Objective To analyse change of neuroendocrine hormone and cardiac ultra ejection force in patients with heart failure before and after treatment with Huangqi Decoction.Methods 100 patients diagnosed with chronic heart failure were collected.According to the different treatment regimens divided into two groups ( control group: conventional western medicine; traditional Chinese medicine group: conventional western medicine treatment +Astragalus Decoction orally for 2 weeks) , before and after treatment,nerve endocrine hormone using echocardiography evaluation index and ejection force were detected in two groups, and compare the results of data.Results Compared with control group, Chinese medicine group with better anti heart failure treatment, performance: the plasma calcitonin gene related peptide (CGRP), atrial natriuretic peptide (ANP), C type natriuretic peptide ( CNP ) , neuropeptide Y ( NPY ) levels were significantly lower ( P<0.05 ) . Capacity index significantly improved cardiac function (P<0.05).Left atrial diameter, left atrial maximum, minimum volume decreased significantly (P<0.05).Ejection force of left atrium and left ventricular increased significantly ( P<0.05 ) .The results were statistically significant.Conclusion Huangqi decoction can inhibit the secretion of CGRP, ANP, CNP,NPY and other neuroendocrine hormones in patients with heart failure, reduce cardiac load, increase cardiac ejection force.

Objective To understand the effects of chemoradiotherapy combined with radiotherapy on peripheral differential blood cell counts on tumor patients.Methods 40 patients receiving concurrent chemoradiotherapy were randomly selected in the radiotherapy department in our hospital.Peripheral blood WBC,PLT radiotherapy for 1-4 weeks were retrospective analysis.Results The first 1-4 week of radiotherapy,peripheral blood cells of WBC,PLT decreased.The difference was statistically significant (WBC:x2 =16.902,P =0.001,PLT:x2 =8.492,P =0.037),and obviously in the second week.Conclusion Tumor radiotherapy decreases blood cell counting and classification in the WBC,PLT,and obviously in the second week.

Objective To explore the correlation between delirium and dementia in elderly patients undergoing hip fracture surgery.Methods A prospective study was conducted on 118 elderly patients with hip fracture who were free from prefracture dementia.Caregivers or the relatives described the patients' prefracture cognition by the Informant Questionnaire on Cognitive Decline in the Elderly-short form (IQCODE SF).Delirium was assessed by the Confusion Assessment Method (CAM).After 6 months,cognition was reassessed by using 5 cognitive tests,taking DSM-Ⅳ as the diagnostic criteria.Results Among 32 cases (27％) suffered from delirium in the acute phase,12 cases (38％) reached to the diagnostic criteria of dementia after 6 months follow up,while there were 6 cases (7％) reached to the diagnostic criteria of dementia after 6 months follow-up in the group without delirium in the acute phase (x2 =16.81,P＜0.001).IQCODE-SF score had no statistically significant difference in the prediction of dementia.Conclusions In elderly patients without the history of dementia,delirium after hip fracture surgery is the major predictor of dementia within half years.

