Objective To observe the effect of febuxostat on IL-1β, TNF-αand COX-2 in gout patients. Methods 82 patients with gout in our hospital from August 2015 to August 2016 were divided into the groups of 40 mg/day of febuxostat and 80 mg/day of febuxostat randomly, 41 cases respectively.Select the same period in our hospital health examination of 41 cases. The levels of serum uric acid were measured before and after one week, two weeks and four weeks after treatment.Serum levels of IL-1β, TNF-αand IL-1βin serum were measured by enzyme-linked immunosorbent assay (ELISA) COX-2 levels. Results The levels of serum uric acid in the group of 40 mg/day vs 80 mg/day of febuxostat were (570.4 ±70.5) μmol/L and (572.8 ±71.2) μmol/L, significantly higher than the normal group (296.5 ±9.7) μmol/L (P<0.05), serum uric acid level decreased gradually with the time of administration, and the serum uric acid levels of the two dosages of fosobutamol at one, two, four weeks were significantly lower than before treatment (P<0.05), but not to below the normal level. The IL-1βof 40 mg/day and 80 mg/day of febuxostat was (40.5 ±6.8) pg/mL and (41.7 ±7.2) pg/mL, COX-2 were (15.5 ±1.7) pg/mL and (15.8 ±1.8) pg/mL respectively, significantly higher than the normal group (8.8 ± 1.7) pg/mL (P<0.05), The expression of IL-1βwas significantly decreased at the second week of IL-1βdecreasing as the time of IL-1βdecreased gradually (P<0.05); TNF-αin 40 mg/day group and 80 mg/day group was significantly lower than that before treatrment (P<0.05), but not to below the normal level. Conclusion Febuxostat can significantly reduce serum uric acid levels in patients with gout, and may reduce IL-1β, TNF-αand COX-2 and other cytokine levels.

Objective To investigate the dose-response of micronuclei (MN) frequency in the lymphocytes irradiated with or without combination of α-particles and γ-rays. Methods Human lymphoblast cells HMy2.CIR were irradiated with 0 - 1 Gy of α-particles,0 - 5 Gy of γ-rays,and 0.025 -0.5 Gy of α-particles followed by different doses of γ-rays,respectively.The micronuclei (MN) in the irradiated cells were measured with the cytokinesis block technique, and the dose-responses of MN were established under different irradiation conditions. Results For γ-ray irradiation, the dose-response of MN was well-fit by the linear-quadratic model with an equation Y =c + αD + βD2.For α-particle irradiation, the MN induction increased linearly with the dose less than 0.250 Gy. But when the dose of α-particles increased continually, the dose-response curve bended and could be well fit with the BaD model Y =c + αD + σ[ 1 - exp( - δD) ] exp( - βD) where radiation-induced bystander effect (RIBE) was indicated. For the combined exposure, the dose-response of MN was similar to that of γ-irradiation when the dose of α-particles was lower than 0.1 Gy,but it was similar to that of α-irradiation when the dose of α-particles was higher. When the dose of α-particles was 0.2 and 0.5 Gy,MN induced by the mixed radiation were significantly higher than the sum of corresponding irradiation alone ( t =5.22 - 11.86,P < 0.05 ).Conclusions The radiation damage of α-particles differs from that of γ-rays,where RIBE may be involved. The combination irradiation of α-particles and γ-rays has a synergistic effect on radiation damage of lymphoblast cells.

Objective:To investigate the current status of primiparas′ postpartum fatigue and paternal involvement, and to explore the relationship between primiparas′ postpartum fatigue and paternal involvement, and to provide reference basis for developing targeted intervention measures to alleviate postpartum fatigue of primiparas. Methods:A cross-sectional survey was conducted on 347 primiparas from Affiliated Hospital of Yangzhou University, Yangzhou Maternal and Child Health Care Hospital from September to December 2020 by convenience sampling. The survey instruments included the general information questionnaire, the Parenting Alliance Inventory (PAI), and the Postpartum Fatigue Scale (PFS).Results:The total score of PAI was (86.51 ± 12.07) points, and the level of paternal involvement was high. The total score of PFS was (16.68 ± 4.12) points. 95.97% (333/347) of primiparas had varying degrees of postpartum fatigue. There was a significant negative correlation between paternal involvement and primiparas′ postpartum fatigue ( r=-0.327, P<0.01). The results of multiple stratified regression analysis showed that paternal involvement was included in the influencing factor model of primiparas′ postpartum fatigue, which could independently explain 9.7% variation of primiparas′ postpartum fatigue. Conclusions:The higher level of paternal involvement could predict the lower level of primiparas′ postpartum fatigue. Medical staff should pay attention to the participation level of the spouses of primiparas in childcare, and improve the participation level of the spouses of primiparas in scientific ways to alleviate the postpartum fatigue of primiparas.

Objective:To explore the application effect of the caregiver self-management support project in main caregivers of adolescents with epilepsy. Methods:Using the convenient sampling method, the main caregivers of adolescents with epilepsy who were enrolled before implementation of the caregiver self-management support project of the First Affiliated Hospital of Zhengzhou University from January to August 2019 were included in the control group ( n=101) , and they were given routine follow-up. The main caregivers of adolescents with epilepsy who were enrolled after the implementation of the project from September 2019 to June 2020 were included in the study group ( n=101) . Before and after the intervention, the Connor-Davidson Resilience Scale (CD-RISC) , Simplified Coping Style Questionnaire (SCSQ) , Zarit Burden Interview (ZBI) , Life Satisfaction Index A (LSIA) and Medical Outcome Study Social Support Survey (MOS-SSS) were used to compare the psychological resilience, coping style, quality of life and social support of caregivers between the two groups. Finally, a total of 98 caregivers in the study group and 97 caregivers in the control group completed the study. Results:After the intervention, the scores of dimensions of CD-RISC, positive coping score of SCSQ, LSIA score and dimensions scores of MOS-SSS of main caregivers in the study group were higher than those in the control group, while the scores of negative coping of SCSQ and ZBI were lower than those in the control group, and the differences were statistically significant ( P<0.05) . Conclusions:The caregiver self-management support project helps to strengthen the psychological resilience of the main caregivers of adolescents with epilepsy, enhance their active coping style and social support and improve their quality of life.

Objective:To explore the application effect of mindfulness meditation training in patients with heart failure treated by left bundle branch area pacing cardiac resynchronization therapy. Methods:Using the convenient sampling method, a total of 86 patients with heart failure who were admitted to the Heart Center of Henan Provincial People's Hospital from March 2019 and March 2020 were selected as the research objects. According to the random number table method, a total of 86 patients were divided into the observation group and the control group, with 43 cases in each group. Patients in the control group were given routine nursing and health education, while patients in the observation group were given mindfulness meditation intervention on this basis. Self-rating Anxiety Scale (SAS) , Self-rating Depression Scale (SDS) and Pittsburgh Sleep Quality Index (PSQI) were used to compare the intervention effects. Results:After intervention, scores of SAS, SDS and PSQI of patients in the two groups were all lower than those before intervention, and scores of SAS, SDS and PSQI of patients in the observation group were all lower than those in the control group, and the differences were statistically significant ( P< 0.05) . Conclusions:Mindfulness meditation intervention in patients with chronic heart failure treated by left bundle branch area pacing cardiac resynchronization therapy can effectively reduce their negative emotions and improve their sleep quality.
