1.Clinical study on influenza viruses infection detected by gold immunchromatographic assay in hospitalized patients

Objective To study the common influenza viruses infection of hospitalized patients admitted for acute respiratory tract infections, using gold immunchromatographic assay ( GICA ) to detect influenza viruses. Methods The result of FluA/B antigen detection in 1145 patients with various types of respiratory diseases from two class-A hospitals were analyzed. Influenza virus detection rates of patients in different seasons,with different gender,age,types of respiratory diseases and whether with foundation diseases were analyzed to identify the common rules and characteristics. Results There were significant differences for Flu A/b detection rate between first quarter and the second or third quarter,P <0.05 by x2 test( FluA x2 = 17. 735, P = 0.000;X2 = 14.855,P = 0. 000;FluB x2 =5. 326,P = 0. 021;x2 = 4.349, P = 0.037 ) . The result was repeated in the comparison between Flu A/B detection rate in the fourth quarter and the second or third quarter,P <0. 05 by x2 test (FluA x2 =19. 480,P= 0.000;x2 =16.771,,P=0. 000;FluB X2 = 6. 885.P = 0. 009;x2 =5. 959,P =0.015). These results indicated the detection rates of the first and fourth quarter were higher than the second and third quarter. Elderly patients (≥65 years old) had higher Flu A/ B detection rate compared with patients below 65 years ( FluA x2 =55. 362,P = 0.000;FluB x2 = 8.984,P = 0.003). The detection rate of Flu A/B in patients without foundation diseases or with one,two or three kinds of foundation diseases had significant differences, which showed with an increase in the number of types of the foundation diseases, FluA/B-positive detection rate increased. In patients with various foundation diseases, the FluA antigen detection rate in group of AECOPD patients was 18.2% and 17.1% in pneumonia group, which were higher than in all other diseases. Conclusions Sporadic cases of influenza were found in general wards, incidence rate was higher in the first and the fourth quarter. There is a higher risk of influenza virus infection for elder patients and patients with foundation diseases.

In this study, we constructed a yeast consortium surface-display expression system by using Flo1 as an anchor protein. Endoglucanase II (EGII) and cellobiohydrolase II (CBHII) from Trichoderma reesei, and β3-glucosidase 1 (BGLI) from Aspergillus aculeatus were immobilized on Saccharomyces cerevisiae Y5. We constructed the cellulose-displaying expression yeast consortium (Y5/fEGII:Y5/fCBHII:Y5/fBGLI = 1:1:1) and investigated the enzymatic ability and ethanol fermentation. The displayed cellulolytic enzymes was stabile during the 96-h fermentation. The yeast consortium produced 0.77 g/L ethanol from 10 g/L phosphoric acid swollen cellulose (PASC) within 96 h. The yield (in grams of ethanol produced per gram of carbohydrate consumed) was 0.35 g/g, which correspond to 68.6% of the theoretical yield.

3.Correlation between white matter hyperintensities and stroke etiology classification in patients with acute isolated penetrating artery territory infarction

Objective:To investigate the correlation between white matter hyperintensities (WMHs) and stroke etiology classification in patients with acute isolated penetrating artery territory infarction.Methods:Patients with first-ever acute isolated penetrating artery territory infarction admitted to the Department of Neurology, the Affiliated Hospital of Xuzhou Medical University from January 2017 to May 2020 were enrolled retrospectively. According to the Chinese Ischemic Stroke Subclassification (CISS) system, they were divided into large artery atherosclerosis (LAA) and perforating artery disease (PAD). According to the distribution of infarcts, they were divided into lenticulostriate artery (LSA) territory infarction and paramedian pontine artery (PPA) territory infarction. The demographics, vascular risk factors, baseline clinical data, WMHs location, and Fazekas Scale scores were documented. Multivariate logistic regression analysis was used to identify the independent influencing factors of stroke etiology classification. Results:A total of 440 patients with acute isolated penetrating artery territory infarction were enrolled, including 120 (27.3%) in the LAA group, and 320 (72.7%) in the PAD group; 213 (48.4%) with LSA territory infarction, and 227 (51.6%) with PPA territory infarction. The proportion of patients with total Fazekas score 3-6 and periventricular WMHs (PWMHs) score 2-3 in the PAD group was significantly higher than those in the LAA group (all P<0.05). In patients with LSA territory infarction, the proportion of the patients with hypertension, WMHs total Fazekas score 3-6 and PWMHs score 2-3 in PAD subgroup was significantly higher than those in the LAA subgroup, while the proportion of the patients with hyperlipidemia was significantly lower than that in LAA subgroup (all P<0.05). In patients with PPA territory infarction, the levels of low-density lipoprotein cholesterol and homocysteine in the PAD subgroup were significantly lower than those in the LAA subgroup. Multivariate logistic regression analysis showed that PWMHs score 2-3 was an independent correlation factor of PAD (odds ratio [ OR] 2.220, 95% confidence interval [ CI] 1.085-4.541; P=0.029). In patients with LSA territory infarction, hyperlipidemia was independently correlated with LAA ( OR 0.432, 95% CI 0.192-0.972; P=0.042), and PWMHs score 2-3 was independently correlated with PAD ( OR 3.846, 95% CI 1.193-12.397; P=0.024). In patients with PPA territory infarction, higher low-density lipoprotein cholesterol ( OR 0.660, 95% CI 0.494-0.883; P=0.005), homocysteine ( OR 0.958, 95% CI 0.930-0.987; P=0.005) and C-reactive protein ( OR 0.987, 95% CI 0.977-0.997; P=0.008) were independently correlated with LAA. Conclusions:WMHs are common in patients with acute isolated perforating territory infarction caused by LAA and PAD, and more severe PWMHs suggest that PAD is more likely to be the cause of the acute isolated perforating territory infarction, especially in patients with LSA territory infarction.
4.Comparative advantages of different methods of collecting nasopharyngeal secretions for rapid detection of influenza virus

