1.Research of the value of ultrasonic imaging in combination with McGill thyroid nodules score (MTNS) in differential diagnosis of thyroid benign and malignant nodules

Objective:To assess the clinical value of ultrasonic imaging in combination with McGill thyroid nodules score (MTNS) system in differential diagnosis of nodule size and benign and malignant tumors.Methods: The clinical data of a total of 112 patients with thyroid nodules were analyzed retrospectively. The nidus of the patients was identified by ultrasound-guided fine needle aspiration biopsy, and then the MTNS, nodule size and false negative rate of the patients were calculated, respectively.Results: The MTNS of the 112 cases of patients with thyroid nodules was within the range of 1 to 18, with an average score of (6.83±2.31). 16 cases with malignant nodules(the percent was 14.29%) were finally diagnosed by pathology, and 96 cases were diagnosed with benign nodules(the percent was 85.71%). The MTNS of patients with malignant nodules was significantly higher than that of those with benign nodules. Ultrasonic imaging showed that the nodule diameter was within the range of 1 to 8.9cm, with an average diameter of (4.13±4.13)cm. MTNS was positively correlation with nodule diameter (r=0.146,P<0.05). Besides, the average diameter of benign nodules was (3.67±1.60)cm, and that of malignant nodules was (4.23±1.51)cm. The missed diagnosed malignant nodules mainly were large diameter nodules.Conclusion: Ultrasound imaging in combination with MTNS can better predict the benign or malignant risk of thyroid nodules.
2.Progress on rehabilitation nursing for hemiplegic shoulder pain after stroke

The article reviewed three aspects of hemiplegic shoulder pain after stroke-introduction, rehabilitation nursing and prospect, in order to provide references for developing effective nursing interventions in the future. We obtained literature from the Cochrane Library, JBI Evidence-based Nursing, PubMed, Wanfang and CNKI Databases between January 2000 and May 2016. According to the principle of three relevant aspects, priority to latest five years and critical appraisal, 39 studies were included. Literature analysis showed that rehabilitation nursing mainly consists of position nursing, supportive devices using, manipulative care and psychological support. Also, it put forward that comprehensive rehabilitation nursing measures have become major means, the combination of Chinese traditional and Western medicine is characteristic therapy in our nation, and nursing practice based on evidence should be set up early to relieve the symptom of hemiplegic shoulder pain and promote quality of life for stroke patients.
3.Characteristics and Drug Resistance of Staphylococcus spp Nosocomial Infection in Suzhou Area 2004-2007

OBJECTIVE To investigate the infection and drug resistance of Staphylococcus spp from hospitalized cases in Suzhou area.METHODS The data from hospitalized cases of 32 hospitals in Suzhou(from 2004 to 2007) were analyzed retrospectively.RESULTS From 2004 to 2007,17 668 cases of nosocomial infection were collected from 32 hospitals in Suzhou area.The infection rate of Staphylococcus aureus was 5.78%,7.11%,8.39% and 7.50%,respectively;the number of meticillin-resistant S.aureus(MRSA) infection cases was 66(34.74%),107(33.86%),138(37.00%) and 219(53.16%) respectively and the total number was 530(41.05%).The nosocomial infection caused by S.epidermidis accounted for 5.99%,5.47%,5.35% and 5.25%,respectively from 2004 to 2007.The number of meticillin-resistant S.epidermidis(MRSE) infection cases were 118(59.90%),128(52.67%),119(50.00%) and 134(46.53%) and the total number was 499(51.66%).The main infection site of S.aureus and S.epidermidis was respiratory tract(74.28% and 71.81%,respectively).Antibiotic resistance strains of S.aureus and S.epidermidis was on rising,and most of them were multi-drug resistance strains.All of the strains were sensitive to vancomycin.CONCLUSIONS In Suzhou area,nosocomial infection and drug resistance of Staphylococcus is on the rise.Evevy hospital must take effective measures to control nosocomial infections of Staphylococcus and drug resistance.
4.Durability of protective effect of resin-based coating material on root surface