BACKGROUND:Alcohol has become pathogenic factors of avascular necrosis, and the alcohol induced
abnormal lipid metabolism in bone marrow may be the important reason for the onset of avascular necrosis, but the mechanism is not clear yet.
OBJECTIVE:To observe the changes of structure and function of fat cel s under the action of alcohol, in order to analyze the pathogenesis of alcoholic femoral head necrosis.
METHODS:Primary adipocytes in vitro culture technique was used to obtain rabbit femoral head intramedul ary adipose tissue, and then the fat cel s were separated, and the phenotype was identified with oil red O staining. The passaged stable intramedul ary fat cel s were col ected. Coverslip was cut into 1 cm × 1 cm in size, and placed in the 24-wel culture plate before planting. The cel s were randomly divided into alcohol group and control group, 24 holes (each hole for a sample) in each group. The control group was without alcohol, while the alcohol group was added with 0.15 mol/L alcohol. At 4, 6, 8 and 10 days, the culture medium was replaced. Medium was changed and no longer adding alcohol, and then cultured for 10 days. When the culture terminated, the coverslip was removed for oil red O staining. Final y, the morphology and the number of the fat cel s were observed under light
microscope.
RESUTLS AND CONCLUSION:With time prolonging, the number of fat cel s in the alcohol group was significantly more than that in the control group (P<0.001). The lipid droplets in the two groups were gradual y increased and enlarged, but more significant in the alcohol group. The number of intramedul ary fat cel s in the alcohol group after cultured for 4, 6, 8 and 10 days was respectively (200.90±24.60), (1 102.30±76.73), (1 160.30±28.37) and (1 199.70±44.74)/cm2;the
number of intramedul ary fat cel s in the control group was respectively (99.80±10.82), (0.40±94.71), (1 000.20± 41.85) and (1 059.80±26.79)/cm2, the number of fat cel s increased with the time of alcohol influence. Alcohol can promote the intramedul ary fat cel s to increase and enlarge, and this may be the main reason for femoral head necrosis, as long-term alcoholism can lead to bone marrow fat tissue increasing, intraosseous pressure increasing and perfusion reducing, thus resulting ischemia.

Objective To detect specific IgM of Lyme disease indirect ELISA using recombinant outer surface protein C(OpsC)of Borrelia burgdorferi in vitro was established. Methods Coated concentration of recombinant OspC and dilution multiple of serum anol concentration of enzyme secondary antibody were determined by block,and degree of percision.specificity interference and interruption test were performed. Results Best concentration of OspC was 150 μg/L.within-run CV was 6.3% between-run CV was 11.8%.Confimred 33 Lyme patients in clinic and 57 controls were examined meanwhile by this method and import ELISA kit,coincidena rate was 97.8%.Conclusion This ELISA using recombinant OspC was a good method for early diagnostic of Lyme disease.

Primary breast lymphoma (PBL) is a kind of rare primary extranodal lymphoma, which is of relatively poor prognosis and so far there is no unified treatment. In the case diagnosed by Our hospital where one patient recurring breast diffuse large B cell lymphoma (DLBCL) many times, has been treated with good therapeutic effect through surgery, chemotherapy, targeted drug therapy and radiotherapy. A multidisciplinary discussion is made concerning the treatment of the patient in this article for the purpose of further discussing the standard treatment procedure for PBL and promoting multi-disciplinary team.

Objective To study the effect of infrared irradiation on the expression of epidermal growth factor EGF) and epidermal growth factor receptor (EGFR) in the gastric mucosa of rats with chronic atrophic gastritis CAG),and to explore its mechanism.Methods Forty healthy adult and clean Wistar male rats were randomized into a normal control group (N group),a model control group (M group) and an infrared group (I group) using a random number table.CAG was induced in the rats of the M and I groups by intra-gastric infusion of 2 ml of a mixture of 30％ alcohol and 2％ sodium salicylate once every day for 8 weeks.This was combined with compulsory running and irregular fasting as pathogenic factors.After the CAG model had been established,the rats in the I group were given infrared irradiation for 10 rain once every day for 20 days.Then all of the rats were sacrificed and the expression of EGF and EGFR was observed using immunohistochemistry.Results The positive staining scores for EGF and EGFR of the M group were 4.91±2.58 and 6.75±2.63,significantly higher than those of the N group (2.10±0.65 and 4.20±2.24 respectively).Those of the I group were 2.75±1.01 and 4.75± 1.66,significantly lower than those of the M group,though not significantly different from the N group's averages.Conclusion Infrared irradiation can inhibit over-expression of EGF and EGFR in the gastric mucosa of rats with chronic atrophic gastritis.This should protect the gastric mucosa and promote tissue repair and healing.

