Objective By analyzing the urinary calculi ingredient and 24-hour urinary stone risk factors of urolithiasis patients in Hengyang area,to investigate the effects of diet intervention on recurrence rate in urolithiasis patients, and provide the measures for prevention and treatment. Methods Prospectively collected 692 patients that permanent residents in Hengyang area from September 2008 to September 2012,who had implementation of minimally invasive operation and taken stone specimens to analyze composition, and also collected 24 hours urine to analyze the urinary stone risk factors. They were divided into test group and control group by random number table method,346 cases in each, control group without diet intervention, and test group was given diet intervention according to the stone composition and urinary stonerisk factors. All patients were followed up for 1 year, the urinary stone recurrence rate in Hengyang area was observed. Results Among 692 urolithiasis patients,663 patients completed the study (test group of 341 cases and control group of 322 cases),the expulsion rate was 4.19%(29/692).The 24-hour urinary stone risk factors in control group before and after diet intervention had no significant difference(P > 0.05).In test group after diet intervention, the excretion of ingredients in urine such as dietary calcium (t =3.412,P < 0.05),oxalate(t =3.018,P < 0.05) and uric acid(t =1.990,P < 0.05) was obviously decreased, and urinary citrate (t =3.174,P < 0.05) was increased, but the excretion of ingredients such as magnesium and phosphorus had no significant difference (P > 0.05).After 1 year after diet intervention, the recurrence rate in test group was lower than that in control group [0.88% (3/341) vs.7.76% (25/322)],there was significant difference (P < 0.01).Conclusion Diet intervention can effectively reduce the risk of urinary stone according to the stone composition and the 24 hours urine stone risk factors, plays an important role on reducing urinary stone recurrence, which is worth clinical promotion.

Objective To detecte the expression of Apelin ( APLN) in prostate cancer tissue and to investi-gate its correlation with prognosis of prostate cancer .Methods Collect prostate cancer patients in Department of Urology of Huadu District People's Hospital for 2014 -2016 years in Guangzhou .The expression of APLN was detected by real-time fluorescence quantitative polymerase chain reaction ( qRT-PCR) in 20 prostate cancer tissues and 20 adjacent normal tissues. The difference between the two groups was compared .And 104 samples of primary prostate cancer and 28 samples of benign paracancerous tissues from the Taylor database were selected to analyze its relationship with the clinical features and prognosis of the patients .Results Compared with the benign paracancerous tissue(7.26 ±0.03),APLN was up-regulated in the prostate cancer tissue (7.62 ±0.42)(t=3.824,P<0.001). The up-regulation of APLN was associated with pathological stage (t=2.942,P=0.003),metastasis(t=3.022, P<0.001),Gleason score (t =2.399,P =0.031),the biochemical recurrence -free survival(t =2.533,P =0.001 ) ,and the biochemical recurrence -free survival of the patients with higher expression of APLN was shorter than that of the patients with lower expression of APLN(χ2 =6.268,P=0.012).Conclusion The abnormal expres-sion of APLN may be associated with tumor formation and malignant progression of prostate cancer .High expression of APLN can predict the biochemical recurrence -free survival in patients with prostate cancer .

OBJECTIVETo study the long-term curative effects of suture plus proximal gastric vagotomy (PGV) and suture plus triad-therapy (omeprazole, amoxycillin and flagyl taken orally) for the treatment of duodenal ulcer with acute perforation.
METHODSThree hundred and twenty-nine patients with duodenal ulcer and acute perforation were treated with 2 different methods, respectively. Method A was suture plus PGV (group A, 153 cases), and method B was suture plus triad-therapy (group B, 176 cases). Follow-up was made by means of correspondence, outpatient reexamination and cooperation with local hospitals in 5 to 8 years after operation. The contents of follow-up included symptom acquisition (such as upper abdominal pain or distention, pyrosis, belch, acid regurgitation, vomiting, diarrhea and conditions of living or working), gastroscopy and Helicobacter pylori (HP) detection. The curative effects were evaluated by the Visick scale.
RESULTSThree hundred and one patients were followed up (group A 142 and group B 159). According to the Visick scale, 97 (68.3%), 19 (13.4%), 13 (9.15%) and 13 (9.15%) patients in group A, and 31 (19.5%), 28 (17.6%), 24 (15.1%) and 76 (47.8%) in group B were classified as Visick I, II, III and IV respectively (Z = -9.818, P < 0.01). As for HP detection, there were 130 (91.5%) patients in group A and 94 (59.1%) in group B (chi(2) = 41.438, P < 0.01).
CONCLUSIONSThe long-term curative effects of suture plus PGV were superior to those of suture plus triad-therapy for duodenal ulcer with acute perforation although HP positive rate was higher in group A than in group B. HP infection is one of the etiological factors of duodenal ulcer. The increased excitability of the vagus nerve remains to play an important role in duodenal ulcer.

Objective:To investigate the workflow, efficacy and safety of MR-Linac in liver malignancies. Methods:Clinical data of 15 patients with hepatocellular carcinomas (HCC) or liver metastases treated with MR-Linac between November 2019 and July 2021 were retrospectively analyzed. The workflow of MR-Linac was investigated and image identification rate was analyzed. Patients were followed up for response and toxicity assessment. Results:Fifteen patients (6 HCC, 8 liver metastases from colorectal cancer, 1 liver metastasis from breast cancer) were enrolled. A total of 21 lesions were treated, consisting of 10 patients with single lesion, 4 patients with double lesions and 1 patient with triple lesions. The median tumor size was 2.4 cm (0.8-9.8 cm). The identification rate for gross tumor volume (GTV) in MR-Linac was 13/15. Although GTV of two patients were unclearly displayed in MR-Linac images, the presence of adjacent blood vessel and bile duct assisted the precise registration. All the patients were treated with stereotactic body radiation therapy (SBRT). For HCC, the median fraction dose for GTV or planning gross tumor volume (PGTV) was 6 Gy (5-10 Gy) and the median number of fractions was 9(5-10). The median total dose was 52 Gy (50-54 Gy) and the median equivalent dose in 2 Gy fraction (EQD 2Gy) at α/ β= 10 was 72 Gy (62.5-83.3 Gy). For liver metastases, the median fraction dose for GTV or PGTV was 5 Gy (5-10 Gy) and the median number of fractions was 10(5-10). The median total dose was 50 Gy (40-50 Gy) and the median EQD 2Gy at α/ β=5 was 71.4 Gy (71.4-107.1 Gy). At 1 month after SBRT, the in-field objective response rate (ORR) was 8/13 and the disease control rate was 13/13. At 3-6 months after SBRT, the in-filed ORR was increased to 6/6. During the median follow-up of 4.0 months (0.3-11.6), 4-month local progression-free survival, progression-free survival and overall survival were 15/15, 11/15 and 15/15, respectively. Toxicities were mild and no grade 3 or higher toxicities were observed. Conclusions:MR-Linac provides a platform with high identification rates of liver lesions. Besides, the presence of adjacent blood vessel and bile duct also assists the precise registration. It is especially suitable for liver malignancies with promising local control and well tolerance.

Ulcerative colitis (UC) is a chronic non-specific intestinal inflammatory disease. Whether surgical treatment should be taken is a difficult and focus decision of clinical practice. Studies have shown that severe complications, combined with severe infection, poor laboratory indices, and ineffective medical treatment are the evaluation elements of UC surgery. This article reviewed the advances in study on high risk factors of surgery with UC.
