1.Recent advance in related factors for ivy sign of moyamoya disease

Ivy sign, as a relatively specific sign in moyamoya disease, has certain clinical value for the diagnosis, preoperative evaluation, and prognostic evaluation of moyamoya disease. However the pathophysiological mechanism of ivy sign is still unclear and this phenomenon is affected by many factors. In this paper, the domestic and foreign literature about ivy sign of moyamoya disease was collected and analyzed and the correlation between different factors and ivy sign was summarized to provide a basis for further study on the correlation between ivy sign and various influencing factors.
2.A cross-sectional study of mental disorders in Ximeng Wa Autonomous County of Yunnan Province

Objective:To describe the epidemiological characteristics of mental disorders in Ximeng Wa Au-tonomous County and explore psychosocial risk factors of mental disorders. Methods:Two thousand three hundred and eighty one residents aged 1 8 years and over were sampled using multistage sampling in Ximeng Wa Autono-mous County in 2013. All respondents were investigated by face-to-face interview. Mental disorders were screened out by using the General Health Questionnaire (GHQ)and diagnosed according to the International Classification of Diseases Checklist (ICD-10-Checklist)criteria. Results:Life time prevalence of any mental disorder was 19. 86%(419/2110). The lifetime prevalence rates of substance use disorder,anxiety disorder,insomnia,mood disorder, schizophrenia were 12. 99%(274/2110),6. 30%(133/2110),2. 94%(62/2110),2. 32%(49/2110)and 1. 00%(21/2110)respectively. Male (OR=0. 43),older age (35 -49 years,OR=1. 78;50 -64 years,OR=2. 59;≥65 years,OR=3. 5 1 ),unmarried and other marital status (OR=0. 3 1 ),non-Wa and non-Lahu ethnic groups (OR=0. 29)were associated with neurotic,stress-related and somatoform disorders. Male (OR=2. 41),older age (35 -49 years,OR=2. 29;50-64 years,OR=3. 20;≥65 years,OR=4. 58),non-farmer and non-self-employed occupation (OR=0. 41),and non-Wa and non-Lahu ethnic groups (OR=0. 32)were associated with psychoactive substance use disorder. Male (OR =0. 35 ) and order age (≥65 years,OR =3. 05 ) were associated with mood disorders. Conclusion:Lifetime prevalence of any mental disorder,substance use disorders and anxiety disorders are high in Ximeng Wa Autonomous County. Measures should be strengthened against prevalence of mental disorders in ethnic minority areas.
3.Effects of nursing practice environment on clinical innovation ability of clinical pediatric nurses

Objective:To understand the current situation of nursing practice environment and clinical innovation ability of pediatric nurses and to explore the effect of nursing practice environment on clinical innovation ability of clinical pediatric nurses.Methods:A cross-sectional survey method was adopted to select 478 pediatric nurses from the Children's Hospital of Nanjing Medical University as the research objects from May to June 2019 by the convenient sampling method. The general data questionnaire, profession practice environment-nursing work index (PES-NWI) and innovation ability of clinical pediatric nurse scale were used to analyze the effect of nursing practice environment on clinical innovation ability of clinical pediatric nurses.Results:The total score of PES-NWI of pediatric nurses was (90.73±13.02) , and the total score of innovation ability of clinical pediatric nurse scale was (120.79±10.84) . There was a positive correlation between the nursing practice environment and the innovation ability score of pediatric nurses ( P<0.05) . The results of stratified regression analysis showed that nurses' participation in hospital affairs and provision of high-quality nursing services in the nursing practice environment of pediatric nurses had a predictive effect on the innovation ability of pediatric nurses ( P<0.05) . Conclusions:The innovation ability of pediatric nurses is at a medium level, and the nursing practice environment has a predictive effect on innovation ability. Nursing managers should pay attention to the improvement of nursing practice environment and take effective measures to improve their innovative ability.
4.The feasibility of cerebral CT angiography in investigating vascular dilatation of the anterior choroidal and posterior communicating artery in Moyamoya syndrome