Objective:To compare the durability of resin-based root-surface coating material and all-in-one self-etching adhesive on root surface in vitro.Methods:Human extracted premolars or molars with intact roots were selected.The cementum was removed using a periodontal scaler to expose root dentin. The root surface was coated with an acid-resistant nail varnish,leaving a window of 3 mm ×3 mm on the exposed dentin.The window was covered with either PRG Barrier Coat (PRG)or Clearfil S3 Bond (CS3).After water aging for 14 d,specimens were immersed in acid buffer at pH 4.5 for 4 d and the demineralization buffer was changed every 24 h.Then the specimen was split longitudinally through the center of the ‘window’and the cross-sectional surface was observed with scanning electron microscope (SEM).After fixed and dehydrated,the prepared samples were coated with platinum.The coating mate-rial,root dentin and the interface was observed by scanning electron microscope (SEM).The thickness of the coating material was measured on the SEMimages.Regarding toothbrush wear test,coronal dentin-disks were prepared and covered with PRG and CS3,respectively.After storage in water for 24 h,the specimen was subjected to the toothbrush wear tester for 100,200,300,500,700,1 500 brushing cy-cles.A slurry of fluoride toothpaste (1 ∶2 ratio of toothpaste and deionized water by weight)was used and the brushing load was 300 N.The surface microstructure of remaining coating material was analyzed using SEM.The wear depths were determined by a profilometer.Statistical analysis was performed with SPSS 20.0 by one-way ANOVA.The level of significance was at 0.05.Results:Application of PRG Barrier Coat produced a coating layer of (47.1 ±27.3)μm,while CS3 presented a thin film of (5.7 ± 2.1)μm in thickness.The exposed dentin was hermetically sealed and no obvious gap was observed at the interface in both PRG and CS3 groups.There was no dentin demineralization observed in both groups after water aging.The wear depths of PRG and CS3 increased along with the numbers of brushing cycles. PRG wore at a significant lower pace than CS3 did (P <0.05).Conclusion:PRG coating resin had similar performances as CS3 on protecting root dentin from demineralization after water aging.What’s more,PRG demonstrated a higher toothbrush wear resistance than CS3.We concluded that PRG Barrier Coat contained S-PRG filler may be an effective coating material for protecting exposed root from both chemical and mechanical challenges.Further studies should be carried out to evaluate the long-term reli-ability of the rootsurface coating materials under the clinical setting.

6.Intervention effect of the mode of monitoring-training-planning on rational use of antibacterials in upper respiratory infection in outpatient clinic of respiratory department

Objective To study the effect of the intervention mode of MTP on use of antibacterials in upper respiratory infection.Methods Adopt retrospective method to select randomly prescriptions of upper respiratory infection from July to September in 2006 for baseline investigation in Outpatient Clinic of Respiratory Department of the First People's Hospital of the city of Zhuhai in Guangdong province.Then aim physicians was interfered by MTP and investigation of post-interference was carried out after a month.The process of intervention and investigation was carried out repeatedly until June in 2007.The ratio of antibacterials use,injection use percentage and average drug fee was observed in pre/post-interference.Results The ratio of antibacterials use in upper respiratory infection in our hospital was decreased from 81.33% to 0,and the ratio percentage of injection use and average drug fee decreased by 81.69% and 35.47% respectively after four MTP circulations.Conclusion The intervention mode of MTP is feasible and effective on promoting the rational use of antibacterials on upper respiratory infection in Outpatient Clinic of Respiratory Department of our hospital.

AIM: To observe the changes of glycemia and serum cholesterol and triglyceride in diabetic nephropathy rats and therapeutic effects of Xiaoke Keli Ji. METHODS: 3/4 nephrectomy was adopted firstly, three weeks later streptozotocin(STZ) was administered intraperitoneally to establish diabetic nephropathy model in rats. Animals were divided into four groups:model group, Xiaoke Keli Ji treatment group, positive control group and sham group. Changes of serum sugar and serum creatinine, cholesterol and triglyceride were examined at 1, 2, 3, 4, 5 weeks after STZ injection. Renal tissue samples were adopted at 6th week and studied by light microscopy. RESULTS: Model group demonstrated different degree of glomerular sclerosis. Lesions in treatment group were alleviated. Serum creatinine, serum sugar and serum cholesterol were higher at 1, 2, 3, 4, 5 weeks after STZ injection in model group than that of the sham group ( P

According to the measured results of ethanol content in vivo, ethanol metabolic distributed rules, mechanisms of ethanol toxicological effect and its production in vivo, this study analysed systematically the time after drinking, total quantity of absorbed ethanol, psychological situations, behavioral dominated ability, death causes and manners in order to find out the implied forensic medical information and provide the reference for colleague.