Objective To study the effects of He-Ne laser irradiation on the pH value of gastric juice, the ultrastructure of gastric parietal cells and the content of nitric oxide ( NO ) in chronic atrophic gastritis ( CAG ) . Methods Sixty healthy male Wistar rats were randomly divided into a normal control group, a model group, a drug group, an acupuncture group and a laser acupoint group, each of 12. A CAG model was successfully intro-duced to all except those in the normal control group through the administration of sodium salicylate and alcohol, combined with fasting and water deprivation, running fatigue and other interventions. The rats of the drug, acupunc-ture and laser acupuncture groups were given medications, acupuncture at Zusanli acupoint and laser irradiation at Zusanli acupoint accordingly. After 14 days of treatment, all of the rats were executed and diluted gastric juice and gastric tissues were collected. NO content in serum was determined using a nitrate enzyme reduction method, the pH value of gastric juice was measured, and the ultrastructure of the gastric mucosa was observed using a transmis-sion electron microscope. Results The average pH of the gastric juice and the average serum NO content were both significantly higher in the model group than in the normal control group. In the model group the membrane structure of the parietal cells was absent or partly dissolved and the mitochondria were swollen. Drugs, acupuncture and laser irradiating at the Zusanli acupoint all corresponded with reduced pH and NO content, but the most signifi-cant improvements were observed in the laser acupuncture group. In the laser acupuncture group the membrane structure of parietal cells was complete, and the cytoplasmic organelles had nearly returned to normal. Conclusions Laser irradiation can promote recovery from CAG, increasing the secretion of gastric acid and de-creasing the serum level of NO. Both have a good curative effect for CAG. Laser irradiation deserves further clinical research and promotion.

Objective To systematically investigate the molecular epidemiological profiles of human papillomavirus (HPV) in patients with condyloma acuminata(CA). Methods Two hundred and one samples of HPV DNA isolated from CA were PCR amplified by the PGMY09/11 primer system. The PCR products were simultaneously hybridized to 37 specific HPV probes immobilized on a nylon strip and then genotyped. All DNA templates were further PCR amplified using HPV 6 and 11 type specific primers for verification. Results All samples were HPV DNA positive consisting of totally 31 genotypes, the types of which were type 11(53.7%, 108/201), 6(43.8%, 88/201), 16(6.5%, 13/201), 52(6.0%, 12/201), 33(5.5%, 11/201), cp6108 (5.5%, 11/201) and 42 (5.0%, 10/201). The samples infected with a single and mixed types of HPV accounted for 60.2% (121/201) and 39.8% (80/201) respectively. Consistent results were found with the detection of HPV6 and 11 between hybridization assay and type-specific PCR. Conclusions At least 31 HPV genotypes are associated with CA. HPV 11 predominates while 68, 40, 54, 67, 73, 82, 35, 64 and 83 are rare in CA. Type cp6108 is detected in CA for the first time with a high prevalence. HPV26, 69, 70, 71,72 and IS39 might be not associated with CA. CA infected with a single and mixed HPV types accounts for 60.2% and 39.8%, respectively.

Objective To explore the effect of low dose ionizing radiation on the thyroid of radiation workers in our country. Methods The literatures were searched of domestic and foreign journals from 1996 to 2017. A total of 15 studies involving 6332 participants were included. These studies were screened according to the inclusion and exclusion criteria. Three indicators of the thyroid function were analyzed in radiation workers, including triiodothyronine ( T3 ) , thyroxine ( T4 ) and thyroid stimulating hormone( TSH) . The Meta analysis was performed using Stata 14. 0 software. Results For radiation workers, the standardized mean difference (SMD) of T3 levels was -0. 32[95% CI ( -0. 61,-0. 04), P=0. 03?, lower than that of the control group. Univariate meta - regression analysis of sources of heterogeneity showed that only gender composition was related to heterogeneity of the study [ adjusted R-squared (Adj R2 ) =29. 99%, P =0. 03]. The SMD of T4 levels was -0. 49 [95%CI ( -0. 94,-0. 03), P=0. 04], lower than that of the control group. Only the gender composition was related to heterogeneity of the study[Adj R2 =26. 60%, P=0. 02]. However, the SMD of TSH levels was 1. 10 [95% CI (0. 32,1. 89), P=0. 006], higher than that of the control group. Conclusions The long term exposure of radiation workers to low dose ionizing radiation may lead to negative impact on thyroid. It is important to strengthen radiation protection of thyroid.
