Objective To discuss influence of different disinfection solution on infection and pain of puncture hole in patients with open fracture of both tibia and fibula treated with external fixation.Methods 41 patients were divided into group A(21 cases)and group B(20 cases).Group A adopted 75% ethanol to sterilize the puncture holes.While the group B used 5%iodide instead.The incidence of infection and pain of puncture holes was observed separately at the 1st week,6th week after operation and removing of fixation pin.Results The incidence of infection of puncture holes was not apparently different from each other(P>0.05)at any other time points.The incidence of paint in group B was lower than group A at the 1st week and 6th week after operation(P<0.05).The incidence of pain was higher in patients with puncture hole infection than those without puncture hole infection(P<0.01).Conclusion No difference was seen in puncture hole infection with either 75% ethanol or 5% iodide.Infection of puncture hole was correlated with pain only at the 1st week after operation.Pain could be alleviated with application of 5% iodide.

Objective To study the chemical constituents from the whole plant of Anisomeles indica. Methods The constituents were isolated and purified by various column chromatographic techniques including silica gel, Sephadex LH-20, and ODS. The structures were identified by spectroscopic analyses and physicochemical properties. Results Ten compounds were isolated from the ethanol extract of the whole plant of Anisomeles indica, in which seven phenylethanoids were identified as epimeredinoside A (1), crenatoside (2), verbascoside (3), cistanoside D (4), 3'-O-methyl isocrenatoside (5), isocrenatoside (6), and glycosmisic acid (7). Three phenolic acids were vanillic acid (8), caffeic acid (9), and ferulic acid (10). Conclusion Compounds 4-7 are isolated from the title plant and Anisomeles genus for the first time.

Objective: To analyze the status quo of assanation and ethics courses and their social requirement, thus to explore a scientific mode for the course setting; Method: The course setting status quo of assanation and ethics is analyzed among 18 colleges in which the nursing speciality is set up. A questionnaire was also conducted to investigate students' inclination towards politics-sorted courses, and relevant data were analyzed. Results: The establishing rate of assanation courses in the higher professional colleges is higher than that in the comprehensive universities. The most favorable subjects among freshmen are the Chinese national political theories, and assanation & ethics, while the counterparts among seniors are assanation & ethics, law basis, and philosophy. Objective To study the pathologic feature and rational diagnosis and treatment of Mirizzi Syndrome. Method The clinical data of 43 cases treated by surgery were retrospectively analysed. Results All the 43 cases underwent operation, including partial cholecystecomy in 8 cases, cholecystectomy in 16cases , cholecystectomy plus common bile duct exploration with T tube drainage in 9 cases, choledochojejunostomy in 10 cases. Of the 43 cases, 36 cases were followed up for 1～5 years. Of them, 29 cases were in excellent, 6 cases in good and 1 case in poor. Conclusions The pathologic type of Mirizzi Syndrome is variant. It is difficult to make a definite diagnoses before operation. So vary imaginal technique should be adopted. Different operative procedures should be used according to patients' pathologic type.

Objective To investigate the prevalence of human metapneumovirus (HMPV)infection in patients with respiratory infection in Fuzhou area and compare their epidemic features and clinical characteristics with those of infected with respiratory syncytial virus (RSV). Methods A total of 153 sputum or pharyngeal swab samples from patients with respiratory tract infection were collected in Fujian Provincial Hospital in consecutive winter and spring seasons from 2005 to 2007. HMPV was detected by nested reverse transcriptase-polymerase chain reaction (RT-PCR) and RSV was tested by RT-PCR. Parts of PCR products were sequenced and analyzed using DNAMAN software. The clinical symptoms, signs and epidemiology of the respiratory tract infections caused by HMPV and RSV were compared. Results In the 153 specimens, 32 (20.9%) were positive for H MPV, 26 (17.0%) were positive for RSV, and 8 were both HMPV and RSV positive. Nucleotide sequences of three 432-bp PCR products were 100% identical and submitted to GenBank (the accession No. DQ887758).Phylogenetic tree analysis of nucleotide sequences revealed that the three isolates clustered in HMPV belonged to genotype A with part of mutation. Twenty-six samples (26. 7%) were HMPV positive from Dec 2005 to Apr 2006 and 6 (10.7%) were positive from Dec 2006 to Apr 2007. The RSV detection rate was opposite of HMPV. The mean age of RSV infection in children was (2.65±2.65)years old and HMPV infection was (4.58 ±3.35) years old. The main clinical manifestations of both RSV and HMPV infections were cough, sore throat and fever. Conclusions Both HMPV and RSV are the major pathogens of respiratory tract infection in Fuzhou area and HMPV and RSV coinfection could be detected. HMPV infected children are older. The clinical features of HMPV and RSV infections are similar. Single genotype of HMPV is detected in Fuzhou area in this study.