Objective To compare the effectiveness of different methods of collecting nasopharyngeal secre-tions by nasopharynx swab and nasopharyngeal underpressure suction catheter for rapid detection of influenza virus. Methods Nasopharyngeal secretions as the experimental samples of 1042 patients with acute respiratory tract disea-ses were collected by nasopharynx swab and nasopharyngeal suction catheter, and gold immunochromatographic assay (GICA) kit was applied for the detection of influenza viruses. Results The use of the above two methods collecting nasopharyngeal secretions as samples for rapid detection of influenza virus would get the same results. The difference between the two methods had no statistical significance( P > 0.05 ). Conclnsions Nasopharynx swab is a reliable method for rapid detection of influenza virus, which is fast and convenient, compared with nasopharyngeal suction catheter.
5.Design, synthesis and biological evaluation of novel diaryl ethers bearing a pyrimidine motif as human Pin1 inhibitors.

Pin1 (peptidyl-prolyl cis-trans isomerase NIMA-interacting 1) belongs to peptidyl-prolyl cis-trans isomerase (PPIase) and is a novel promising anticancer target. Based on the lead structure of benzophenone, a series of novel diarylether derivatives containing a pyrimidine ring were designed and synthesized. The inhibitory activities on Pin1 of compounds 5a-5d and 6a-6i were evaluated by a protease-coupled enzyme assay. Of all the evaluated compounds, 6 compounds displayed inhibitory activities. Molecular docking was performed using FlexX algorithm to explore the binding mode of the active molecules.
6.The effects of whole-phase interval reconstruction and four dimensional mode on the imaging quality of dual-source CT coronary angiography:an initial study

ObjectiveTo evaluate the multi-phase imaging ability of dual source CT coronary angiography and the effects of whole-phase interval reconstruction and the four dimensional(4D)mode of"Inspace"software on the imaging quality of dual source CT coronary angiography.MethodsFifty patients performed coronary CT angiography(CTA),and the images were reconstructed from 10% to 100% of the R-R interval(the reconstruction interval was 10%).The imaging quality scores of the three main coronary arteries were assessed by two experienced radiologists using the 4D mode of"Inspace"software On workstation.ResultsThe average scores of imaging quality in the RCA.LAD and LCX at all 10 reconstruction phases were 1.71,2.57 and 2.03 respectively. The scores of imaging quality in the three main coronary arteries were all over 2.0 when the reconstruction phase were at 30%,40%and 70%of the cardiac circle.The imaging quality scores in the three main coronary arteries reached 3 at least one phase both in systole(10%-40%)and in diastole(50%-100%)in 41 cases(82%).The imaging quality scores in the left anterior descending artery reached 2 at all phases in 25 cases(50%),and reached 3 in 5 cases(10%).Conclusion Dual source CT coronary angiography can achieve good image quality in mid-late systole(30%-40%)or mid-diastole(70%).
7.Effects of the serum in shock rats and shenfu injection on the endothelial cell protein C receptor

Objective To investigate the effects of the serum from rats with hemorrhagic shock and Shenfu injection, on the expression of endothelial cell protein C receptor (EPCR) in human umbilical vein endothelial cells (HUVECs) cultured with rat serum. Method The soluble endothelial protein C receptor (sEPCR) in supernatant, the expression of EPCR mRNA and protein level of EPCR in HUVECs were detected by using enzyme linked immunosorbent assay ( ELISA), reverse transcription polymerase chain reaction (RT-PCR), and western bloting (WB) in normal control group, hemorrhagic shock serum (3 h, 12 h, 24h, 72 h) group, and Shenfu-treated (3 h, 12 h, 24 h, 72 h) group, respectively. Results The mean levels of sEPCR and the expression of EPCR mRNA were significantly higher in hemorrhagic shock serum (12 h, 24 h) group, and Shenfu -treated(24 h)group than those in normal control group (all P <0.01 ),the mean levels of sEPCR and the expressions of EPCR mRNA were significantly higher in Shenfu-treated ( 12 h) group than those in normal control group ( all P <0. 05 ), while the levels of protein were lower in hemorrhagic shock serum ( 12 h, 24 h) group and in Shenfu-treated(24 h)group than those in normal control group ( both P <0.01 ), and the level of EPCR protein was lower in Shenfu-treated( 12 h) group than that in normal control group ( P < 0. 05) . The mean levels of sEPCR and the expressions of EPCR mRNA were significantly lower in Shenfu-treated ( 12 h, 24 h) group than those in hemorrhagic shock serum ( 12 h,24 h) group (all P <0.05), while the levels of EPCR protein were higher in Shenfu-treated ( 12 h, 24 h)group than those in hemorrhagic shock serum ( 12 h, 24 h) group ( P < 0.05 ). Conclusions These data suggest that Shenfu injectio could affect the expression of EPCR mRNA and the level of EPCR protein, thereby it might be effective in prevention of development of hemorrhagic shock.
8.Application of TG-ROC analytical method in detection reagent quality evaluation

