1.Separation and determination of aconitine in medicine wine by reversed-phase high performance liquid chromatography.

OBJECTIVE:
For the purpose of more accurate and rapid analysis of aconitine in medicine wine, a reversed-phase High Performance Liquid Chromatography (HPLC) method was developed.
METHODS:
Standard aconitine was added to the blank wine when the wine sample was pretreated. The pretreatment method of samples, the linear range, the precision, the recoveries in the plasma were investigated by using of white wine plasma spiked with standard aconitine.
RESULTS:
The linear range was 0.45 approximately 9.0 microg x mL(-1), r=0.998 8. The detective limit was 0.45 microg x mL(-1), The intra and inter-day precision of assay for aconitine were less than 3.1% and 4.7% (n=5) in wine respectively. The recoveries of aconitine were more (97.3+/-2.8)% in medicine wine. The HPLC method has been used to investigate the concentration of aconitine in one forensic medicine case.
CONCLUSION
The HPLC method of quantitative analysis aconitine is rapid and sensitive. It is only in the 2.0 h that determination of aconitine in the medicine wine.

2.Evaluation of myocardial ischemia caused by left anterior descending coronary artery disease using coronary flow reserve

Objective To explore the value of coronary flow reserve(CFR) evaluating myocardial ischemia measured by adenosine triphosphate (ATP) stress transthoracic Doppler echocardiography (TTDE),and the feasibility of CFR to predict coronary stenosis.Methods Fifty-four patients suffering chest pain with known or suspected coronary artery disease were performed ATP stress TTDE to measure resting and maximum expansion coronary blood flow velocity and calculate CFR.all patients were performed by coronary angiography (CAG) and single-photon emission computed tomography (SPECT) myocardial perfusion imaging.Results ① To evaluate myocardial ischemia,there was not statistical significant difference between non-invasive CFR and SPECT myocardial perfusion imaging(P >0.05).CFR≤2.0 was the best cutoff value for evaluating myocardial ischemia which yielded a sensitivity of 93.3 % and specificity of 89.7%.②Coronary artery stenosis was negatively correlated with CFR (P <0.001).ROC curve analysis demonstrated that CFR≤ 1.60 yielded a sensitivity of 92.3% and specificity of 73.3% to predict coronary stenosis significantly.Conclusions CFR measured by ATP stress TTDE can evaluate myocardial ischemia of coronary artery disease and predict LAD significant stenosis before CAG.Using CFR and CAG has important clinical value for choosing treatment of stable coronary artery disease.
3.Effect of acupuncture--general anesthesia on hemodynamics in perioperative period of patients undergoing cholecystectomy.

OBJECTIVETo observe the effect of acupuncture-general anesthesia on hemodynamics in the perioperative period of patients undergoing laparoscopic cholecystectomy.
METHODSSixty patients of ASA class I-II scheduled to receive laparoscopic cholecystectomy were randomly assigned to two groups equally, Group A, the control group, and Group B, the compound anesthesia group. The general anesthesia applied on them during operation was induced by combined midazolam, fentanyl, propofol and vecuronium bromide, but for those in Group B, 15 min of electric stimulation at bilateral Neiguan (PC6), Hegu (LI4) and Quchi (LI11) was given with an acupuncture anesthesia apparatus before the drug induction as acupuncture anesthesia induction, then the general anesthesia started and maintained the same as that in Group A. Changes of cardiac function and hemodynamics were monitored using thoracic electrical bio-impedance method at different time points of operation, i.e. the baseline before induction (T0), after induction when loss of consciousness (T1), during intubation (T2), beginning of operation (T3), ending of operation (T4), during extubation (T5) and 10 min after extubation (T6).
RESULTSNo significant difference between the two groups was found in operation lasting time and anesthesia maintaining time (P > 0.05). The amount of vecuronium bromide consumed in an unit of time in Group B was significantly lower than that in Group A (P < 0.05). The mean arterial pressure (MAP), heart rate (HR), rate pressure product (RPP), cardiac output (CO), systemic vascular resistance (SVR), and blood flow acceleration index (ACI) in Group A at T1 were all significantly lower (P < 0.05 or P < 0.01), but became significantly higher at T2 and T5 as compared with those of baseline (P < 0.05 or P < 0.01), while no significant change of them was found in Group B. Moreover, MAP, RPP, SVR and ACI were significantly higher in Group A than those in Group B at T2 and T5 (P < 0.05 or P < 0.01).
CONCLUSIONThe acupuncture-general anesthesia shows a bi-directional effect for diminishing the fluctuation of hemodynamics and stabilizing the blood circulation, and also can reduce the consumption of narcotics during operation, so it has an extensive value in clinical practice.

4.Effect of combined therapy of modified kangjia recipe combined with western medicine on hyperthyroidism.