Objective:To explore the feasibility of CT angiography (CTA) in investigating vascular dilatation of anterior choroidal artery (AChA) and posterior communicating artery (PComA) in patients with Moyamoya syndrome (MMS).Methods:From July 2017 to July 2018, the clinical and imaging data of MMS patients with brain CTA and DSA performed were analyzed retrospectively. According to DSA results, 71 MMS patients were divided into unilateral MMS group (20 cases, 20 hemispheres) and bilateral MMS group (51 cases, 102 hemispheres). There were 20 cases in unilateral MMS group, 10 males and 10 females, with an average age of (45±9) years; 51 cases in bilateral MMS group, 24 males and 27 females, with an average age of (44±12) years. The hemispheres were divided into dilated group and non-dilated group according to the dilatation of AChA or PComA. Kappa analysis was used to evaluate the consistency of two inspection methods to judge the expansion of AChA. The lumen diameters of PComA, P1 and P2 segments of posterior cerebral artery were measured on CTA images, and the ratio of PComA/P1 and PComA/P2 were calculated. The repeatability of CTA measures was evaluated by intra-group correlation coefficient. Independent sample t-test was used to compare CTA measurement results between PComA dilated group and non-dilated group, and ROC curve was drawn to calculate the best threshold for diagnosis of PComA expansion. The sensitivity, specificity, accuracy, positive predictive value and negative predictive value of CTA measures were calculated. Results:The sensitivity, specificity, accuracy, positive predictive value and negative predictive value of CTA diagnosis of AChA expansion inunilateral MMS were all 100.00%. The sensitivity, specificity, accuracy, positive predictive value and negative predictive value of CTA diagnosis of AChA expansionin bilateral MMS were 90.00%, 93.90%, 93.14%, 78.26% and 97.47%. Compared with DSA, there was no significant difference in the diagnostic performance of AChA expansion between single and bilateral MMS diagnosed by CTA ( P>0.05). The two methods had strong consistency (Kappa value was 1.00 and 0.79 respectively, P<0.01). A total of 46 patients (69 cerebral hemispheres) were included in the evaluation of PComA. PComA/P1 (1.09±0.41) and PComA/P2 (0.86±0.13) in the dilated group were significantly higher than those in the non-dilated group (0.71±0.21 for PComA/P1 and 0.75±0.23 for PComA/P2). The differences were statistically significant ( t=-4.59, -2.50, P<0.05). The best threshold in diagnosing PComA expansion was 0.87 (PComA/P1) and 0.76 (PComA/P2), and the sensitivity, specificity, accuracy, positive predictive value and negative predictive value were 84.62%, 83.33%, 84.06%, 86.84%, 80.65% and 79.49%, 60.00%, 71.01%, 72.09% and 69.23%, respectively. Compared with DSA, the Kappa value of CTA measures in diagnosis of PComA expansion was 0.68 (PComA/P1) and 0.40 (PComA/P2), respectively, and the difference was statistically significant ( P<0.05). Conclusions:CTA has a strong consistency with DSA in evaluating the AChA expansion in MMS. When the PComA/P1 ratio on CTA is greater than 0.87, it can be used as the diagnosis criterion for PComA expansion.
5.Assessment of effect of encephalo-duro-arterio-synangiosis on moyamoya disease by CT perfusion