9.Evaluation of the degree of white matter damage in patients with white matter hypertensities and cognitive impairment

Objective:To analyze the characteristic of nerve tracts in different stages of cognitive impairment caused by white matter hypertensities, applying diffusion tensor imaging-peak width of skeletonized mean diffusivity as evaluation indicator.Methods:Demographic characteristics and vascular risk factors of all subjects were recorded.Screened out 50 subjects without WMH and whose MoCA scale was more than or equal 26 points as normal control group.Then, we assessed 85 patients with WMH using the MoCA scale and clinical dementia assessment. Of 85 patients, 45 cases were divided into the normal cognition group, 23 cases were assigned into non-dementia vascular cognitive impairment group, and 17 cases into the vascular dementia group.The diffusion tensor imaging-peak width of skeletonized mean diffusivity marking method was used to compare the integrity of white matter fibers in WMH patients with varying degrees of cognitive impairment and normal control group.Results:The difference of PSMD among the four groups was statistically significant(CN group: 3.558±0.157, VCIND group: 4.128±0.222, VaD group: 5.469±0.000, NC group: 2.612±0.105, F=42.479, P<0.05). PSMD in WMH groups were higher than that in NC group (all P<0.05). There was no significant difference between WMH-VCIND group and WMH-CN group ( P=0.067). The PSMD of WMH-VAD group was higher than that of WMH-CN group and WMH-VCIND group (both P<0.05). Conclusion:PSMD can accurately detect white matter fiber damage caused by WMH.The degree of WMH combined cognitive decline was not completely consistent with the degree of PSMD increase.Compared with the cases with normal WMH cognition, PSMD was not significantly increased when WMH was combined with VCIND, while PSMD was significantly increased when WMH was combined with VaD.DTI-PSMD value as imaging marker can provide objective basis for clinical diagnosis of WMH-VaD.
10. Regulating effect of glycyrrhetinic acid on bronchial asthma smooth muscle proliferation and apoptosis as well as inflammatory factor expression through ERK1/2 signaling pathway

Objective To study the influence of glycyrrhetinic acid (GA) on bronchial asthma (BA) smooth muscle proliferation and apoptosis as well as inflammatory factor expression and its molecular mechanism. Methods Male SD guinea pigs were selected and made into asthma models, bronchial asthma smooth muscle cells were cultured and divided into BA group, GA group and GA + LM group that were treated with serum-free RPMI1640 culture medium, serum-free RPMI1640 culture medium containing 50 ng/mL glycyrrhetinic acid, serum-free RPMI1640 culture medium containing 50 ng/mL glycyrrhetinic acid and 100 ng/mL LM22B-10 respectively; normal guinea pigs were collected and bronchial smooth muscle cells were cultured as control group. The cell proliferation activity as well as the expression of proliferation and apoptosis genes, inflammatory factors and p-ERK1/2 was determined. Results Proliferation activity value and mRNA expression of Bcl-2, TNF-α IL-4, IL-6, YKL-40, protein expression of p-ERK1/2 of airway smooth muscle cell in BA group were significantly higher than those of control group while mRNA expression levels of Bax, caspase-9 as well as caspase-3 were significantly lower than that of control group (P < 0.05); proliferation activity value and mRNA expression of Bcl-2, TNF-α IL-4, IL-6, YKL-40, protein expression of p-ERK1/2 of airway smooth muscle cell in GA group were significantly lower than those of BA group (P < 0.05) while the mRNA expression levels of Bax, caspase-9 as well as caspase-3 were significantly higher than those of BA group (P < 0.05); proliferation activity value and mRNA expression of Bcl-2, TNF-α IL-4, IL-6, YKL-40 of airway smooth muscle cell in GA + LM group were significantly higher than those of GA group (P < 0.05) while mRNA expression levels of Bax, caspase-9 as well as caspase-3 were significantly lower that of GA group (P < 0.05). Conclusion GA can inhibit the proliferation of bronchial smooth muscle cells and reduce the expression of inflammatory factors by inhibiting the phosphorylation of ERK1/2.
