1.Effectiveness of clinical pharmacists 'intervention in antimicrobial prophylaxis in type Ⅰ incisional surgery

Objective To evaluate effectiveness of clinical pharmacists'intervention in perioperative antimicrobial use in type Ⅰ incisional surgery.Methods Clinical data of 1398 patients undergoing type Ⅰ incisional surgery in a hospital between January and December 2014 were analyzed retrospectively,633 patients who underwent surgery between January and June were as pre-intervention group,765 patients who underwent surgery between July and December were as post-intervention group (clinical pharmacists were assigned to strengthen the management and control of rational antimicrobial use),antimicrobial prophylaxis between pre-intervention group and post-interven-tion group were compared.Results In pre- and post-intervention groups,antimicrobial prophylaxis were 56.71%and 28.37% respectively(χ2= 113.26,P<0.05). Rational selection rates of antimicrobial types were 60.45% and 89.86% respectively,rational rates of administration time were 49.58% and 82.49% respectively,the rates of anti-microbial prophylaxis course≤24 hours were 11.42% and 29.95% respectively,>24 hours were 88.58% and 70.05% respectively,combination antimicrobial use were 10.31% and 0 respectively,the differences between before and after intervention were all statistically significant (all P<0.05). Incidence of surgical site infection in pre-and post-intervention groups were 0.79% (5/633)and 0.26% (2/765)respectively(P= 0.255).Conclusion Through clini-cal pharmacists'intervention in perioperative antimicrobial prophylaxis in type Ⅰ incisional surgery,antimicrobial prophylax-is rate decreased dramatically,antimicrobial management is more standardized,scientific and systematic.
2.Effect of nitric oxide on myocardial cell injury after hypoxia in rats

Objective To clarify the effect of NO on hypoxic myocardium in rats.Methods Myocardial cells from neonatal Wistar rats aged 1-2 days cultured for 4-6 days,then pretreated with GSNO(30,200,4 000 ?mol/L)were exposed to normoxia(N)or hypoxia(H)(1%O2,5%CO2 and 94%N2)for 4 h.The NO content and LDH in the supernatant were detected.Results NO of 1.43,7.2 and 70 ?mol/L were detected respectively in the supernatant of hypoxic myocardial cells treated by GSNO(30,200 and 4 000 ?mol/L).Compared with normoxic group,LDH activity increased and myocardial cell activity decreased markedly in H+G30 and H+G4000 groups but less injury was observed in H+G200 group.Conclusion NO of certain concentration can markedly abate myocardial cell injury induced by hypoxia.
3.Investigation on 6 genetical traits in Yi and Bai ethnic groups in Guizhou province

Objective To study distributive frequencies and the corelation among 6 genetical traits in Yi and Bai ethnic groups in Guizhou province. Methods According to the techniques of human population genetics and cluster random sampling,a survey on 6 genetical characters of 879 cases (Yi ethnic group 472 and Bai ethnic group 407) was carried out in October 2008,then dealt with data using SPSS13.0 software. 4.p38 kinase pathway mediated cardiomyocyte injury in rats due to hypoxia and burn serum

Objective To investigate the roles of the activated p38 kinase in cell injury by observation of the effects of hypoxia and burn serum on cardiomyocyte p38 kinase and JNK activation. Methods Phosphorylation of p38 kinase and JNK in primary cultured neonatal rat cardiomyocytes before and after hypoxia and burn serum was determined by Western blotting. Effects of pretreatment with SB203580 at the dose of 10 ?mol/L on the changes of phosphorylation of p38 kinase in cardiomyocytes, lactate dehydrogenase (LDH) activity, cell vitality and apoptosis were investigated, respectively. Results Exposure of rat neonatal cardiomyocytes to hypoxia and burn serum resulted in a rapid and long lasting activation of p38 kinase but no significant activation of JNK. SB203580(10 ?mol/l), a selective inhibitor of p38 kinase, could inhibit p38 kinase activation dramatically, decrease the LDH activity in culture media and cell apoptosis significantly and improve cell vitality. Conclusion In the two stress activated signal pathways of MAPKs family, p38 kinase pathway, but not JNK, is the major pathway activated by hypoxia and burn serum and participates in the cardiomyocyte injury.