Objective:To evaluate the utility of CT perfusion (CTP) for assessment of effect of encephalo-duro-arterio-synangiosis (EDAS) on moyamoya disease (MMD).Methods:Thirty-eight adult MMD patients, who underwent EDAS in our hospital from March 2014 to October 2019, were chosen in our study. All patients received CTP and digital subtraction angiography (DSA) before and after surgery; cerebral blood flow (CBF), cerebral blood volume (CBV), mean transit time (MTT) and time to peak (TTP) in the operative cerebral cortices and their contralateral mirror areas, as well as brainstems were measured respectively; their relative parameter values (rCBF, rCBV, rMTT and rTTP) were calculated with the brainstem as the reference; perfusion changes of cerebral tissues were observed before and after surgery, and efficacies of these parameters in evaluating the surgical treatment effect were compared. According to the degrees of collateral formation detected by postoperative DSA, the 42 hemispheres of these patients were divided into group of good collateral formation and group of poor collateral formation, and the differences of perfusion changes in the two groups were further discussed.Results:CTP revealed that cerebral perfusion at surgical side after EDAS was significantly improved in all the 42 hemispheres; significantly increased rCBF, and significantly decreased rMTT and rTTP were noted as compared with those before surgery ( P<0.05). Receiver operating characteristic (ROC) curve showed that the area under the curve of rTTP (0.897) was the largest among all parameters, with diagnostic sensitivity of 87.5% and specificity of 80.8%. Twenty-six patients (61.9%) were into the group of good collateral formation and 16 (38.1%) were into the group of poor collateral formation; changed values of rCBF and rMTT in the group of good collateral formation were significantly better than those in the group of poor collateral formation ( P<0.05). Conclusion:CTP can quantitatively evaluate cerebral hemodynamic changes after EDAS in moyamoya disease patients, and postoperative rTTP changes can reflect the degrees of collateral vessel formation.
6.Influence of nursing lack on burden of main caregivers among children hospitalized

Objective To explore the influence of nursing lack of nurses at one children's hospital on burden of family members among children hospitalized so as to provide a theoretical basis for lowering the burden of main caregivers in children hospitalized. Methods From June 2018 to April 2019, we selected main caregivers of children hospitalized and responsible nurses at Children's Hospital of Nanjing Medical University as subjects by convenience sampling. The Miss Care Survey and Caregiver Burden Inventory (CBI) were used to investigate the nursing lack of nurses and burdens of family members. Pearson correlation was used to analyze the influence of nursing lack on burden of main caregivers in children hospitalized. A total of 150 questionnaires were sent out and 146 valid questionnaires were collected with 97.33% for the valid recovery rate. Results Among those 146 nurses, the score of the missed nursing care was (99.11±12.85). A total of 146 nurses were divided into group of nursing lack (n=65) and group of non-nursing lack (n=81) by taking 96 as cutoff value. The scores of the missed nursing care of group of nursing lack and group of non-nursing lack were (92.97±11.37) and (102.31±14.72) respectively. The term scores of the reasons for missed care from low to high were the labor resources, material resources and communication. There were statistical differences in the total score of CBI and scores of time-dependence, developmental, physical, social burden of main caregivers between group of nursing lack and group of non-nursing lack (P<0.05). There was no statistical difference in the score of emotional burden between two groups (P>0.05). Pearson correlation analysis showed that scores of time-dependence, developmental, physical, emotional and social burden had negative correlations with the total score of the miss care survey (P<0.05). Conclusions The missed nursing care of nurses at Nanjing Children's Hospital is widespread. The missed nursing care has negative correlations with burden of main caregivers in children. Hospital managers should take measures to reduce the missed nursing care so as to lower the burden of main caregivers in children and contradictions between nurses and patients.
7.Motion freeze technology for improving quality of cone-beam CT images of renal artery aneurysms

Objective To observe the value of motion freeze technology(MFT)for improving the quality of cone-beam CT(CBCT)images of renal artery aneurysms.Methods Twenty patients with renal artery aneurysms who would receive transcatheter arterial embolization were prospectively enrolled.Renal artery angiography was performed under the guidance of CBCT,and motion artifacts were processed using MFT.Then subjective and objective evaluation of images before and after MFT correction were performed,aimed on displaying of renal artery,parent artery and vascular tree in target area,and the mean value(mean)and standard deviation(SD)of unit density pixels,margin sharpening error rate of blood vessels were measured and calculated.Results After MFT correction,the displaying of renal artery on maximum intensity projection images and parent artery on reconstructed CBCT images were significantly better than those before correction(both P<0.05),clear vascular trees could be automatically extracted using Flightplan for Liver software,SD and margin sharpening error rate of blood vessels of renal artery aneurysm significantly decreased but mean significantly increased on CBCT images(all P<0.05).Conclusion MFT could effectively improve the quality of CBCT images of renal artery aneurysms.
8.Comparison of clinically relevant factors in bipolar disorder patients with different age of onset

