, the fracture line became vague, and the marginal bone density was increased. The percentage of new bone area was highest in the experimental group, successively followed by the control group and blank control group. (2) Micro-CT three-dimensional reconstruction: new bones in the blank control group were mainly distributed on the both sides of the defect, but those in the control and experimental groups distributed in the defect region. The bone regeneration capacity was ranked as follows: experimental group > control group > blank control group (P < 0.05). (3) Histological analysis: new bone ingrowth was visible in all the three groups to different extents. Compared with the other two groups, new bone formation and microvessel density were significantly higher in the experimental group (P < 0.05). To conclude, this novel honeycomb-like polycaprolactone-calcium silicate crystal compound scaffold can obviously promote bone formation in the skull defect region.
8.Investigation of present situation and analysis of positive practice environments of doctors in public hospital in China

Objective: This study aims to investigate the positive practice environments evaluation of doctors in public hospital and provide suggestions for Chinese health care reform from the perspective of health service providers.Methods: Using self-designed questionnaire, a national cross-sectional survey was performed in 77 public hospitals across seven provinces/ metropolis, involving a total of 3564 doctors.Positive practice environment includes organizational management and doctor-patient two-dimension relationships.Results: The doctor''s mean score of positive practice environment evaluation was 18.02±4.86 (out of 40).Doctors had a higher evaluation (score) of organizational management than doctor-patient relationship.In the organizational management dimension, the personal advice got the lowest scores(2.24±1.04);and in doctor-patient relations, evasion of medical risk got the lowest scores(1.59±0.81).There was a difference in the positive practice scores of physicians in different regions, and the scores of doctors from the western China were the lowest (P<0.001.Doctors from western medicine hospitals had lower evaluation of positive practice environments than ones from Chinese Traditional Medicine hospitals(P=0.002.Conclusion: As per the analysis conducted in this investigation, the current medical practice environment of doctors in China is not a promising one.Doctor-patient relationship is worse than organizational management.Hospitals should adopt a participatory decision-making model in management and establish a broadband performance based compensation to motivate junior doctors.Besides, government should establish reasonable compensation incentive mechanism, with the aim of promoting doctor-patient trust.
9.Effect of different postures for PICC intubation through basilic vein in very low birth weight infants

Objective To explore effect of different postures for PICC intubation through basilic vein in very low birth weight infants. Methods Eighty infants from Oct. 2013 to Sep. 2014 with PICC via basilic vein were set as the control group in which traditional method of preventing catheter displacement was applied, another 80 cases from Oct. 2014 to Sep. 2015 were divided to the observation group. And improved method was applied in the experiment group besides the traditional method. The ectopic occurrence rates of the two groups were compared. Result The ectopic rate of PICC catheters in the observation group was significantly lower than that of the control group (P<0.05). Conclusion When the catheter tip reaches the shoulder of the very low birth weight infants, the method of raising their upper limbs slightly to the head can reduce the ectopic incidence of catheters and thereby improve the success rate of PICC intubation.
10.Variation and Functional Study of Q Promotor of Epstein-Barr Virus in Nasopharyngeal Carcinoma

[Objective] To investigate the variation of Q promoter (Qp) in nasopharyngeal carcinoma (NPC) cells, and to compare the existing two mutant sites [62 225 site(g→a)and 62 422 site (g→c) ] Qp in NPC cells with the Qp in B95.8 cell line in the functional and biological difference. [Methods] The Qp sequence was amplified in the samples from 29 cases of paraffin-embedded tissues of NPC suffers and 14 cases of peripheral blood of healthy adults by polymerase chain reaction (PCR) method (totally 43 cases). The point mutations on specified sites were analyzed and statistically compared from sequencing results. The sequences of variant and prototype Qp were amplified by PCR and cloned into luciferase reporter vector (pGL3-basic), then transfected into HaCat cells respectively. The transcriptional activity was compared between variant and prototype Qp using luciferase reporter system. The DNA binding affinity of mutant and prototype Qp to Sp1 was compared through chromatin immunoprecipitation (CHIP) method since mutation of nt 62 225 located in a Spl binding site. [Results] The mutation rate of Qp was significantly higher in NPC compared with healthy controls (P=0.039 5, <0.05), which suggested the variant Qp was closely associated with NPC. The transcription of the luciferase gene promoted by variant Qp was significant more than that of prototype Qp in transient transfection assay (2.5:1, P<0.05). The binding affinity of variant Qp to Sp1 was about 1.52 times higher than that of prototype Qp as determined by quantitative ChIP assay. [Conclusions] The transcriptional activity was enhanced in variant Qp in NPC cells compared with prototype, which possibly through the higher binding affinity to Sp1. We suggest that the mutated Qp may play an important role during the EBV infection and transformation of nasopharyngeal epithelium.