Giardia lamblia is an important intestinal protozoan which can cause diarrhea in humans. The detection of Giardia infection is performed through the detection methods of pathogen,immunoassay and molecular biology. Currently,the immunodiagnostic methods have good application and development prospect because of high sensitivity and specificity,simple and convenient,and time saving. In this article,we review the main progress and application of immunodiagnostic methods for Giardia infection.
6.Pharmacokinetic study of loganin in LIUWEI DIHUANG DECOCTION~* in mice

Object To study the pharmacokinetics of loganin in LIUWEI DIHUANG DECOCTION (LWDHD) in mice. Methods HPLC-UV detection was used to determine the loganin levels in biological samples. Results After ig LWDHD in mice, the plasma concentration-time course fitted well to one-compartment model with the 1st order absorption and with the following pharmacokinetic parameters; Ka= 0.04min -1, Ke=0.019 min -1, T (peak)=42.4 min, t 1/2ka=17.4 min, t 1/2ke=35.75 min. Conclusion The pharmacokinetic parameters of loganin obtained in the study may be attributed to the effect of other constituentsin the DECOCTION.
7.Effects of microtubule depolymerization on spontaneous beating and action potential of cardiac myocytes in rats and its mechanism.

OBJECTIVETo explore the effects of microtubule depolymerization (MD) on the spontaneous beating rate, action potential (AP), and oxygen consumption of cardiac myocytes in rats and its mechanism.
METHODSOne-hundred and eighty neonatal SD rats divided into 12 batches were used in the experiment, and 15 rats in each batch were sacrificed for the isolation and culture of cardiac myocytes after the heart tissues were harvested. The cardiac myocytes were respectively inoculated in one 12-well plate filled with 6 round cover slips, one 12-well plate filled with 6 square cover slips, two cell culture flasks, and two cell culture dishes. After routine culture for three days, the cardiac myocytes from all the containers were divided into normal control group (NC, routinely cultured with 3 mL DMEM/F12 solution rewarmed at 37 °C for 3 h) and group MD (routinely cultured with 3 mL DMEM/F12 solution rewarmed at 37 ° and containing 8 µmol/L colchicine for 3 h) according to the random number table, with 3 holes, 1 flask, or 1 dish in each group. The morphological changes in microtubules were observed with confocal laser scanning microscope after immunofluorescent staining. The content of polymerized or dissociative α-tubulin was determined by Western blotting. Spontaneous beating rate of the cells was observed and calculated under inverted microscope. Dissolved oxygen concentration of DMEM/F12 solution containing cardiac myocytes was determined by oxygen microelectrode system before and after the addition of colchicine. Additionally, dissolved oxygen concentration of DMEM/F12 solution and colchicine + DMEM/F12 solution was determined. The whole-cell patch-clamp technique was used to record AP, delayed rectifier K+ current (I(K)), and L-type Ca2+ current (I(Ca-L)) in cardiac myocytes; current density-voltage (I-V) curves were drawn based on the traces. Data were processed with independent or paired samples t-test.
RESULTS(1) In group NC, microtubules of cardiac myocytes were around the nucleus in radial distribution with intact and clear linear tubiform structure. The microtubules in group MD were observed in dispersive distribution with damaged structure and rough linear tubiform structure. (2) In group MD, the content of dissociative α-tubulin of cells (0.61 ± 0.03) was obviously higher than that in group NC (0.46 ± 0.03, t = -6.99, P < 0.05), while the content of polymerized α-tubulin (0.57 ± 0.04) was significantly lower than that in group NC (0.88 ± 0.04, t = 9.09, P < 0.05). (3) Spontaneous beating rate of cells was (59 ± 8) times per min in group MD, which was distinctly higher than that in group NC [(41 ± 7) times per min, t = 5.62, P < 0.01]. (4) Dissolved oxygen concentration of DMEM/F12 solution containing cardiac myocytes was (138.4 ± 2.5) µmol/L, and it was reduced to (121.7 ± 3.6) µmol/L after the addition of colchicine ( t = 26.31, P < 0.05). There was no obvious difference in dissolved oxygen concentration between DMEM/F12 solution and colchicine + DMEM/F12 solution (t = 0.72, P > 0.05). (5) Compared with that of group NC, AP morphology of cells in group MD changed significantly, with unobvious repolarization plateau phase and shorter action potential duration (APD). The APD20, APD50, and APD90 were respectively (36.2 ± 3.8), (73.7 ± 5.7), and (115.1 ± 8.0) ms in group MD, which were significantly shorter than those of group NC [(40.2 ± 2.3), (121.4 ± 7.0), and (169.4 ± 5.6) ms, with t values respectively 2.61, 15.88, and 16.75, P values below 0.05]. (6) Compared with that of group NC, the I-V curve of I(K) of cells in group MD moved up with higher current density under each test voltage (0 to 40 mV) after activation ( with t values from 2. 70 to 3. 76, P values below 0.05) . (7) There was not much alteration in current density of I(Ca-L) under each test voltage (-30 to 50 mV) between 2 groups (with t values from -1.57 to 1.66, P values above 0.05), and their I-V curves were nearly overlapped.
CONCLUSIONSAfter MD, the I(K) is enhanced without obvious change in I(Ca-L), making AP repolarization faster and APD shortened. Then the rapid spontaneous beating rate increases oxygen consumption of cardiac myocytes of rats.