OBJECTIVETo observe the therapeutic effect of combined therapy of modified Kangjia Recipe (KR) and western medicine propylthiouracil (PTU) on hyperthyroidism and to assess its advantage.
METHODSNinety patients with hyperthyroidism were divided into the treatment group (TG) and the control group (CG). The 60 patients in TG treated with KR and PTU, and the 30 patients in CG treated with PTU alone. The treatment course of both groups was 3 months. The serum thyroid hormone (TH) level, clinical symptoms and signs, and adverse reaction were observed before and after treatment.
RESULTSThe serum TH level, clinical symptoms and signs were significantly improved after treatment in both groups (all P < 0.01). The total effective rate was 93.3% in TG, which was higher than that in CG (83.3%, P < 0.01); and the efficacy in improving palpitation, fidget, aversion to heat, sweating and insomnia was better in the TG than that in the CG respectively (P < 0.05). As for the incidence of adverse reaction, it that was lower in the TG than in the CG.
CONCLUSIONKD has good efficacy in treating hyperthyroidism, and the combined treatment of Chinese and Western medicine shows obvious advantages.

5.Clinical Study on Minchuanke Granules Combined with Conventional Western Medicine for the Treatment of Allergic Rhinitis-Asthma Syndrome

Objective To observe the efficacy of Minchuanke Granules combined with conventional Western medicine in the treatment of allergic rhinitis-asthma syndrome(CARAS);To explore the mechanism of its efficacy.Methods Totally 69 patients with CARAS were divided into 34 cases in the control group and 35 cases in the treatment group using random number table method.Both groups were given oral theophylline extended-release tablets and topical budesonide nasal spray.The treatment group was given Minchuanke Granules on the basis of the control group,one dosage per day,twice a day,orally.30 d was one treatment course for both groups,and two courses were observed.Asthma control efficacy and clinical efficacy were observed.TCM symptom scores and sign scores,serum IgE,interleukin(IL)-4,IL-33,γ interferon(IFN-γ),and tumor necrosis factor-α(TNF-α)levels before and after treatment in two groups were compared.Safety indicators and adverse reactions of both groups were monitored.Results Two Cases were lost in each group.The total effective rate of asthma control in the treatment group was 87.88%(29/33),while in the control group it was 65.63%(21/32),without statistical significance(P>0.05).The total effective rate of clinical efficacy in the treatment group was 87.88%(29/33),while that in the control group was 56.25%(18/32),with statistical significance(P<0.05).Compared with before treatment,the total score of TCM symptoms decreased in both groups after treatment(P<0.05);after treatment,the total score of TCM symptoms in the treatment group was lower than that in the control group(P<0.05).Compared with before treatment,there was no statistically significant difference in nasal congestion,itching,runny nose,cough,wheezing,shortness of breath and chest tightness of the two groups after treatment(P>0.05);after treatment,there was no statistical significance(P>0.05)in the grading of the above symptoms in the treatment group.Compared with before treatment,the serum IgE,IL-4,and IL-33 levels in the treatment group significantly decreased after treatment,while TNF-α and IFN-γ levels significantly increased(P<0.05);after treatment,the treatment group showed better improvement in IgE,IL-4,IL-33,TNF-α,and IFN-γ compared to the control group(P<0.05).There were no significant abnormalities in the safety indicators of the two groups,and no obvious adverse reactions were observed.Conclusion Minchuanke Granules combined with conventional Western medicine can effectively improve the clinical symptoms of CARAS patients by lowering IgE,regulating Th1/Th2 levels,and inhibiting the release of inflammatory mediators,thereby exerting the effects of treating CARAS.
6.Tetramethylpyrazine accelerated spinal cord repair through regulation of caspase-3 and neurofilament protein expression: an acute spinal cord injury model in rats.

OBJECTIVE:
To investigate the effect of tetramethylpyrazine on spinal cord repair through hypothesized pathways including regulation of caspase-3, neurofilament protein NF-L, NF-H, and NF-M expression.
METHODS:
Allenos weight drop method was used to establish acute spinal cord injury (SCI) rat model at T10 section and was conducted in 80 adult Sprague Dawley rats that were subsequently divided into pseudo-therapy group (sodium chloride group, n=40) and tetramethylpyrazine group (treatment group, n=40). Eight normal rats were included into the pseudo-surgery normal group (n=1 for each time point). The function of spinal cord was evaluated with animal behavioral scores by measuring modified Rivilin loxotic plate degree and counting BBB score at 1, 3, 7, 14, and 21 d postoperatively. The injured spinal cord tissue samples were harvested at 1 h, 3 h, 6 h, 1 d, 3 d, 7 d, 14 d, and 21 d postoperatively (n=5 for each time point) for the preparation of continuous histological sections that were analyzed the expression of caspase-3,NF-L,NF-H, and NF-M by immunohistochemistry method.
RESULTS:
At postoperative Day 7, 14, and 21, animal behavioral scores revealed higher modified Rivlin loxotic plate degree and BBB scores in the treatment group when compared with those of the control group (P<0.05). The cells in the injured spinal cord tissue expressed significantly less caspase-3 and significantly more NF (NF-L, NF-H and NF-M) in the treatment group than those in the control group at Day 3, 7, and 14 (P<0.05). There was positive correlation between modified Rivlin loxotic plate degree and NF expression, BBB score and NF expression; and negative correlations between BBB score and caspase-3 expression, caspase-3 expression and NF expression.
CONCLUSION
Tetramethylpyrazine improves spinal cord healing through regulation of caspase-3 and neurofilament protein expression.