Objective To explore the relationship between the types of female breast density and age and breast cancer .Methods By accepting the digital mammography X -ray examination for 5 006 women cases and according to the ACR BI -RADS standard in the fourth edition ,the breast density assessment was quantified . We analysed the relationship between the breast density and age and breast cancer .Results In 5 006 cases,the average female age was between 44.22 ±8.09 years old,median age was 43 years old.The components of the breast density were fat type , small amount type , large amount type and compact type each count were 256 (5.11%),726(14.51%),3 719(74.29%),305(6.09%)respectively.By dividing into different age -group to analyze the breast density,there was significant statistical differences of the breast density among age -groups(P<0.001).Among them the breast cancer were 184 cases.Age was between 51.26 ±10.15 years old.Breast cancer in each breast density were fat type 10.16%(26/256),small amount type 9.09%(66/726),large a-mount type 2.45%(91/3719)and compact type 0.33%(1/305).There were statistical differences among age -groups and breast densities and breast (P<0.001).Conclusion Age plays a very important effects on the fe-male breast density .The lower breast density is a high risk factor to breast cancer occurrence .

Objective To evaluate the effect of Xingqi Tongfu Pad on the intestinal functions of children with acute abdomen after surgery. Methods The random digit table was used to randomize 146 patients with acute abdomen into treatment group (n=71) and control group (n=75) according to their registration number. In the control group, the patients only received normal treatment and nursing, while those in the treatment group received Xingqi Tongfu Pad for the acupoint application in addition to routine treatments. The two groups were compared in terms of the time of bowel sound recovery , the exhaust defecation time and gastrointestinal dysfunction score. Results The bowel sounds and the first exhaust and defecation in the treatment group appeared obviously earlier than those in the control group . The score on gastrointestinal dysfunction in the treated group was significantly higher than that of the control group (P<0.05). Conclusions Xingqi Tongfu Pad can promote the recovery of gastrointestinal function after abdominal operation. It deserves popularization for its positive effect, cheap price and easy operation.

OBJECTIVETo detect the expression levels of CD4(+) CD25(+) CDl27(low) Treg cells, TGF-β and Notch1 mRNA in peripheral blood of the patients with idiopathic thrombocytopenic purpura (IPT) before and after treatment, and to investigate their significance in the pathogenesis of ITP.

METHODSPeripheral blood was collected from 30 newly diagnosed patients with ITP and 20 normal controls, then the number of CD4(+) CD25(+) Treg and CD4(+) CD25(+) CDl27(low) Treg were detected by the flow cytometry. Plasma TGF-β level was determined by ELISA. Total RNA was extracted and the expression level of Notch1 mRNA was measured by real-time Q-PCR.

RESULTSThe expression levels of CD4(+) CD25(+) CDl27(low) Treg and CD4(+) CD25(+) Treg in newly diagnosed ITP group were significantly lower than those in normal controls. After treatment, the proportion of Tregs increased to (5.17% ± 0.74%) and (4.16% ± 0.68%), and was higher than that in newly diagnosed patients. The TGF-β level in peripheral blood of newly-diagnosed patients was obviously lower than that in normal controls, and was (961.53 ± 60.10) ng/L after treatment and was significantly higher than that in newly diagnosed patients; the expression level of Notch1 mRNA in peripheral blood of patients in newly-diagnosed group was obviously lower than that in control, and was (1.35 ± 0.10) after treatment that was higher than that in newly-diagnosed group. After treatment, the proportion of Treg cells, level of TGF-β and erpression level of Notch1 mRNA in effective group were higher than those in effective group, improved group and ineffective group, and there was significant difference (P <0.01). The expression level of TGF-β and Notch1 mRNA in ITP patients possitively correlated to CD4(+) CD25(+) CDl27(low) (P <0.01).

CONCLUSIONSThe levels of CD4(+) CD25(+) CDl27(low) Treg, TGF-βand Notch1 mRNA in peripheral blood of the patients with ITP are significantly lower than those of normal control, suggesting that there is significant abnormal immunoregulation in ITP patients. In the ITP patients the levels of CD4(+) CD25(+) CDl27(low) Treg postively correlated with Notch1 mRNA expression, indicating that Notch signal may be revalent to Treg's immunosuppression function.

ObjectiveTo investigate the therapeutic value of noninvasive mechanical ventilation in patients with conscious disturbance due to severe chronic obstructive pulmonary disease (COPD) with respiratory failure. MethodsForty-two patients with conscious disturbance due to COPD complicated with respiratory failure were selected in the study. GALILEO or PAPHAEL large EMT ventilator produced by Switzerland Hamilton Company was used for noninvasive mechanical ventilation with P-SIMV+PSV+PEEP mode. Meanwhile, the change of vital signs, consciousness, the degree of inspiratory muscle fatigue and blood gas indexes were observed before and after treatment. Glasgow coma scale (GCS) was applied to evaluate the consciousness, and scale for accessory muscle use was adopted to measure the degree of inspiratory muscle fatigue. ResultsAll the patients were treated successfully. Compared with admission, the pH value, the pressure of arterial carbon dioxide (PaCO2) and the arterial oxygen pressure/inhaled oxygen concentration (PaO2/FiO2)were significantly improved 2 hours, 4 hours and 24 hours after treatment. The respiration rate and heart rate were markedly decreased 24 hours after treatment, which indicated that the condition of the patients tended to be stable and the patients could endure the therapy. The average GCS was increased from 5.69-1-0.93 to 10.45±1.23 (t= 31.68, P<0.001), and the state of consciousness was improved significantly. The scale for accessory muscle use was decreased from 3.70±0. 45 to 2. 06±0. 52 (t = 31.21, P < 0. 001), and the respiratory muscle fatigue was relieved. ConclusionsNoninvasive mechanical ventilation has obvious clinical curative effect on severe infection and conscious disturbance due to severe COPD complicated with respiratory failure.