8.Setup of professional positions in stomatological hospitals

This paper introduced the position setup and initial recruitment initiated in publicfunded institutions in Beijing, i.e. Beijing Stomatological Hospital. It stated that position setup should fit actual needs and recruitment standards should comply with long-term development strategy. In addition, the paper highlighted such principles as transparency and all-staff involvement, along with the supervising function of departments and experts of the hospital. Thus, position setup and initial recruitment can be smoothly accomplished, forming a long-standing incentive for medical staff.
9.Distribution of Bacteria Detected from Blood Culture of Pediatric Patients and Analyses of Antibiotic Resistance

OBJECTIVE To investigate the distribution of bacteria detected from blood culture of pediatric patients and to observe the antibiotic resistance of these bacteria. METHODS The BacT/Alert blood culture system was applied for culture.Species identification and antibiotic resistance tests were performed by the VITEK automicroscan system. RESULTS Coagulase-negative staphylococcus(CNS) and Staphylococcus aureus accounted for 67.2% and 7.8%, respectively.The proportion of Gram-negative bacteria was 14.4%.The resistant rate of CNS and S.aureus to oxacillin was 85% and 36%,respectively.The strains producing the extended spectrum beta-lactamases(ESBLs) of Gram-negative bacteria were often detected. CONCLUSIONS Gram-postive bacteria are the main pathogens detected from the blood culture of pediatric patients.Detection rate of CNS is the highest.Multiresistant strains are common.
10.Acetylized Racemization Process and Kinetics of D-penicillamine

OBJECTIVE:To study the process of acetylized racemization of D-penicillamine in the acidic condition. METHODS:The acetylized racemic mixture of D-penicillamine was prepared by racemizing acetyl chloride in acetic acid solu_tion with D-penicillamine as feedstock.The preparation process was optimized with the quantities of solvent and acetyl chloride,the reaction temperature,the reaction time etc.as parameters.The influence of reaction temperature,reaction time on the specific optical rotation in the acetylized process was determined and the kinetic equation of acylation process was computed. RESULTS:The optimal condition for racemization was the following,the quantity ratios of D-penicillamine-acetic acid -acetyl chloride were 1∶7∶2,the reaction temperature was 80℃ and the reaction time was 5h.The kinetic equation of acylation process fitted first order linear relation. CONCLUSION:This preparation process is mild and simple,and it offers direct feedstock for the preparation of D,L-penicillamine.