8.Exercise-induced muscle damage and physiotherapy: a visualized analysis

ObjectiveTo conduct a visualized analysis of the researches related to the physiotherapy for exercise-induced muscle damage, identify the present situation, and predict the hotspots and developing trends. MethodsRelevant literature about physiotherapy of exercise-induced muscle damage were collected in Web of Science Core Collection from the establishment of the database to October 31, 2022, and CiteSpace 6.1.R3 software was used for visualized analysis. ResultsA total of 357 articles were included, involving 51 countries/regions, 532 authors and 346 institutions. The annual number of documents issued showed an overall upward trend. The country with the highest number of documents was the United States, the organization was Edith Cowan University, and the author was Howatson Glyn. Keywords with high attention in the last three years were performance, eccentric exercise, recovery and soreness. Cited document clustering words were exercise-induced muscle damage, delayed-onset muscle soreness, foam rolling and exercise performance. ConclusionExercise-induced muscle damage is a muscle micro-injury, and the most important concerns are post-injury muscle soreness, myofascial pain and ultrastructural changes. Researches about physiotherapy of exercise-induced muscle damage are gradually increasing. Main physiotherapy treatments include cryotherapy, cold water therapy, massage, foam rolling and eccentric exercise. Presently, eccentric exercise is a hot topic in this field, and foam rolling would be hot in the future.
9.Clinical Study of Shengxue Mixture (生血合剂) in Treating Aplastic Anemia

Objective: To observe the clinical effect of Shengxue Mixture (SXM) in treating aplastic anemia and study the possible mechanism. Methods: Eighty-four patients in the treated group with Spleen-Kidney Yang deficiency syndrome and Spleen-Kidney Yin deficiency syndrome were treated with SXM-Ⅰ and SXM-Ⅱ respectively, and 30 patients in the control group were treated with Stanozolol. The clinical effect and several experimental parameters were also observed. Results: The basic cure was gained in 18 cases, remission in 23, markedly improved in 32, ineffective 11, total effective rate and cure remission rate of the treated group were 86.90% and 48.81% respectively, which were obviously better than those of the control group (P<0.05) with no obvious side-effect. While the patient's symptoms were alleviated, the peripheral blood cells increased, the ratio of T lymphocyte subsets tended to balance, the level of natural killer cell activity increased, interleukin-2 reduced, and reproduction of the bone marrow were markedly improved in most of the patients treated by SXM. Conclusions: SXM is an effective and safe drug for aplastic anemia. Its mechanism might be likely due to its regulating the immune function, which facilitates the recovery of the bone marrow hematopoiesis function.
10.Clinical Observation of Shengxiantang in Treatment of Early Parkinson's Disease with Autonomic Dysfunction of Qi Deficiency Pattern

ObjectiveTo investigate the efficacy and safety of Shengxiantang in the treatment of early Parkinson's disease with autonomic dysfunction of Qi deficiency pattern. MethodA total of 82 eligible patients were randomized into control group (41 cases) and traditional Chinese medicine (TCM) group (41 cases). On the basis of standardized treatment of western medicine, TCM group was prescribed Shengxiantang while control group were treated with placebo for 12 consecutive weeks additionally. Scale for Outcomes in Parkinson's Disease-Autonomic (SCOPA-AUT) questionnaire, Unified Parkinson's Disease Rating Scale (UPDRS), Traditional Chinese Medicine Qi Deficiency Symptom Score of Parkinson's Disease (TCMQDSSPD), serum levels of glutathione peroxidase (GPx) and superoxide dismutase (SOD), daily dosage of pramipexole and levodopa and benserazide hydrochloride, and safety index were evaluated both before and after treatment. ResultAfter treatment, the total score of SCOPA-AUT, gastrointestinal score, urinary score, and thermoregulatory score in the control group were higher than those before treatment (P<0.01), while the above sores in the TCM group were lower than those before treatment (P<0.05, P<0.01). In addition, the cardiovascular score, pupillomotor score, and sexual score in two groups showed no significant difference from those before treatment. After treatment, the total score of SCOPA-AUT, gastrointestinal score, urinary score, and thermoregulatory score in the TCM group were lower than those in the control group (P<0.05, P<0.01), and cardiovascular score, pupillomotor score, and sexual score showed no significant difference between two groups. After treatment, the total score of TCMQDSSPD, main symptom scores, and minor symptom scores in the control group had no significant difference from those before treatment. The total score of TCMQDSSPD and minor symptom scores of TCM group were lower than those before treatment (P<0.01), while the main symptom scores of the TCM group showed no significant difference from those before treatment. After treatment, the UPDRS score, serum GPx and SOD levels, and daily dosage of pramipexole and levodopa and benserazide hydrochloride demonstrated no significant difference from those before treatment in the two groups and between the two groups. No abnormality was found in the safety indexes. ConclusionBased on the standardized treatment of western medicine, Shengxiantang can effectively and safely improve the autonomic symptoms of gastrointestinal system, urinary system, and thermoregulation, as well as the symptoms of Qi deficiency syndrome in early Parkinson's disease with autonomic dysfunction.