Objective To compare the relationship between the enzyme‐linked immunosorbent assay(ELISA) reagent and West‐ern blot(WB) confirmation reagent for analyzing the quality lever of human T‐cell lymphotropic virus(HTLV) detection reagent . Methods The WB confirmation reagent was used to detect anti‐HTLV antibody in 156 human serum samples of ELISA prelimina‐ry screening positive .The ELISA cut‐off value(optimal value) was selected by using the two‐graph receiver operating characteristics (TG‐ROC) analytical method .The two‐by‐two table analysis was constructed to analyze the consistency of results detected by the two methods ,moreover the McNemar test was used to evaluate the consistency of detection results .The quality level of HTLV de‐tection reagent was comprehensively evaluated .Results Among 156 serum samples of ELISA preliminary screening positive ,only 40 samples were positive by the WB confirmation ,and other 116 samples were negative .The sensitivity and specificity of ELISA de‐tection reagent obtained by TG‐ROC analysis were 97 .5% and 45 .7% respectively ,the TG‐ROC test also indicated that the detec‐tion results had significant difference between ELISA and WB(P<0 .05) .By adjusting the cut‐off value ,the sensitivity and specific‐ity of ELISA were increased to 88 .8% (parametric method) .In the comparison of the parametric method and the non‐parametric method ,the obtained areas under the curve(AUC) was 0 .923 5(parametric method) ,their results were basically consistent .Conclu‐sion Although above results indicate that the detection results of ELISA reagent are different from those of WB ,but adjusting the cut off value can increase its sensitivity and specificity ,thus increases the reliability of diagnosis result .
9.A clinical analysis of subclinical hypothyroidism during pregnancy and the effect of thyroxine replacement therapy

Objective To investigate the maternal and fetal outcomes of pregnant women with subclinical hypothyroidism, and clinical observation of thyroxine replacement. Methods From March 2014 to March 2015, the clinical records of 216 women with subclinical hypothyroidism(including 166 cases with thyroxine replacement), and hypothyroidism(n=69)during pregnancy who delivered at our hospital were reviewed. The maternal complications and neonatal outcomes were compared with 406 healthy women who delivered during the same period. Results The age, number of fetus, and morbidity rate of gestational hypertension were without significant differences in those groups. The morbidity of gestational diabetes in subclinical hypothyroidism group and hypothyroidism group were significantly higher than those in control group(13.4%, 17.4% vs 0.2%, P<0.05). The rate of thyroid peroxidase antibody and thyroglobulin antibody positive in subclinical hypothyroidism group and hypothyroidism group were significantly higher than those in control group(26.9%, 23.2% vs 9.9%; 15.7%, 23.2% vs 8.1%, all P<0.05). No matter treated or not treated in subclinical hypothyroidism group, the preterm birth, Apgar score, low birth weight, birth defects, and infant congenital hypothyroidism were without significant differences as compared to the control group(P>0.05). Further compared those between treated and untreated subclinical hypothyroidism, the results were also without significant difference(P>0.05). Conclusions Subclinical hypothyroidism had no significant influence on pregnancy outcomes and perinatal events. Thyroxine replacement in subclinical hypothyroidism pregnant women also had no significant influence on pregnancy outcomes and perinatal events.
10.DUOX2 mutations are frequently associated with congenital hypothyroidism in a Chinese Patients

Objective To investigate the prevalence of DUOX2 mutations in Chinese patients with congenital hypothyroidism (CH) and to discuss the inheritance pattern of DUOX2 gene.Methods Blood samples were collected from 91 CH children and their genomic DNA was extracted from peripheral blood leukocytes.All exons and exon-intron boundaries of DUOX2 were analyzed by target next-generation sequencing and family trios was established to study the inheritance pattern of DUOX2 gene.Results Fifty-four out of 91 children with CH carried DUOX2 mutation, with a prevalence of 59.34%.Of the 54 CH children, 36 carried DUOX2 biallelic mutations.In all 12 family trios with probands carrying biallelic DUOX2 mutations, the parents carried heterozygous DUOX2 mutations while still showing normal thyroid function, suggesting that CH caused by DUOX2 mutations is inherited in an autosomal recessive manner.Conclusion DUOX2 gene is one of the most frequently mutated genes in Chinese CH patients and its inheritance pattern is an autosomal recessive one.