Objective:To compare clinical characteristics,treatment patterns and physiological indicators in bipolar disorder(BD)patients with different age of onset.Methods:Totally 380 patients with DSM-5 BD were se-lected in this study.Psychiatrists diagnosed the patients using the Mini International Neuropsychiatric Interview.The clinical information questionnaire and the Global Assessment of Functioning scale were utilized to collected clinical characteristics,treatment status,and physiological indicators.The onset age of BD was divided into 21 and 35 years as cut-off points.Multivariate logistic regression and linear regression were used to analyze related factors.Results:Among the 380 patients with BD,199 cases were early-onset group(52.4%),121 cases were middle-onset group(31.8%),and 60 cases were late-onset group(15.8%).There were 26.6%of patients in the early-onset group in-itially diagnosed as depression,23.1%in the middle-onset group,and 11.7%in the late-onset group.Multivariate analysis revealed that compared to the early-onset group of BD,the middle-onset(OR=2.22)and late-onset(OR=4.99)groups had more risk to experience depressive episodes,and the late-onset group(OR=6.74)had 6.74 times of risk to suffer from bipolar Ⅱ disorder.Additionally,patients in the middle-onset(β=-1.52)and late-on-set(β=-4.29)groups had shorter durations of delayed treatment,and those in the middle-onset(β=-1.62)and late-onset(β=-3.14)groups had fewer hospitalizations.Uric acid levels were lower in both the middle-onset(β=-28.39)and late-onset(β=-31.47)groups,and total cholesterol level was lower in the middle-onset group(β=-0.23).Conclusion:Patients with BD in different age of onset show significant differences in clinical charac-teristics,treatment conditions and physiological indicators.
9.Compliance of residents with repeated screening for colorectal cancer in Jiading District, Shanghai

ObjectiveTo explore the compliance related factors of repeated screening for colorectal cancer in Jiading District, and to provide a scientific basis for the prevention and control of colorectal cancer. MethodsBased on the natural population cohort in Jiading District, and the screening situation in 2017‒2019 and 2020‒2022, the study subjects were divided into the groups of never participating in screening and participating in screening. Subjects in the participating group were further divided into participating in one round of screening or having repeated screening. SPSS 21.0 software was used to analyze the demographic characteristics of each group. χ2 test or Fisher precise probability test were used to conduct univariate analysis of the factors such as gender, age, education level, marital status, retirement status, and type of medical insurance. Factors with the significant difference (P<0.05) were selected for inclusion in multivariate analysis, and factors related to compliance with repeated screening were analyzed by multivariate logistic regression. ResultsA total of 8 179 subjects were included in the study, including 3 323 males (40.6%) and 4 856 females (59.4%). The average age of the subjects was (61.26±6.06) years old. A total of2 652 (32.4%) had educated in primary school or below, 4 242 (51.9%) in secondary school, and 1 285 (15.7%) in higher secondary school. Mostly, 7 579 (92.7%) were married. Among the participants, 4 062 people had never participated in screening, 4 117 people had participated in screening, and 1 485 of them had repeated screening, with a repeated screening rate of 18.2%. Multivariate logistic regression analysis showed that women had better compliance with repeated screening than men (OR=1.31, 95%CI: 1.14‒1.50). Compared with the population aged 50 to 54 years, the population aged 55‒59 years (OR=1.57, 95%CI: 1.19‒2.08), 60-64 years (OR=2.77, 95%CI: 2.13‒3.61), and 65-69 years (OR=3.31, 95%CI: 2.51‒4.36) had higher compliance with repeated screening. Compared with employees' medical insurance, residents' medical insurance group had worse compliance with repeated screening (OR=0.76, 95%CI: 0.66‒0.87). People with a history of intestinal polyps were more likely to undergo repeat screening than those without (OR=2.07, 95%CI: 1.50‒2.87). ConclusionCompliance with repeated screening for colorectal cancer still needs to be improved, and there are differences in compliance with repeated screening for different populations with different characteristics. Identifying groups that are unlikely to adhere to community-based colorectal cancer screening and taking targeted interventions can help improve the continued compliance of residents with colorectal cancer screening.
